Assessment of Satisfaction in the Reception and Emergency Department of a Health Center

Introduction The main objective of any healthcare establishment must be to ensure the quality of patient care and customer satisfaction. It is necessary to regularly assess patient satisfaction. The aim of this study was to assess the level of satisfaction of customers aged over 18 years attending the emergency department of the health center. methodology This was a descriptive and analytical cross-sectional study of patients aged 18 years and over, who attended the Samu Municipal emergency department between 02 and 30 May 2023. The satisfaction index was determined using the adapted 2009 SAPHORA-MCO questionnaire and the Likert satisfaction scale. Results A total of 400 patients were surveyed. The average age was 35 years, with a standard deviation of 14.7. Of those surveyed, 51% were women, 87% were educated, 50% lived in Grand Yoff and 59.5% were unemployed. Satisfaction levels linked to perception of the cost of care (72%), waiting time (64.3%), information given to patients (69.1%) and pain management (74 .5%) are fair. On the other hand, the levels of satisfaction linked to administrative procedures (82.5%), staff attitudes towards patients (84%), staff availability (86.4%), patient privacy (89.2%), general atmosphere (87.2%), staff competence (87.3%), and the effectiveness of care (89.4%) were satisfactory. The average waiting time was 38 minutes. However, 32% of patients waited less than 30 minutes and 92% less than an hour. The satisfaction index linked to administration and reception was 72.9% and 79.85%, respectively. The satisfaction index linked to the administration and technical quality of care is equal to 85.8% and 83.7%, respectively. The overall satisfaction index is equal to 80.6%;the level of satisfaction of users of the health structure is satisfactory. Conclusion Patient satisfaction is an essential part of quality care. DEXA Contribution in Bone Densitometry Evaluation on Osteoporosis in Senegal: Results of a Study Realized in African Black Populations

Osteoporosis, a public health problem is able to occur severe fractures and its prevalence increases in developing countries. Several factors involved in its pathogenesis. These factors are: Heredity, elderly, race, ethnicity, hormones, physical inactivity, sunlight exposure, vitamin D deficiency and certain drugs. In Africa, few data are available and the disease prevalence is maybe underestimated. In Senegal, measure of bone mineral density is recent. In this study, our objective was to determine DEXA patients profile and epidemiological, clinical characteristics of patients for densitometry examination. Thus, 43 cases were collected;lumbar and femoral sites were evaluated. median lumbar T-score was 0.91 and the femoral T-score was 0.16. Osteopenia was found in 35% of patients and osteoporosis in 26%. Bone Mineral Density was abnormal in 60% of cases. This study is another proof of documented reality of osteoporosis in Senegal. It determines frequency in a population at risk. For the future, it opens way for further larger studies to get better profile of the patients and prevalence osteoporosis in Senegal.

PI3K/SHIP2/PTEN pathway in cell polarity and hepatitis C virus pathogenesis

Hepatitis C virus(HCV) infects hepatocytes, polarized cells in the liver. Chronic HCV infection often leads to steatosis, fibrosis, cirrhosis and hepatocellular carcinoma, and it has been identified as the leading cause of liver transplantation worldwide. The HCV replication cycle is dependent on lipid metabolism and particularly an accumulation of lipid droplets in host cells. Phosphoinositides(PIs) are minor phospholipids enriched in different membranes and their levels are tightly regulated by specific PI kinases and phosphatases. PIs are implicated in a vast array of cellular responses that are central to morphogenesis, such as cytoskeletal changes, cytokinesis and the recruitment of downstream effectors to govern mechanisms involved in polarization and lumen formation. Important reviews of the literature identified phosphatidylinositol(Ptd Ins) 4-kinases, and their lipid products Ptd Ins(4)P, as critical regulators of the HCV life cycle. SH2-containing inositol polyphosphate 5-phosphatase(SHIP2), phosphoinositide 3-kinase(PI3K) and their lipid products Ptd Ins(3,4)P2 and Ptd Ins(3,4,5)P3, respectively, play an important role in the cell membrane and are key to the establishment of apicobasal polarity and lumen formation. In this review, we will focus on these new functions of PI3 K and SHIP2, and their deregulation by HCV, causing a disruption of apicobasal polarity, actin organization and extracellular matrix assembly. Finally we will highlight the involvement of this pathway in the event of insulin resistance and nonalcoholic fatty liver disease related to HCV infection.

