BACKGROUND Little is known about the safety and efficacy of using two or more biologics for the treatment of immune-mediated diseases,including Crohn’s disease(CD).CAsE sUMMARY This case report and narrative review demonstrate the potential safety of dual biologic therapy(DBT)in a 45-year-old female with two separate immunemediated *** had a history of multiple sclerosis for which she was receiving treatment with ocrelizumab,and she had been recently diagnosed with CD after presenting with *** CD diagnosis was confirmed radiologically,endoscopically,histologically,and *** patient received treatment with vedolizumab,a gut-specific inhibitor of theα4β7 integrin on *** adverse reactions were observed for the duration of *** safety of ocrelizumab and vedolizumab for the treatment of different immune-mediated diseases was *** DBT may be a safe and effective option for the treatment of refractory disease or multiple immunemediated *** biologics,which have improved safety profiles and gut specificity,may provide promising avenues for ***,caution must be exercised in the appropriate selection of biologics given their inherent immunosuppressive properties,side effects,and efficacy *** evidence suggests that biologic therapy is not associated with a worse prognosis in patients with coronavirus disease 2019,but treatment decisions should be made in a multidisciplinary *** research from controlled trials is needed to better understand the safety profile of DBT in *** immunopathological mechanisms underlying DBT also remain to be clarified.

Cardanol is a natural phenol which is obtained from high vacuum distillation of cashew nut shell *** contains a hydrocarbon chain of 15 carbon atoms in the meta position,either with one,two or three nonconjugated double *** article describes thermal thiol-ene reaction to synthesize new cardanol-based polyols for polyurethanes with aromatic-aliphatic *** hydroxyl group was blocked by alkoxylation and 2-mercaptoethanol was added to the double bonds of propoxylated *** resultant product is a mixture of polyols that may contain one,two,three or four hydroxyl groups,as a function of the number of double bonds reacted with *** polyols,but with much higher functionality,were also synthesized from condensation of cardanol-based novolacs with *** cardanolbased polyols were further utilized to prepare rigid polyurethane foams(PUs)with excellent physical and mechanical properties,useful for various applications in chemical and food industries.

Photometric observations in sloan g′and i′ bands of W UMa binaries NsVs 4340949,T-Dra0–00959,GsC 03950–00707,NsVs 4665041,NsVs 4803568,MM Peg,MM Com and NsVs4751449 are *** light curve solutions revealed that the components of each target are of G and K spectral *** binaries of the sample have middle-contact configurations whose fillout factors are within the range 0.2–*** only exception is NsVs 4751449 which is in deeper contact（fillout factor of 0.55）.It precisely obeys the relation between mass ratio and fillout factor for deep,low mass ratio overcontact *** of the eclipses of almost all targets（except MM Peg）is an occultation and their photometric mass ratios and solutions could be accepted with *** found that the target components have almost equal temperatures but differ considerably in size and *** components of the partially-eclipsed MM Peg have close *** solutions reveal that NsVs 4340949,T-Dra0–00959,NsVs 4803568 and MM Com are of W subtype while GsC 03950–00707,NsVs 4665041,MM Peg and NsVs 4751449 are of A *** subclassification is well-determined for all totallyeclipsed *** targets confirm the trends in which W-subtype systems have smaller periods and lower temperatures than A subtype binaries.

Photometric observations in sloan 9＇ and i＇ bands of four W UMa stars, NsVs 2244206, NsVs 908513, Css J004004.7＋385531 and VsX J062624.4＋570907, are presented. The light curve solutions reveal that all targets have overcontact configurations with fillout factors within 0.15-0.26. Their components are G-K spectral types and are almost in thermal contact. They are also relatively close in size and luminosity： the radius ratios r2/r1 are within 0.75-0.90; the luminosity ratios 12/11 are within 0.53-0.63. The results of the light curve solution of Css J004004.7＋385531 imply the weak limb-darkening effect of its primary component and possible presence of additional absorbing features in the system.

