Approximately, 20% of all cancer patients will end up having brain metastases. This is especially the case of patients in which the primary tumor is lung cancer, breast and colorectal cancers, melanoma, or renal cell carcinoma. Development of brain metastases contributes substantially to overall cancer mortality in patients with advanced-stage cancer given the associated poor prognosis despite multimodal treatments and advances in systemic therapies. The objective of this retrospective single center study is to describe the experience of the Department of Neurosurgery of the Ibn Sina University Hospital in Rabat in the management of patients with brain metastases, highlighting the therapeutic choices as well as the various challenges encountered during treatment.

Introduction: Soft tissue chondroma is a very rare, slow progressing, benign tumor. It has a specific tendency for hands and feet. It can exhibit worrisome radiographic and histologic features mimicking chondrosarcomas. Case 1: A 54-year-old man was presented with a big mass in the left thigh. At magnetic resonance imaging (MRI), the lesion exhibited a heterogeneous pattern, localized in muscle without infiltration. Open biopsy was performed showed a soft tissue chondroma. Case 2: A 40 years old man presented with a hard mass on the right iliac fossa. TDM showed a well circumscribed soft tissue mass with a heterogeneous pattern. Histological assessment of the resected specimen confirmed a soft tissue chondroma. Conclusion: Soft tissue chondroma is a very rare benign tumor. In our case, the tumor presented with an unusual location and radiological appearance, suggesting a malignant process. The diagnosis was rectified thanks to pathological examination.

Quality assessment of intraoperative frozen sections: An analysis of 261 consecutive cases in a resource limited area: Morocco

Purpose: To evaluate the accuracy and limits of a frozen section analysis in our department in order to develop a quality control. Materials and methods: all frozen sections reported at our department between 1st January 2010 and 31st December 2010, have been retrospectively evaluated. The frozen section results were compared to permanent paraffin section results. Results: Frozen sections and final diagnosis agreed in 95% and disagreed in 5% (13 cases), 6.6% of the cases were deferred (17 cases). The most common pathological processes encountered were presence/typing of neoplasm (85.4%) and assessment of surgical margins (7.3%). Conclusion: The accuracy of frozen sections diagnosis at our department can be interpreted as comparable with the most international quality control series and is valuable to assist surgeons in their decision.

Background: Cystic meningioma is a rare variety of meningioma. It represents 4% to 7% of all meningiomas. The authors report a case of intracranial cystic meningioma with a review of literature. Case Report: A 59-year-old female presented with a history of headache followed by left side hemiplegia. CT scan and MRI of the brain showed a right parietal tumor with double solid and cystic components thought to be pilocytic astrocytoma or metastasis preoperatively. At surgery, the extraaxial solid and cystic lesion had a well defined capsule that could be easily separated from the perilesional cortical surface. The tumor was totally removed and the cyst was punctioned. The histological study showed a cystic meningioma WHO Grade I. A follow-up at 10 months after surgery, the patient was going well and recovered from the hemiplegiaa. Conclusion: Cystic meningioma is an uncommon tumor that should be considered in the differential diagnosis of brain tumors with a cystic component. The preoperative diagnosis and management could be in some cases challenging.

Molecular Particularity in Rare Tumour of Buttock: Case Report and Literature Review

Introduction: Hyalinizing spindle cell tumor with giant rosettes (HSCT) is a very uncommon mesenchymal tumor that has similar morphological and biological features to the low-grade fibromyxoid sarcoma (LGFMS). Case Report: Reported herein is a case of primary tumour of buttock HSCT that had rare FUS-CREB3L1 fusion transcripts, a product of characteristic chromosomal abnormality t (7;16) (q33;p11) of HSCT and LGFMS. The patient was a 48-year-old man who had a large solitary mass in the buttock. Histologically, it was composed of bland spindle cells with variable cellularity deposited in a densely hyalinized stroma alternating with myxoid areas. Characteristic collagen rosettes were scattered in the cellular areas. Reverse transcription-polymerase chain reaction (RT-PCR) assay using formalin-fixed, paraffin-embedded tissue detected FUS-CREB3L1 fusion transcripts. In our knowledge is the second case may display a variant FUS/CREB3L1 fusion transcript in international literature. Conclusion: LGFMS and HSCT probably have a wider spectrum of morphologic features than previously thought, the awareness of which will help pathologists to avoid diagnostic pitfalls. Demonstration of the t (7;16) translocation will help to diagnose difficult cases with unusual histologic features.

