背景:导师评估是胃肠专科医生内镜培训的一个关键部分。本研究旨在建立和验证一个简明的、基于网络的上消化道内镜培训效果实时评估工具。方法:我们建立了针对上消化道内镜的专科医生内镜培训技能评估(SAFE-T)工具,在一个5部分的问卷中获取总结性和格式化的反馈。该工具与既往已获验证的结肠镜SAFE-T相似,是基于网络开展应用的。我们应用该工具对2018-2019学年的15名胃肠专科医生(培训期1、2、3年各5名)。为确保评估的可靠性,由一名独立评审人对不同的上消化道内镜操作技术进行评估,完成SAFE-T和内镜能力评估(ACE)两份量表。结果:20名高年资医生在研究期间对15名专科医生进行了413次SAFE-T评估。不同培训学年的专科医生平均SAFE-T评分存在显著差异,第一学年医生的评分低于第二学年(3.31 vs 4.25,P<0.001),第二学年低于第三学年(4.25 vs 4.56,P<0.001)。随着内镜操作难度的增加,平均SAFE-T评分下降,简单操作的评分高于中等操作(3.98 vs 3.39,P<0.001),中等操作高于困难操作(3.39 vs 2.84,P=0.042)。导师与独立评审人同时观察评估一项内镜操作,其SAFE-T评分一致性良好(Kappa=0.815,P=0.001)。SAFE-T评分与ACE评分高度相关(r=0.76,P=0.011)。结论:我们建立并验证了SAFE-T这种简明的、基于网络的工具,用以实时评估胃肠专科医生上消化道内镜培训效果的有效性。

Introduction One of the key objectives of gastroenterology(GI)fellowships is to teach fellows how to perform high-quality endoscopic procedures[1].The primary model for this education is the apprenticeship approach,in which fellows perform endoscopic procedures on patients under the supervision of an attending gastroenterologist[2].Critical to the success of this teaching model,faculty should deliver timely and actionable feedback based on direct observation of the fellow’s endoscopic performance[3].

Background:There is mounting evidence that regular physical activity is an important prerequisite for healthy cognitive aging.Consequently,the finding that almost one-third of the adult population does not reach the recommended level of regular physical activity calls for further public health actions.In this context,digital and home-based physical training interventions might be a promising alternative to center-based intervention programs.Thus,this systematic review aimed to summarize the current state of the literature on the effects of digital and home-based physical training interventions on adult cognitive performance.Methods:In this pre-registered systematic review(PROSPERO;ID:CRD42022320031),5 electronic databases(PubMed,Web of Science,Psyclnfo,SPORTDiscus,and Cochrane library)were searched by 2 independent researchers(FH and PT)to identify eligible studies investigating the effects of digital and home-based physical training interventions on cognitive performance in adults.The systematic literature search yielded 8258 records(extra17 records from other sources),of which 27 controlled trials were considered relevant.Two reviewers(FH and PT)independently extracted data and assessed the risk of bias using a modified version of the Tool for the assEssment of Study qualiTy and reporting in EXercise(TESTEX scale).Results:Of the 27 reviewed studies,15 reported positive effects on cognitive and motor-cognitive outcomes(i.e.,performance improvements in measures of executive functions,working memory,and choice stepping reaction test),and a considerable heterogeneity concerning study-related,population-related,and intervention-related characteristics was noticed.A more detailed analysis suggests that,in particular,interventions using online classes and technology-based exercise devices(i.e.,step-based exergames)can improve cognitive performance in healthy older adults.Approximately one-half of the reviewed studies were rated as having a high risk of bias with respect to completion adherence(≤85%)and monitoring of t

Bacterial infections and multidrug-resistant bacteria are major health burdens in wound care.Biocompatible antimicrobial agents,e.g.,ε-polylysine(ε-Pl),provide a broad spectrum of antibacterial properties and support dermal cell growth.Here,ε-Pl was incorporated into polycaprolactone(PCl)/gelatin electrospun scaffolds collected at varying rotation speeds.Then,the samples were crosslinked using dopamine hydrochloride to provide highly proliferative dressings with broad antimicrobial activity.The morphological study showed that the electrospun wound dressings were smooth,continuous,and bead-free,with a mean diameter ranging from 267±7 to 331±8 nm for all random and aligned nanofibers.The fiber alignment of the electrospun PCl/gelatin scaffolds improved their tensile strength and modulus.Moreover,nanofiber mats are highly hydro-philic,which is crucial for an efficient wound dressing.The samples also demonstrated high antimicrobial properties against common wound bacterial strains,including methicillin-resistant Staphylococcus aureus(MRSA),Staphylococcus aureus(SA),Escherichia coli(EC),Acinetobacter baumannii(AB),and Pseudomonas aeruginosa(PA).Mammalian cell prolifera-tion and morphology assays involving primary human dermal fibroblasts(hDFs)and immortalized keratinocytes(HaCaT)showed excellent biocompatibility of the electrospun mats and remarkably aligned mats.Furthermore,aligned mats showed more cell migration than randomly oriented mats,which is desirable for more efficient wound healing.Therefore,it can be concluded that aligned PCl/gelatin mats containingε-Pl are promising for potential use in wound dressings.

This process describes a novel technique for effective trapping of “Pd” metal through resins during the process development of l-valine, 2-[(2-amino-1,6-dihydro-6-oxo-9H-purin-9-yl) methoxy] ethyl ester, and mono hydrochloride known as a Valacyclovir hydrochloride (1). This technique is suitable for large-scale production of 1 and it is described here Pd metal trapping by using different resins.

Purpose: The study of the sentinel lymph node is the best technique to stage, have a prognosis and decide the adequate treatment in breast cancer. The usual technique implies studding the axillary lymph node. Our work tries to identify affected nodes in other regions apart from the axilla and its possible impact in staging and treatment. Methods: The sentinel lymph node technique was performed on 1660 patients included in an observational and multicentric study designed to observe the presence of metastatic cells in axillary and non-axillary lymph nodes. Results: In 19% of the patients the sentinel lymph node was detected in non-axillary regions. In these cases metastatic cells were more frequent which could suppose a change in the stage and/or treatment. As protective factor against non-axillary nodes involvement we found the localization of the cancer in external quadrants while youth and injecting the tracer inside the tumor were found to be risk factors. Conclusions: Detecting and studding non-axillary lymph nodes in breast cancer leads to a more precise staging of the disease which could imply a change in the optimal treatment.
