In Kenya, demand for biomass energy has increased due to increasing population and urbanization and high cost of alternative energy sources. This coupled with use of inefficient production and utilization technologies, has led to increased deforestation, environmental degradation and increased health impacts. In Kenya, a number of improved technologies have been developed and promoted;however, adoption still remains low. This study sought to assess factors influencing adoption of biomass energy conservation technologies in four selected areas of Kitui County, Kenya. Data were collected using a structured questionnaire and guidelines for institutional and focus group discussions. Sampling was done through purposive and stratified random sampling. Results revealed three categories of biomass energy conservation technologies: energy saving stoves (46%), woodlots (9%) and improved charcoal conversion kilns (2%). The mean percentage adoption rate in the four study areas stood at 48% with Chuluni having the highest (51%) while Kitui West had the least (46%). Agriculture (28%), forestry (26%) and NGOs (Non-Governmental Organizations)/CBOs (Community-Based Organizations)(23%) were indicated as the main sources of information, while field days (35%), community trainings (30%) and demonstrations sites (22%) were the most preferred dissemination channels. High cost of technologies, lack of awareness, financial constraints, cultural preferences, gender aspects, lack of follow up programmes and inadequate capacity to enforce implementation of existing energy policies and regulation were indicated as key constraints to adoption of technologies. The study recommends development of a joint implementation strategy and follow up programmes that will look at the cost of technologies, dissemination channels and involvement of stakeholders’ in development and dissemination of biomass energy conservation technologies.

Enterococcus faecalis and Enterococcus faecium rank among the leading causes of nosocomial bacteremia and urinary tract infections. They often persist on hospital surfaces due to their ability to withstand adverse environmental conditions (low or high temperatures, high pH, and high salinity). The global Enterococcus faecalis-Enterococcus faecium ratio is currently shifting towards Enterococcus faecium. Enterococci present variable levels of resistance to certain families of antibiotics. This is the case for aminoglycosides, beta-lactams and cephalosporins. In 2017, WHO ranked Enterococci among priority pathogens for research and development of new antibiotics. The objective of our study was to determine the antibiotic resistance profile of Enterococcus faecalis and Enterococcus faecium isolates from urine and pleural fluid in two hospitals in Cameroon. This cross-sectional and analytic study was carried out between June to August 2023 on hospitalized and day patients in which a cytobacteriological test of urine and pleural fluid was done. The samples were inoculated on CLED Agar for urine and on Chocolate + polyvitex and blood agar (prepared from Columbia agar) for pleural fluid samples and incubated at 37℃ for 18 to 24 hours. Identification of isolates was carried out using the API 20 STREP micro gallery (Biomerieux, France) and tested for antimicrobial susceptibility. The data on socio-demographical and potential risk factors were recorded using self-administered questionnaires and data from laboratory analyses of the specimen were collected in a data capture sheet. Potential risk factors associated with the presence of Enterococci, were evaluated using the logistic regression in univariate and multivariate analysis. P value < 0.05 was considered as significant. A total of 511 patients were recruited who were predominantly females. Enterococcus spp were isolated in 27.79% of our samples with Enterococcus faecalis mostly encountered. 目的探索人工智能(AI)初筛分流在大规模糖尿病视网膜病变(DR)筛查中的应用。方法2018年5至7月在新疆维吾尔自治区喀什市和克孜勒苏柯尔克孜自治州,8005例糖尿病患者参加了DR筛查,所有患者均行免散瞳的眼底彩照检查,每眼采集2张眼底彩照(分别以视盘和黄斑为中心)。拍照完成后,首先使用AI系统对每例患者的4张眼底彩照进行判定,如均为可分级且无DR时,直接产生AI报告,并随机抽取1/3至读片中心由眼科医师组分级以作质量控制(组A)。此外,如4张眼底彩照中,任意一张图片判别为无法分级或存在DR时,该患者的所有眼底彩照均需行人工分级(组B)。从组A和组B中分别随机抽取300例患者的眼底照片,由眼科医师组作出最终判别,确定AI和人工分级判定需转诊DR(增殖前或增殖期DR,或糖尿病黄斑水肿)的准确性。结果在8005例参加筛查的糖尿病患者中,男3220例(40.2%),女4785例(59.8%),年龄(58.3±10.6)岁。经AI初筛后,AI直接产生报告5267例(65.8%),另外2738例(34.2%)需要进行人工分级。在组A中,AI和人工判定需转诊DR的准确性和特异度均为100.0%。在组B中,AI和人工判定需转诊DR的准确性分别为75.8%和90.3%,灵敏度分别为100%和79.1%。结论在大规模的DR筛查中,使用AI作为DR的初筛手段,可在不遗漏需转诊DR病例的情况下,减少约60%的图片分级工作量。

