Patch-based denoising methods have recently emerged due to its good denoising performance. In this paper, based on analysis of the optimal over-complete patch aggregation, we highlight the importance of a local transform for good image features representation. A finite Radon transform （FRAT） based two-stage over-complete image denoising algorithm is then proposed for obtaining good visual quality of denoised images. Experimental results demonstrate good performance in that the denoised images obtained by the proposed method are less influenced by artifacts.

Sudden cardiac death （SCD） in children and adolescents is uncommon and yet it is devastating for both victim＇s family and the society. Recently, it was increasingly recognized that SCD in young patients with structurally normal heart may be caused by inheritable primary electrical diseases due to the malfunction of cardiac ion channels, a disease entity known as the ion channelopathies. Catecholaminergic polymorphic ventricular tachycardia （CPVT） is a specific form of ion channelopathy which can cause cardiac syncope or SCD in young patients by producing catecholamine-induced bi-directional ventricular tachycardia （BiVT）, polymorphic VT and ventricular fibrillation （VF） during physical exertion or emotion. We reported here an index case of CPVT caused by cardiac ryanodine receptor gene （hRyR2） mutation which presented as cardiac syncope and sudden cardiac arrest in a Chinese adolescent female.

Incorporating rate and state friction laws, stability of linearly stable （i.e., with stiffness greater than the critical value） spring-slider systems subjected to triggering perturbations was analyzed under variable normal stress condition, and comparison was made between our results and that of fixed normal stress cases revealed in previous studies. For systems associated with the slip law, the critical mag- nitude of rate steps for triggering unstable slips are found to have a similar pattern to the fixed normal stress case, and the critical velocity steps scale with a/（b - a） when k = kcr for both cases. The rate-step boundaries for the variable normal stress cases are revealed to be lower than the fixed normal stress case by 7 %-16 % for a relatively large ct = 0.56 with （b - a）/a ranging from 0.25 to 1, indicating easier triggering under the variable normal stress condition with rate steps. The difference between fixed and variable normal stress cases decreases when the α value is smaller. In the same slip- law-type systems, critical displacements to trigger instability are revealed to be little affected by the variable normal stress condition. When k 〉 kcr（V,）, a spring-slider system with the slowness law is much more stable than with the slip law,suggesting that the slowness law fits experimental data better when a single state variable is adopted. In stick-slip motions, the variable normal stress case has larger stress drops than the constant normal stress case. The variable normal stress has little effect on the range of slip velocity in systems associated with the slowness law, whereas systems associated with the slip law have a slowest slip velocity immensely smaller than the fixed normal stress case, by ~ 10 orders of magnitude.

Cellular deformability is a promising biomarker for evaluating the physiological state of cells in medical *** has emerged as a powerful technique for measuring cellular ***,existing microfluidic-based assays for measuring cellular deformability rely heavily on image analysis,which can limit their scalability for high-throughput ***,we develop a parallel constriction-based microfluidic flow cytometry device and an integrated computational framework(ATMQcD).The ATMQcD framework includes automatic training set generation,multiple object tracking,segmentation,and cellular deformability *** system was validated using cancer cell lines of varying metastatic potential,achieving a classification accuracy of 92.4%for invasiveness assessment and stratifying cancer cells before and after hypoxia *** ATMQcD system also demonstrated excellent performance in distinguishing cancer cells from leukocytes(accuracy=89.5%).We developed a mechanical model based on power-law rheology to quantify stiffness,which was fitted with measured data *** model evaluated metastatic potentials for multiple cancer types and mixed cell populations,even under realworld clinical *** study presents a highly robust and transferable computational framework for multiobject tracking and deformation measurement tasks in *** believe that this platform has the potential to pave the way for high-throughput analysis in clinical applications,providing a powerful tool for evaluating cellular deformability and assessing the physiological state of cells.

Congenital central hypoventilation syndrome （CCHS, OMIM #209880） is a rare autosomal dominant disorder of the autonomic nervous system （ANS） characterized by an abnormal autonomic ventilatory response to progressive hypercarbia and sustained hypoxemia. Patients typically present in the newborn period with hypoventilation or apnea asleep, awake, or both, without any associated cardiac, pulmonary, neuromuscular or brainstem lesions. Rarely, some patients may present at a later age and are diagnosed to have late onset central hypoventilation syndrome （LOCHS）.1 Other features of ANS dysfunction such as feeding difficulty due to oesophageal dysmotility, severe constipation in the absence of Hirschsprung disease, poor regulation of basal body temperature, episodes of profuse sweating, pupillary and ocular abnormalities, decreased beat-to-beat variability of heart rate, attenuated response of heart rate to exercise, abnormal fluctuation of blood pressure, decreased perception to pain, and decreased perception to anxiety may be variably present but not essential for diagnosis Furthermore, this central hypoventilation can occur as an isolated feature or in association with a number of neurocristopathies, notably Hirschsprung disease （Haddad syndrome, OMIM #209880） and tumours of the sympathetic nervous system particularly neuroblastoma, ganglioneuro- blastoma, and ganglioneuroma, which were found in 20% and 5%--10% of CCHS patients, respectively.2 Studies of genes pertinent to the early embryologic development of the neural crest cells, specifically the endothelin and the RET-GDNF signaling pathways, have recently led to the identification of PHOX2B as the major disease causing gene for CCHS.2-6 PHOX2B was mapped to chromosome 4p12 and consists of 3 exons. It encodes a highly conserved paired-like homeobox transcription factor of 314 amino acids linked to the RET-GDNF signaling

AIMTo evaluate the efficacy of 5 mL simethicone solution in decreasing gastric foam if given at least 30 min before gastroscopy. METHODSThis was a randomized, placebo controlled, endoscopist blinded study performed at Changi General Hospital. Patients were at least 21 years old, had no prior surgical resection of the upper gastrointestinal tract, and scheduled for elective diagnostic gastroscopies. The primary outcome was the total mucosal visibility score (TMVS) which was evaluated using McNally score. The sample size was calculated to be 24 per group (SD 2.4, 80% power, P t test). RESULTSFifty-four patients were randomised to receive either simethicone [1 mL liquid simethicone (100 mg) in 5 mL of water] or placebo (5 mL of water) at least 30 min before their gastroscopy. Six accredited consultants conducted the gastroscopy, and the interobserver agreement of scoring TMVS was good with a Kappa statistic of 0.73. The simethicone group had significantly better mean TMVS compared to placebo (5.78 ± SD 1.65 vs 8.89 ± SD 1.97, P CONCLUSIONWith a premedication time of at least 30 min, 5 mL simethicone can significantly decrease gastric foam, decrease the volume of additional flushes, and shorten gastroscopy time.
