Coronal shear fractures of the distal humerus are rare,complex fractures that can be technically challenging to manage. They usually result from a low-energy fall and direct compression of the distal humerus by the radial head in a hyper-extended or semi-flexed elbow or from spontaneous reduction of a posterolateral subluxation or dislocation. Due to the small number of soft tissue attachments at this site, almost all of these fractures are displaced. The incidence of distal humeral coronal shear fractures is higher among women because of the higher rate of osteoporosis in women and the difference in carrying angle between men and women. Distal humeral coronal shear fractures may occur in isolation, may be part of a complex elbow injury, or may be associated with injuries proximal or distal to the elbow. An associated lateral collateral ligament injury is seen in up to 40% and an associated radial head fracture is seen in up to 30% of these fractures. Given the complex nature of distal humeral coronal shear fractures, there is preference for operative management. Operative fixation leads to stable anatomic reduction, restores articular congruity, and allows initiation of early range-of-motion movements in the majority of cases. Several surgical exposure and fixation techniques are available to reconstruct the articular surface fol owing distal humeral coronal shear fractures. The lateral extensile approach and fixation with countersunk headless compression screws placed in an anterior-to-posterior fashion are commonly used. We have found a two-incision approach(direct anterior and lateral) that results in less soft tissue dissection and better outcomes than the lateral extensile approach in our experience. Stiffness, pain, articular incongruity, arthritis, and ulnohumeral instability may result if reduction is non-anatomic or if fixation fails.

Purpose: To improve liver iron load assessment by investigating the precision of different approaches of T2* Measurement. Background: Iron overload is a major problem in the treatment of thalassemic patients. Liver iron concentration (LIC) is an important index toward the management of body iron load. The accuracy of iron load estimation may suffer from the methodology of T2* measurement and there is no complete agreement upon the best approach of T2* calculation. Methods: 32 β-thallasemic patients (18 male) with the mean age of 20.0 ± 6.5 years were involved in this study. A multi-echo fast gradient-echo technique on a 1.5 T MRI system was used to measure liver iron overload and the T2* map of liver was reconstructed on a pixel-by-pixel basis. The T2* map and MRI images were utilized to deter- mine accurate location of ROI (region of interest). The mean of T2* were computed from the ROIs. The reproducibility of calculated T2* values in two methods were obtained. Moreover, the mean of the pixel’s T2* was calculated in the entire liver parenchyma of one slice. The T2* value of the entire slice was compared with the ROI approach. Results: In the ROI based method, the CoV for the intra-observer reproducibility was 8.5% and for the inter-observer was 9.78%. In the pixel based method, the CoVs for intra-observer and inter-observer reproducibility were 2.79% and 3.91%. There was an acceptable correlation (r = 0.96) between the T2* values calculated by the ROI and the entire slice. Conclusions: The pixel-based approach is more precise to determine the appropriate placement of the ROI. The assessment of T2* in the entire slice reduces the user-based errors significantly.

Accuracy of Medical Oxygen Flowmeters: A Multicentric Field Study

The accuracy of 476 oxygen flowmeters was investigated using a thermal mass flowmeter in eight hospitals in France and Belgium. Different oxygen flow rates (2 to 15 l/min) were evaluated at the patient’s bed. When the sample was considered as a whole, the accuracy of delivered flow was acceptable but precision was poor. The variability of the delivered flow between devices was greater when a low flow rate was required. Compensated-pressure oxygen flowmeters for these low rates were more accurate than their non-compensated counterparts. This study emphasizes the need to individually adapt the oxygen flow rate each time a patient has to move from one flowmeter to another.

Objective:To identify the incidence rate,relative risk,hotspot regions and incidence trend of COVID-19 in Qom province,northwest part of Iran in the first stage of the ***:The study included 1125 officially reported PCR-confirmed cases of COVID-19 from 20 February 2020 to 20 April 2020 in 90 regions in Qom city,*** Bayesian hierarchical spatial model was used to model the relative risk of COVID-19 in Qom city,and the segmented regression model was used to estimate the trend of COVID-19 incidence *** Poisson distribution was applied for the observed number of COVID-19,and independent Gamma prior was used for inference on log-relative risk parameters of the ***:The total incidence rate of COVID-19 was estimated at 89.5 per 100000 persons in Qom city(95%CI:84.3,95.1).According to the results of the Bayesian hierarchical spatial model and posterior probabilities,43.33%of the regions in Qom city have relative risk greater than 1;however,only 11.11%of them were significantly greater than *** on Geographic Information Systems(GIS)spatial analysis,10 spatial clusters were detected as active and emerging hotspot areas in the south and central parts of the *** downward trend was estimated 10 days after the reporting of the first case(February 7,2020);however,the incidence rate was decreased by an average of 4.24%per day(95%CI:−10.7,−3.5).Conclusions:Spatial clusters with high incidence rates of COVID-19 in Qom city were in the south and central regions due to the high population *** GIS could depict the spatial hotspot clusters of COVID-19 for timely surveillance and decision-making as a way to contain the disease.

