Edible mushrooms, known for many years and considered as functional foods, contain a wide variety of biomolecules with nutritional and medicinal properties. Pleurotus species, especially Pleurotus pulmonarius, Pleurotus floridanus and Pleurotus sajor-caju are the most commonly consumed in Cameroon. The present work aims to study the “in vitro” antiradical activity and ferric reducing antioxidant power of extracts of formulations of these three species. Mushrooms were harvested in two mycicultures from Cameroon, then dried and crushed finely to obtain powders. In well-defined proportions, ten different formulations were made and their aqueous, ethanol and hydroethanol extracts prepared. These were used to determine total phenolic content by using Folin-Ciocalteu reagent method, the antioxidant activity determined by 2,2-diphenyl-1-picrylhydrazyl radical (DPPH) scavenging activity and ferric-reducing antioxidant power (FRAP) assays, with vitamin C (ascorbic acid) as standard. The results obtained from the ten formulations, including thirty extracts, showed that all the extracts had quite significant ferric reducing power at C = 1 mg/ml, (7.35 - 26.02 mg EAA/g). They also had good percentages of DPPH radical inhibition at C = 25 mg/ml (60.40% - 77.72%) and a significant polyphenol content at C = 100 mg/ml (13.73 - 21.15 mg CE/g). The aqueous extracts had the best activities compared to the standard. In addition, a strong positive linear correlation exists between polyphenol content and the scavenging property (R2 = 0.9063;p = 0.0003) also with the ferric reducing test (R2 = 0.9253;p = 0.0001);suggesting the responsibility of these compounds for these activities. Surgical Treatment of Hemorrhoidal Disease at Brazzaville University Hospital Center

Introduction: Hemorrhoidal disease is a common pathology and its surgical treatment is based, among other things, on pedicular resection after failure of medical and instrumental treatment. The aim of this study is to analyze the epidemiological, clinical, therapeutic and evolving aspects of hemorrhoidal disease at the stage of surgical treatment at the University Hospital Center of Brazzaville. Patients and Methods: We conducted a retrospective and descriptive study carried out from January 2020 to December 2021, a 24 months period, in the Digestive surgery department of the University Hospital Center of Brazzaville. It concerned patients who underwent a surgical procedure for hemorrhoidal disease. Results: 21 cases were collected, representing a hospital frequency of 2.3%, with a sex ratio of 4.3 in favor of men. The average age of patients was 42.2 ± 11.9 years. The symptoms were mainly proctalgia, mass sensation and rectal bleeding. We recorded five cases (19.1%) of hemorrhoidal thrombosis and 16 cases (80.9%) of hemorrhoidal prolapse including 12 cases requiring manual integration (Goligher grade III) and four irreducible permanent cases (Goligher grade IV). The Grade III prolapse was associated with a polyp in one patient and with posterior anal fissure in another patient. Out of the 21 patients, 14 underwent a tripedicular hemorrhoidectomy according to Milligan and Morgan. Two patients underwent mono- and bipedicular hemorrhoidectomy with resection of associated lesions and five patients underwent emergency thrombectomy. The outcome was favorable for all our patients. The average length of hospital stay was 1.5 ± 2.1 days. Conclusion: In the event of failure of medical and instrumental treatment, the hemorrhoidal cure according to Milligan and Morgan is the surgical treatment of reference for hemorrhoidal disease at the University Hospital Center of Brazzaville.

Diarrheic Escherichia coli: A Predominant Etiological Agent of Gastroenteritis, a Case Study in Douala, Cameroon

