Fragility of statistically significant findings from randomized clinical trials of surgical treatment of humeral shaft fractures:A systematic review

BACKGROUND Despite recent meta-analyses of randomized controlled trials(RCTs),there remains no consensus regarding the preferred surgical treatment for humeral shaft *** fragility index(FI)is an emerging tool used to evaluate the robustness of RCTs by quantifying the number of participants in a study group that would need to switch outcomes in order to reverse the study *** To investigate the fragility index of randomized control trials assessing outcomes of operative fixation in proximal humerus *** We completed a systematic review of RCTs evaluating the surgical treatment of humeral shaft *** criteria included:articles published in English;patients randomized and allotted in 1:1 ratio to 2 parallel arms;and dichotomous outcome *** FI was calculated for total complications,each complication individually,and secondary surgeries using the Fisher exact test,as previously *** Fifteen RCTs were included in the analysis comparing open reduction plate osteosynthesis with dynamic compression plate or locking compression plate,intramedullary nail,and minimally invasive plate *** median FI was 0 for all parameters *** individual outcomes,the FI was 0 for 81/91(89%)of *** FI exceeded the number lost to follow up in only 2/91(2%)*** The FI shows that data from RCTs regarding operative treatment of humeral shaft fractures are fragile and does not demonstrate superiority of any particular surgical technique.

Investigation on Degradation Path of SF_(6)in Packed-bed Plasma:Effect of Plasma-generated Radicals

SF_(6)degradation mechanism in non-thermal plasma(NTP)systems is not fully understood due to the formation of a complex physico-chemical reaction network,especially when reactive gases and packing materials are *** this work,we conduct a combined experimental and theoretical study to unravel the SF_(6)degradation path in a-Al_(2)O_(3)packed plasma in the presence of h_(2)O or O_(2).Our experimental results show that both h_(2)O and O_(2)have a synergetic effect with-Al_(2)O_(3)packing on promoting SF_(6)degradation,leading to higher stable gas yields than typical spark or corona discharges.h_(2)O or O_(2)addition promotes SO_(2)or SO_(2)F_(2)selectivity,*** functional theory(DFT)calculations reveal that SO_(2)generation corresponding with the highest activation barrier is the most critical step toward SF_(6)*** like h and O generated from h_(2)O or O_(2)discharge can significantly promote the degradation process via Eley-Rideal mechanism,affecting key reactions of stable product generation,advancing degradation *** results of this work could provide insights on further understanding SF_(6)degradation mechanism especially in packed-bed plasma systems.

Carbonaceous aerosols affect air quality adversely,affect global warming,and human ***,our understanding of the impact of ultrafine(PM_(0.1))carbonaceous particulate matter is incomplete,particularly the effects during haze *** study monitored diurnal variations in PM_(0.1) in Chiang Mai,Thailand,from March to April *** investigated carbonaceous PM_(0.1) collected by an ambient nano-sampler and evaluated their effect by using a carbon analyzer(IMPROVE_TOR).The results showed that burning large open areas in the dry season was crucial for increasing the particle mass concentration because of the large open burnings that occurred in this *** majority of biomass fires near the sampling site occurred during the night,which would allow more particles to be released thus resulting in higher concentrations of PM_(0.1).hence,the release of PM_(0.1) during the night would obviously result in higher concentrations than that during the *** the eight carbon profiles,organic carbon 3(OC3)was predicted to be a marker of biomass *** carbon indices displayed that PM_(0.1) was influenced by biomass burning both daytime and *** findings reported herein should be of great impor-tance in terms of establishing biomass burning control policies for mitigating heavy haze pollution in Thailand and elsewhere.

