This paper proposes the fractal patterns classifier for multiple cardiac arrhythmias on field-programmable gate array (FPGA) device. Fractal dimension transformation (FDT) is employed to adjoin the fractal features of QRS-complex, including the supraventricular ectopic beat, bundle branch ectopic beat, and ventricular ectopic beat. FDT with fractal dimension (FD) is addressed for constructing various symptomatic patterns, which can produce family functions and enhance features, making clear differences between normal and unhealthy subjects. The probabilistic neural network (PNN) is proposed for recognizing multiple cardiac arrhythmias. Numerical experiments verify the efficiency and higher accuracy with the software simulation in order to formulate the mathematical model logical circuits. FDT results in data self-similarity for the same arrhythmia category, the number of dataset requirement and PNN architecture can be reduced. Its simplified model can be easily embedded in the FPGA chip. The prototype classifier is tested using the MIT-BIH arrhythmia database, and the tests reveal its practicality for monitoring ECG signals.

The data rate of a visible light communication(VLC) system is basically determined by the electrical-to-optical(E-O) bandwidth of its light-emitting diode(LED) source. In order to break through the intrinsic limitation of the carrier recombination rate on E-O bandwidth in conventional c-plane LEDs based on In Ga N quantum wells,a blue micro-LED with an active region of nano-structured In Ga N wetting layer is designed, fabricated, and packaged to realize a high-speed VLC system. The E-O bandwidth of the micro-LED can reach up to 1.3 GHz. Based on this high-speed micro-LED, we demonstrated a data rate of 2 Gbps with a bit error rate(BER) of 1.2×10^(-3) with simple on-off keying signal for a 3-m real-time VLC. In addition, a 4-Gbps VLC system using quadrature phase shift keying-orthogonal frequency-division multiplexing with a BER of 3.2×10^(-3) is also achieved for the same scenario. Among all the point-to-point VLC systems based on a single-pixel LED,this work has the highest distance-bandwidth product of 3 GHz·m and the highest distance-rate product of 12 Gbps·m.

Background Congenital long QT syndrome （LQTS） is an ion channelopathy associated with genetic mutations. It is well known that most LQTS patients （91%） have a single mutation. The purpose of this study was to investigate the clinical characteristics of congenital LQTS patients with bigenic mutations in Taiwan, China. Methods Congenital LQTS patients were recruited consecutively at Taiwan University Hospital in Taiwan from 2003 to 2009. The diagnosis of LQTS was defined by an LQTS Schwartz score greater than 4. Mutation screening in KCNQ1, KCNH2, KCNE1, and SCN5A was performed using direct sequencing. Results Three of 16 LQTS patients （18.7%） were identified with bigenic mutations. One patient had missense mutations in KCNQ1 and KCNH2, the second in KCNQ1 and KCNE1, and the third in KCNH2 and SCN5A. The mean age at onset of LQTS for patients with bigenic mutations was （17±3） years, and all of these patients were female. Two of them experienced seizure and one presented with syncope, although one of them had a family history of syncope. The mean QTc interval was （515＋17） ms, similar to those with single mutation or SNPs （（536~74） ms, P=-0.63）. Compared to those LQTS patients with single mutation or SNPs, a significantly higher percentage of LQTS patients with bigenic mutations presented with seizure and were younger at onset of the first index event （P=0.03 and 0.001, respectively）, but lower percentage of them presented with sudden cardiac death （P=0.03）. Conclusions Although the percentage of bigenic mutations in LQTS is less than 10% in Caucasian populations, we identified 3 of 16 LQTS patients （18.7%, 95% confidence interval： 0.04-0.46） with bigenic mutations in Taiwan. However, the sevedtv of their clinical presentations was not hioher than those patients with sinele mutation or SNPs.

目的:建立二维液质联用方法确定多黏菌素E甲磺酸钠(CMS)杂质的结构及来源,进而用于药物质量控制研究。方法:一维系统:采用Acquity UPLC~?Peptide CSH C_(18)(150 mm×2.1 mm, 1.7μm)色谱柱,以磷酸盐缓冲液(7.8 g·L^(-1)磷酸二氢钠,用1 mol·L^(-1)氢氧化钠溶液调节pH至6.4)-乙腈(19∶1)为流动相A,磷酸盐缓冲液-乙腈(1∶1)为流动相B,梯度洗脱,流速0.3 mL·min^(-1),柱温30℃;二维系统:采用Acquity BEH C_(18)柱(50 mm×2.1 mm, 1.7μm)色谱柱,以甲酸铵(A)-乙腈(B)为流动相,梯度洗脱,流速0.2 mL·min^(-1),柱温40℃,检测波长210 nm。质谱检测器采用ESI源,负离子扫描模式。结果:采用2D-LC-Q TOF MS法,推定了CMS中55个杂质的结构,并推测其主要来源为多黏菌素E1-I、多黏菌素E1-7MOA、多黏菌素E3及多黏菌素E6。结论:利用二维液质联用技术推定CMS中杂质峰的结构及来源,并用来评价不同厂家、生产工艺的原料药中杂质含量变化,有利于改进生产工艺,从源头控制药物质量。

目的:针对新加坡的花岗岩地质结构,研究将典型马蹄形隧道挖掘成双拱隧道时岩石支撑的设计问题。利用离散元法分析侧边导坑对隧道主体开口的影响。创新点:1.基于平面应变假设,提出一个针对性的离散元模型以分析侧边导坑的设置对主体隧道支撑的影响;2.此模型适用于解决底下长跨度开口的支撑设计问题,如大众捷运系统隧道和紧贴式双孔隧道的建造,补充了传统经验主义的设计方法。方法:1.采用离散元法对马蹄形隧道开口支撑问题进行非连续分析;2.采用二维平面应变模型简化问题。结论:1.离散元法可用于分析有限岩石覆盖层的失效机制,单隧道支撑要求以及增加侧边导坑后的支撑要求;2.模型计算结果显示,侧边导坑的挖掘使得支柱总的螺栓连接作用力增加了一倍。

Objective: This study aims to evaluate the impact and potential prognostic roles of the pre- and post-treatment Glasgow prognostic score (GPS) and the change thereof in patients with advanced head and neck cancer undergoing concurrent chemoradiotherapy (CCRT). Methods: We collected GPS and clinicopathological data of 139 stage III, IVA, and IVB head and neck cancer patients who underwent CCRT between 2008 and 2011. Their GPSs pre- and post-CCRT and the change thereof were analyzed for correlations with recurrence and survival. Results: The GPS changed in 72 (51.8%) patients, with worse scores observed post-CCRT in 65 (90.3%) of the GPS changed patients. Patients in the improved GPS group showed a tendency toward better survival. From the multivariate analysis, the post-CCRT GPS level was an independent prognostic factor in addition to tumor stage. Conclusions: After CCRT, a high GPS was revealed to be an important predictor of survival for advanced head and neck cancer.
