The Evaluation of Tolvaptan Therapy and Long-Term Prognosis in Hepatocellular Carcinoma with Refractory Ascites

＜正＞In this paper, pure Aluminum and cluster diamond-dispersed aluminum composites were made by the compression shearing method. The friction coefficient, density, mechanical properties, and microstructures of compacted powders were investigated. The addition of 1vol. % CD to Al improved the friction coefficient. The average friction coefficients of Al, 1 vol. % CD sample, and 5vol. % CD sample indicate 0.91, 0.71 and 0.83, respectively. Comparing with one of Al, these values are reduction of 22% and 9% , respectively. However, the 5vol % CD solidified by compression shearing method and dynamic compaction method shows an increase in friction coefficient. The pores can be observed at 5% CD sample. It is considered that the lubricating ability of the Al composite containing 5% CD decreases by the pores, consequently the friction coefficient is increased. In compression shearing, the oxidation layer formed on the Al powder surface was destroyed. However, the bonding of the powders was lower in hot press and dynamic compacting, since it was not possible to sufficiently destroy the oxidation layer. Therefore, the friction coefficient of composite material molded by compression shearing is lower than the one of composite material molded by hot press and dynamic compacting.

We investigated Tolvaptan efficacy and long-term prognosis with focus on nutrition in 20 patients with refractory hepatic ascites in hepatocellular carcinoma (HCC). Bloating improved in 55% of patients, as determined using a Japanese version of the Support Team Assessment Schedule. Nutritional status improved with Tolvaptan treatment, based on the Controlling Nutritional Status score and Onodera’s prognostic nutritional index. Long-term prognosis was better in responders than in non-responders (mean survival time: 308 days vs. 97 days, p = 0.031). Tolvaptan was even effective in many patients with HCC, with additional improvement in long-term prognosis expected with improvement in the nutritional status.

A laser trapping-microspectroscopy technique combined with excitation energy transfer from a fluorescent cationic dye (Rhodamine B, RB+) to a non-fluorescent cationic dye (Malachite Green, MG+) was employed to study pH effects on the diffusion coefficients of MG+ (D(MG+)) in single cation-exchange resin microparticles with the diameters of 16 μm. When RB+-pre-adsorbed resin particles were soaked in an aqueous MG+ solution, the RB+ fluorescence was quenched gradually with the soaking time. The time course of the quenching efficiency of RB+ by MG+ was then used to evaluate the D(MG+) value in the particle. The D(MG+) value increased from 1.1 × 10-11 to 4.3 × 10-11 cm2.s–1 on going the solu- tion pH value from 9 to 4. The results were explained reasonably by a Donnan electric potential model.

This paper focuses on the study and control of a non-linear mathematical epidemic model ( SSvihVELI ) based on a system of ordinary differential equation modeling the spread of tuberculosis infectious with HIV/AIDS coinfection. Existence of both disease free equilibrium and endemic equilibrium is discussed. Reproduction number R0 is determined. Using Lyapunov-Lasalle methods, we analyze the stability of epidemic system around the equilibriums (disease free and endemic equilibrium). The global asymptotic stability of the disease free equilibrium whenever Rvac is proved, where R0 is the reproduction number. We prove also that when R0 is less than one, tuberculosis can be eradicated. Numerical simulations are conducted to approve analytic results. To achieve control of the disease, seeking to reduce the infectious group by the minimum vaccine coverage, a control problem is formulated. The Pontryagin’s maximum principle is used to characterize the optimal control. The optimality system is derived and solved numerically using the Runge Kutta fourth procedure.

Symplectic Numerical Approach for Nonlinear Optimal Control of Systems with Inequality Constraints

This paper proposes a system representation for unifying control design and numerical calculation in nonlinear optimal control problems with inequality constraints in terms of the symplectic structure. The symplectic structure is derived from Hamiltonian systems that are equivalent to Hamilton-Jacobi equations. In the representation, the constraints can be described as an input-state transformation of the system. Therefore, it can be seamlessly applied to the stable manifold method that is a precise numerical solver of the Hamilton-Jacobi equations. In conventional methods, e.g., the penalty method or the barrier method, it is difficult to systematically assign the weights of penalty functions that are used for realizing the constraints. In the proposed method, we can separate the adjustment of weights with respect to objective functions from that of penalty functions. Furthermore, the proposed method can extend the region of computable solutions in a state space. The validity of the method is shown by a numerical example of the optimal control of a vehicle model with steering limitations.

