Paradoxical Low Flow Low Gradient Severe Aortic Stenosis—An Under-Recognized Entity

Some patients with severe aortic stenosis (AS), due to restrictive cardiac physiology, paradoxically have relatively low flow and low gradients across stenotic aortic valves despite preserved left ventricular (LV) systolic function. It results in symptoms and reduced quality of life and carries a high mortality. Whilst this form of severe AS, termed paradoxical low flow low gradient (pLFLG), is well reported, patients with this diagnosis experience inappropriate barriers to aortic valve replacement (AVR), the only efficacious treatment. We present the case of an 88-year-old female with 12 months of exertional dyspnoea on a background of hypothyroidism and hypercholesterolemia. Transthoracic echocardiogram (TTE) revealed LV hypertrophy, with a small LV cavity size and reduced stroke volume, yet normal systolic function. A heavily calcified aortic valve was identified with severe aortic stenosis, based on valve area, yet with incongruous mean transvalvular gradient of 25 mmHg (severe ≥ 50 mmHg). Following exclusion of other differential diagnoses, her symptoms were attributed to paradoxical LFLG severe AS. She was however declined definitive transcatheter aortic valve implantation (TAVI) due to her paradoxically low mean aortic gradient. Following further deterioration in her symptoms and supportive quantification of poor exercise performance, she was ultimately re-referred, accepted, and underwent TAVI. Following her AVR, the patient experiences significant improvement in both symptoms and quality of life after only one month. Paradoxical LFLG severe AS remains a well-documented yet under recognized disease. It carries high morbidity and mortality if untreated, yet is significantly less likely to be referred and accepted for intervention. With its prevalence expected to rise with an ageing population, this case serves as a timely reminder for clinicians to address the under recognition of important pathology.

Relative abundance of Phlebotominae sandflies with emphasis on vectors of kala-azar

Objective:To identify potential vectors of kala-azar from Northern plains of Uttar Pradesh, ***:The collections were made using sticky paper traps,CDC light-traps,and aspirators from outdoors as well as *** female sandflies were dissected and identified. Results:During January 2009 to July 2009,528 phlebotomine specimens were collected including 329 males(62.3%) and 199 females(37.7%),approximately 238(45%) of them were captured from indoor and 290(55%) from outdoor resting *** species belonging to the genera Phlebotomus(8%) and Sergentomyia(92%) were ***:Public health measures such as case detection and treatment,the control of sandflies,the conjunction elimination of infected stray dogs and health education can be effective in controlling the disease.

Design and optimization analysis of dual material gate on DG-IMOS

An impact ionization MOSFET （IMOS） is evolved for overcoming the constraint of less than 60 mV/decade sub-threshold slope （SS） of conventional MOSFET at room temperature. In this work, first, the device performance of the p-type double gate impact ionization MOSFET （DG-IMOS） is optimized by adjusting the device design parameters. The adjusted parameters are ratio of gate and intrinsic length, gate dielectric thickness and gate work function. Secondly, the DMG （dual material gate） DG-IMOS is proposed and investigated. This DMG DG-IMOS is further optimized to obtain the best possible performance parameters. Simulation results reveal that DMG DG-IMOS when compared to DG-IMOS, shows better IoN, ION/IoFF ratio, and RF parameters. Results show that by properly tuning the lengths of two materials at a ratio of 1.5 in DMG DG-IMOS, optimized perform- ance is achieved including ION/IoFF ratio of 2.87 × 10^9 A/μm with/ON as 11.87 × 10^-4 A/μm and transconductance of 1.06× 10^-3 S/μm. It is analyzed that length of drain side material should be greater than the length of source side material to attain the higher transconductance in DMG DG-IMOS.

Analysis of Genotoxicity Induced by Food Dyes in Root Meristem of Salvia hispanica L.

Food dyes are chemicals either natural or synthetic that were used by humans to give attractive colors to numerous food products. Colorful food products may cause several health-related issues to us because they may be prepared through synthetic food dyes. The color which is intended to attract people to eat is given to them by the use of some artificial food dyes such as Erythrosine, Allura red, Indigostine, Tartrazine, Sunset yellow, etc. These artificial food dyes are synthetic chemicals that can cause numerous health-related problems if they were taken in by individuals more than the limited amount. Artificial food dyes are becoming very common in use in different ways to provide an attractive and soothing color to food products such as sweets, drinks, candies, etc. The food products we consume with very good and attractive colors are not as good for our health as they appear. Here in this study, we are observing the cytological effect of two different synthetic food dyes (Brilliant blue and Sunset yellow) in the root meristem of Salvia hispanica L. (Chia plant) which is a major source of α-linolenic acid, mainly omega 3 fatty acids and dietary fibers found in the seed. For this purpose, germinated seeds of Chia are in the germinator. After the emergence of the young roots, we have given the treatment of the respective dyes of 0.5%, 1%, 1.5% & 2% respectively with a control set. After treatment, the roots were fixed in Carnoy’s fixative and preserved in alcohol for further analysis. When the slides of the treated roots were prepared and analyzed we observed that numerous cytological abnormalities such as (stickiness, fragmentation, bridge, laggards, disorientation, etc.) were there in the root cells and the level of abnormalities are increasing with increasing treatment period due to the exposure of them to respective dyes. These results are enough to conclude that the consumption of these dyes is harmful to the health of humans.

Microscopic level interaction between fusion-peptides and lipid bilayer membranes plays a crucial role in membrane fusion,a key step of viral *** this paper,we use coarse-grained molecular dynamics(CGMD)simulations to study the interaction between hemagglutinin fusion-peptides and phospholipid bilayer *** CGMD,we are able to simulate the interaction of fusion peptides with a relatively large piece of membrane for a sufficiently long time period,which is necessary for a detailed understanding of the fusion process.A conformation of the peptide with a kink at the level of phosphate group is obtained,consistent with NMR and EPR *** results show that the N-terminal segment of the peptide inserts more deeply into the membrane bilayer compared to the C-terminal segment,as observed in previous *** simulations also show that the presence of fusion peptides inside the membrane may cause bilayer thinning and lipid molecule ***,our results reveal that peptides tend to aggregate,indicating cluster formation as seen in many experiments.

Energy loss at bends in the pneumatic conveying of fly ash

An accurate estimation of the total pressure drop of a pipeline is important to the reliable design of a pneumatic conveying system. The present paper presents results from an investigation into the modelling of the pressure drop at a bend in the pneumatic conveying of fly ash. Seven existing bend models were used （in conjunction with solids friction models for horizontal and vertical straight pipes, and initial acceleration losses） to predict the total pipeline pressure drop in conveying fly ash （median particle diameter： 30 }zm; particle density： 2300 kg/m^3; loose-poured bulk density： 700 kg/m3） in three test rigs （pipelines with dimensions of 69 mm inner diameter （I.D.） × 168 m length; 105 mm I.D. × 168 m length; 69 mm I.D. × 554 m length）. A comparison of the pneumatic conveying characteristics （PCC） predicted using the seven bend models and experimental results shows that the predicted total pipeline PCC and trends depend on the choice of bend model. While some models predict trends that agree with the experimental results, other models predicted greater bend pressure drops for the dense phase of fly ash than for the dilute phase.
