Heterogeneity of clinical features, EEG and brain imaging findings in anti‑leucine‑rich glioma‑inactivated protein 1 autoimmune encephalitis: a retrospective case series study and review of the literature

Background Anti-leucine-rich glioma-inactivated 1(LGI-1)autoimmune encephalitis(AE),characterized by rapid decline of memory,seizures,and neuropsychiatric abnormalities,is a rare but devastating disorder.Early diagnosis and treatment are essential to prevent long-term sequelae.In this report,we provide a detailed description of clinical characteristics,laboratory test results,imaging,and electroencephalography(EEG)findings,as well as treatment responses of eight patients with anti-LGI-1 AE treated at our center.Case presentation At the onset,all eight patients presented with confusion/memory deterioration,seizures(including faciobrachial dystonic seizures or other types of seizure),and behavioral changes such as hallucination,paranoia,and anxiety.Four patients were found with severe hyponatremia.Anti-LGI1 antibodies were detected in the cerebrospinal fluid and/or serum of all patients.For patients with faciobrachial dystonic seizures,no discernible scalp EEG change was detected,while EEG recording of patients experiencing other types of seizure showed focal slowing,focal epileptiform discharges,and focal onset seizures.All patients showed abnormal brain magnetic resonance imaging signals,mainly involving the mesial temporal lobe and the hippocampus.In addition,one patient also experienced fulminant cerebral edema during the acute phase of the illness.All patients received immunotherapy and anti-seizure medications and achieved good seizure control.Nevertheless,these patients continued to experience cognitive impairment during their long-term follow-ups.Conclusions The care of anti-LGI1 AE patients requires rapid evaluation,prompt initiation of immunotherapy,and long-term follow-up.The long-term presence of neurocognitive complications observed in these patients underline the importance of developing reliable biomarkers that can distinguish between different subtypes of this disease with heterogeneous clinico-electrographico-radiological features.Further research is needed to understand the molecular mechanisms un

In this paper,we study the asymptotic relation between the first crossing point and the last exit time for Gaussian order statistics which are generated by stationary weakly and strongly dependent Gaussian sequences.It is shown that the first crossing point and the last exit time are asymptotically independent and dependent for weakly and strongly dependent cases,respectively.The asymptotic relations between the first crossing point and the last exit time for stationary weakly and strongly dependent Gaussian sequences are also obtained.

A New Noise-Tolerant Dual-Neural-Network Scheme for Robust Kinematic Control of Robotic Arms With Unknown Models

Taking advantage of their inherent dexterity,robotic arms are competent in completing many tasks efficiently.As a result of the modeling complexity and kinematic uncertainty of robotic arms,model-free control paradigm has been proposed and investigated extensively.However,robust model-free control of robotic arms in the presence of noise interference remains a problem worth studying.In this paper,we first propose a new kind of zeroing neural network(ZNN),i.e.,integration-enhanced noise-tolerant ZNN(IENT-ZNN)with integration-enhanced noisetolerant capability.Then,a unified dual IENT-ZNN scheme based on the proposed IENT-ZNN is presented for the kinematic control problem of both rigid-link and continuum robotic arms,which improves the performance of robotic arms with the disturbance of noise,without knowing the structural parameters of the robotic arms.The finite-time convergence and robustness of the proposed control scheme are proven by theoretical analysis.Finally,simulation studies and experimental demonstrations verify that the proposed control scheme is feasible in the kinematic control of different robotic arms and can achieve better results in terms of accuracy and robustness.

De novo ADGRV1 variant in a patient with ictal asystole provides novel clues for increased risk of SUDEP

Background Various cardiac and autonomic manifestations are frequently reported during seizures.Among the seizure-related arrhythmia,ictal tachycardia is the most common,followed by ictal bradycardia,with ictal asystole being the rarest.The occurrence of ictal asystole may obscure the clinical presentation and delay the diagnosis,representing a life-threatening presentation of epilepsy,with an elevated risk of sudden unexpected death in epilepsy patients(SUDEP).These cardiac abnormalities are being increasingly recognized as the key to elucidating the mechanisms of SUDEP.Case presentation We present a 35-year-old man with a history of focal-onset seizures with impaired consciousness since his mid-20 s.He developed different types of seizures for 2 years,described as tonic seizure and atonic seizure(drop attack).During such clinical events,he suffered from falls and cardiac arrest.However,thorough cardiac electrophysiology and imaging workup failed to reveal a cardiac etiology.Subsequent video electroencephalograph(EEG)monitoring was performed,and ictal bradycardia and ictal asystole were discovered.A cardiac pacemaker was implanted,and at 3-year follow-up,the patient did not suffer more atonic seizures,or falls.Genetic tests discovered a de novo variant of Adhesion G Protein-Coupled Receptor V1(ADGRV1),which may provide a clue for the patient’s ictal asystole and the increased risk of SUDEP.Conclusions Considering the important impact of ictal bradycardia and asystole on the morbidity and potential mortality of epileptic patients,it is important to simultaneously utilize EEG and electrocardiogram to confirm the diagnosis.This case report highlights the link between the de novo variant of ADGRV1 and the ictal bradycardia/asystole phenotype and implicates the importance of genetic testing in adult epilepsy patients.