Context: Gastroenteritis remains an infectious disease with high morbidity and mortality particularly in low incomes countries, where the capacity to search all etiological agents, especially pathogenic Escherichia coli, is very limited. We investigated the contribution of pathogenic Escherichia coli and their antibiotic resistance profiles in cases of gastroenteritis. Methods: A cross-sectional study was carried out on human stool samples from October 2021 to June 2022 at Laquintinie Hospital. Samples were received from patients of all age groups and screened for bacteriological and parasitological identification by microscopy, bacterial culture, biochemical identification, and antimicrobial susceptibility tests. Results: A total of 296 patients with gastroenteritis complaints, were enrolled in the study with ages ranging from 5 months to 90 years old (Median = 35.5;SD = 20.8). Among the samples analyzed, 1.7% (n = 5/296) were positive for parasites and 27% (n = 80/296) were positive for bacterial pathogens. Parasites were found in mono parasitism, mainly Entamoeba histolytica (60%;n = 3/5), followed by Trichomonas intestinalis (20%;n = 1/5), and Giardia intestinalis (20%;n = 1/5). Three species of bacterial pathogens were identified with no co-infection: diarrheic Escherichia coli (DEC), Salmonella sp, and Shigella sp with respective proportions of 90% (n = 72/80), 6.3% (n = 5/80), and 3.7% (n = 3/80). For antibiotic resistance profiles (ARPs) of the 72 isolates of DEC, high levels of resistance were observed globally with amoxicillin (93.1%;n = 67/72), followed by ciprofloxacin (75%;n = 54/72), and to trimethoprim + sulfamethazole (73.6%;n = 53/72). In contrast, DEC showed low resistance rates with nitrofurans (6.9%;n = 5/72) and imipenem (2.8%;n = 2/72). The strains had 56 distinct ARPs, of which 88.9% (n = 64/72) were MDR. Acute and Sub-Chronic Toxicity Evaluation of the Crude Methanolic Bark Extract of Bridelia micrantha (Hochst.) Baill. (Phyllanthaceae) and Its Fraction

Bridelia micrantha, commonly known as coastal golden leaf, is a member of the family Phyllanthaceae. In preliminary studies nine fractions, named F1 - F9, were obtained by fractionating the crude methanol extract of the stem bark of Bridelia micrantha using column chromatographic techniques. The fraction F6 was the most active when tested for antibacterial activity. Thus, toxicity of this fraction was investigated for further use. The present study evaluated the acute and sub-chronic toxicity of the crude methanolic bark extract of Bridelia micrantha and its fraction. The acute toxicity was carried out according to the experimental protocol of Organization for Economic Co-operation and Development (OECD). The plant extract or the fraction F6 was administered orally to female mice at a single dose of 2000 mg/kg and the animals were observed for any behavioral changes or mortality for 14 days. In the sub-chronic toxicity study, the extract and fraction were administered orally at 200, 400 and 800 mg/kg bw/day for 28 days to healthy Wistar rats. The general behavior and body weight of the rats were recorded daily. At the end of the experimental period, hematological and biochemical analyses, changes in vital organ weight (liver, lung, heart, spleen and kidney), and histopathological examination of the liver and kidney were performed. no mortality or adverse effects were noted at the 2000 mg/kg dose during the oral acute toxicity test. In the sub-chronic study, the crude methanolic bark extract of Bridelia micrantha and the fraction F6 induced no mortality or treatment-related adverse effects on body weight, general behavior, relative organ weights, hematological and biochemical parameters. Histopathological examination of the liver and kidney showed normal architecture suggesting no morphological alterations. Profiling and Determinants of Impaired Lipid Profile Parameters among Breast Cancer Women of Childbearing Age Living in Douala, Cameroon

Objective: Breast cancer in women of childbearing age (WCBA) is a major public health concern. This study aimed to determine variation and determinants in lipid profile among Cameroonian WCBA diagnosed with breast cancer. Materials and Methods: A case-control study took place at two reference hospitals in Douala, Cameroon. A total of 176 WCBA (88 cases and 88 controls) were finally enrolled. Interviewer-administered questionnaires were used to collect sociodemographic, behavioural, clinical and anthropometric data. Three millilitres of venous blood were collected for analysis of total cholesterol (TC), high density lipoprotein cholesterol (HDL-c), low density lipoprotein cholesterol (LDL-c) and triglycerides (TG). Results: Overweight was predominantly seen in cases and controls. Serum levels of TC and LDL-c were significantly lower in cancer women (167.02 ± 45.46 vs 183.12 ± 27.38 mg/dL, p = 0.005 for TC;85.83 ± 40.34 vs 105.25 ± 29.95 mg/dL, p = 0.0004 for LDL-c), while HDL-c levels were higher in controls (45.09 ± 7.20 vs 38.24 ± 11.14 mg/dL, p Conclusion: This study suggests the influence of breast cancer on lipid profile parameters especially HDL-c and LDL-c, and possibly their putative clinical utility for early diagnosis in premenopausal women in Cameroon.

