目的:评估伴牙科畏惧症(DF)的重度低龄儿童龋(S-ECC)患儿在牙科全身麻醉技术(DGA)治疗后的口腔健康相关生活质量(OHRQoL)。方法:选择2016年6月至2017年10月在广西医科大学附属口腔医院儿童牙病科接受DGA的3~5岁、伴DF的S-ECC的93名儿童为研究对象。采用学龄前儿童口腔健康影响量表(ECOHIS)评估治疗前和治疗后1个月OHRQoL的情况。结果:S-ECC儿童DGA治疗后,对儿童的影响和对家庭的影响两部分的得分及总分均低于治疗前(P<0.001)。结论:DGA技术有助改善伴DF的S-ECC儿童的OHRQoL。

本综述旨在探讨一些HIF信号通路上的重要基因,包括EGLN1和EPAS1,这些基因可以解释喜马拉雅地区世居人群所具有的与其高原适应有关的一些复杂特征。目前,全基因组扫描和深度测序等高通量技术已经证明了EPAS1和EGLN1是高原世居人群适应高原低压低氧环境的两个最重要的正选基因。世居高原人群在进化过程中出现的对EGLN1和EPAS1基因的特殊选择性变异和单倍体使得世居高原人群远离了慢性高原病等其他高原相关疾病的侵扰。未来基于EGLN1和EPAS1的遗传学和表遗传学的深入研究可能会发现更多与高原适应相关的重要生物标志物。

Antioxidant enzyme profile of two clinical isolates of Entamoeba histolytica varying in sensitivity to antiamoebic drugs

AIM To study the sensitivity and antioxidant enzyme response in two clinical isolates of Entamoeba histolytica(E. histolytica) during treatment with antiamoebic drugs,auranofin and metronidazole. METHODS E. histolytica were isolated from stool samples and maintained in Robinson's biphasic culture medium. Clinial isolates were maintained in xenic culture medium,and harvested for determination of minimum inhibitory concentrations to the two antiamoebic drugs,Metronidazole and Auranofin using microtiter plate tests. The percent survival of the two isolates were determined using the trypan blue cell count. Isolate 980 was treated with 70 μmol/L and 2 μmol/L while isolate 989 was treated with 20 μmol/L and 0.5 μmol/L of metronidazole and auranofin respectively for 24 h. Fifty thousand cells of each isolate were harvested after 24 h of treatment for analysis of the mRNA expressions of the antioxidant enzymes,thioredoxin reductase,peroxiredoxin and FeSOD using the specific primers. Cell lysate was used for determination of enzyme activity of thioredoxin reductase by measuring DTNB reduction spectrophotometrically at412 *** Minimum inhibitory concentration of the clinical isolates 980 and 989 for auranofin was 3 μmol/L and 1 μmol/L respectively while that for metronidazole was 80 μmol/L and 30 μmol/L respectively. Thioredoxin reductase,peroxiredoxin and FeSOD expression levels were significantly reduced in the isolate 980 when treated with Auranofin. Metronidazole treatment showed a down regulation of thioredoxin reductase. Though not significant both at the mRNA and the enzyme activity levels. Peroxiredoxin and FeSOD however remained unchanged. Auranofin treatment of isolate 989,showed an upregulation in expression of thioredoxin reductase while Peroxiredoxin and FeSOD did not show any change in expression. Upon treatment with metronidazole,isolate 989 showed an increase in thioredoxin reductase expression. Peroxiredoxin and FeSOD expressions however remain unchange

Overview of extended release tacrolimus in solid organ transplantation

Tacrolimus(Prograf?, Astellas Pharma Europe Ltd, Staines, United Kingdom; referred to as tacrolimusBID) is an immunosuppressive agent to prevent and treat allograft rejection in kidney transplant recipients in combination with mycophenolate mofetil, corticosteroids,with or without basiliximab induction. The drug has also been studied in liver, heart and lung transplant; however, these are currently off-label indications. An extended release tacrolimus formulation(Advagraf?, Astagraf XL?) allows for once-daily dosing, with the potential to improve adherence. Extended release tacrolimus has similar absorption, distribution, metabolism and excretion to tacrolimus-BID. Phase Ⅰ pharmacokinetic trials comparing extended release tacrolimus and tacrolimus-BID have demonstrated a decreased maximum concentration(C max) and delayed time to maximum concentration(t max) with the extended release formulation; however, AUC0-24 was comparable between formulations. Overall extended release tacrolimus has a very similar safety and efficacy profile to tacrolimus-BID. It is not recommended in the use of liver transplant patient's due to the increased risk of mortality in female recipients. There has been minimal data regarding the use of extended release tacrolimus in heart and lung transplant recipients. With the current data available for all organ groups the extended release tacrolimus should be dosed in a 1:1 fashion, the exception may be the cystic fibrosis population where their initial dose may need to be higher.

Background: Endoscopic procedures are frequently performed to rule out any disease process which eventuates with either a positive or negative outcome. Association of patient demographics, clinical features with endoscopic outcomes are of paramount importance for better understanding and practice of endoscopy. Objectives: This study aimed to determine the frequency of positive endoscopic findings in patients with gastrointestinal symptoms compared to those with negative findings. As a secondary objective, the association between relevant demographics, symptoms, laboratory investigations and procedural outcomes of the participants and positive endoscopic findings were also assessed. Methods: A retrospective analysis of all the patients who underwent esophagogastroduodenoscopy from January 2017 to December 2017 was conducted. A total of 1066 endoscopy records were retrieved and using a convenience sampling technique, relevant data were manually entered to the questionnaires. Records with incomplete or inconsistent data were disregarded as per the exclusion criteria, resulting in 1011 patient records ultimately utilized for the study. All data were entered and analyzed using IBM SPSS 23.0. Results: The study comprised a total of 1011 patients where 52.2% were females and 47.8% males while the mean age of patients was 42.16 ± 15.45. Positive endoscopic findings were observed in 88.1%, where the most common finding was gastritis (49.1%). Approximately 14.5% had positive investigation findings for HBsAg and 23.1% were seropositive for anti-HCV antibody. P-values statistically significant were for age (p = 0.044), hematemesis (p = 0.002), betel quid addiction (p = 0.044), anti-HCV antibody (p = 0.043), HCV-RNA (p = 0.041) and “H. pylori” antigen (p = 0.032). Conclusion: Both genders were almost equally included where most patients presented between the 3rd and 5th decades of life. A high incidence of gastritis was noted.