Prognostic Factors for the Occurrence of Blood Exposure Accidents among Health Personnel at the Kaolack Regional Hospital (Senegal)

Caregivers are highly exposed to blood exposure accidents (BEA). The objective of our study is to determine the frequency of BEA and to identify the predictive factors for the occurrence of BEAs among caregivers at the Kaolack Regional Hospital (KRH). It is a descriptive and analytical cross-sectional study of KRH’s caregivers. The data were collected during the period from 16 to 26 April 2018. The multivariate analysis was carried out using the binary logistic regression model with a dependent variable (occurrence of blood exposure accidents) and 5 explanatory variables (sex, age, service of belonging, professional category and seniority in the profession). A total of 115 caregivers were surveyed out of 144. It is noted that 68 caregivers (59.1%) have had at least one BEA in the previous 12 months. The age of caregivers significantly reduces the risk of developing a BEA (p = 0.004, CI [0.04 - 0.21]);belonging to the age group [20 - 40 years] increases the risk of a BEA by 6.66. Sex significantly influences the occurrence of BEA with a (p = 0.013, CI [1.47 - 19.4]);men are 5 times more likely to develop BEA. The risk of a BEA occurring varies according to the professional category. Senior health technicians have a significantly 50 times lower risk of occurrence of a BEA (p = 0.007, CI [0.00 - 0.24)]. Nurses and midwives have a 5.8 lower risk of developing BEAs (p = 0.031, CI [0.03 - 0.75]). The risk of occurrence of BEAs varies according to the service of belonging;medicine service caregivers have a significantly 50-fold lower risk of developing BEAs (p = 0.004, CI [0.00 - 0.17]). The identified risk factors will be used to better guide our BEAs prevention interventions.

Petrography and Mineralogy of the Eocene Phosphate Deposit of Tobène (Ta&#239ba, Senegal)

The Tobène deposit forms, with those of ndomorDiop and KeurMor Fall, the large phosphate deposit of Ta?ba. The Tobène site has been the subject of lithostratigraphic, biostratigraphic, mineralogical, petrographic and geochemical studies. The lithostratigraphic and biostratigraphic studies dated the series from Lutetian to Bartonian composed of five lithological units, which are assembled by [1] in three formations. The mineralogical study shows three groups of minerals associated with the different phosphatic facies identified in the various sectors of Tobène: 1) characteristic minerals of sedimentological conditions;2) minerals of diagenetic origin;3) minerals of alteration. The petrographic study allowed an inventory of the constituting visible grains of the microfacies as well as the different phases of diagenetic and post-diagenetic transformations that may have affected them. It thus appears that the phosphatic ore of Tobène has undergone an extensive diagenesis (compaction, dissolution-recrystallization, epigenesis) to which a relatively intense ferruginization has been added.

From Myeloma to Plasma Cell Leukemia, Persistent Inequalities

Multiple myeloma (MM) is both a complex and heterogeneous disease. Cytogenetic and molecular abnormalities lead to resistance to treatment and transformation to plasma cell leukemia, which is defined by the presence in circulating blood of plasma cells over 2 G/L, or more than 20% of leukocytes. It is an uncommon hematological malignancy with a poor prognosis. Against this backdrop, we report an observation of multiple myeloma transformed into plasma cell leukemia diagnosed at the Hôpital Principal de Dakar (HPD) that occurred on a 64-year-old man with a history of thyroidectomy followed for multiple myeloma presenting with Salmon et Durie stage IIIA and ISS stage I. Despite a marked improvement in management strategy, myeloma remains an almost invariably incurable disease. However, the development of genetic and molecular biomarkers is necessary to improve its prognosis.

Factors Associated with Full Vaccination of Children Aged 12 to 23 Months in the Commune of Thiomby (Kaolack/Senegal)

Introduction: Data on the complete vaccination of children in rural areas and the factors associated with it are poorly known. Knowledge of these factors is necessary for the adoption of effective vaccination strategies. The aim of our study was to determine the proportion of children aged 12 to 23 months fully vaccinated before the age of 12 months in the commune of Thiomby and to identify associated factors. methods: A descriptive and analytical cross-sectional study was conducted from 15 January to 15 February 2020 in Thiomby among of children aged 12 to 23 months and their mothers/caregivers. The sampling was carried out in two-step clusters. Bivariate analysis was done with Epi-Info version 7.***.2.16. Results: The sample included 380 mothers/caregivers with children aged on average 24.7 years. Among them, 93.2% were housewives, 29.4% had attended school and 12.8% had a monthly income of more than 100,000 CFA francs. In total, 296 children were fully inoculated i.e. 77.9% of children aged 12 - 23 months had received all the appropriate vaccines by the age of 12 months. In addition, 42% of mothers and babysitters had a good level of knowledge about vaccination (benefits, side effects, etc.). The primary source of information for mothers about vaccination was the midwife, with 88 percent of women being informed through this channel. The age of mothers less than or equal to 30 years would significantly reduce (p Conclusion: Factors such as a good level of knowledge of mothers and access to information through midwives during prenatal and postnatal consultations contribute to an improvement in complete vaccination coverage among children aged 12 to 23 months.

