The aim of this research was to assess the knowledge and practices of radiological versus non-radiological health professionals on the optimisation of radiation protection in paediatric and adult radiology in BUKAVU hospitals. To achieve this, we surveyed a convenience sample of 73 health professionals including 23 radiologists working in the hospitals surveyed to assess knowledge and level of implementation of radiation protection principles. Also, physical parameters were taken for the calculation of entry doses in paediatric and adult radiology units for comparison with the International Commission on Radiological Protection (ICRP) diagnostic reference levels. After analysis of the data, the following was found: although radiologists have sufficient knowledge of radiation protection standards, technical constraints do not allow them to observe the dose limitation principle recommended by the ICRP. This is why several radiology departments, including those of the HPGRB, the MWANZI clinic and CIRIRI hospital, have proved to be very irradiating for children. However, radiologists and non-radiologists alike do not contribute positively to the optimisation of radiation protection in the diagnostic use of X-rays. Therefore, support in the implementation of radiation protection principles and regular monitoring of the units as well as replacement of non-standard equipment is necessary to promote patient and environmental safety by optimising radiation protection.

随着社会的发展,对交通运输工具的安全、高速、及运载力的要求日益增加。高速铁路是供乘客使用的轨道交通,其运行速度比普通速度的铁路交通有着显著的提高。在安全运行的条件下,其速度可达400-450km/h。然而,横风、大雨等自然现象会对高速列车的安全行驶起到负面影响。本文旨在通过研究现代化高速运载工具,从而对经济、环境及技术的可行性的研究提供依据。高速列车的空气动力学特性不仅与其运行效益紧密相关,还与安全性,以及在极端天气条件和其他外部因素下的运行性能密切相关。一些学者研究了横风和大雨对高速列车气动力的影响,但仍然需要进一步的深入研究。本研究的目的在于：1)分析安全行驶的速度范围及其特性;2)研究大雨对高速列车的影响,加深对这项对安全至关重要的因素的认识;3)分析表面压强分布及其对几何设计和运行性能的影响;4)对数值模拟结果进行分析,并与其他研究者在相关研究中提及的概念与理论框架进行验证。在此背景下,现有的知识与研究为进一步的研究提供了基础与依据。本文涉及空气动力学、流体力学、流体动力学、数值分析、计算流体动力学和多相流分析的理论与计算,列车车身绕流考虑了横向风的作用和雨的作用。采用多相流混合模型理论、realizable k-∈湍流模型、多面体网格和标准壁面函数对各项参数进行分析。使用高端台式电脑客户端,数值模拟计算软件使用ANSYS WoRKBENCH-FLUENT,结构设计使用整合了Autodesk Inventor与Nx7.5的标准CAD平台。本研究使用了三组不同大小的网格,其单元数量分别为：1824488;2285764;和3555243。这三个有限元组的选择是根据计算和时间的限制确定的。列车的性能一部分是由它的结构决定的,还有很重要的一部分是由外部条件决定的,例如横风和雨。在研究横风影响时,选取以下速度：24m/s,20m/s,16m/s,10m/s,4m/s。参照邵雪明等人的相关研究,降雨量取60mm/h,每滴雨速度5m/s。考虑到网格设计理论与模型,使用y+壁面函数,在列车模型的重要部分,如车头和车尾,y+取30-60;考虑到计算能力的限制,列车其他部分y+选取120-200。通过研究,本文得到以下结论： ●由于数值模拟的计算时间与计算条件有限,圆柱形计算域与多面体网格对本文的数值模拟最为合适。 ·最大压强和最小压强分别分布在车头和车尾。 ·阻力、升力、侧向力、滚动力矩和偏航力矩在横风和大雨的环境下显著增大。 ·高速列车系统在大雨条件下运行是不安全的,其出轨和倾覆的可能性很高。 ●在使用CFD进行数值模拟计算时,为避免结果失真,例如出现回流和残差过大,计算区域的大小、松弛因子、以及离散方法的选择至关重要。

Preliminary Network Centric Therapy for Machine Learning Classification of Deep Brain Stimulation Status for the Treatment of Parkinson’s Disease with a Conformal Wearable and Wireless Inertial Sensor

The concept of Network Centric Therapy represents an amalgamation of wearable and wireless inertial sensor systems and machine learning with access to a Cloud computing environment. The advent of Network Centric Therapy is highly relevant to the treatment of Parkinson’s disease through deep brain stimulation. originally wearable and wireless systems for quantifying Parkinson’s disease involved the use a smartphone to quantify hand tremor. Although originally novel, the smartphone has notable issues as a wearable application for quantifying movement disorder tremor. The smartphone has evolved in a pathway that has made the smartphone progressively more cumbersome to mount about the dorsum of the hand. Furthermore, the smartphone utilizes an inertial sensor package that is not certified for medical analysis, and the trial data access a provisional Cloud computing environment through an email account. These concerns are resolved with the recent development of a conformal wearable and wireless inertial sensor system. This conformal wearable and wireless system mounts to the hand with the profile of a bandage by adhesive and accesses a secure Cloud computing environment through a segmented wireless connectivity strategy involving a smartphone and tablet. Additionally, the conformal wearable and wireless system is certified by the FDA of the United States of America for ascertaining medical grade inertial sensor data. These characteristics make the conformal wearable and wireless system uniquely suited for the quantification of Parkinson’s disease treatment through deep brain stimulation. Preliminary evaluation of the conformal wearable and wireless system is demonstrated through the differentiation of deep brain stimulation set to “on” and “off” status. Influence of CRT Glass Quantity on the Properties of Red Mud-CRT Glass Ceramics Fired at Different Temperatures

