Backgr.und: Acute lower.r.spir.tor. tr.ct illness is a major.cause of hospitalization, r.spir.tor. failur., and death in childr.n wor.dwide. However. infectious agents ar. not identified in >50% of these clinical illnesses using tr.ditional methods such as r.pid antigen detection assays or.vir.l cultur.. Objective: To conduct a pilot study using r.-PCr.to deter.ine the fr.quency of r.spir.tor. vir.ses in cr.tically ill childr.n with r.spir.tor. compr.mise. Methods: Cr.tically ill childr.n admitted to the Pediatr.c Cr.tical Car. Unit dur.ng 2 winter.seasons with r.spir.tor. compr.mise or.failur. wer. pr.spectively enr.lled. r.spir.tor. tr.ct specimens (tr.cheal lavage or.nasal wash) wer. tested for.11 differ.nt vir.ses using r.altime r.-PCr. including r.spir.tor. syncytial vir.s (r.V), human metapneumovir.s (HMPV), influenza A (IAV) and B (IBV), par.influenza vir.ses (PIV) 1-3, cor.navir.ses OC43, 229E, and Nether.ands, and human r.inovir.s (Hr.). r.sults: Thir.y-two childr.n wer. enr.lled, 28 (88%) of which r.quir.d mechanical ventilation. Clinical diagnoses included br.nchiolitis, cr.up, asthma, apnea, and pneumonia. Of these patients, 28/32 (88%) tested positive for.one of eleven vir.ses by r.-PCr. Compar.d to antigen testing for.r.V, IAV, and IBV, r.-PCr.detected an additional nine infections for.an incr.ased yield of 53%. r.-PCr.also detected cor.navir.s infections in five patients, including thr.e patients with Nether.ands cor.navir.s. Co-infection with mor. than one vir.s was detected in 8/32 (25%). None of the 32 patients tested positive for.HMPV, PIV 1, or.PIV 3. Conclusions: We detected a r.spir.tor. vir.s in 88% of childr.n with r.spir.tor. failur.. r.-PCr.significantly incr.ased the identification of vir.ses compar.d to antigen testing. r.V, influenza A, and cor.navir.ses wer. the most common vir.ses detected. Our.data suggest that a vir.l etiology can be identified in the major.ty of childhood episodes of sever. r.spir.tor. disease and that r.-PCr.is a u
