Background: Secondary alveolar bone graft has been widely used for the closure of maxillary bone defects in patients with cleft lip and palate (CLP). However, the log-term stability of grafted bone in detail remains unclear. Purpose: To evaluate the nature of longitudinal changes in the grafted bone from autogenous iliac bone in patients with CLP for more than 2 years after the surgery. Methods and Subjects: The subjects were 124 CLP patients treated with iliac bone graft in Hiroshima University Hospital from 1997 to 2007. The height and location of the bone bridge was evaluated radiographically before and after surgery. Results: 1) Bone graft was performed at a mean age of 12.5 (±4.2) years old. The canines on the cleft side were not erupted at the surgery in 60.0%. All canines erupted except for the congenital missing and impacted ones (11.3%) within 2 years after bone graft;2) The height of the bone bridge was more than 11 mm in 71.8% of all patients 1 month after surgery. In addition, the location of bone bridge in 66.1% of all patients were clinically satisfactory. However, these rates decreased time-dependently owing to bone resorption after the bone graft. Conclusions: In conclusion, it is shown that autogenous bone graft produce a sufficient bone bridge in the jaw cleft area by the long-term evaluation of the changes in height and location of bone bridge, although some amount of time-dependent bone resorption is inevitable.

We report a case of postoperative refractory bile leakage managed successfully by intrahepatic biliary ablation with ethanol. A 75-year-old man diagnosed with hepatocellular carcinoma underwent extended posterior segmentectomy including the caudate lobe and a part of the anterior segment. The hepatic tumor attached to the anterior branch of the bile duct was detached carefully and resected. Fluid drained from the liver surface postoperatively contained high concentrations of total bilirubin, at a constant volume of 150 mL per day. On d 32 after surgery, a fistulogram of the drainage tube demonstrated an enhancement of the anterior bile duct. Endoscopic retrograde cholangiography demonstrated complete obstruction of the proximal anterior bile duct and no enhancement of the peripheral anterior bile duct. On d 46 after surgery, a retrograde transhepatic biliary drainage （RTBD） tube was inserted into the anterior bile duct under open surgery. However, a contrast study of RTBD taken 7 mo post-surgery revealed that the fistula remained patent despite prolonged conservative man- agement, so we decided to perform ethanol ablation of the isolated bile duct. Four mL pure ethanol was injected into the isolated anterior bile duct for ten minutes, the procedure being repeated five times a week. Following 23 attempts, the volume of bile juice reached less than 10 mL per day. The RTBD was clamped and removed two days later. After RTBD removal, the patient had no complaints or symptoms. Follow-up magnetic resonance imaging demonstrated atrophy of the ethanol-injected anterior segment without liver abscess formation.

To understand the role of forest ecosystems in the global carbon cycle, it is important to clarify the factors affecting the carbon balance of forest ecosystems. However, little is known about the direct effect of forest types, especially dominant species, on their different carbon dynamics. To clarify the effect of difference in forest types, an experiment was conducted in three forests, which were located in the same place and exposed to the same climate conditions. These forests were middle-aged (40 - 45 years) and dominated by Quercus serrata (Q forest), Larix kaempferi (L forest) and Pinus densiflora (P forest). Net primary production (NPP), heterotrophic respiration (HR) and net ecosystem production (NEP) were estimated in each forest, using a biometric method over one year. For NPP estimated from the annual growth of tree biomass (ΔB) and amount of litter (LF), P forest NPP (5.3 MgC·ha-1·yr-1) was higher than Q and L forest NPP (4.6 and 3.2 MgC·ha-1·yr-1). The difference was affected by a significant difference in ΔB (p = 0.032) and LF (p -1·yr-1) was higher than L and P forest (2.3 and 2.1 MgC·ha-1·yr-1). This difference could result from the amount of litter (respiration substrate) and chemical properties of litter (lability of decomposition). The NEP, which was calculated from the difference between NPP and HR, varied widely among the forest types (0.5, 0.9 and 3.2 MgC·ha-1·yr-1 in Q, L and P forests, respectively). The range of values among the forest types was comparable to those among age sequences and climate zones in previous studies. These results suggest that the difference in forest types (especially dominant species) can potentially lead to a large variation in carbon dynamics, in ecosystems located in the same place.