Previously, Polyvinyl Alcohol (PVA) and phenolic resin were used for resin impregnated bamboo fiber reinforced PP composites which was manufactures for resin impregnated bamboo fiber with polypropylene (PP). Resin impregnation method can show improvement on tensile strength of fiber. However, to reduce the contact surface area and low inter-facial shear strength (IFSS) between impregnated resin and matrix, using 40% weight fraction of bamboo fiber in PP matrix, PVA impregnated composites with mean flexural and tensile strength 10% higher than untreated composites were produced butphenolic resin impregnated fiber reinforced composition’s mechanical properties were decreased. In this study maleic anhydride grafted polypropylene (MAPP) was used to increase interfacial shear strength between resin impregnated fiber and PP. With 10% MAPP, IFSS between resin impregnated fiber and PP increased more than 100% and reinforced composites. MAPP with untreated, phenolic resin and PVA impregnated cases showed similar mechanical properties. Yet in water absorption test, the PVA treatment with bamboo/PP composites increased water absorption ratio. But with 10% MAPP, matrix PP water absorption ratio decreased like phenolic resin impregnated fiber reinforced composites. 10% MAPP with resin impregnated bamboo fiber reinforced PP composites can improve IFSS, mechanical properties of composite and can decrease water absorption PVA resin impregnated bamboo fiber reinforced composites.

Oscillating Mechanical Stimulation of the Craniocervical Region as Physical Therapy for Chronic Migraine: A Pilot Trial

Objective: We conducted a prospective trial of oscillating mechanical stimulation (OS) of the craniocervical region as treatment for drug-refractory chronic migraine (CM). Methods: Ten patients (8 women, 2 men;mean age 47.0 ±15.1 years) were enrolled. The treatment was administered over an 8-week period to 13, 4, and 9 sites on the face and head, neck, and upper back, respectively, at 5- to 15-pound intensity. The primary outcome measure was the number of days patients suffered a migraine (hereafter “number of migraine days”), and the secondary outcome measures were the six-item Headache Impact Test (HIT-6) and Visual Analog Scale (VAS) scores for migraine pain intensity and the nine-item Patient Health Questionnaire (PHQ-9) and the seven-item Generalized Anxiety Disorder (GAD-7) scale scores. Results: Nine patients completed treatment. The number of migraine days remained unchanged, from a mean 21.7 ±11.6 days/month before treatment to 19.3 ±7.3 days/month upon completion of treatment. However, the HIT-6 scores improved from 67.0 ±8.2 to 61.4 ±7.1 (p = 0.007) after 3 weeks, 61.1 ±11.5 (p = 0.01) after 6 weeks, and 59.9 ±11.6 (p = 0.035) upon completion of treatment. Similarly, the VAS scores improved significantly from 7.3 ±1.7 to 5.7 ±3.1 (p = 0.018) at 6 weeks and 4.8 ±2.8 (p = 0.011) upon completion of treatment. The GAD-7, PHQ-9, and allodynia scale scores remained unchanged. Conclusion: Our data suggest that OS is well tolerated and may become a feasible form of treatment for drug-resistant CM.

AIM: To employ pulse wave Doppler ultrasonography to evaluate the changes in portal blood flow velocity in patients with chronic hepatitis C (CHC) receiving interferon (IFN) treatment. METHODS: The subjects in this study were 14 patients (13 men and l woman) with CHC who received IFN treatment. Portal blood flow velocity was measured in the vessels at the porta hepatis at four time points: before IFN administration (pre-IFN), 2 wk after the start of administration (wk 2), 24 wk after the start of administration (wk 24, i.e., the end of IFN administration), and 24 wk after the end of administration (wk 48). RESULTS: The patients with CHC in whom IFN treatment resulted in complete elimination or effective elimination of viruses showed a significant increase in portal blood flow velocity at the end of IFN treatment compared with that before IFN treatment. In contrast, when IFN was ineffective, no significant increase in portal blood flow velocity was observed at wk 24 or 48 compared with the pre-IFN value. In addition, the patients with CHC in whom IFN was ineffective showed significantly lower portal blood flow velocity values than control subjects at all measurement time points. CONCLUSION: Pulse wave Doppler ultrasonography is a noninvasive and easily performed method for evaluating the effects of IFN treatment in patients with CHC. This technique is useful for measuring portal blood flow velocity before and 24 wk after IFN administration in order to evaluate the changes over time, thus assessing the effectiveness of IFN treatment.
