Hypothalamic neuropeptides named hypocretin/orexins which were identified in 1998 regulate critical functions such as wakefulness in the central nervous *** past 20 years had revealed that orexins/receptors system was also present in the peripheral nervous system where they participated to the regulation of multiple functions including blood pressure regulation,intestinal motility,hormone secretion,lipolyze and reproduction *** to these peripheral functions,it was found that orexins and their receptors were involved in various diseases such as acute/chronic inflammation,metabolic syndrome and *** present review suggests that orexins or the orexin neural circuitry represent potential therapeutic targets for the treatment of multiple pathologies related to inflammation including intestinal bowel disease,multiple sclerosis and septic shock,obesity and digestive cancers.

This paper investigates the inverse problem of bi-revealed utilities in a defaultable universe,defined as a standard universe(represented by a filtration F)perturbed by an exogenous defaultable time τ.We assume that the standard universe does not take into account the possibility of the default,thus τ adds an additional source of *** defaultable universe is represented by the filtration G up to time τ(τ included),where G stands for the progressive enlargement of F by *** basic assumption in force is that τ avoids F-stopping *** bi-revealed problem consists in recovering a consistent dynamic utility from the observable characteristic of an *** general results on bi-revealed utilities,first given in a general and abstract framework,are translated in the defaultable G-universe and then are interpreted in the *** decomposition of G-adapted processes X^(G) provides an interpretation of a Gcharacteristic X^(G)_(τ) stopped at τ as a reserve *** to the characterization of G-martingales stopped at τ in terms of F-martingales,we establish a correspondence between G-bi-revealed utilities from characteristic and F-bi-revealed pair of utilities from characteristic and *** a financial framework,characteristic can be interpreted as wealth and reserves as *** result sheds a new light on the consumption in utility criterion:the consumption process can be interpreted as a certain quantity of wealth,or reserves,that are accumulated for the financing of losses at the default time.

The tricarboxylic acid （TCA） cycle is a central route for oxidative phosphorylation in cells, and fulfills their bioenergetic, biosynthetic, and redox balance require- ments. Despite early dogma that cancer cells bypass the TCA cycle and primarily utilize aerobic glycolysis, emerging evidence demonstrates that certain cancer cells, especially those with deregulated oncogene and tumor suppressor expression, rely heavily on the TCA cycle for energy production and macromolecule synthesis. As the field progresses, the importance of aberrant TCA cycle function in tumorigenesis and the potentials of applying small molecule inhibitors to perturb the enhanced cycle function for cancer treatment start to evolve. In this review, we summarize current knowledge about the fuels feeding the cycle, effects of oncogenes and tumor suppressors on fuel and cycle usage, common genetic alterations and deregulation of cycle enzymes, and potential therapeutic opportunities for targeting the TCA cycle in cancer cells. With the application of advanced technology and in vivo model organism studies, it is our hope that studies of this previously overlooked biochemical hub will provide fresh insights into cancer metabolism and tumorigenesis, subsequently revealing vulnerabilities for thera- peutic interventions in various cancer types.

Outpatient competence restoration: A model and outcomes

AIM: To describe a model outpatient competencerestoration program(OCRP) and provide data on time to restoration of adjudicative ***: The authors tracked the process by which individuals are referred for outpatient competence restoration(OCR) by courts in the United States capital, describing the unique requirements of American law, and the avenues available for compelling adherence. Competence to stand trial is a critical gate-keeping function of the judicial and forensic communities and assures that defendants understand courtroom procedures. OCR is therefore an effort to assure fairness and protection of important legal rights. Multimedia efforts are described that educate patients and restore competence to stand trial. These include resources such as group training, use of licensed clinicians, visual aids, structured instruments, and cinema. Aggregate data from the OCRP's previous 4 years of OCR efforts were reviewed for demographic characteristics, restoration rate, and time to restoration. Poisson regression modeling identified the differences in restoration between sequential 45-d periods after entrance into the ***: In the past 4 years, the DC OCRP has been successful in restoring 55 of 170 participants(32%), with an average referral rate of 35 persons per year. 76% are restored after the initial 45 d in the program. Demographics of the group indicate a predominance of African-American men with a mean age of 42. Thought disorders predominate and individuals in care face misdemeanor charges 78% of the time. Poisson regression modeling of the number attaining competence during four successive 45-d periods showed a substantial difference among the time periods for the rate of attaining competence(P = 0.0011). The three time periods after 45 d each showed a significant decrease in the restoration rate when compared to the initial 0 to 45 d period- their relative rates were only 22% to 33% as high as the rate for 0-45 d(all P-values, com

The dietary relationship study between marine sponge Xestospongia *** its nudibranch predators jorunna funebris based on the discovery of isoquinolinequinones has long been *** this study,chemical investigation of the sponge Xestospongia *** nudibranch *** from the South China Sea yielded a new marine alkaloid neopetroside C(1),together with nine known alkaloids(2-10).The chemical structures of all the compounds were elucidated by extensive spectroscopic *** C(1)featured a riboside of nicotinic acid with a rareα-N glycosildic linkage and an acyl residue of(Z)-2-methylbut-2-enoic acid attached to C-5′.The plausible chemical ecology relationship between sponge Xestospongia *** its nudibranch predator *** was proposed based on the biogenetic relationship of the common marine *** observation of two structural fragments,(Z)-2-methylbut-2-enoyloxy and trigonelline groups in both sponge and nudibranch,indicated that nudibranch might uptake chemicals from sponge and then modify and transform them into chemical weapons to defend against predators.

BACKGROUND:The Pediatric Infectious Disease Society(PIDS)and Infectious Disease Society of America(IDSA)published an evidence-based guideline for the treatment of uncomplicated communityacquired pneumonia(CAP)in children,recommending aminopenicillins as the first-line *** guideline compliance with 10%–50%of patients admitted to the hospital receiving narrow-spectrum antibiotics has been reported.A new clinical practice guideline(CPG)was implemented in our emergency department(ED)for uncomplicated *** aim of this study was to examine baseline knowledge and ED provider prescribing patterns pre-and post-CPG ***:Prior to CPG-implementation,an anonymous case-based survey was distributed to evaluate knowledge of the current PIDS/IDSA guideline.A retrospective chart review of patients treated in the ED for CAP from january 2015 to February 2017 was performed to assess prescribing patterns for intravenous(IV)antibiotics in the ED at Children’s National Health System pre-and post-CPG ***:ED providers were aware of the PIDS/IDSA guideline recommendations,with 86.4%of survey responders selecting ampicillin as the initial antibiotic of ***,only 41.2%of patients admitted to the hospital with uncomplicated CAP pre-CPG received ampicillin(P<0.01).There was no statistically signifi cant increase in ampicillin prescribing post-CPG(P=0.40).CONCLUSIONS:Providers in the ED are aware of the PIDS/IDSA guideline regarding the first-line therapy for uncomplicated CAP;however,this knowledge does not translate into clinical *** of a CPG in isolation did not significantly change prescribing patterns for uncomplicated CAP.
