AIM： To systematically examine the extent of correlation of risk factors, such as age, consumed dietary habit and familial predisposition with somatic Tp53 molecular lesion causal to elevate carcinogenesis severity of esophageal squamous cell carcinoma （ESCC） among the Kashmiri population of Northern India. METHODS： All cases （n = 51） and controls （n = 150） were permanent residents of the Kashmir valley. Genetic alterations were determined in exons 5-8 of Tp53 tumor suppressor gene among 45 ESCC cases histologically confirmed by PCR-SSCP analysis. Data for individual cancer cases （n = 45） and inpatient controls （n = 150） with non-cancer disease included information on family history of cancer, thirty prevailing common dietary risk factors along with patient＇s age group. Correlation of genetic lesion in p53 exons to animistic data from these parameters was generated by Chi-square test to all 45 histologically confirmed ESCC cases along with healthy controls.RESULTS： Thirty-five of 45 （77.8%） histologically characterized tumor samples had analogous somatic mutation as opposed to 1 of 45 normal sample obtained from adjacent region from the same patient showed gerrnline mutation. The SSCP analysis demonstrated that most common p53 gene alterations were found in exon 6 （77.7%）, that did not correlate with the age of the individual and clinicopathological parameters but showed significant concordance （P 〈 0.05） with familial history of cancer （CD = 58）, suggesting germline predisposition at an unknown locus, and dietary habit of consuming locally grown Brassica vegetable ＂Hakh＂（CD = 19.5）, red chillies （CD = 20.2）, hot salty soda tea （CD = 2.37） and local baked bread （CD = 1.1）. CONCLUSION： Our study suggests that somatic chromosomal mutations, especially in exon 6 of Tp53 gene, among esophageal cancer patients of an ethnically homogenous population of Kashmir valley are closely related to continued exposure to various common dietary risk factors, especially hot sa

Primitive neuroectodermal tumor (PNET) of uterus is the rare of the rarest among the tumors of female genital tract. We herein reported a 32 years old female operated for lower abdominal lump,which was diagnosed,as PNET later confirmed by immunohistochemistry (IHC). She was lost to follow-up and presented 10 months later with recurrence. She was treated with cyclical combination chemotherapy followed by definitive surgery and is in complete remission at present.

Background The development of modern agriculture has significantly contributed to improving global food security and safety, alleviating poverty, and enhancing human health and livelihoods. However, the rapid advancement of modern agriculture has also brought about various challenges that limit its sustainable development. This commentary aims to discuss these issues through the One Health lens, and provide valuable insights for balancing modern agricultural activities with the need to protect and promote the health of all the *** text This commentary explores the multifaceted impacts of modern agriculture on social development, as well as the associated various health challenges and environmental impacts within the One Health framework. Key issues include ecosystem degradation, increased risk of interspecies disease transmission like zoonoses, reverse zoonoses, and vector-borne diseases, and the escalated threat of antimicrobial resistance due to intensified agricultural production and increased antimicrobial use. To address these challenges, this commentary outlines potential solutions anchored in the development and implementation of modern technologies and good agricultural practices, such as precision farming, integrated pest management, biosecurity measures, vaccination programs, as well as surveillance and early detection of health *** Good agricultural practices supported by scientific and technological advancements are essential for aligning productivity with the One Health vision, ensuring the health and resilience of all the sectors. Enhancing stakeholder education, strengthening regulatory frameworks, and providing supportive policies and infrastructure for farmers to adopt sustainable practices are crucial for the long-term viability of agrifood systems. The Food and Agriculture Organization of the United Nations plays a pivotal role in guiding this sustainable transformation through the One Health approach.

Synchronous Adenocarcinomas of the Jejunum and Sigmoid Colon—A Rare Case

Primary adenocarcinomas of the small bowel are rare. Because of their rarity, non-specific symptoms and diagnostic difficulty, small bowel tumors are often diagnosed and treated late in their course. The diagnostic difficulty is increased when these tumors arise in association with primary synchronous tumors of the colon. We present a case of a 17-year-old female who was evaluated for anemia and bleeding per rectum. Colonoscopy revealed multiple colonic polyps. Intraoperatively, she was surprisingly found to be having a synchronous jejunal growth apart from multiple colonic polyoid lesions. Total colectomy was performed along with resection of jejunal growth. Histopathology of colonic specimen revealed multiple polypoid projections with one polyp showing adenocarcinoma and rest severe dysplasia. Histopathology of jejunal growth revealed poorly-differentiated adenocarcinoma. Very rarely, cases have been reported in literature, where the primary adenocarcinoma of the jejunum developed synchronously with colonic adenocarcinoma, and both treated surgically on the same occasion.

Etiology and Management of Small Bowel Obstruction with Special Reference to Role of Laparoscopy—A Single Centre Experience

Background: Small bowel obstruction (SBO) is the commonly diagnosed disease in the emergency department. The diagnosis and management of small bowel obstruction varies among institutions. The role of laparoscopy in the management of small bowel obstruction is debatable. Aim: To study the profile, clinical features and management options of patients of small bowel obstruction with special emphasis on role of laparoscopy. Material and Methods: 51 consecutive patients with documented small bowel obstruction admitted in our department were studied from June 2016 to December 2018. All the patients above 12 years of age with small bowel obstruction were included. The diagnosis of small bowel obstruction (SBO) was made in these cases by detailed history, clinical examination and investigations. All these patients were received and adequately resuscitated. Non-operative (conservative) treatment was confirmed to those who met the standard parameters for such treatment and early surgical treatment was carried out whenever indicated. The operative procedure done was laparoscopy or open depending on the clinical features and condition of patient. Results: In our study, 51 patients mean age was 44.7 years with 27 Males and 24 females. 28 (54.9%) patients of our study presented with continuous abdominal pain followed by colicky pain in 23 (45.1%) patients. 34 (66.7%) patients on admission gave history of effortless vomiting. Distension of abdomen was present in 30 (58.8%) patients and constipation was present in 40 (78.4%) of our studied patients. On CT scan interloop fluid was present in 23 (45.1%) patients while transition point was present in 31 (60.8%) patients. Adhesion obstruction was found to be most common cause of obstruction 27 (52.9%) patients, followed by malignancy in 8 (15.7). 25 (49.0%) patients were managed conservatively and 26 (51%) were operated. 11 (21.6%) patients underwent laparoscopic adhesiolysis and 15 (29.4%) underwent exploratory laparotomy.