Cervical Spondylodiscitis Revealed by Tracheostomy Infected: A Rare Case Report Management and Outcome

Background: Spondylodiscitis is a spinal infection rare affecting primarily the intervertebral disk and the adjacent vertebral bodies. His diagnosis is difficult and often delayed or missed due to the rarity of the disease. Cervical spondylodiscitis has quite rare findings regarding the common location of spinal abscesses in the lumbar and thoracic regions. To obtain the best patient outcomes, these spinal infections require prompt diagnosis and appropriate treatment. Case description: A 44-year-old boy was admitted to the neurosurgery department of our hospital for heaviness in 4 limbs without gait disorders in context infected tracheostomy after staying in intensive care. MRI spine showed a spine deformity with lyse C4C5 epiduritis and spinal cord compressed. Antibiotics intravenous were started for 1 month the patient benefited from a corpectomy with an iliac graft and anterior cervical plate. The anatomopathological examination revealed an inflammation not specific. He was therefore put on antibiotics for 6 weeks. Three months later the neck pain and limb pain resolved after treatment and a complete return of lower extremity strength. Conclusion: Cervical spondylodiscitis has increased and become more aggressive. While radical surgical debridement, stable reconstruction together with antibiotic therapy remained a reliable approach to achieve complete healing of the inflammation, anterior alone surgery became more applicable.

Introduction: Breast involvement by non-Hodgkin lymphoma is particularly rare in women. We describe the case of a patient with a rapidly growing, nodule in the right breast. On ultrasonography, the nodule was suspicious for breast carcinoma. Case presentation: A breast biopsy from a 73-year-old Moroccan women answered invasive ductal carcinoma grade 3 from elston and ellis. Patey was performed. Microscopic examination showed lymphoid proliferation confirmed on immunohistochemical analysis. Our patient was treated with chemotherapy. Conclusions: The rarity of breast lymphomas, and the problems related to the diagnosis and therapeutic choices with these tumors require molecular techniques in association with classical histological diagnosis.

Dermoid Cysts of the Conus Medullaris Associated with Sacral Meningocele. A Case Report

Spinal dermoid cysts are gradually progressive benign tumors that may be congenital or acquired. They account for 0.8% to 1.1% of all primary spinal tumors. MRI is the gold standard for the radiological assessment. Management is difficult for tumors involving highly eloquent areas such as the conus medullaris. In this report, we present a rare case of an adult woman with dermoid cysts in the conus medullaris associated with a sacral meningocele which was treated with a midline myelotomy that drained yellowish keratinous fluid and decompressed the cyst. No aggressive attempt at complete resection of the cyst wall was undertaken. The patient fully recovered, and her condition considerably improved after surgery and remains good at a 6-month follow-up.

Respiratory epithelial adenomatoid hamartoma (REAH) is an uncommon lesion of the upper aerodigestive tract first described by Wenig and Heffner in 1995 as prominent glandular proliferations lined by ciliated respiratory epithelium originating from the surface epithelium. Case Report: We report a case of 48-year-old women with nasal polyposis history, which consults for nasal obstruction, with suspicion of malignancy on CT. Surgical resection showed a respiratory epithelial adenomatoid hamartoma (REAH) of the nasal cavity. Conclusion: REAH is a recently described pathologic entity that can present rhinorrhea, epistaxis, hyposmia, and headaches. It is a rare lesion of nasal and paranasal sinuses, but should be considered in the differential diagnosis because it is a benign lesion and complete surgical resection is curative.
