Psychiatric comorbidities in epilepsy:population co-occurrence,genetic correlations and causal effects

Background Psychiatric comorbidities are common in patients with epilepsy.Reasons for the co-occurrence of psychiatric conditions and epilepsy remain poorly understood.Aim We aimed to triangulate the relationship between epilepsy and psychiatric conditions to determine the extent and possible origins of these conditions.methods Using nationwide Swedish health registries,we quantified the lifetime prevalence of psychiatric disorders in patients with epilepsy.We then used summarydata from genome-wide association studies to investigate whether the identified observational associations could be attributed to a shared underlying genetic aetiology using cross-trait linkage disequilibrium score regression.Finally,we assessed the potential bidirectional relationships using two-sample mendelian randomisation.Results In a cohort of 7628495 individuals,we found that almost half of the 94435 individuals diagnosed with epilepsy were also diagnosed with a psychiatric condition in their lifetime(adjusted lifetime prevalence,44.09%;95%confidence interval(Cl)43.78%to 44.39%).We found evidence for a genetic correlation between epilepsy and some neurodevelopmental and psychiatric conditions.For example,we observed a genetic correlation between epilepsy and attention-deficit/hyperactivity disorder(r,=0.18,95%Cl 0.09 to 0.27,pmore pronounced in focal epilepsy(r=0.23,95%CI 0.09 to 0.36,pm mendelian randomisation using common genetic variants did not support bidirectional effects between epilepsy and neurodevelopmental or psychiatric conditions.Conclusions Psychiatric comorbidities are common in patients with epilepsy.Genetic correlations may partially explain some comorbidities;however,there is little evidence of a bidirectional relationship between the genetic liability of epilepsy and psychiatric conditions.These findings highlight the need to understand the role of environmental factors or rare genetic variations in the origins of psychiatric comorbidities in epilepsy.

Purpose:The study aimed to examine the reporting completeness of randomized controlled trials(RCTs)of non-pharmacological interventions following concussion.methods:We searched mEDLINE,Embase,PsycInfo,CINAHL,and Web of Science up to may 2022.Two reviewers independently screened studies and assessed reporting completeness using the Template for Intervention Description and Replication(TIDieR),Consensus on Exercise Reporting Template(CERT),and international Consensus on Therapeutic Exercise aNd Training(i-CONTENT)checklists.Additional information was sought my study authors where reporting was incomplete.Risk of bias(ROB)was assessed with the Cochrane ROB-2 Tool.RCTs examining non-pharmacological interventions following concussion.Results:We included 89 RCTs(n=53 high ROB)examining 11 different interventions for concussion:sub-symptom threshold aerobic exercise,cervicovestibular therapy,physical/cognitive rest,vision therapy,education,psychotherapy,hyperbaric oxygen therapy,transcranial magnetic stimulation,blue light therapy,osteopathic manipulation,and head/neck cooling.median scores were:TIDieR 9/12(75%;interquartile range(IQR)=5;range:5-12),CERT 17/19(89%;IQR=2;range:10-19),and i-CONTENT 6/7(86%;IQR=1;range:5-7).Percentage of studies completely reporting all items was TIDieR 35%(31/89),CERT 24%(5/21),and i-CONTENT 10%(2/21).Studies were more completely reported after publication of TIDieR(t_(87)=2.08;p=0.04)and CERT(t_(19)=2.72;p=0.01).Reporting completeness was not strongly associated with journal impact factor(TIDieR:rs=0.27;p=0.01;CERT:r_(s)=-0.44;p=0.06;i-CONTENT:r_(s)=-0.17;p=0.48)or ROB(TIDieR:rs=0.11;p=0.31;CERT:rs=0.04;p=0.86;i-CONTENT:rs=0.12;p=0.60).Conclusion:RCTs of non-pharmacological interventions following concussion demonstrate moderate to good reporting completeness,but are often missing key components,particularly modifications,motivational strategies,and qualified supervisor.Reporting completeness improved after TIDieR and CERT publication,but publication in highly cited journals and low ROB