Propose:In this study,we re-assessed the criteria defined by the radiological society of North America(RSNA)to determine novel radiological findings helping the physicians differentiating COVID-19 from pulmonary ***:All trauma patients with blunt chest wall trauma and subsequent pulmonary contusion,COVID-19-related signs and symptoms before the trauma were enrolled in this retrospective study from February to May *** patients(Group P)were then classified into two groups based on polymerase chain reaction tests(Group Pa for positive patients and Pb for negative ones).Moreover,44 patients from the prepandemic period(Group PP)were *** were matched to Group P regarding age,sex,and trauma-related *** radiologists blindly reviewed the CT images of all enrolled patients according to criteria defined by the RSNA *** radiological findings were compared between Group P and Group PP;statistically significant ones were re-evaluated between Group Pa and Group Pb ***,the sensitivity and specificity of each significant findings were *** Chi-square test was used to compare the radiologica丨findings between Group P and Group ***:In the Group PP,73.7%of all ground-glass opacities(GGOs)and 80%of all multiple bilateral GGOs were detected(p<0.001 and p=0.25,respectively).Single bilateral GGOs were only seen among the Group *** Chi-square tests showed that the prevalence of diffused GGOs,multiple unilateral GGOs,multiple consolidations,and multiple bilateral consolidations were significantly higher in the Group P(p=0.001,0.01,0.003,and 0.003,respectively).However,GGOs with irregular borders and single consolidations were more significant among the Group PP(p=0.01 and 0.003,respectively).Of note,reticular distortions and subpleural spares were exclusively detected in the Group ***:We concluded that the criteria set by RSNA for the diagnosis of COVID-19 are not appropriate in trauma ***

The used water for human consumption must be free of microorganisms and chemicals that cause risk in the human health. In this study, water quality of 18 rural area of Abarkouh was determined and compared the conventional monitoring method (According to ISIR (Institute of Standards and Industrial Research of Iran), 1053 and 4208) and use of electronic system method (Patent in industrial property general office of Iran, 77815). Free chlorine monitoring and pH test done by health workers in the conventional method and the results will be sent to the Health Network monthly. Sampling for microbiological testing is done monthly based on population (According to ISIR, 4208). On the electronic system, the procedure is also done by health workers, but the result will be sent to the receiver device by using a cell phone. According to the chlorine test results if the free chlorine residual reported zero, microbiological sampling was done by a health expert. Finally, the number of chlorine test and microbiological sampling and the results of these experiments collected in the both methods and recorded in SPSS 22 then were analyzed by using chi-square test and Fisher exact test. The result of microbiological experiments shows that the sampling rate decreased 29% in using of electronic system method in comparison to the conventional monitoring method while the number of microbial defect detection increased 19% in drinking water networks monitoring by electronic system. Using of electronic system monitoring can reduce the rate and cost of microbiological sampling and its experiments and increase accuracy of these tests, in this way it will increase the quality and safety of drinking water in distribution network in small and dispersed rural communities.

AIM: To explore the role of mammalian target of rapamycin(m TOR) in the pathogenesis of cirrhotic cardiomyopathy and the potential of rapamycin to improve this pathologic ***: Male albino Wistar rats weighing 100-120 g were treated with tetrachloride carbon(CCl_4) for 8 wk to induce cirrhosis. Subsequently, animals were administered rapamycin(2 mg/kg per day). The QT_c intervals were calculated in a 5-min electrocardiogram. Then, the left ventricular papillary muscles wereisolated to examine inotropic responsiveness to β-adrenergic stimulation using a standard organ bath equipped by Powerlab system. Phosphorylated-m TOR localization in left ventricles was immunohistochemically assessed, and ventricular tumor necrosis factor(TNF)-α was measured. Western blot was used to measure levels of ventricular phosphorylated-m TOR ***: Cirrhosis was confirmed by hematoxylin and eosin staining of liver tissues, visual observation of lethargy, weight loss, jaundice, brown urine, ascites, liver stiffness, and a significant increase of spleen weight(P < 0.001). A significant prolongation in QTc intervals occurred in cirrhotic rats exposed to CCl_4(P < 0.001), while this prolongation was decreased with rapamycin treatment(P < 0.01). CCl_4-induced cirrhosis caused a significant decrease of contractile responsiveness to isoproterenol stimulation and a significant increase in cardiac TNF-α. These findings were correlated with data from western blot and immunohistochemical studies on phosphorylated-m TOR expression in left ventricles. Phosphorylated-m TOR was significantly enhanced in cirrhotic rats, especially in the endothelium, compared to controls. Rapamycin treatment significantly increased contractile force and myocardial localization of phosphorylated-m TOR and decreased cardiac TNF-α concentration compared to cirrhotic rats with no treatment.
