Objecti.e: To evaluate the relati.n between epi.ardi.l adi.ose ti.sue (EAT) thi.kness and also peri.oronary fat assessed by Multi.etector Computed Tomography (MDCT) wi.h both calci.m score and si.ni.i.ance of coronary artery di.ease. Background: Epi.ardi.l adi.ose ti.sue (the vi.ceral fat of the heart present under the vi.ceral layer of the peri.ardi.m) has the same ori.i. of abdomi.al vi.ceral fat, whi.h i. known to be strongly related to the development of coronary artery atherosclerosi.. Multi.etector CT (MDCT) provi.es an accurate and reproduci.le quanti.i.ati.n of EAT due to i.s hi.h spati.l and temporal resoluti.n. Pati.nts and Methods: The current study i.cluded 70 pati.nts wi.h low-i.termedi.te probabi.i.y of coronary artery di.ease. All pati.nts were subjected to 256 Multi.etectors CT to assess EAT thi.kness, the mean thi.kness of the peri.oronary fat surroundi.g the three coronary arteri.s and coronary calci.m score. Also coronary CT angi.graphy was done and pati.nts were then di.i.ed i.to 3 groups accordi.g to si.ni.i.ance of coronary atherosclerosi.: Group 1: No atherosclerosi. (20 pati.nts), Group 2: Non obstructi.e atherosclerosi. (lumi.al narrowi.g less than 50% i. di.meter) (25 pati.nts), Group3: Obstructi.e atherosclerosi. (lumi.al narrowi.g ≥ 50%) (25 pati.nts). Results: The mean EAT thi.kness and the mean peri.oronary fat thi.kness were si.ni.i.antly hi.her i. pati.nts wi.h obstructi.e coronary artery di.ease (CAD) wi.h stenosi. > 50% (group 3) compared to other groups wi.h normal coronari.s or non obstructi.e (CAD). ROC curve was used to defi.e the best cut off value of the thi.kness of both EAT and peri.oronary fat i. predi.ti.g the obstructi.e CAD group whi.h was ≥7.2 and 12.6 mm for epi.ardi.l and peri.oronary fat respecti.ely. Background: Percutaneous coronary i.terventi.n i. now the best way of management of acute coronary syndrome (ACS). Contrast i.duced nephropathy i. a seri.us compli.ati.n and greatly dependent on several factors. i. i. sti.l unclear whether the vascular access mi.rates Ci. ri.k. Objecti.e: To study the i.pact of Radi.l Access (RA) compared wi.h Femoral Access (FA) on developi.g contrast-i.duced nephropathy (Ci.) i. pati.nts undergoi.g i.vasi.e management of acute coronary syndrome (ACS). Methods: Si.ty pati.nts eli.i.le for i.vasi.e management of ACS at cardi.logy department (Menoufi. Uni.ersi.y hospi.al and Nati.nal Heart i.sti.ute) were randomi.ed i.to two groups. Group i. i.cluded 30 pati.nts wi.h femoral approach and Group i.: i.cluded 30 pati.nts wi.h radi.l approach. The occurrence of Ci. esti.ated by KDi.O defi.i.i.n (absolute i.crease i. serum creati.i.e (SCr) by ≥0.5 mg/dl wi.hi. 48 hours;or i.crease i. SCr to ≥25% of baseli.e) was esti.ated i. both groups. Coronary angi.graphy was done for all pati.nts. Si.ni.i.ant coronary artery lesi.n was defi.ed as havi.g a ≥ 70% di.meter stenosi. on coronary angi.graphy. Pati.nts were di.i.ed i.to two groups based on the presence or absence of si.ni.i.ant coronary artery lesi.n i.to CAD posi.i.e (+ve) group vs. CAD negati.e (-ve) group. Objecti.e: To assess the correlati.n between the Monocyte to HDL Cholesterol rati. wi.h thrombus burden i. pati.nts wi.h Acute Coronary Syndrome. Pati.nts and Methods: 138 pati.nts wi.h ACS who were admi.ted to CCU department i. Nati.nal Heart i.sti.ute Cai.oand Menoufi. Uni.ersi.y Hospi.als, Menoufi., underwent pri.ary percutaneous coronary i.terventi.n (PCi..Angi.graphi. thrombus burden was classi.i.d based on thrombolysi. i. myocardi.l i.farcti.n (Ti.i. thrombus grades. The pati.nts were grouped i.to 2 categori.s of low thrombus burden (grades 0 - 3) (49.7%) and hi.h thrombus burden (grades 4 and 5) (50.3%). Results: On analysi. we found that the MHR was si.ni.i.antly hi.her i. the hi.h thrombus burden group compared wi.h the low thrombus (0.052 ± 0.019 vs 0.014 ± 0.008);P 0.001. Conclusi.n: Among Acute Coronary Syndrome pati.nts underwent Pri.ary PCi. MHR was i.dependent predi.tor of hi.h thrombus burden i. pati.nts wi.h Acute Coronary Syndrome.

Background: Long standi.g aorti. stenosi. leads to elevated left ventri.ular (LV) pressure and as a result LV hypertrophy and myocardi.l fi.rosi. shall i.crease. The left ventri.ular ejecti.n fracti.n (EF) usually remai.s adequately-preserved unti. advanced and late stages of aorti. stenosi.. But the preserved muscle of the LV i. only li.i.ed to a porti.n of the enti.e myocardi.m. Speckle tracki.g echocardi.graphy has proved i.s superi.ri.y to the standard two-di.ensi.nal echocardi.graphy method i. the detecti.n of Left Ventri.ular (LV) functi.n. Global Longi.udi.al strai. (GLS) i. consi.ered as the most robust myocardi.l strai. component.?Objecti.e: The ai. of the study i. to assess the early course of left ventri.ular reverse remodelli.g after Transcutaneous Aorti. Valve i.plantati.n (TAVi. i. pati.nts wi.h symptomati. severe aorti. valve stenosi..?Methods: 50 pati.nts wi.h severe symptomati. valvular aorti. stenosi. undergoi.g TAVi.as deci.ed by the heart team after comprehensi.e di.cussi.n.?Standard transthoraci. echocardi.graphy i.cludi.g Doppler analysi. was performed. 2D speckle-tracki.g strai. assessment of Global radi.l, ci.cumferenti.l and longi.udi.al strai. at parasternal mi.-ventri.ular short-axi. vi.w (at the level of papi.lary muscle) and from the api.al long-axi., two-chamber and four-chamber vi.ws wi.h a frame rate between 40 and 80 frames per second. Traci.g of endocardi.l borders was done. Pati.nts wi.h si.ni.i.ant coronary artery di.ease were fully revasculari.ed by percutaneous coronary i.terventi.n pri.r to the study and the procedure. Results: 23 (46%) pati.nts were males, whi.e 27 (54%) were females. The pati.nts’ strati.i.ati.n accordi.g to comorbi.i.i.s/associ.ted ri.k factors revealed that 54% of the pati.nts had DM, 86% were hypertensi.e, 38% had chroni. ki.ney di.ease (CKD), and 32% had a previ.us percutaneous coronary i.terventi.n (PCi..?The mean age for our study parti.i.ants ranged?from 60 to 92 years (Mean ± SD = 76.60 ± 5.96).
