乳腺癌前哨淋巴结活检

目的　评价前哨淋巴结活检预测腋窝淋巴结有无肿瘤转移的准确性及其临床意义。方法　用99m锝硫胶体作为示踪剂 ,用γ探测仪导向 ,对 70例临床分期为T1 2 N0 M0 的乳腺癌患者进行前哨淋巴结活检 ,所有的患者均同时行腋窝淋巴结清扫。H... 详细信息

目的　评价前哨淋巴结活检预测腋窝淋巴结有无肿瘤转移的准确性及其临床意义。方法　用99m锝硫胶体作为示踪剂 ,用γ探测仪导向 ,对 70例临床分期为T1 2 N0 M0 的乳腺癌患者进行前哨淋巴结活检 ,所有的患者均同时行腋窝淋巴结清扫。HE染色阴性的前哨淋巴结再切片 ,用CK8、CK19、KP 1行免疫组织化学染色。　结果　 70例患者中成功发现前哨淋巴结的有 6 7例 ,发现率为95 7% (6 7/ 70 )。前哨淋巴结的数量为 1～ 5枚 ,平均每例 1 6枚。非前哨淋巴结 5～ 2 0枚 ,平均每例12 3枚。 6 7例前哨淋巴结活检成功的患者中 ,2 9例患者 (43 3% )有腋窝淋巴结转移 ,其中前哨淋巴结有转移者 2 4例 (35 8% ) ,前哨淋巴结未发现转移而非前哨淋巴结有转移者 5例 (7 5 % )。 7例患者(10 4 % )只有前哨淋巴结为阳性淋巴结。前哨淋巴结活检的准确性为 92 5 % (6 2 / 6 7) ,假阴性率为7 5 % (5 / 6 7)。原发灶≤ 2cm的患者 ,前哨淋巴结活检的准确性为 10 0 %。 4 3例患者的 6 5枚HE染色阴性的前哨淋巴结 ,CK8及CK19免疫组织化学染色均为阴性。　结论　前哨淋巴结活检能较准确地预测腋窝淋巴结转移情况 ,对原发灶为T1的乳腺癌 ,前哨淋巴结活检的准确性为 10 0 %。同一层面切片行免疫组织化学染色并不能提高淋巴结微转移?

Betalains are natural coloring pigments with betalamic acid as the core structure of all *** their coloring properties,betalains exhibit various biological activities,including antioxidant and anti-inflammatory properties,which are highly *** in-vivo studies reported that betalains protect various body organs,leading to health *** organs,including the heart,liver,kidney,lung,etc.,are important for a healthy ***,these organs can be affected or damaged by various stress factors,toxicants,and harmful *** studies have claimed that betalains could protect all vital organs of the body through antioxidant and anti-inflammatory *** review article described the in-vivo antioxidant and anti-inflammatory activities of betalains in various cell-line or animal models.A comprehensive discussion has been provided on the mechanism of action of betalains in protecting various body organs,including cardio-protective effect,hepato-protective ability,renal protection capacity,repro-protective ability,neuro-protective effect,lung protection,and gut protection ***,future research directions and conclusions have been outlined.

This study presents a neural network-based model for predicting linear quadratic regulator(LQR)weighting matrices for achieving a target response *** on the expected weighting matrices,the LQR algorithm is used to determine the various responses of the *** responses are determined by numerically analyzing the governing equation of motion using the state-space *** training a neural network,four input parameters are considered:the time history of the ground motion,the percentage reduction in lateral displacement,lateral velocity,and lateral acceleration,Output parameters are LQR weighting *** study the effectiveness of an LQR-based neural network(LQRNN),the actual percentage reduction in the responses obtained from using LQRNN is compared with the target percentage ***,to investigate the efficacy of an active control system using LQRNN,the controlled responses of a system are compared to the corresponding uncontrolled *** trained neural network effectively predicts weighting parameters that can provide a percentage reduction in displacement,velocity,and acceleration close to the target percentage *** on the simulation study,it can be concluded that significant response reductions are observed in the active-controlled system using ***,the LQRNN algorithm can replace conventional LQR control with the use of an active control system.

Patients with malignant biliary obstruction,following endoscopic retrograde cholangiopancreatography(ERCP)failure could be referred for endoscopicultrasound-guided biliary drainage through electrocautery-enhanced(ECE)lumen-apposing metal stent(LAMS)***,the efficacy and safety of ECE-LAMS in this scenario have remained debatable due to minimal scientific *** current confirmed 91.0%clinical success,96.7%technical success,7.3%reintervention rate,and 17.5%adverse events,following the treatment of malignant biliary obstruction with ECE-LAMS ***,ECE-LAMS proved to be a generalizable strategy for managing biliary obstruction for patients who were excluded from ERCP.

This study involves A356 alloy molded through ultrasonically vibrated cooling *** slope alongside ultrasonic power enables indispensable shear for engendering slurry from which the semisolid cast/heat treated billets got *** examination demonstrates ultrasonically vibrated cooling slope influencing the liquid fraction/microstructure/physical characteristics of stated *** investigation encompasses five diverse ultrasonic powers(0,75,150,200,250 W).The ultrasonic power of 150 W delivers finest/rounded microstructure with enhanced physical *** modifications reason physical transformations because of grain refinement and grain boundary/Hall-Petch strengthening.A smaller grain size reasons a higher strength/shape factor and an increased homogeneity reasons a higher *** characteristics get improved by *** is owing to coalescence throughout temperature *** physical characteristics is improved by reheating because of a reduced porosity and enhanced dissolution besides augmented homogeneity.A direct comparison remains impossible owing to unavailability of researches on ultrasonically vibrated cooling slope.

Molecular spectroscopy is a branch of physics which deals with the interaction of electromagnetic radiation with *** the new theoretical approach,i.e.,Lie algebraic approach,we calculate the infrared spectra of phosphine in the range from 3000 cm^(-1) to 9500 cm^(-1) and nitrogen trifluoride in the range from 900 cm^(-1) to 4500 cm^(-1).The model Hamiltonian constructed seems to describe the P–H and N–F stretching modes accurately with only four numbers of *** present calculation not only predicts the higher overtones but also shows good agreement with the few observed data.