Modulation of human B lymphocyte differentiation by therapeutic immunoglobulins: from protein to mRNA levels

Several groups are investigating the mechanisms of action of therapeutic immunoglobulins (IVIg) in order to improve their use. In vitro models such as CD40-CD154 interaction are necessary to study the physiological response of human B lymphocytes to IVIg. Human B lymphocytes treated with IVIg triggers a rapid phospho-rylation (<1 h) of extracellular-regulated kinases 1 and 2 (ERK1/2), which subsequently results in increased differentiation and decreased pro-liferation. However, the modulation of human lymphocyte physiology by IVIg is a gradual and cumulative process and requires long-term experimentation. Differentiation of human B lymphocytes into Ig-secreting cells can be evaluated both at the transcription and translation levels. The secretion of immunoglobulins can be assessed using ELISA or ELISPOTS whereas expression of immunoglobulin genes can be measured using semi-quantitative or quantitative PCR methods. We hereby report a comparison of these methods to explain how contradictory observations towards IVIg effects could result from their use. Our results indicate that ELISA and ELISPOTS will provide consistent observations by opposition to real-time PCR quantification. Besides, the reliability of each of these me-thods remained dependent on the stimulation period as well as the preparation of cellular extracts or cell samples following IVIg-treatment.

In dialysis treatment, the radio-cephalic arteriovenous fistula (RCAVF) is a commonly used fistula, yet its low maturation rate remains a challenge. To enhance surgical outcomes, the relationship between stenosis-prone locations and RCAVF anastomosis angle is studied during maturation by developing two sets of RCAVF models for early (non-mature) and mature RCAVFs at five anastomosis angles. The impact of hemodynamics and wall shear stress (WSS) is examined to determine optimal anastomotic angles. Results indicate that acute angles produce more physiological WSS distributions and fewer disturbed regions, with early stenosis-prone regions located near the anastomosis that shift to the bending venous segment during remodeling. A pilot study comparing clinical and numerical results is conducted for validation.

The main reason for the early failure of radio-cephalic arteriovenous fistula (RCAVF) is non-maturity, which means that the blood flow rate in the fistula cannot increase to the expected value for dialysis. From a mechanical perspective, the vascular resistance at the artificially designed anastomosis causes an energy loss that affects blood flow rate growth and leads to early failure. This research studied how to maximize the RCAVF maturity and primary patency by controlling the energy loss rate. We theoretically analyzed and derived a model that evaluates the energy loss rate Eavf in RCAVF as a function of its blood vessel geometric parameters (GPs) for given flow rates. There was an aggregate of five controllable GPs in RCAVF: radial artery diameter (Dra), cephalic vein diameter (Dcv), blood vessel distance between artery and vein (h), anastomotic diameter (Da), and anastomotic angle (θ). Through this analysis, it was found that Eavf was inversely proportional to Dra, Dcv, Da, and θ, whereas proportional to h. Therefore, we recommended surgeons choose the vessels with large diameters, close distance, and increase the diameter and angle of the anastomosis to decrease the early failure of RCAVF. Simultaneously, we could explain the results of many clinical empiricisms with our formula. We found that increasing Dcv and θ was more significant in reducing Eavf than increasing Dra and Da.