This report describes a method for percutaneous transhepatic biliary stenting with a BLAKE Silicone Drain, and discusses the usefulness of placement of the drain connected to a J-VAC Suction Reservoir for the treatment of stenotic hepaticojejunostomy. Percutaneous transhepatic biliary drainage was performed under ultrasonographic guidance in a patient with stenotic hepaticojejunostomy after hepatectomy for hepatic hilum malignancy. The technique used was as follows. After dilatation of the drainage root, an 11-FF tube with several side holes was passed through the stenosis of the hepaticojejunostomy. A 10-Fr BLAKE Silicone Drain is flexible, which precludes onestep insertion. One week after insertion of the 11-Fr tube, a 0.035-inch guidewire was inserted into the tube. After removal of the 11-Fr tube, the guidewire was put into the channel of a 10-Fr BLAKE Silicone Drain. The drain was inserted into the jejunal limb through the intrahepatic bile duct and was connected to a J-VAC Suction Reservoir. Low-pressure continued suction was applied. Patients can be discharged after insertion of the 10-Fr BLAKE Silicone Drain connected to the J-VAC Suction Reservoir. Placement of a percutaneous transhepaUc biliary stent using a 10-Fr BLAKE Silicone Drain connected to a J-VAC Suction Reservoir is useful for the treatment of stenotic hepaticojejunostomy.

We determined the sequence of mitochondrial Cytochrome oxidase subunit I （CO1） in two Drosophila melanogaster strains originating at ＂Evolution Canyon＂, Israel. CO1 nucleotide sequences from two iso-female strains, 2-1 and 6-1, were 1,534 and 1,543 base-pairs, respectively. In each strain, ATAA was used in initiation of translation. Exchange rates for nucleotide and amino acid sequences were larger in the 6-1 strain than in the 2-1 strain when Oregon-R was used as the standard. Non-synonymous exchange rate was larger than synonymous exchange rate among the three strains.