Reflected shock/turbulent boundary layer interaction structure analysis based on large eddy simulation

In order to understand the physical phenomenon of the reflected shock/turbulent boundary layer interaction,the Large Eddy Simulation(LES)is conducted to investigate shock wave and turbulent boundary layer interaction in a 12°compression ramp with inlet high Mach number of 2.9.Rescaling/recycling method is used as inflow turbulence generation technique and validated on a supersonic flat plate turbulent boundary layer.The flow field of recycling plane in the plate computation domain is obtained to give the inlet boundary condition for the LES computation.This paper focuses on the reflected shock/turbulent boundary layer interaction region,where the fine flow structure and instantaneous flow field are analyzed in detail.It is found that the unsteady motion of the shock wave leads to the increase of wall pressure fluctuation.

Mn^(2+)-doped lead halide perovskites either in 3D or 2D have been extensively explored due to their rich energy-transfer behaviors.While their application on LED is still lagging behind in comparison to non-doped 3D perovskite due to inferior film-formation and low luminescent efficiency.Here we report an in-situ-formed Mn^(2+)doped 2D perovskite nanocrystal(NCs)film by introducing quaternary phosphonium salt during the crystallizing process.The as-formed film shows improved luminescent efficiency with emission peaked at 600 nm and photo-luminescence quantum yield(PLQY)of as high as 73.37%,which is about 1.3 times higher than that of pristine film.Further characterizations confirm the enhanced confinement effect from smaller size particle is responsible for the improved luminescent efficiency.The perovskite LEDs based on pristine and phosphonium passivated thin films were fabricated,and a great improvement in the external quantum efficiency of these LEDs(from 0.0017%to 0.12%)is observed due to the improved morphology and enhanced luminescent efficiency.

Dear editor,The left atrial appendage(LAA)is the most common site for thrombus formation in patients with atrial fibrillation(AF).[1]We present the case of a patient with AF who underwent LAA closure(LAAC)with Watchman(Atritech,Boston Scientific,USA),in whom extracranial systemic embolic events(SEEs)with left atrial thrombus were observed after 4 years,and discuss relevant literature to improve the understanding of SEEs.

Applying Big Data Based Deep Learning System to Intrusion Detection

With vast amounts of data being generated daily and the ever increasing interconnectivity of the world’s internet infrastructures,a machine learning based Intrusion Detection Systems(IDS)has become a vital component to protect our economic and national security.Previous shallow learning and deep learning strategies adopt the single learning model approach for intrusion detection.The single learning model approach may experience problems to understand increasingly complicated data distribution of intrusion patterns.Particularly,the single deep learning model may not be effective to capture unique patterns from intrusive attacks having a small number of samples.In order to further enhance the performance of machine learning based IDS,we propose the Big Data based Hierarchical Deep Learning System(BDHDLS).BDHDLS utilizes behavioral features and content features to understand both network traffic characteristics and information stored in the payload.Each deep learning model in the BDHDLS concentrates its efforts to learn the unique data distribution in one cluster.This strategy can increase the detection rate of intrusive attacks as compared to the previous single learning model approaches.Based on parallel training strategy and big data techniques,the model construction time of BDHDLS is reduced substantially when multiple machines are deployed.

Renormalization group theory for temperature-driven first-order phase transitions in scalar models

We study the scaling and universal behavior of temperature-driven first-order phase transitions in scalar models. These transitions are found to exhibit rich phenomena, though they are controlled by a single complex-conjugate pair of imaginary fixed points of φ3 theory. Scaling theories and renormalization group theories are developed to account for the phenomena, and three universality classes with their own hysteresis exponents are found： a field-like thermal class, a partly thermal class, and a purely thermal class, designated, respectively, as Thermal Classes I, II, and III. The first two classes arise from the opposite limits of the scaling forms proposed and may cross over to each other depending on the temperature sweep rate. They are both described by a massless model and a purely massive model, both of which are equivalent and are derived from φ3 theory via symmetry. Thermal Class III characterizes the cooling transitions in the absence of applied external fields and is described by purely thermal models, which include cases in which the order parameters possess different symmetries and thus exhibit different universality classes. For the purely thermal models whose free energies contain odd-symmetry terms, Thermal Class III emerges only at the mean-field level and is identical to Thermal Class II. Fluctuations change the model into the other two models. Using the extant three- and two- loop results for the static and dynamic exponents for the Yang-Lee edge singularity, respectively, which falls into the same universality class as φ3 theory, we estimate the thermal hysteresis exponents of the various classes to the same precision. Comparisons with numerical results and experiments are briefly discussed.