背景：冠心病是以冠状动脉发生粥样硬化而引起管腔狭窄及闭塞为特点的疾病。其中动脉粥样斑块是动脉粥样硬化的主要病理特点,其使冠脉逐渐狭窄,从而减少冠脉血流量。动脉粥样硬化作为一种慢性炎症疾病,是以动脉粥样斑块中持续存在着大量吞噬胆固醇的巨噬细胞为特征的。这些巨噬细胞被认为是泡沫细胞的主要来源,其通过募集更多的白细胞及血管平滑肌细胞使得斑块进展。一般来说,急性炎症反应的消退过程包括单核细胞来源的炎症细胞从邻近的淋巴管迁移出炎症部位。在动脉粥样硬化中此途径的受损,在一定程度上导致了巨噬细胞中胆固醇含量的增多,从而使其转变为固着性表型的细胞。在动脉粥样斑块中大量吞噬胆固醇的巨噬细胞高表达轴突诱向因子-1(netrin-1),促进动脉粥样斑块的进展,从而加重动脉粥样硬化的现象已被体外及动物试验所证实。然而,在人体内的相关研究甚少。因此,本研究的目的在于运用Gensini评分系统来评估血浆神经生长因子-1(netrin-1)的水平与冠状动脉疾病严重程度的关系。方法：本群组由88名来自在长沙中南大学湘雅二医院心内科接受冠脉造影术的患者组成。根据冠脉造影结果患者可分成两组：冠心病组(n=59)及非冠心病组(n=29),其中冠心病组又分为3个亚组(稳定性心绞痛组(SA)=14,不稳定性心绞痛(UA)=26,急性心肌梗死(MI)=19)。冠心病患者为冠脉造影术提示至少有1支主要的冠脉(包括左主干,左前降支,左回旋支,右冠)其狭窄大于或等于50%的。而非冠心病患者则为冠脉造影术提示冠脉狭窄小于50%的。包括年龄,性别,临床表现在内的患者信息已被记录在册。外周血标本取自患者的股动脉/静脉或者桡动脉于择期冠脉造影术前或急性心肌梗死患者急诊介入术时。在采血前,患者至少禁食8小时,通过标准的实验室方法测定血常规,尿常规,血脂全套(包括总胆固醇,总甘油三酯,低密度脂蛋白胆固醇,高密度脂蛋白胆固醇)及空腹血糖。血浆神经生长因子-1(netrin-1)水平通过酶联免疫吸附试验(ELISA)双抗体夹心法测定。全部数据应用SPSS16.0版本的统计软件进行分析。通过单样本t检验方法来比较非冠心病对照组及冠心病实验组的临床数据及血浆神经生长因子-1(netrin-1)水平。应用Fisher确切检验进行各组之间类别分布的比较。通过应用方差分析中最小二乘法差异检验对多个Gensini评分组中血浆神经生长因子-1(netrin-1)浓度进行比较。在变量之间的双变量相关分析通过Pearson相关分析检验或非参数的Spearman的相关检验实现。P值(双尾)均小于0.05,被认为具有统计学意义。结果：血浆中神经生长因子-1(netrin-1)的水平相当低,对于某些患者甚至难以测出。通过与对照组的比较以及不同亚组相互之间的比较发现,其血浆神经因子-1(netrin-1)的浓度并无显著差异,而且血浆神经因子-1的浓度与性别(p=0.168)及年龄亦并无显著相关性。不稳定性心绞痛组具有最高的Gensini评分(47.38±40.45),而稳定性心绞痛组则具有最低的Gensini评分(10.28±12.12),三个亚组之间的Gensini评分有显著的差异性(p=0.01)。所有患者均进行了Gensini评分与神经生长-1(netrin-1)之间以及二者与各心脏代谢指标的相关分析。并未发现Gensini评分与神经生长-1(netrin-1)之间具有显著地相关性(r=-0.129p=0.329),也未发现各心脏代谢指标与神经生长-1(netrin-1)以及Gensini评分有显著相关性。结论：我们的研究再次证实了其他相关研究之前证实的结论：尽管在粥样斑块的形成及其不稳定性中,神经生长因子-1(netrin-1)具有重要作用,但是冠心病人群与无冠心病人群之间血浆神经生长因子-1(netrin-1)的浓度却并无显著差异性。神经生长因子-1(netrin-1)与Gensini评分及相关心脏代谢参数之间也无相关性。这一发现提示当前神经生长因子-1(netrin-1)还不能作为诊断冠心病的标志物。但是,神经生长因子-1仍是心血管领域中值得探索的物质,因为其不仅与动脉粥样斑块的形成,不稳定性及脆性相关,而且在心肌梗死后血管新生及抑制和心肌保护方面亦有广阔的探索空间。