AIM: To investigate changes over time in, and effects of sealing technology on, milk test results following pancreatectomy. METHODS: From April 2008 to October 2013, 66 pancreatic resections were performed at the Iwakuni Clinical Center. The milk test has been routinely conducted at the institute whenever possible during pancreatectomy. The milk test comprises the following procedure: A nasogastric tube is inserted until the third portion of the duodenum, followed by injection of 100 mL of milk through the tube. If a chyle leak is present, the patient tests positive in this milk test based on the observation of a white milky discharge. Positive milk test rates, leakage sites, and chylous ascites incidence were examined. Liga Sure?(LS; Covidien, Dublin, Ireland), a vessel-sealing device, is routinely used in pancreatectomy. Positive milk test rates before and after use of LS, as well as drain discharge volume at the 2^(nd) and 3^(rd) postoperative days, were compared retrospectively. Finally, positive milk test rates and chylous ascites incidence were compared with the results of a previous ***: Fifty-nine milk tests were conducted during pancreatectomy. The positive milk test rate for all pancreatectomy cases was 13.6%(8 of 59 cases). One case developed postoperative chylous ascites(2.1% among the pancreatoduedenectomy cases and 1.7% among all pancreatectomies). Positive rates by procedure were 12.8% for pancreatoduodenectomy and 22.2% for distal pancreatectomy. Positive rates by disease were 17.9% for pancreatic and 5.9% for biliary diseases. When comparing results from before and after use of LS, positive milk test rates in pancreatoduodenectomy were 13.0% before and 12.5% after, while those in distal pancreatectomy were 33.3% and 0%. Drainage volume tended to decrease when LS was used on the 3^(rd) postoperative day(volumes were 424 ± 303 mL before LS and 285 ± 185 mL after, P = 0.056). Aim:The present study was designed to evaluate the feasibility of preoperative liver functional volumetry performed by 3D-technetium-99m-diethylenetriaminepentaacetic acid-galactosyl-human serum albumin(99mTc-GSA)scintigraphy/vascular fusion imaging using SYNAPSE VINCENT and to examine the discrepancy between conventional and functional ***:The study group comprised 15 patients who underwent preoperative 3-dimensional(3D)-99mTc-GSA scintigraphy/vascular fusion imaging using SYNAPSE VINCENT software before hepatectomy between July 2014 and August *** diagnosis was hepatocellular carcinoma(n=4),metastatic liver tumor(n=10),or intrahepatic cholangiocarcinoma(n=1).Right hepatectomy was performed in 2 patients,left hepatectomy in 3 patients,right posterior sectionectomy in 3 patients,segmentectomy in 2 patients,and partial hepatectomy in 4 patients.99mTc-GSA scintigraphy and computed tomography(CT)were performed to construct 3D-99mTc-GSA scintigraphy/vascular fused *** conventional volume ratio of the planned resection region without tumor(%CT),and the functional volume ratio of the planned resection region without tumor(%GSA)were *** discrepancy ratio was calculated as follows:discrepancy ratio=100-%GSA/%CT×100(%).Results:The%GSA(17.9±16.7%)was significantly lower than the%CT(21.5±17.6%)(P<0.036).In all except 2 patients,the%GSA was lower than the%*** discrepancy ratio ranged from-4%to 75%(median,20.7%).Conclusion:3D-99mTc-GSA scintigraphy/vascular fused images constructed using SYNAPSE VINCENT were useful for noninvasively performing functional liver volumetry in patients scheduled to undergo various patterns of *** planned resection regions without tumor,the functional volume ratio was about 20%lower than the conventional volume ratio.

It is known that cochlear implantation for deaf patients with eosinophilic otitis media (EOM) is safe and can provide good speech perception. However, the best timing of implant surgery in patients with EOM is not yet known. The aim of this case report is to suggest the appropriate timing of the surgery in EOM patients with deaf. Cochlear implantation was indicated in two patients with EOM. One underwent cochlear implantation in the absence of any ear discharge. In the other case, implant surgery was delayed for three years due to persistent ear discharge. No complications related to implant device or skin flap were observed in either case. The speech recognition score after implantation was good in the first case and poor in the second case. Perioperative complications were manageable even in the patient with persistent ear discharge. However, the delay in implant surgery due to the persistent ear discharge resulted in a poor speech recognition score. Early implantation should be considered even in EOM patients with ear discharge, although the presence of active middle ear inflammation is regarded as one of the contraindications for implantation according to the current Japanese guidelines.

目的了解小鼠睾丸组织内血清白蛋白和IgG渗透性的时相性变化,揭示支持细胞间紧密连接的屏障功能。方法运用活体内冷冻固定技术对小鼠睾丸进行快速冷冻固定和冷冻置换,并作石蜡包埋。5μm厚连续切片用于抗鼠血清白蛋白和IgG的免疫组化染色和H.E染色。结果H.E染色切片显示距冷冻组织表面300-400μm深度范围内的精曲小管结构保存完好,无明显冰晶形成所致的人工损伤。血清白蛋白和IgG免疫反应产物主要定位于精曲小管管周肌样细胞周围,以及间质细胞之间和毛细血管内,部分免疫反应产物呈拱型出现在精曲小管上皮内,并严格限定在基底室,沿精原细胞表面分布。拱型免疫反应产物的数量与精曲小管上皮周期的发育时相密切相关。结论活体内快速冷冻固定和冷冻置换结合血清白蛋白和IgG的免疫组化方法,可很好地保存精曲小管组织结构并清楚地揭示支持细胞间紧密连接结构的时相变化。