This paper reports the production of ceramics using CRT(cathode ray tube)glass and red mud by varying the amount of waste glass and firing from 700-950℃.The raw materials were characterized using chemical composition,IR(infra-red spectroscopy),XRD(X-ray diffraction),DSC-TGA(differential scanning calorimetry-thermo gravimetric analyses)and SEM(scanning electron microscopy)*** ceramic specimen was produced by mixing a constant quantity of red mud and 0%-40%of waste glass,then firing from 700-950℃.The linear shrinkage,water absorption,apparent porosity,bulk density,weight loss on ignition,flexural strength and chemical resistance test were used to evaluate the produced *** materials had good fluxing properties which were improved with *** the specimens produced had water absorption values below the maximum value of 20% specified by the norm for roofing tiles while specimens with 30%-40% CRT glass and fired at 850°C,and specimens with 20%-40% CRT glass and fired at 950℃ had water absorption values below 10%,which is the norm for ceramic *** specimens 0%-40% CRT glass fired at 950℃,specimens 5%-40% CRT glass fired at 850℃,specimens 10%-40% CRT glass fired at 750℃ and specimen with 20%CRT glass fired at 700℃,water absorption values were all lower than 17% which is ASTM C62 specifications for *** norm of flexural strength for floor tiles>25 MPa but all values were less than 25 *** ceramic shows a maximum loss of 0.10%in HNo3 against 0.06%for *** two waste materials can effectively be used to produce ceramic materials with good properties.

BACKGRoUND Identifying a potential single monogenetic disorder in healthy couples is costly due to the Assisted Reproduction facilities'current methodology for screening,which focuses on the detecting multiple genetic disorders at ***,we report the successful application of a low-cost and fast preimplantation genetic testing for monogenic/single gene defects(PGT-M)approach for detecting propionic acidemia(PA)in embryos obtained from a confirmed heterozygous propionyl-CoA carboxylase alpha subunit(PCCA)*** SUMMARY A fertile 32-years old Mexican couple with denied consanguinity sought antenatal genetic *** were suspected obligate PA carriers due to a previous deceased PA male newborn with an unknown PCCA/propionyl-CoA carboxylase beta subunit(PCCB)***-Generation Sequencing revealed a heterozygous genotype for a pathogenic PCCA variant(c.2041-1G>T,ClinVar:RCV-000802701.1;dbSNP:rs1367867218)in both *** couple requested in vitro fertilization(IVF)and PGT-M for *** IVF,12 oocytes were collected and fertilized,of which two resulted in high-quality *** biopsies and Whole Genome Amplification by a fragmentation/amplification-based method were performed and revealed that the two embryos were *** polymerase chain reaction and further Sanger sequencing of the exon-intron borders revealed a wild-type PCCA male embryo and a heterozygous c.2041-1G>T female *** embryos were transferred,resulting in a clinical pregnancy and the delivery of a healthy male newborn(38 wk,weight:4080 g,length:49 cm,APGAR 9/9).The absence of PA was confirmed by expanded newborn *** We show that using PGT-M with Whole Genome Amplification templates,coupled with IVF,can reduce the transmission of a pathogenic variant of the PCCA gene.

Introduction: The therapeutic pathway affects the patient who changes care facilities. This change is linked to factors such as financial means, geographical accessibility of care facilities, quality of care and the socio-cultural perception of the patient. In countries with limited resources such as Guinea, the therapeutic itinerary remains multiple and hampered by obstacles. The objective of this study was to describe the therapeutic itinerary of patients living with hepatitis B and C at the Donka National Hospital. Material and methods: This was a retrospective, descriptive study lasting 43 months (24 January 2017 - 27 August 2020);it focused on patients living with hepatitis B or C or viral cirrhosis. We collected sociodemographic, clinical and treatment history variables. The data were analysed with SPSS software version 21.0. Results: out of 5400 patients, the proportion of viral hepatitis B or C represented 393 patients (7.3%). The mean age of our patients was 35 ± 10 years, with extremes of 16 - 77 years. The sex ratio was 2.5. Three hundred and forty-two patients (87%) had recourse to modern medicine, mostly in private facilities (50.6%). one hundred and twenty-five patients (31.8%) had received previous treatment, half of whom (57.6%) had received inappropriate treatment. The delay in treatment was long in 45% of cases. The main reasons for delaying treatment were feeling unwell in 33.3%;not knowing to consult a specialist in 29.9% and feeling that they were being treated with useful drugs in 24.3%. Conclusion: The lack of training of general practitioners on the management of viral hepatitis and the scarcity of awareness campaigns on viral hepatitis contributed to the inadequacy of the management.