The influence of closed and open surface flaws on the stress distribution and failure in rock specimens is *** finite element models are developed to simulate the compression tests on flawed rock *** simulated specimens include those with closed flaws and those with open flaws on the *** analyses are conducted to investigate the influences of the flaw inclination,friction coefficient and the confining stress on failure *** results show significant differences in the stress,displacement,and failure behavior of the closed and open flaws when they are subjected to pure compression;however,their behaviors under shear and tensile loads are *** to the results,when compression is the dominant mode of stress applied to the flaw surface,an open flaw may play a destressing role in the rock and relocate the stress concentration and failure *** presented results in this article suggest that failure at the rock surface may be managed in a favorable manner by fabricating open flaws on the rock *** insights gained from this research can be helpful in managing failure at the boundaries of rock structures.

Calculation of hildebrand Solubility Parameters of Some Polymers Using QSPR Methods Based on LS-SVM Technique and Theoretical Molecular Descriptors

In this work, some chemometrics methods are applied for the modeling and prediction of the hildebrand solubility parameter of some polymers. A genetic algorithm （GA） method is designed for the selection of variables to construct two models using the multiple linear regression （MLR） and least square-support vector machine （LS-SVM） methods in order to predict the hildebrand solubility parameter. The MLR method is used to build a linear relationship between the molecular descriptors and the hildebrand solubility parameter for these compounds. Then the LS-SVM method is utilized to construct the non-linear quantitative structure-activity relationship （QSAR） models. The results obtained using the LS-SVM method are then compared with those obtained for the MLR method; it was revealed that the LS-SVM model was much better than the MLR one. The root-mean-square errors of the training set and the test set for the LS-SVM model were 0.2912 and 0.2427, and the correlation coefficients were 0.9662 and 0.9518, respectively. This paper provides a new and effective method for predicting the hildebrand solubility parameter for some polymers, and also reveals that the LS-SVM method can be used as a powerful chemometrics tool for the quantitative structure-property relationship （QSPR） studies.

AIM:To compare effectiveness,safety,and cost of photodynamic therapy(PDT)and radiofrequency ablation(RFA)in treatment of Barrett’s dysplasia(BD).METhODS:Consecutive case series of patients undergoing either PDT or RFA treatment at single center by a single investigator were ***-three patients with high-grade dysplasia(hGD)had treatment with porfimer sodium photosensitzer and 630 nm laser(130 J/cm),with maximum of 3 treatment ***-three patients with BD(47 with low-grade dysplasia-LGD,6 with hGD)had step-wise circumferential and focal ablation using the hALO system with maximum of 4 treatment *** groups received proton pump inhibitors twice *** biopsies were acquired at 2 and 12 mo after enrollment,with 4-quadrant biopsies every 1 cm of the original BE extent.A complete histological resolution response of BD(CR-D)was defined as all biopsies at the last endoscopy session negative for ***’s exact test was used to assess differences between the two study groups for primary *** all outcomes,a two-sided P value of less than 0.05 was considered to indicate statistical ***:Thirty(91%)PDT patients and 39(74%)RFA were men(P=0.05).The mean age was 70.7±12.2 and 65.4±12.7(P=0.10)year and mean length of BE was 5.4±3.2 cm and 5.7±3.2 cm(P=0.53)for PDT and RFA patients,*** CR-D was(18/33)54.5%with PDT vs(47/53)88.7%with RFA(P=0.001).One patient with PDT had an esophageal perforation and was managed with non-surgical measures and no perforation was seen with *** was five times more costly than RFA at our *** two groups were not randomized and had different BD grading are the limitations of the ***:In our experience,RFA had higher rate of CR-D without any serious adverse events and was less costly than PDT for endoscopic treatment of BD.

Numerical study on rock failure around a tunnel destressed by a conceptualized notched technique

In this study,a harmonic oscillator with position-dependent mass is ***,as an introduction,we give a full description of the system by constructing its classical Lagrangian;thereupon,we derive the related classical equations of motion such as the classical Euler–Lagrange ***,we fractionalize the classical Lagrangian of the system,and then we obtain the corresponding fractional Euler–Lagrange equations(FELEs).As a final step,we give the numerical simulations corresponding to the FELEs within different fractional *** results based on the Caputo and the Atangana-Baleanu-Caputo(ABC)fractional derivatives are given to verify the theoretical analysis.