Evaluation of Acute and Sub-Acute Oral Toxicities of Momordica foetida Schumach. (Cucurbitaceae) Leaves Methanol Extract in Wistar Rats

Momordica foetida is a plant widely used in tropical Africa to manage gastroenteric diseases. Previous studies demonstrated interesting antibacterial activity against human pathogenic bacteria. However, the security or toxicity of methanol leaf extract has not been determined yet. The aim of this study was to evaluate the acute and sub-acute toxicity of the leaf extract of Momordica foetida. In the acute toxicity study, a single oral dose of 5000 mg/kg body weight was administered to rats which were observed for 14 days in order to identify signs of toxicity or death. In the sub-acute toxicity, the animals were treated with 250, 500 and 1000 mg/kg of the extract for 28 consecutive days. Body weights and behavior were noted throughout the experiment. Upon treatment, blood and urine were collected for hematological and biochemical analysis. Liver, lungs, heart, kidneys, testes and ovaries were analyzed for relative weights and histopathology. The acute toxicity study of M. foetida leaf extract revealed no signs of toxicity related to the treatment, indicating that the median-lethal-dose (LD50) value is greater than 5000 mg/Kg of body weight. In the sub-acute toxicity assay, the extract did not affect the general behavior of animals, meanwhile, it led to a significant increase in the levels of red blood cells, platelets, hemoglobin, granulocytes and Mid-Cells (MIDs). Biochemical parameters showed an increase in total cholesterol, HDL cholesterol, serum urea, serum and urinary glucose and a decrease in urinary proteins, serum creatinine, urinary urea levels, serum activities of AST, ALT and proteins levels, as well as increases in lung, spleen and ovaries relative weight were noticed, all compared to control animals. Histological analysis revealed a normal architecture of kidneys, liver, heart, lung, ovaries and testes. Introduction: Jaundice is a major symptom during neonatal time. It is defined by a bilirubin ≥ 50 mg/l. Objectives: To determine the frequency of neonatal jaundice at the Brazzaville University Hospital, to determine the main etiologies, to evaluate the management, to specify the evolution. Patients and Method: It was a prospective and descriptive study that was conducted in the neonatology Department and Biochemistry Laboratory of the University Hospital of Brazzaville, from 1st August 2015 to 31st July 2016. It included newborn children admitted into the neonatal unit for treatment of jaundice. The variables studied were epidemiological, clinical, etiological therapy and outcome. Results: The frequency of the neonatal jaundice is 7.2%. The hyperbilirubinemia as not conjugated was found in 99.1% of cases. The main causes are bacterial neonatal infection n = 102 (47.9%), the physiological icterus n = 46 (21.6%) and the ABO incompatibility n = 40 (18.8%). The treatment consists of the phototherapy in all cases. Each phototherapy session lasts 3 hours n = 99 (56.9%), 6 hours n = 58 (33.3%) and 12 hours n = 17 (9.8%). The average duration of the administering of the therapy is of 3 ± 2 days. Hyperbilirubinemia encephalopathy occurred in 62 (29.1%) cases, including 49 cases of kernicterus. The death occurred in 70 (32.9%) children. Conclusion: The importance and the gravity of the neonatal jaundice require primary preventive means based on proper care in pregnancy and the birth. While the secondary prevention includes high index of suspicion, comprehensive diagnostic equipment availability to prevent acute bilirubin encephalopathy.

Aim: To determine etiological, therapeutic aspects and issues of AGP in intensive care unit (ICU) at University Hospital of Brazzaville (UHB). Materials and Methods: It was a retrospective and descriptive study carried out from January to December 2016 in ICU at UHB. We collected completed medical records of patients admitted and operated for AGP regardless of age or sex. The parameters studied were age, sex, admission’s reasons, etiologies, management, post-operative complications, length of hospital and mortality. Data were treated in Excel 2010 and Epi info 2007. Results: Thirty-one complete medical records were identified (mean age: 40.6 ± 22.0 years). The sex ratio was 2.4. Shock was the most common reason for admission with 67.7% of the cases. The etiologies of AGP wn>ere dominated by gastroduodenal perforating ulcer (41.9%) followed by complicated appendicitis (19.4%). The management of all patients was medico-surgical. The bi antibiotic ceftriaxone-metronidazole was administered in 29 patients (93.6%). 18 patients (59.1%) received vasopressor therapy. The complications had occurred among 9 patients i.e. 29% of the cases;parietal suppurations represented 44.5% of the complications. The average length of hospitalization was 5.2 ± 4.6 days.