Evaluation of the Therapeutic Management of Hypercalcemia in Myeloma at the Nephrology Department of Louis Pasteur Hospital of Chartres: About 8 Cases

Introduction: Hypercalcemia is the most common metabolic complication in myeloma. The aim of this study was to evaluate the management strategy of hypercalcemia in myeloma at the nephrology department of Louis Pasteur hospital of Chartres. Patients and Methods: We carried a retrospective study of patients treated for myeloma-related hypercalcemia between January 1, 2019 to December 31, 2021. The clinical, paraclinical, therapeutic and evolutive characteristics were studied. Results: Eight patients were included in this study with a median age of 67 years [41 - 85] and a sex ratio (M/F) of 1. A quarter of patients were known with chronic kidney disease. Four patients (50%) had symptoms of hypercalcemia. Biologically, the mean hemoglobin was 9.8 ± 2.7 g/dl, all patients had an acute kidney injury with a mean creatinine level of 364.1 ± 173.3 mmol/l, a mean serum calcium of 3.42 ± 0.59 mmol/l and three quarter of patients had bone lesions. Five patients (62.5%) were rehydrated with a mean volume of saline of 2700 ± 836.7 ml/24h. Seven patients (87.5%) received biphosphonates and none received diuretics. The mean normalization time of the serum calcium was 5 days. Conclusion: Hypercalcemia is frequent in malignancy and represents a poor prognosis factor of the disease. A well-conducted therapeutic strategy allows rapid normalization.

Health Providers’ Points of View on the Impact of the Project to Strengthen the Expanded Program of Routine Vaccination from 2014 to 2018 in the Barumbu Health Zone in Kinshasa, Democratic Republic of Congo

According to the World Health organization, approximately 3 million deaths are prevented each year in the world thanks to vaccination and then, in addition, each year, it makes it possible to avoid nearly 750,000 children suffering from serious physical, mental or neurological handicaps and 1.5 million other deaths could be avoided if the vaccination coverage is improved. However, vaccination coverage is often not achieved due to various difficulties encountered by health providers in the expanded program on vaccination activities. Yet, it is important to strengthen the program. This research paper aimed to assess health providers’ points of view on the impact of the project to strengthen the routine expanded program on vaccination in the Barumbu health zone in Kinshasa, Democratic Republic of Congo (RDC). To do so, we surveyed a convenience sample of 234 health providers involved in the implementation of the project, including 210 community relays, 9 permanent nurses, 9 health committee chairpersons and 6 health authorities. A questionnaire related to the objective of the research was submitted to them. After analyzing the data, we found that the points of view of health providers are negative. The performance of the vaccination program was not great. Several reasons were given for this, including the non-involvement of the political and administrative authorities, the failure to respect the funds disbursement circuit and the non-effective financing of vaccination activities in the Barumbu health zone in Kinshasa, DRC.

Autolytic and Proteolytic Properties of Strains of<i>Lactococcus lactis</i>Isolated from Different Vegetables, Raw-Milk Cheeses and Commercial Starter Cultures

The autolysis and proteolysis are important features in the strains of L. lactis used in the manufacture of cheese. The autolytic and proteolytic activity of L. lactis has been linked with the development of flavor and texture in the cheese. on the other hand, there is a growing interest in new strains isolated from raw-milk cheeses and vegetables. These wild-strains have showed different features of industrial importance in comparison with those observed in commercial cultures. However, it still not clear if the autolytic and proteolytic properties of these wild-strains differ from the industrial strains. The objective of this work was to assess the autolytic and proteolytic activities of 21 strains of L. lactis isolated from diverse sources. The rates of autolysis and proteolysis observed in vitro were highly strain-dependent. The pH and the NaCl concentration in the media affected significantly the autolysis of L. lactis. The strains isolated from vegetable showed in general low and medium autolytic activity, whereas the strains isolated from raw-milk cheeses had medium to high autolytic activity. The strain with highest proteolytic activity was a strain isolated from corn leaves. Although still not clear how this strain acquired this pronounced characteristic.

The waste derived from Electrical and Electronic Equipment (WEEE) is of interest now worldwide, in some countries due to the rapid technological development and in others due to technological dependence through the acquisition of second hand equipment. Therefore it is necessary to implement strategies for the sustainable management of electrical and electronic equipment at the end of its useful life. This paper presents the results of an investigation that was conducted in a city located in the Northwest of Mexico, which seeks to understand the management practices of household WEEE generators. A survey was applied to a representative sample of the population with a confidence level of 90%. Six home appliances were quantified and it was found that approximately 21% of them are disposed directly as part of the household flow of Municipal Solid Waste (MSW), while 79% are storage for a latter sale, repair or reuse. The results obtained support the need to develop evaluations and to implement possible scenarios for the sustainable management of Electrical and Electronic Waste.
