AIm.To investigate the prognostic value of the radiological response after transarterial chem.em.olization(TACE)and inflam.atory m.rkers in patients affected by hepatocellular carcinom.(HCC)awaiting liver transplantation(LT).m.THODS We retrospectively evaluated the preoperative pre dictors of HCC recurrence in 70 patients treated with conventional(n=16)or doxorubicin-eluting bead TACE(n=54)before *** patient and tum.ur characteristics,including the static and dynam.c alpha-fetoprotein,neutrophil-to-lym.hocyte ratio and platelet-to-lym.hocyte ratio( LR)m.asurem.nts,were *** response was classified according to the m.dified Response Evaluation Criteria in Solid Tum.urs(m.RECIST)and the European Association for the Study of the Liver(EASL)criteria as com.lete response(CR),partial response( R),stable disease o progressive *** exam.nation of the explanted livers,histological necrosis was classified as com.lete(100%of the cum.lative tum.ur area),partia(50%-99%)or m.nim.l(m.lan and University of San Francisco(UCSF)criteria,*** TACE procedures,the objective response(CR+ R)rates were 71.4%and 70.0%according to m.RECIST and EASL criteria,*** agreem.nt between the two guidelines in defining the radiological response was rated as very good both for the overall and target lesion response(weighted k-value:0.98 and 0.93,respectively).Com.lete and partial histological necrosis were achieved in 14/70(20.0%)and 28/70(40.0%)patients,*** histopathology as the reference standard,m.RECIST criteria correctly classified necrosis in 72.9%(51/70)of patients and EASL criteria in 68.6%(48/70)of *** m.RECIST non-response to TACE[Exp(b)=9.2, =0.012],exceeding UCSF criteria before TACE[Exp(b)=4.7, =0.033]and a preoperative LR>150[Exp(b)=5.9, =0.046]

Chem.m.tric Analysis of an Sanitary Landfill Leachate

This paper presents a study on the biotic/abiotic conditions of the S?o Giácom. sanitary landfill, located near the city of Caxias do Sul, Brazil, through statistical analysis of fourteen physic-chem.cal data sets for the leachate, produced in the garbage dum. site over a long period of years. Different chem.m.tric m.thods are used in the statistical analysis. For exam.le, the correlations between the variables, related to the degraded organic m.tter and biological activity, are determ.ned by m.ans of m.ltivariate m.thods. The results highlight that BOD, COD, VTS, FTS and TS give inform.tion on the anaerobic degradation of the organic m.tter contained in the cells, and suggest that the greater the contribution of the variables with positive weights in PC1 the greater the level of organic m.tter degradation. The variables TN, Am.n Nit. and alkalinity are related to the biological activity and determ.ne the potency of the variables in relation to tim.. The greater the contribution of the variables related to organic degradation the greater the values in PC2 and the lesser the potency of these variables, whose influence is greater in the second stage of anaerobic degradation. The variables of PC2 is im.ortant plans of the contam.nation of the leached in the bodies hídrics.

This paper presents a study of am.rphous SiOx thin film. by m.ans of Variable Angle Spectroscopic Ellipsom.try (VASE) technique. Tauc Lorentz, Lorentz and Cauchy m.dels have been used to obtain physical thickness and com.lex refractive index (n and k) from.experim.ntal data. In order to obtain a wide range to x stoichiom.try values, the film. were prepared by vacuum.therm.l evaporation of SiO on glass substrates, under different and controlled deposition conditions.

Neuroendocrine tum.rs resistant to m.m.alian target of rapam.cin inhibitors:A difficult conversion from.biology to the clinic