BACKGROUND Pancreaticoduodenectomy(PD)is a technically complex operation,with a re-latively high risk for complications.The ability to rescue patients from post-PD complications is as a recognized quality measure.Tailored protocols were instituted at our low volume facility in the year 2013.AIm To document the rate of rescue from post-PD complications with tailored protocols in place as a measure of quality.mETHODS A retrospective audit was performed to collect data from patients who experienced major post-PD complications at a low volume pancreatic surgery unit in Trinidad and Tobago between January 1,2013 and June 30,2023.Stan-dardized definitions from the International Study Group of Pancreatic Surgery were used to define post-PD complications,and the modified Clavien-Dindo classification was used to classify post-PD complications.RESULTS Over the study period,113 patients at a mean age of 57.5 years(standard deviation[SD]±9.23;range:30-90;median:56)underwent PDs at this facility.major complications were recorded in 33(29.2%)patients at a mean age of 53.8 years(SD:±7.9).Twenty-nine(87.9%)patients who experienced major morbidity were salvaged after aggre-ssive treatment of their complication.Four(3.5%)died from bleeding pseudoaneurysm(1),septic shock secondary to a bile leak(1),anastomotic leak(1),and myocardial infarction(1).There was a significantly greater salvage rate in patients with American Society of Anesthesiologists scores≤2(93.3%vs 25%;P=0.0024).CONCLUSION This paper adds to the growing body of evidence that volume alone should not be used as a marker of quality for patients requiring PD.Despite low volumes at our facility,we demonstrated that 87.9%of patients were rescued from major complications.We attributed this to several factors including development of rescue protocols,the competence of the pancreatic surgery teams and continuous,and adaptive learning by the entire institution,cul-minating in the development of tailored peri-pancreatectomy protocols.

Gabonese’s estuary is an important coastal mangrove setting and soil plays a key role in mangrove carbon storage in mangrove forests. However, the spatial variation in soil organic carbon (SOC) storage remain unclear. To address this gap, determining the SOC spatial variation in Gabonese’s estuarine is essential for better understanding the global carbon cycle. The present study compared soil organic carbon between northern and southern sites in different mangrove forest, Rhizophora racemosa and Avicennia germinans. The results showed that the mean SOC stocks at 1 m depth were 256.28 ± 127.29 mgC ha−1. Among the different regions, SOC in northern zone was significantly (p p mgC ha−1) than upper layers (55.42 ± 25.37 mgC ha−1). The study highlights that low deforestation rate have led to less CO2 (705.3 mg CO2e ha−1 - 922.62 mg CO2e ha−1) emissions than most sediment carbon-rich mangroves in the world. These results highlight the influence of soil texture and mangrove forest types on the mangrove SOC stocks. The first national comparison of soil organic carbon stocks between mangroves and upland tropical forests indicated SOC stocks were two times more in mangroves soils (51.21 ± 45.00 mgC ha−1) than primary (20.33 ± 12.7 mgC ha−1), savanna and cropland (21.71 ± 15.10 mgC ha−1). We find that mangroves in this study emit lower dioxide-carbon equivalent emissions. This study highlights the importance of national inventories of soil organic carbon and can be used as a baseline on the role of mangroves in carbon sequestration and climate change mitigation but the variation in SOC stocks indicates the need for further national data.

Narrative minireview of the spatial epidemiology of substance use disorder in the United States:Who is at risk and where?

Drug overdose is the leading cause of death by injury in the United States.The incidence of substance use disorder(SUD)in the United States has increased steadily over the past two decades,becoming a major public health problem for the country.The drivers of the SUD epidemic in the United States have changed over time,characterized by an initial heroin outbreak between 1970 and 1999,followed by a painkiller outbreak,and finally by an ongoing synthetic opioid outbreak.The nature and sources of these abused substances reveal striking differences in the socioeconomic and behavioral factors that shape the drug epidemic.moreover,the geospatial distribution of the SUD epidemic is not homogeneous.The United States has specific locations where vulnerable communities at high risk of SUD are concentrated,reaffirming the multifactorial socioeconomic nature of this epidemic.A better understanding of the SUD epidemic under a spatial epidemiology framework is necessary to determine the factors that have shaped its spread and how these patterns can be used to predict new outbreaks and create effective mitigation policies.This narrative minireview summarizes the current records of the spatial distribution of the SUD epidemic in the United States across different periods,revealing some spatiotemporal patterns that have preceded the occurrence of outbreaks.By analyzing the epidemic of SUD-related deaths,we also describe the epidemic behavior in areas with high incidence of cases.Finally,we describe public health interventions that can be effective for demographic groups,and we discuss future challenges in the study and control of the SUD epidemic in the country.

