In this work, Bioactive-functionalized interpenetrating network (IPNs) hydrogel (BIOF-INPs) were prepared and investigated in vitro for the free radical detection/defense, therapeutic release as well as shear bond strength to dentine, ability to re-mineralize surface of the dentin after application of these bio-inspired materials using a biologically inspired mineralization process in vitro as well as investigating antimicrobial properties of the BIOF-INPs against S. aureous. The aim of this investigation was to evaluate the suitability and flexibility of the designer materials to act as an “in vitro” probe to gain insights into molecular origin of TMD and associated disorders.

Restorative materials in the new era aim to be “bioactive” and long-lasting. The purpose of the study was to design and evaluate novel chitosan hydrogels containing melatonin and/or propolis (antioxidant containing material), nystatin (antifungal), naproxen (pain relieve medication) and combinations thereof (chitosan-H-melatonin, chitosan-H-melatonin-naproxen, chitosan-H-propolis, chitosan-H-propolisnaproxen, chitosan-H-naproxen-propolis-melatonin, Chitosan/Propolis/Nystatin, Chitosan/Melatonin/Propolis/Nystatin, Chitosan/Propolis/BSA/Nystatin, Chitosan/Melatonin/BSA/Nystatin) as functional additive prototypes for further development of “dual function restorative materials”, to determine their effect on the dentin bond strength of a composite, to evaluate stability of the encapsulated antioxidants as well as evaluate antimicrobial properties of the selected group of “designer” functional materials. Materials and Methods: The above mentioned hydrogels were prepared by dispersion of the corresponding component in glycerol and acetic acid with the addition of chitosan gelling agent. The surface morphology (SEM), drug-polymer solid state interaction (FT-IR spectroscopy), released behaviours (physiological pH and also in acidic conditions), stability of the therapeutic agent-antioxidant-chitosan and the effect of the hydrogels on the shear bond strength of dentin were also evaluated. Results: The release of naproxen confers the added benefit of synergistic action of a functional therapeutic delivery when comparing the newly designed chitosan-based hydrogel restorative materials to the commercially available products alone. Neither the release of naproxen or the antioxidant stability was affected by storage over a 6-month period. Dielectric relaxation spectroscopy（DRS） of poly（ε-caprolactone） with different draw ratios showed that the mobility of polymer chains in the amorphous part decreases as the draw ratio increases. The activation energy of the α process, which corresponds to the dynamic glass transition, increases upon drawing. The enlarged gap between the activation energies of the α process and the β process results in a change of continuity at the crossover between the high temperature a process and the α and β processes. At low drawing ratios the a process connects with the β process, while at the highest drawing ratio in our measurements, the a process is continuous with the αprocess. This is consistent with X-ray diffraction results that indicate that upon drawing the polymer chains in the amorphous part align and densify upon drawing. As the draw ratio increases, the α relaxation broadens and decreases its intensity, indicating an increasing heterogeneity. We observed slope changes in the α traces, when the temperature decreases below that at which τα≈ 1 s. This may indicate the glass transition from the ‘rubbery＇ state to the non-equilibrium glassy state.

This is a prospective controlled randomized trial conducted in 92 women with singleton pregnancies in preterm labor. The tocolytic efficacy and safety of combination atosiban and nifedipine was compared with that of the single agent, atosiban. Both lines of intervention was administered for 48 hours. Progression of labor was assessed by the frequency of uterine contractions and cervical changes. For statistical purpose, intent-to-treat (ITT) analysis was used throughout. Efficacy, as determined by the proportion of women in each group who did not deliver after therapy initiation, was comparable with no significant differences, at 48 hrs (91.5% vs. 91.1%) and at 7 days (90.7% vs. 85.7%) for the atosiban and the combination groups respectively. Safety was assessed by the numbers of adverse events. Maternal side effects were reported more in the combination group (34% vs. 64%;P = 0.006). Perinatal outcomes were similar between the groups. We conclude that the addition of nifedipine did not substantially improve the clinical outcomes beyond that were achieved with atosiban alone. Moreover, it has increased maternal side effects. Future research could focus on combination of other tocolytics.

糖尿病足溃疡(DFU)是糖尿病严重并发症之一,具有高致残率、高病死率、高患病率的特点。DFU易合并感染,部分DFU患者感染症状不明显,造成延误治疗,截肢风险升高,高达50%的DFU患者合并周围动脉病变(PAD),PAD造成动脉管腔狭窄、闭塞,导致溃疡愈合不良,DFU患者同时伴有PAD及感染时截肢、死亡风险明显升高。本文重点介绍DFU患者合并PAD及感染的临床特点及感染细菌谱、抗生素耐药情况,如何规范诊治此类患者,改善糖尿病足溃疡感染合并PAD的预后,以期降低DFU患者的截肢率,提高患者生活质量。

The purpose of the present study was to assess the reproducibility of voxel placement for GABA-edited MRS. GABA-edited MRS data were acquired in 13 healthy volunteers from (3 cm)3 voxel;and within the same session a second acquisition was independently prescribed. A three-dimensional voxel mask image was reconstructed in T1-image-space using the SVMask tool (in house software). Reproducibility of voxel placement was assessed using the Dice overlap coefficient, both within-subj.ct and between-subj.ct following co-registration of T1 images and transformation of voxel mask images to standard space. Within-subj.ct overlap coefficients were 86% ± 5%. Between-subj.ct overlap coefficients were 75% ± 10%. For the two voxel locations considered (occipital and sensorimotor), voxel overlap was very similar. Between-subj.ct values are higher due to between-session effects, anatomical variability and volume mismatch in standard space. While surprisingly low in terms of volume overlap, the overlap coefficients correspond to acceptable linear displacements.

糖尿病足不仅给患者带来巨大的折磨,还给社会带来沉重的经济负担。本指南是以目标为导向,以循证医学为依据,且在全球得到合理实施。践行国际糖尿病足病指南有良好的经济-效益比。国际糖尿病足工作组(IWGDF)从1999年起开始发布和更新国际指南。IWGDF在着眼于相关临床问题和结局、严谨的文献系统评价、最大可能给予明确而清晰的推荐及其依据的基础上发布了2019年版指南更新。相关推荐均基于推荐评估和评价系统(GRADE)。本文阐述《IWGDF:糖尿病足防治国际指南(2019)》(简称《IWGDF指南》)的制定过程。《IWGDF指南》中指南共6个部分,分别由6个国际专家组编写,从预防、减压、周围动脉病变、感染、创面愈合干预及糖尿病足溃疡分级6个方面提出推荐意见。IWGDF编辑委员会分别从上述6个方面制定了一系列的实践指南,每条推荐意见均经过IWGDF编辑委员会成员和相关领域国际专家的深入评估。我们相信,只要相关专业人员遵循2019版IWGDF指南推荐,并将这些推荐与本地具体情况相结合,就能够进一步改善糖尿病足的预防和治疗水平,同时也会减轻糖尿病足给全世界患者和社会所带来的负担。