Objective:To describe the main epidemiological,clinical,and laboratory features,treatment options and outcome in children with ***:Retrospectively evaluated data were obtained from 317 pediatric patients with brucellosis that were treated at the University Clinic for Infectious Diseases and Febrile Conditions in skopje,during the period from 1989 to *** medical records and follow-up protocols were used for ***:Childhood brucellosis composed 317(18.7%) of 1 691 patients with *** patients were median 9 years old,ranging from 7 months to 14 years,and 201(63.4%) were *** history was present in 197(62.1%),and direct contact with animals occurred in 140(44.2%) of the *** dominant manifestations were fever in 248(78.2%),joint pain in228(71.9%) and hepatomegaly in 216(68.1%).Organ affection was present in 206(65.0%)of the *** hundred and six(33.4%) of the patients were treated with combination composed of *** 211(66.6%) with three antimicrobial *** were registered in 21(6.6%).and therapeutic failures in 3(0.9%) of the ***:In endemic regions childhood brucellosis represents a significant part of human *** spectrum of clinical manifestations,frequent affection of various organ systems and possibility of relapses show that brucellosis could be a serious disease in this age *** presence of fever,joint pain,sweating,and affection of various systems in children from endemic regions should alert pediatricians for the possibility of brucellosis.

AIM: To assess whether depression and anxiety are more expressed in patients with the first episode of inflammatory bowel disease (IBD) than in individuals with newly discovered cancer of the colon (CCa). METHODs: A total of 32 patients with IBD including 13 males and 19 females, aged 27 to 74, and 30 patients with CCa including 20 males and 10 females, aged 39-78, underwent a structured interview, which comprised Hamilton’s Depression Rating Inventory, Hamilton’s Anxiety Rating Inventory and Paykel’s stressful Events Rating scale. REsULTs: Patients of the IBD group expressed both depression and anxiety. Depressive mood, sense of guilt, psychomotor retardation and somatic anxiety were also more pronounced in IBD patients. The discriminant function analysis revealed the total depressive score was of high importance for the classification of a newly diagnosed patient into one of the groups. CONCLUsION: Newly diagnosed patients with IBD have higher levels of depression and anxiety. Moreover, a psychiatrist in the treatment team is advisable from the beginning.

The present study was aimed at revealing the responses of metabolite pools to selection for alternative reproductive schedules in the seed beetle, Acanthoscelides obtectus say （Coleoptera： Chrysomelidae： Bruchinae）. The levels of metabolites （free sugars, glycogen, lipids, soluble and hydrophobic proteins） that were determined in virgin females and males at three ages from adult eclosion onwards were compared among the base population （B） and two derived lines that were selected for either early （Y） or late （O） reproduction. The results showed differences in the accumulation of metabolites during pre-adult development, as well as in the pattern of their changes during adult ageing, Generally, in comparison to the B population, the short-lived beetles from the Y line showed increased protein content and reduced carbohydrate and lipid content, whereas the opposite was true for the long-lived beetles from the O line. Females from the O line exhibited slower utilization of energy reserves and a slower increase in protein contents than females from the Y line. Females contained higher levels of free sugars, glycogen and hydrophobic proteins and lower levels of lipids and soluble proteins than males, although the sexual dimorphism was not evenly expressed among lines. Age-specific changes in metabolite contents were slower in females than males. Our findings suggest that trade-offs among capital resources are a physiological basis of early/late fitness trade-offs and point to a conservation of resources that can be used for somatic maintenance.

AIM To examine whether high-flow nasal oxygen(HFNO) availability influences the use of general anesthesia(GA) in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) and endoscopic ultrasound(EUs) and associated *** In this retrospective study, patients were stratified into 3 eras between October 1, 2013 and June 30, 2014 based on HFNO availability for deep sedation at the time of their endoscopy. During the first and last 3-mo eras(era 1 and 3), no HFNO was available, whereas it was an option during the second 3-mo era(era 2). The primary outcome was the percent utilization of GA vs deep sedation in each period. secondary outcomes included oxygen saturation nadir during sedation between periods, as well as procedure duration, and anesthesia-only time between periods and for GA vs sedation cases *** During the study period 238 ERCP or EUs cases were identified for analysis. statistical testing was employed and a P s significant unless the Bonferroni correction for multiple comparisons was used. General anesthesia use was significantly lower in era 2 compared to era 1 with the same trend between era 2 and 3(P = 0.012 and 0.045 respectively). The oxygen saturation nadir during sedation was significantly higher in era 2 compared to era 3(P s 1 and 2(P = 0.028) or 1 and 3(P = 0.069). The procedure time within each era was significantly longer under GA compared to deep sedation(P ≤ 0.007) as was the anesthesia-only time(P ≤ 0.001).CONCLUsION High-flow nasal oxygen availability was associated with decreased GA utilization and improved oxygenation for ERCP and EUs during sedation.
