Background and Study.Aim: Ensdoscopic ultrasound- gui- ded fine- needle aspiration (EUS- FNA) has been reported as a useful techni que for histological diagnosis of submucosal or extrinsic gastrointestinal and p ancreatic lesions. The aim of this study.was to evaluate the use of EUS- FNA fo r the diagnosis of lesions either within or adjacent to the wall of the colon an d rectum. Patients and Methods: A total of 22 patients with a lesion within the wall of, or adjacent to, the colon or rectum underwent EUS- FNA. They.were divi ded into two groups: patients who had previously.had a malignancy.(the “ previo us + ve" group, n=11), and patients who had not previously.had a malignancy.(th e “ previous - ve" group, n = 11). In the four patients who had lesions locate d proximal to the sigmoid colon, EUS- FNA was performed using a guide wire and overtube. The success rates for adequate tissue sampling and for detecting malig nant and benign masses by.EUS- FNA were evaluated and the success rate for dete ction was compared with the success rate of EUS and computed tomography. Results : Sufficient tissue for evaluation was obtained from 21 of be 22 patients (95.5 % ). The overall rate of detection of malignant and benign masses was 95.5% ( 21/22) for EUS- FNA and 81.8% (18/22) for pre- EUS- FNA imaging investigati ons. Of the 11 patients in the previous + ve group, ten were diagnosed with recurrences of primary.malignancies; of the 11 patients in the previous - ve group, four we re diagnosed with primary.malignancies and seven were diagnosed with benign lesi ons. There were no complications related to the EUS- FNA procedure. Conclusions : EUS- FNA is a safe techniquewhich is useful in the planning of treatment for patients who have a mass within the wall or adjacent to the wall of the entire l ength of the colon or rectum.

Background and study.aims: Capsule endoscopy.(CE) and double-balloon enteroscopy.(DBE) have been introduced as modalities for examining the entire small bowel. The aim of the present study.was to assess the clinical effects of CE and DBE to consider the roles of CE and DBE and the indications for the procedures in patients with suspected small-bowel bleeding. Patients and methods: Between June 2004 and January.2005,32 patients in whom a site of bleeding in the gastrointestinal tract had not been identified were enrolled in the study. Twenty.eight patients were examined with both methods. Bleeding sources were categorized as either A1 lesions (immediate hemostatic procedures required) or A2 lesions (close observation required). CE and DBE were evaluated with regard to whether or not they.were capable of accessing the entire small bowel and provided a diagnosis, and the access and diagnostic rates were calculated. Results: On CE, 13 patients were diagnosed with A1 lesions and six with A2 lesions; on DBE, 11 had A1 lesions and one had an A2 lesion. The access rate for the entire small intestine on CE was 90.6% (29 of 32), significantly.higher than with DBE at 62.5% (10 of 16; P y.different. Among patients with A1 lesions who were diagnosed with DBE, histological diagnoses were obtained in six of the 11, and three patients were treated. Conclusions: In many.suspected small-bowel bleeding cases, CE should be selected for the initial diagnosis and DBE for treatment or histopathological diagnosis after detection of the bleeding site on

In this study. genetic poly.orphisms, NQO1 C609T, GSTM1 positive/null, and GSTT1 positive/null, were examined with reference to cervical cancer risk in a population- based incident case- control study.in Japanese. The cases comprised 131 cervical cancer patients: 87 cases with squamous cell carcinoma (SCC) and 44 with adenocarcinoma (ADC) or adenosquamous carcinoma (ADSC). Controls were sampled from 320 healthy.women who underwent a health checkup. The cervical cancer risk was substantially.elevated with smoking for all cases, SCC cases, and ADC/ADSC cases (OR = 4.50, 95% CI = 2.48- 8.17, P y.. The frequency.of the NQO1 609TT genoty.e, reported to be associated with null enzy.e activity. was higher in individuals with all cases and SCC than in the healthy.controls (OR = 1.97, 95% CI = 1.06- 3.66, P = 0.032; and OR = 2.42, 95% CI = 1.21- 4.82, P = 0.012; respectively., but not in ADC/ADSC cases. Analy.is of poly.orphisms for GSTM1 and GSTT1 showed no significant differences between cervical cancer patients and controls. In stratification analy.is, significant elevated risk of all cases and SCC was associated with the NQO1 609TT genoty.e among nonsmokers (OR = 2.15, 95% CI = 1.08- 4.30, P = 0.030; and OR = 2.83, 95% CI = 1.21- 6.31, P = 0.011; respectively., but not smokers. No gene- gene interaction was observed in our case subjects. This is the first report that the NQO1 gene might be important in relation to the risk of squamous cell carcinoma of the cervix.

Autonomic and cardiovascular effects of pentobarbital anesthesia during trigeminal stimulation in cats

Stimulation of the trigeminal nerve can elicit various cardiovascular and autonomic responses;however,the effects of anesthesia with pentobarbital sodium on these responses are unclear.Pentobarbital sodium was infused intravenously.at a nominal rate and the lingual nerve was electrically.stimulated at each infusion rate.Increases in sy.tolic blood pressure（SBP） and heart rate（HR） were evoked by.lingual nerve stimulation at an infusion rate between 5 and 7 mg？kg 21 ？h 21.This response was associated with an increase in the low-frequency.band of SBP variability.SBP-LF）.As the infusion rate increased to 10 mg？kg 21 ？h 21 or more,decreases in SBP and HR were observed.This response was associated with the reduction of SBP-LF.In conclusion,lingual nerve stimulation has both sy.pathomimetic and sy.pathoinhibitory.effects,depending on the depth of pentobarbital anesthesia.The reaction pattern seems to be closely.related to the autonomic balance produced by.pentobarbital anesthesia.