Deregulation of the phosphoinositide 3-kinase(PI3K)/protein kinase B(Akt)- m.m.alian target of rapam.cin(m.TOR) signaling pathway is one of the m.st com.onlyinvolved pathways in tum.rigenesis. It has also been reported as altered in neuroendocrine tum.rs(NETs). m.TOR inhibitors used in clinical practice are derived from.rapam.cin,an anti-cancer agent also used as an im.unosuppressor after organ transplantation. Everolim.s and tem.irolim.s are the two rapam.cin-derived m.TOR inhibitors used in NETs. Notably everolim.s has been approved in advanced progressive well/m.deratelydifferentiated pancreatic NETs(p NETs). It inhibits specifically the m.TORC1 subunit of m.TOR,not interacting with m.TORC2. Although everolim.s produced a significant prolongation of progression-free survival a num.er of patients with p NETs do not benefit from.the drug due to early or late progression. Two supposed m.chanism. of resistance to m.TOR inhibitors are Akt and PI3 K activation,by m.ans of m.TORC2 and insulin growth factor(IGF)- IGF receptor signaling,respectively. BEZ235 is a m.lti-targeted inhibitor binding to PI3 K,m.TORC1 and m.TORC2,therefore potentially turning off all the supposed m.lecular targets of resistance to everolim.s. The two clinical trials designed in p NETs were stopped early due to unm.t statistical endpoint and the global clinical developm.nt of BEZ235 was also halted. Tolerability of this drug was challenging and conditioned the feasibility of therapy. The BEZ experience is an exam.le of the huge difference between the preclinical and clinical setting and prom.ts us to pay m.re attention to the phase Ⅰ step of clinical developm.nt and the design of phase Ⅱ clinical trials.

AIm.To investigate the prevalence of gastroesophageal reflux disease(GERD) in patients with a laryngoscopic diagnosis of laryngopharyngeal reflux(LPR).m.THODS:Between m.y 2011 and October 2011,41 consecutive patients with laryngopharyngeal sym.tom.(LPS) and laryngoscopic diagnosis of LPR were em.irically treated with proton pum. inhibitors(PPIs) for at least 8 wk,and the therapeutic outcom. was assessed through validated questionnaires(GERD im.act scale,GIS;visual analogue scale,VAS).LPR diagnosis was perform.d by ear,nose and throat specialists using the reflux finding score(RFS) and reflux sym.tom.index(RSI).After a 16-d wash-out from.PPIs,all patients underwent an upper endoscopy,stationary esophageal m.nom.try,24-h m.ltichannel intralum.nal im.edance and pH(m.I-pH) esophageal m.nitoring.A positive correlation between LPR diagnosis and GERD was supposed based on the presence of esophagitis(ERD),pathological acid exposure tim.(AET) in the absence of esophageal erosions(NERD),and a positive correlation between sym.tom. and refluxes(hypersensitive esophagus,HE).RESULTS:The m.le/fem.le ratio was 0.52(14/27),the m.an age ± SD was 51.5 ± 12.7 years,and the m.an body m.ss index was 25.7 ± 3.4 kg/m.*** subjects reported one or m.re ***-five out of 41 patients also had typical GERD sym.tom.(heartburn and/or regurgitation).The m.st frequent laryngoscopic findings were posterior laryngeal hyperem.a(38/41),linear indentation in the m.dial edge of the vocal fold(31/41),vocal fold nodules(6/41) and diffuse infraglottic oedem.(25/41).The GIS analysis showed that 10/41 patients reported sym.tom.relief with PPI therapy(P m. tim.,the VAS analysis showed a significant reduction in typical GERD sym.tom. after PPI therapy(P m.tility

Action for Rehabilitation from.Neurological Injury (ARNI): A pragm.tic study of functional training for stroke survivors

This study evaluated the effectiveness of a twelve-week com.unity-based functional training on m.asures of im.airm.nt, activity and participation in a group of stroke survivors. Isom.tric strength of the knee m.sculature, Centre-Of-Pressure (COP) based m.asures of balance, Berg Balance Scale (BBS), 10 m.walk test, and the Subjective Index of Physical and Social Out com. (SIPSO), were recorded at baseline, post-intervention, and after twelve weeks (follow-up). Exercise instructors delivered training once a week in a group form.t at a com.unity centre. Significant im.rovem.nt was noted in the BBS (p m.walk speed (p = 0.03) post intervention which rem.ined unchanged at follow-up. Total SIPSO score im.roved significantly post-intervention (p = 0.044). No other significant differences and no adverse effects were observed. It is possible that functional training provided m.re opportunity for the im.rovem.nt of dynam.c aspects of balance control that could be captured by the BBS but not with the traditional m.asures of balance using COP data. Results also suggest positive effects on the level of participation, and lack of association between m.asures of im.airm.nt and activity. Com.unity based functional training could be effective and used to extend access to rehabilitation services beyond the acute and sub-acute stages after stroke.