Study of Factors Associated with Knowledge, Attitudes and Practices Regarding Rabies in the Commune of Niakhéne in Senegal, in 2022

Introduction: Rabies is a serious disease, as it is always fatal, but it can be prevented by sero-vaccination. It is a neglected tropical disease endemic in Asia and Africa. The aim of this study was to assess knowledge, attitudes and practices regarding rabies and to determine the factors associated with them among people aged 18 and over in the commune of Niakhène. methods: This was a cross-sectional, descriptive and analytical survey of subjects aged 18 and over living in the commune of Niakhène. A sample of 300 individuals was drawn from a two-stage cluster survey stratified by age and sex. Bivariate analysis was performed using association tests. Results: The mean age of respondents was 35.3 ± 16.9 years. It was noted that 67% (201) of respondents had a good knowledge of rabies. The results showed that 7.3% (22) of respondents owned a dog. Of the 278 people who did not own a dog, 78.4% (218) said they would have vaccinated their dog if they had had one. It should be noted that 83.7% (251) of respondents said they would go to a health facility if an animal bit them. None of the dog owners had vaccinated their dogs against rabies. Of the 41 people exposed to rabies, 39% went to a health facility. The age and education of the respondents had statistically significant associations with knowledge of rabies. Respondents’ age and education were statistically significantly related to whether they had vaccinated a domestic dog. The age, education and economic well-being quintile of respondents’ households had statistically significant associations with the use of a health facility in the event of being bitten or scratched by an animal vector. The education of respondents who had been bitten by an animal vector was statistically significantly associated with the use of a health facility. Introduction: metabolic neonatal adaptation is a complex phenomenon and metabolic disorders can be frequent in immature newborns or in life-threatening situations. In Low and Middle income countries (LMIC) the difficult access to some diagnostic tests makes the management of the metabolic emergencies challenging. The main objectives of this study were to assess the frequency and circumstances of occurrence and to describe the clinical picture associated with glucose, sodium and potassium disorders in neonates. Patients and methods: Our study was a retrospective and descriptive study conducted in the neonatology unit of National Children Hospital Albert Royer in Dakar (Senegal) from January 1 to December 31, 2014. Results: The prevalence of the studied metabolic disorders was 46.7%. The most common metabolic disorder noted was Hyperglycemia followed by Hyponatremia. Thermoregulation disturbances were found particularly in newborns with serum sodium disorders (hyponatremia 33.5% and hypernatremia 59.7%). Neurological signs were noted in case of blood sugar abnormalities (hypoglycemia 26.1% and hyperglycemia 29.8%). Half of the newborns with hyperglycemia (82 cases/50%) had blood sugar levels greater than or equal to 2 g/l. Hypernatremia was severe (Serum sodium> 180 mmol/l) in 12 neonates (16.7%). The main diagnoses retained were sepsis (159 cases/45.4%), prematurity (96 cases/27.4%), intrauterine growth retardation (66 cases/18.9%), malformations (63 cases/18%), perinatal asphyxia (44 cases/12.6%) and malnutrition (36 cases/10.3%). For most metabolic disorders, the correction was late and was done beyond 48 hours. On average, the correction time varied between 3 hours and 6 days. The most frequent complications were cerebral edema (12 cases), brain death (8 cases) and increased intracranial pressure (3 cases). Management of Severe Acute Malnutrition in a Senegalese Paediatric Hospital

Malnutrition is a global health problem. It is very common in developing countries where it contributes to an increase in morbidity and mortality, especially among children under five years of age. The main objective of this study was to describe the management of severe acute malnutrition (SAM) in children six months to five years of age hospitalized at Diamniadio Children’s Hospital (DCH). This was an epidemiological, retrospective, descriptive and cross-sectional study of children aged six to 59 months hospitalized at DCH for SAM. The study took place over a 12-month period (from January 1, 2018 to December 31, 2018). During the study period, 67 children were admitted for SAM, representing a hospital prevalence of 8.4%. The majority (88%) children were less than 24 months old. Reasons for consultations were dominated by diarrhea (57.5%) vomiting (38.8%). Complications related to SAM were mainly: anaemia (74.6%), infections (85%) and severe acute dehydration (40.3%). Factors associated with SAM were young age (infants under 24 months of age), poverty, the first four children of a sibling, poor behaviour of food diversification. The average length of hospitalization was 12 days. The nutritional recovery rate was 73.1% and lethality was 3%. Conclusion: The prevention of malnutrition must involve the fight against poverty, an optimal and well-diversified diet, monitoring the nutritional status of children up to five years of age.