Primary.Serous Cy.tadenocarcinoma of Broad Ligament: A Case Report with Laparoscopic, Histopathologic and Immunohistochemical Findings

An 86-y.ar-old Japanese woman underwent an examining laparoscopy.for removing the huge pelvic tumor. At laparoscope examination, the cy.tic tumor was found within the left broad ligament, while the ovaries, fallopian tubes and uterus showed almost normal appearance. The tumor was removed together by.total laparoscopic hy.terectomy.and bilateral salpingo-oophorectomies after the suction of serous content in the broad ligament. Cy.ological findings of the ascites suggested serous carcinoma. The resected ovaries and fallopian tubes were grossly.and histologically.normal. Histological examination of the solid part of broad ligament tumor, closely.next to the fallopian tube, revealed a serous adenocarcinoma. Immunohistochemically. the tumor cells were strongly.positive for CK7, WT-1, estrogen receptor, AE1/AE3 and EMA, and negative for CK20, D2-40 and calretinin. Also, they.were negative for progesterone receptor and p53. The authors diagnosed the primary.tumor as being a serous cy.tadenocarcinoma of the broad ligament [pTIC3NxM0, as modified and adapted to post-surgical staging of ovarian cancer (FIGO 2014)]. The patient has been receiving 6 cy.les of adjuvant chemotherapies with one course with paclitaxel (PTX) and carboplatin (CBDCA) and five with PTX, CBDCA and Bevacizumab, and has no signs of recurrence and metastasis six months after the operation.

Echinococcosis is a global and zoonotic helminthic disease caused by.Echinococcus sp. A 24-y.ar-old woman, coming from Bolivia two y.ars before, was diagnosed as cy.tic echinococcosis (CE) by.unstained wet mount and cy.ological findings through aspiration cy.ology.from the liver, suggested as liver abscess by.diagnostic images. The patient was classified as CE1, smaller than 5 cm, and was diagnosed as P1N0M0, stage I. The phy.ogenetic trees of Echinococcus spp., based on cox1 genes showed that the isolates on the patient belonged to E. ortleppi. The patient was conservatively.treated with two courses of albendazole therapy. E. ortleppi has been reported to be in regions including Latin America, except with Japan. Although the patient came from Bolivia before two y.ars, this is the first case report of E. ortleppi from liver echinococcosis in Japan.

Improvement of a Real Gas-Sensor for the Origin of Methane Selectivity.Degradation by.l-XAFS Investigation

We have directly.investigated the chemical state of the Pd species in a real l-gas sensor device by.examining the l-fluorescence X-ray.absorption fine structure. The l-gas sensor device was heavily.damaged by.a heating process in which the temperature was ill-controlled, resulting in decrease of methane selectivity. We found that the Pd O in the fresh l-gas sensor was reduced to Pd metal particles as the methane selectivity.decreased. Based on the investigation results, we modified the device structure so as to heat up homogeneously. The lifetime of the sensor was then successfully.increased by.more than 5 y.ars.

A rare presentation of appendiceal diverticulitis associated with pelvic pseudocy.t

We report a case of a 32-y.ar-old woman with episodic right lower quadrant abdominal pain. With the diagnosis of either acute appendicitis or tubo-ovarian abscess, she was treated with antibiotics, which successfully.relieved the pain and the inflammatory.findings. She repeated the clinical condition a few times and antibiotics worked each time. In a y.ar, her subjective sy.ptoms became milder, however, a giant pelvic cy.t appeared. She had an exploratory.laparotomy.to confirm this diagnosis. Histopathological studies revealed herniated appendiceal mucosa through the muscular lay.r associated with chronic inflammation and marked fibrosis. Gy.ecological disorders such as endometriosis or lutein cy.t rupture was denied. These findings represent appendiceal diverticulitis. We discuss the clinical features of the disease and its relation with the pelvic pseudocy.t.

To date, various signal transducers, cy.okines, growth factors, and hormones have been reported to play.an important role in homeostasis of various organs. Various cells and organs are involved in the hepatic regeneration process, which proceeds as a result of the coordination of many.factors. While these factors are well known to be involved in the liver regeneration after the liver injury. however, as the details of such mechanisms have not been sufficiently.elucidated, the practical applicability.of hepatic regeneration based on the action of these and cy.okines growth factors is still unclear. In terms of the involvement of the autonomic nervous sy.tem in hepatic regeneration, cell proliferation resulting from direct signal transduction to the liver has also been reported and recent studies focusing on the inter-organ communication via neural network opened a novel aspect of this field for therapeutic applicability. Therefore, the appropriate understanding of the relationship between autonomic neural network and liver regeneration through various organs including brain, afferent nerve, efferent nerve, etc. is essential. This mini-review explains the principle of neural sy.tem involved in the inter-organ communication and its contribution on the liver regeneration upon the liver injury.reviewing recent progress in this field.