monitoring changes in Annual Net Primary Productivity(ANPP)is required for reporting on UN Sustainable Development Goal(SDG)Indicator 15.3.1:the proportion of land that is degraded over the total land area.Calibrating time-series observations of ANPP to derive Water Use Efficiency(WUE;a measure of ANPP per unit of evapotranspiration)can minimize the influence of climate factors on ANPP observations and highlight the influence of non-climatic drivers of degradation such as land use changes.Comparing the ANPP and WUE time series may be useful for identifying the primary drivers of land degradation,which could be used to support the Land Degradation Neutrality objectives of the UN Convention to Com.at Desertification(UNCCD).This paper presents an algorithm for the Google Earth Engine(freely and openly available upon request-http://***/10.5281/zenodo.4429773)to calculate and compare ANPP and WUE time series for Santa Cruz,Bolivia,which has recently experienced an intensification in its land use.This code builds on the Good Practice Guidance document(ver-sion 1)for monitoring SDG Indicator 15.3.1.We use the mODIS 16-day average,250 m resolution to demonstrate that the Enhanced Vegetation Index(EVI)responds faster to changes in water avail-ability than the Normalized Difference Vegetation Index(NDVI).We also consider the relationships between ANPP and WUE.Significant and concordant trends may highlight good agricultural practices or increased resilience in ecosystem structure and productivity when they are positive or reducing resilience and functional integrity if negative.The sign and significance of the correlation between ANPP and WUE may also diverge over time.With further analysis,it may be possible to interpret this relationship in terms of the drivers of change in plant productivity and ecosystem resilience.

NmR relaxation analysis provides a unique and non-invasive probe of fluid dynamics within porous materials,and may be applied to the interpretation of a wide variety of material and interfacial characteristics.Here,we report two-dimensional^(1)H T_(1)-T_(2)relaxation correlation measurements of a range of three-carbon adsorbates(1-propanol,2-propanol and propanoic acid)imbibed within the mesoporous metal oxide gamma-alumina.Our data,acquired across field strengths of 2 mHz,12.7 mHz and 43 mHz,clearly reveal two populations in each measurement,identified as the alkyl and hydroxyl moieties of each adsorbate.These results expand the range of materials in which such functional group resolved relaxation is known to occur,and demonstrate the clear persistence of such phenomena using a range of typical benchtop NmR systems employed to study fluid-saturated porous media.

multi-objective optimization of the cathode catalyst layer micro-composition of polymer electrolyte membrane fuel cells using a multi-scale,two-phase fuel cell model and data-driven surrogates

Polymer electrolyte membrane fuel cells(PEmFCs)are considered a promising alternative to internal combustion engines in the automotive sector.Their commercialization is mainly hindered due to the cost and effectiveness of using platinum(Pt)in them.The cathode catalyst layer(CL)is considered a core component in PEmFCs,and its composition often considerably affects the cell performance(V_(cell))also PEmFC fabrication and production(C_(stack))costs.In this study,a data-driven multi-objective optimization analysis is conducted to effectively evaluate the effects of various cathode CL compositions on Vcelland Cstack.Four essential cathode CL parameters,i.e.,platinum loading(L_(Pt)),weight ratio of ionomer to carbon(wt_(I/C)),weight ratio of Pt to carbon(wt_(Pt/c)),and porosity of cathode CL(ε_(cCL)),are considered as the design variables.The simulation results of a three-dimensional,multi-scale,two-phase comprehensive PEmFC model are used to train and test two famous surrogates:multi-layer perceptron(mLP)and response surface analysis(RSA).Their accuracies are verified using root mean square error and adjusted R^(2).mLP which outperforms RSA in terms of prediction capability is then linked to a multi-objective non-dominated sorting genetic algorithmⅡ.Compared to a typical PEmFC stack,the results of the optimal study show that the single-cell voltage,Vcellis improved by 28 m V for the same stack price and the stack cost evaluated through the U.S department of energy cost model is reduced by$5.86/k W for the same stack performance.

A bentonite-humic acid(B-HA) mixture added to degraded soils may improve soil physical and hydraulic properties, due to effects such as improved soil structure and increased water and nutrient retention, but its effect on soil physicochemical and biological properties, and grain quality is largely unknown. The effect of B-HA, added at 30 mg ha^(-1), was studied at 1,3, 5 and 7 years after its addition to a degraded sandy soil in a semi-arid region of China. The addition of B-HA significantly increased water-filled pore space and soil organic carbon, especially at 3 to 5 years after its soil addition to the soil. Amending the sandy soil with B-HA also increased the content of microbial biomass(mB)-carbon,-nitrogen and-phosphorus, and the activities of urease, invertase, catalase and alkaline phosphatase. The significant effect of maize(Zea mays L.) growth stage on soil mB and enzyme activities accounted for 58 and 84% of their total variation, respectively. In comparison, B-HA accounted for 8% of the total variability for each of the same two variables. B-HA significantly enhanced soil properties and the uptake of N and P by maize in semi-arid areas. The use of B-HA product would be an effective management strategy to reclaim degraded sandy soils and foster sustainable agriculture production in northeast China and regions of the world with similar soils and climate.
