Background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private sector.Methods Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in Ha Noi, Hai Phong and Ho Chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate regression.Results The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). However, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;P<0.001). This resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;P<0.001) and experiencing TB-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the public or private sector. This study examined the feasibility of using the grey wolf optimizer(GWO)and artificial neural network(ANN)to predict the compressive strength(CS)of self-compacting concrete(SCC).The ANN-GWO model was created using 115 samples from different sources,taking into account nine key SCC factors.The validation of the proposed model was evaluated via six indices,including correlation coefficient(R),mean squared error,mean absolute error(MAE),IA,Slope,and mean absolute percentage error.In addition,the importance of the parameters affecting the CS of SCC was investigated utilizing partial dependence plots.The results proved that the proposed ANN-GWO algorithm is a reliable predictor for SCC’s CS.Following that,an examination of the parameters impacting the CS of SCC was provided.

BACKGROUND Twin reversed arterial perfusion(TRAP) sequence is an extremely rare congenital anomaly in monochorionic(MC) twins.The condition is characterized by a malformed fetus(acardiac twin) without cardiac activities being perfused by a structurally normal one(pump twin) via an artery-to-artery anastomosis in a reverse direction.CASE SUMMARY We described the first case of TRAP to receive laser surgery in Vietnam.The 26-wk pregnancy was originally misdiagnosed in another hospital as MC twins with single intrauterine fetal death.Following admission to our center,the diagnosis was amended to a 26-wk TRAP sequence stage IIb.The acardiac twin was 7.5 cm at the longest length,the ratio of the weight of the acardiac twin to the weight of the pump twin was more than 90%,the pump twin showed fetal distress with absent diastolic flow in umbilical artery of pump twin,and the peak systolic velocity in the middle cerebral artery = 1.6 Mo M.We performed emergency laser photocoagulation of the acardiac twin’s umbilical cord.After surgery,we successfully maintained the pregnancy for 8 wk and ended it electively by cesarean section at 34 wk of gestation due to rupture of membranes.CONCLUSION TRAP should be appropriately diagnosed and treated early to avoid complications of the pump twin.Fetoscopic laser photocoagulation is a new and effective treatment for this condition.

Importance:Nutritional status of infants,measured by birth weight and length,is an essential factor in neonatal development.Malnutrition in newborns may lead to a higher risk of mortality,neurological and cognitive impairment,and poor language development.Objective:This study aims to assess the nutritional status of infants and related factors regarding maternal anthropometric characteristics and medical history.Methods:A cross-sectional study was conducted at the National Hospital of Obstetrics and Gynecology,Vietnam from May 2021 to May 2022 on 340 infants and mothers.Low birth weight was defined following the Intergrowth-21 standards.Stunting was evaluated using the Fenton growth chart when the length was below the 10%percentile line of the gestational week.Multivariate regression models were applied to identify factors associated with the nutritional status of infants.Results:We found that 12.4%and 14.1%of infants in our study fell into stunted and underweight categories,respectively.Infants of mothers over 35 years old,having a height lower than 150 cm or experiencing anemia during pregnancy were more likely to be stunted or have low birth weight.Serum albumin deficiency during pregnancy was strongly associated with the infant being underweight(odds ratio[OR]=2.8,95%confidence interval[95%CI]1.1–7.3).Newborns were more likely to be stunted if their mothers had a history of preterm birth(OR=3.3,95%CI 1.1–10.2).Interpretation:Maternal nutritional status is closely related to infant malnutrition,particularly in preterm infants.Improving the understanding of mothers regarding prenatal care,reproductive healthcare,adequate nutritional diet,and multi-micronutrient supplements during pregnancy is therefore important.

In this paper,we establish a new algorithm to the non-overlapping Schwarz domain decomposition methods with changing transmission conditions for solving one dimensional advection reaction diffusion problem.More precisely,we first describe the new algorithm and prove the convergence results under several natural assumptions on the sequences of parameters which determine the transmission conditions.Then we give a simple method to estimate the new value of parameters in each iteration.The interesting advantage of our method is that one may update the better parameters in each iteration to save the computational cost for optimizing the parameters after many steps.Finally some numerical experiments are performed to show the behavior of the convergence rate for the new method.

To the Editor:Recent randomized controlled trials,as well as high-quality systematic reviews and meta-analyses,strongly support the use of corticosteroids in patients with coronavirus disease 2019(COVID-19)who require oxygen support.[1]The use of pulse cor-ticosteroid therapy has been reported in case series and studies,showing significant improvement in patients and a reduction in mortality rate.[2]In this letter,we report the mortality rate among patients with severe COVID-19 receiving pulse corticos-teroids with methylprednisolone(500 mg/day)for≥3 days and≥24 h before intubation(median duration of therapy:3 days)compared with standard of care.

