BACKGROUND: Recent trials have demonstrated better outcomes with intensive than with moderate statin treatment. Intensive treatment produced greater reductions in both low-density lipoprotein(LDL)choles.e.ol and C-reactive protein(CRP), suggesting a relationship between these two biomarkers and disease progression. MeTHODs: We performed intravascular ultrasonography in 502 patients with angiographically documented coronary disease. Patients were randomly assigned to receive moderate treatment(40 mg of pravastatin orally per day)-or intensive treatment(80 mg of atorvastatin orally per day). Ultrasonography was repeated after 18 months to measure the progression of atherosclerosis. Lipoprotein and CRP levels were measured at baseline and followup. ResULTs: In the group as a whole, the mean LDL choles.e.ol level was reduced from 150.2 mg per deciliter(3.88 mmol per liter)at baseline to 94.5 mg per deciliter(2.44 mmol per liter)at 18 months(Pe geometric mean CRP level decreased from 2.9 to 2.3 mg per liter(Pe correlation between the reduction in LDL choles.e.ol levels and that in CRP levels was weak but significant in the group as a whole(r=0.13, P=0.005), but not in either treatment group alone. In univariate analyses, the percent change in the levels of LDL choles.e.ol, CRP, apolipoprotein B-100, and nonhigh-density lipoprotein choles.e.ol were related to the rate of progression of atherosclerosis. After adjustment for the reduction in these lipid levels, the decrease in CRP levels was independently and significantly correlated with the rate of progression. Patients with reductions in both LDL choles.e.ol and CRP that were greater than the median had significantly slower rates of progression than patients with reductions in both biomarkers that were less than the median(P=0.001). ACAT抑制对冠状动脉粥样硬化进展的影响

Background: The enzyme acyl-coenzyme A:choles.e.ol acyltrans.e.ase(ACAT) es.e.ifies choles.e.ol in a variety of tiss.e.. In some animal models, ACAT inhibitors have antiatherosclerotic effects. Methods: We performed intravascular ultrasonography in 408 patients with angiographically documented coronary disease. All patients received usual care for secondary prevention, including statins, if indicated. Patients were randomly assigned to receive the ACAT inhibitor pactimibe(100 mg per day) or matching placebo. Ultrasonography was repeated after 18 months to measure the progression of atherosclerosis. Results: The primary efficacy variable analyzing the progression of atherosclerosis-the change in percent atheroma volume-was similar in the pactimibe and placebo groups(0.69 percent and 0.59 percent, res.e.tively; P=0.77). However, both secondary efficacy variables as.e.sed by means of intravascular ultrasonography showed unfavorable effects of pactimibe treatment. As compared with baseline values, the normalized total atheroma volume showed significant regression in the placebo group(-5.6 mm3, P=0.001) but not in the pactimibe group(-1.3 mm3, P=0.39; P=0.03 for the comparison between groups). The atheroma volume in the most diseased 10-mm subsegment regres.e. by 3.2 mm3 in the placebo group, as compared with a decrease of 1.3 mm3 in the pactimibe group(P=0.01). The combined incidence of adverse cardiovascular outcomes was similar in the two groups(P=0.53). Conclusions: For patients with coronary disease, treatment with an ACAT inhibitor did not improve the primary efficacy variable(percent atheroma volume) and adversely affected two major secondary efficacy measures as.e.sed by intravascular ultrasonography. ACAT inhibition is not an effective strategy for limiting atherosclerosis and may promote atherogenesis.(*** number, NCT 00268515).

The main objective of this paper is to investigate the influence of inertia of nonlinear springs on the dis.e.sion behavior of discrete monoatomic chains with lumped and distributed *** developed model can represent the wave propagation problem in a non-homogeneous material consisting of heavy inclusions.e.bedded in a *** inclusions are idealized by lumped mas.e.,and the matrix between adjacent inclusions is modeled by a nonlinear spring with distributed ***,the model is capable of depicting the wave propagation in bi-material bars,wherein the first material is represented by a rigid particle and the second one is represented by a nonlinear spring with distributed *** discrete model of the nonlinear monoatomic chain with lumped and distributed mas.e. is first considered,and a closed-form expression of the dis.e.sion relation is obtained by the second-order Linds.e.t-Poincare method(LPM).Next,a continuum model for the nonlinear monoatomic chain is derived directly from its discrete lattice model by a suitable continualization *** subsequent use of the second-order method of multiple scales(MMs)facilitates the derivation of the corresponding nonlinear dis.e.sion relation in a closed *** novelties of the present study consist of(i)considering the inertia of nonlinear springs on the dis.e.sion behavior of the discrete mass-spring chains;(ii)developing the second-order LPM for the wave propagation in the discrete chains;and(iii)deriving a continuum model for the nonlinear monoatomic chains with lumped and distributed ***,a parametric study is conducted to examine the effects of the design parameters and the distributed spring mass on the nonlinear dis.e.sion relations and phase velocities obtained from both the discrete and continuum *** parameters include the ratio of the spring mass to the lumped mass,the nonlinear stiffness coefficient of the spring,and the wave amplitude.

This paper presents an investigation of the seismic behavior of reinforced concrete(RC)structures in which s.e.r walls are the main lateral load-resisting elements and the participation of flat slab floor sys.e.s is not considered in the seismic design *** this regard,the behavior of six prototype structures(with different heights and plan layouts)is investigated through nonlinear static and time history analyses,implemented in the Opens.e. *** results of the analyses are presented in terms of the behavior of the slab-column connections and their mode of failure at different loading ***,the global response of the buildings is discus.e. in terms of some parameters,such as lateral overstrength due to the gravity flat slab-column *** to the nonlinear static analyses,in structures in which the slab-column connections were designed only for gravity loads,the slab-column connections.e.hibited a punching mode of failure even in the early stages of ***,the punching failure was.e.iminated in structures in which a minimum trans.e.se reinforcement recommended in ACI 318(2019)was provided in the slabs at joint ***,despite neglecting the contribution of gravity flat slab-column frames in the lateral load resistance of the structures,a relatively significant overstrength was imposed on the structures by the gravity frames.

KLnF_(4):Yb^(3+),er^(3+)/Ho^(3+)(Ln=Y,Yb,Gd)phosphors were prepared by solid-state reaction *** each of the Yb^(3+)/er^(3+)and Yb^(3+)/Ho^(3+)co-doped phosphors independently,phase confirmation and morphological examination were accomplis.e. using the X-ray diffraction technique and scanning electron microscope analysis.X-ray photoelectron s.e.troscopy analysis shows the charge state and exis.e.ce of lanthanides and *** lumines.e.ce emissions of green and red colour were detected for the synthesized *** changes in the upconverted emission intensities and colour output pertaining to the change of the lanthanide host matrix along with dopants were investigated for the prepared *** the upconversion lumines.e.ce,erbium-doped samples.e.hibit a predominance of red emission while holmium-doped samples display a prevalence of green *** rare-earth er/Ho doped KYbF4shows strong red emission compared to other ***_(4):Yb,er/Ho exhibits relatively significant green upconve rsion emissions,while Yb,er-doped KGdF_(4)host displays a 10-fold increase in green/red emission ratio with an increase in the input power of 20-780 *** photolumines.e.ce studies and decay curve analysis were carried out using UV light excitation for the developed phosphors.A higher lifetime of 2.2 ms was measured for the downshifting red emission in the Ho doped KYbF_(4)*** study suggests a potential for colour tunability in the upconversion and downshifting emissions through host modifications to the fluoride phosphor.
