Life style modification improves insulin resistance and liver histology in patients with non-alcoholic fatty liver disease

AIM: To study the effect of regular aerobic exercise on insulin resistance, serum aminotransferase and liver histology in nonalcoholic fatty liver disease (NAFLD) patients. METHODS: Sixty (mean age 40.0 ± 8.5 years, 75% male) NAFLD patients were included in the study. After baseline anthropometric measurement i.e., body mass index (BMI), waist circumference (WC); all patients were advised regular aerobic exercise for 30 min/d, for at least 5 d/wk and trained to achieve around 70% of his maximal heart rate. In addition, moderately energy restricted diet was advised to patients with high BMI (> 25 kg/m 2 ). Monthly follow up was done by measuring BMI, WC, aspartate aminotransferase, and alanine aminotransferase (ALT). Insulin resistance was calculated using homeostasis model assessment (HOMA) of insulin resistance (HOMA-IR) model, at baseline and after 6mo. Insulin resistance was arbitrarily considered altered when it was ≥ 2. Liver biopsy was done in a section of patients at baseline and after 6 mo. RESULTS: Seventy percent (42/60) patients were overweight or obese; 95% (57/60) had central obesity (WC > 90 cm in men, > 80 cm in women). In the 45 exercise compliant patients insulin resistance decreased from 6.4 ± 6.1 to 1.3 ± 1.0, BMI from 26.7 ± 3.3 kg/m 2 to 25.0 ± 3.3 kg/m 2 , WC from 95.7 ± 8.9 cm to 90.8 ± 7.3 cm and ALT from 84.8 ± 43.5 U/L to 41.3 ± 18.2 U/L (P < 0.01). In 15 exercise noncompliant patient's insulin resistance, BMI, WC and ALT did not show significant change at 6 mo follow up. Six of 8 patients in compliant group on repeat liver biopsy showed significant change in steatosis and necroinflammation. Nonalcoholic steatohepatitis scores improved form 5.3 ± 1.5 to 3.35 ± 1.5. The decline in insulin resistance correlated with decline in ALT (P = 0.01, r s = 0.90) and liver histology (P = 0.03, r s = 0.73). CONCLUSION: Life style modification improves insulin resistance resulting in improvement in ALT and liver histology in NAFLD patients.

We present the results obtained from detailed timing and spectral studies of a black hole candidate MAXI J1813-095 using Swift,NICER,and NuSTAR observations during its 2018 *** timing behavior of the source is mainly studied by examining NICER light curves in the 0.5−10 keV *** did not find any signature of quasi-periodic oscillations in the power density spectra of the *** carry out spectral analysis with a combined disk blackbody&power law model,and physical two-component advective flow(TCAF)*** the combined disk blackbody&power-law model,we extracted thermal and non-thermal fluxes,photon index and inner disk *** also find evidence for weak reflection in the *** have tested the physical TCAF model on a broadband spectrum from NuSTAR and Swift/*** parameters like mass accretion rates,the size of Compton clouds and the shock strength are *** result affirms that the source remained in the hard state during the entire outburst which indicates a‘failed’*** estimate the mass of the black hole as 7.4±1.5M⊙from the spectral study with the TCAF *** apply the LAOR model for the Fe K line *** this,the spin parameter of the black hole is ascertained as a^(∗)>*** inclination angle of the system is estimated to be in the range of 28°−45°from the reflection *** find the source distance to be∼6 kpc.

Roles of syndecan-1, bcl6 and p53 in diagnosis and prognostication of immunoproliferative small intestinal disease

AIM： To evaluate roles of syndecan-1, bcl6 and p53 in diagnosis and prognostication of immunoproliferative small intestinal disease （IPSID） and to study profiles of kappa （κ） and lambda （λ） light chains and IgA heavy chain. METHODS： The study consisted of 11 cases of IPSID and similar number of controls which included 11 of normal intestinal mucosa and 11 of high grade B cell lymphoma of ileum. The parameters analyzed included clinical profiles, biochemical and other laboratory investigations, radiologic and histological findings including immunohistochemistry. RESULTS： All IPSID cases had demonstrable serum IgA heavy chain and heavy mucosal plasma cell infiltration. According to Galian＇s histological staging, there were 4 patients with stage A and 7 with stage B. κ and ;λ light chains were over-expressed in 7 patients; 1 stage A patient had H pylori-positive active gastritis and eradication of H pylori led to disease remission. Stage A biopsies had higher expression for syndecan-1, while stage B had higher expression for bcl6 and p53. Syndecan-1,κ and λ light chains and IgA heavy chain showed inverse relationship with bcl6 and p53. All patients were treated with doxycycline. CHOP regime was added in 5 patients who developed frank lymphoma. Three died of the disease due to extensive organ infiltration. CONCLUSION： Certain immunomarkers like syndecan-1,κ and λ light chains and IgA heavy chain could be of much help in identifying early stage IPSID. Stage B IPSID showed higher expression for bcl6 and p53 than stage A IPSID. bcl6 and p53 expressions correlated with a more advanced disease stage and aggressive tumour behavior.

The objective of this paper is to provide a better rendition of Generation Z purchase intentions of retail products through *** study gyrated around the favorable attitude formation of Generation Z translating into intentions to purchase retail products through *** role of antecedents of attitude,namely enjoyment,credibility,and peer communication was also *** main purpose was to analyze the F-commerce pervasiveness(retail purchases through Facebook)among Generation Z in India and how could it be materialized effectively.A conceptual fac¸ade was proposed after trotting out germane and urbane *** study focused exclusively on Generation Z *** data were statistically analyzed using partial least squares structural equation *** study found the proposed conceptual model had a high prediction power of Generation Z intentions to purchase retail products through Facebook verifying the materialization of ***,credibility,and peer communication were proved to be good predictors of attitude(R^(2)=0.589)and furthermore attitude was found to be a stellar antecedent to purchase intentions(R^(2)=0.540).