Background:Strongyloidiasis,a neglected disease caused by intestinal nematodes of the genus,is endemic to tropical and subtropical areas such as Vietnam.Morphological methods only identify the genus,while DNA-molecular techniques are susceptible in Strongyloides spp.detection.The study aims to determine the prevalence of dominant Strongyloides species among the population in Duc Hoa district,Long An,Vietnam.Methods:A cross-sectional study used 1190 stool specimens collected from July 2017 to November 2018.All samples were transported within 2 h,stored at 2-8℃,and processed within 48 h for microscopy smear and culture at the Laboratory of Medical Parasitology,Pham Ngoc Thach University of Medicine(PNT).Then all positive samples with the above 2 methods were verified by real-time PCR technique.Real-time PCR amplification was conducted at the Laboratory of Molecular Biology,PNT.Results:Direct microscopy and modified Harada-Mori culture detected Strongyloides spp.larvae in 79/1190 sam-ples(6.6%).About 94.2%of the DNA samples were Strongyloides stercoralis,2.9%were co-infections with Strongy-loides ratti and S.stercoralis,and 2.9%were patients with S.ratti.The identity of 12/14 sequences was confirmed as S.stercoralis with a high level of similarity(91.3%-100%)and over 98%for S.ratti.Conclusion:DNA-molecular techniques and sequence analysis are highly suitable for identifying Strongyloides species isolated from stool samples.It is remarkable evidence of the presence of zoonosis S.ratti disease in human,not just the known S.stercoralis.It is likely to result in a certain proportion of people being infected by this animal-borne infectious pathogen.

Water level predictions in the river,lake and delta play an important role in flood management.Every year Mekong River delta of Vietnam is experiencing flood due to heavy monsoon rains and high tides.Land subsidence may also aggravate flooding problems in this area.Therefore,accurate predictions of water levels in this region are very important to forewarn the people and authorities for taking timely adequate remedial measures to prevent losses of life and property.There are so many methods available to predict the water levels based on historical data but nowadays Machine Learning(ML)methods are considered the best tool for accurate prediction.In this study,we have used surface water level data of 18 water level measurement stations of the Mekong River delta from 2000 to 2018 to build novel time-series Bagging based hybrid ML models namely:Bagging(RF),Bagging(SOM)and Bagging(M5P)to predict historical water levels in the study area.Performances of the Bagging-based hybrid models were compared with Reduced Error Pruning Trees(REPT),which is a benchmark ML model.The data of 19 years period was divided into 70:30 ratio for the modeling.The data of the period 1/2000 to 5/2013(which is about 70%of total data)was used for the training and for the period 5/2013 to 12/2018(which is about 30%of total data)was used for testing(validating)the models.Performance of the models was evaluated using standard statistical measures:Coefficient of Determination(R2),Root Mean Square Error(RMSE)and Mean Absolute Error(MAE).Results show that the performance of all the developed models is good(R2>0.9)for the prediction of water levels in the study area.However,the Bagging-based hybrid models are slightly better than another model such as REPT.thus,these Bagging-based hybrid time series models can be used for predicting water levels at Mekong data.

Carboxymethyl starch (CMS) can be cross-linked by electron beam radiation to form a biocompatible and environment-friendly hydrogel at a high absorbed dose and a condensed CMS concentration. Acrylic acid (AAc) can be used as a sensitizer in order to reduce the absorbed doses to an acceptable certain level. At an absorbed dose of 3-4 kGy, the gel content of crosslinked CMS can be obtained about 50% with 5% (w/w) AAc concentration used. The compressive strength of CMS samples increased with increasing their cross-linked densities due to raising absorbed doses. The swelling ratio of cross-linked CMS was also attainable at a maximum of 50 times in the distilled water. The enzymatic degradation of cross-linked CMS was carried out in acetate buffer pH 4.6 with 0.1% α-amylase enzymatic solution incubated at 40°C for 6 h. The crosslinked CMS samples were degraded slower than uncrosslinked CMS ones. The results indicated that the highly cross-linked CMS was almost fully degradable when the enzymatic hydrolysis was performed during 6 h. The FT IR spectra of cross-linked CMS in the presence of AAc were examined to observe the carboxyl group of AAc in the structure of cross-linked CMS. The hydrophilic of cross-linked CMS surface was determined by a contact-angle analysis. © 2013, Science Press. All rights reserved.
