Induction of ischemic tolerance in rat liver via reduced nicotinamide adenine dinucleotide phosphate oxidase in Kupffer cells

AIM: To elucidate the mechanisms of hepatocyte preconditioning by H2O2 to better understand the pathophysiology of ischemic preconditioning. METHODS: The in vitro effect of H2O2 pretreatment was investigated in rat isolated hepatocytes subjected to anoxia/reoxygenation. Cell viability was assessed with propidium iodide fluorometry. In other experiments, rat livers were excised and subjected to warm ischemia/ reperfusion in an isolated perfused liver system to determine leakage of liver enzymes. Preconditioning was performed by H2O2 perfusion, or by stopping the perfusion for 10 min followed by 10 min of reperfusion. To inhibit Kupffer cell function or reduced nicotinamide adenine dinucleotide phosphate (NADPH) oxidase, gadolinium chloride was injected prior to liver excision, or diphenyleneiodonium, an inhibitor of NADPH oxidase, was added to the perfusate, respectively. Histological detection of oxygen radical formation in Kupffer cells was performed by perfusion with nitro blue tetrazolium. RESULTS: Anoxia/reoxygenation decreased hepatocyte viability compared to the controls. Pretreatment with H2O2 did not improve such hepatocyte injury. In liver perfusion experiments, however, H2O2 preconditioning reduced warm ischemia/reperfusion injury, which wasreversed by inhibition of Kupffer cell function or NADPH oxidase. Histological examination revealed that H2O2 preconditioning induced oxygen radical formation in Kupffer cells. NADPH oxidase inhibition also reversed hepatoprotection by ischemic preconditioning. CONCLUSION: H2O2 preconditioning protects hepato- cytes against warm ischemia/reperfusion injury via NADPH oxidase in Kupffer cells, and not directly. NADPH oxidase also mediates hepatoprotection by ischemic preconditioning.

Elastography for the pancreas: Current status and future perspective

Elastography for the pancreas can be performed by either ultrasound or endoscopic ultrasound(EUS). There are two types of pancreatic elastographies based on different principles, which are strain elastography and shear wave elastography. The stiffness of tissue is estimated by measuring the grade of strain generated by external pressure in the former, whereas it is estimated by measuring propagation speed of shear wave, the transverse wave, generated by acoustic radiation impulse(ARFI) in the latter. Strain elastography is difficult to perform when the probe, the pancreas and the aorta are not located in line. Accordingly, a fine elastogram can be easily obtained in the pancreatic body but not in the pancreatic head and tail. In contrast, shear wave elastography can be easily performed in the entire pancreas because ARFI can be emitted to wherever desired. However, shear wave elastography cannot be performed by EUS to date. Recently, clinical guidelines for elastography specialized in the pancreas were published from Japanese Society of Medical Ultrasonics. The guidelines show us technical knacks of performing elastography for the pancreas.

Enzyme-instructed self-assembly of peptides containing phosphoserine to form supramolecular hydrogels as potential soft biomaterials

Enzyme-instructed self-assembly （EISA） offers a facile approach to explore the supramolecular assemblies of small molecules in cellular milieu for a variety of biomedical applications. One of the commonly used enzymes is phosphatase, but the study of the substrates of phosphatases mainly focuses on the phos- photyrosine containing peptides. In this work, we examine the EISA of phosphoserine containing small peptides for the first time by designing and synthesizing a series of precursors containing only phosphoserine or both phos- phoserine and phosphotyrosine. Conjugating a phospho- serine to the C-terminal of a well-established self- assembling peptide backbone, （naphthalene-2-1y）-acetyl- diphenylalanine （NapFF）, affords a novel hydrogelation precursor for EISA. The incorporation ofphosphotyrosine, another substrate of phosphatase, into the resulting precursor, provides one more enzymatic trigger on a single molecule, and meanwhile increases the precursors＇ propensity to aggregate after being fully dephosphorylated. Exchanging the positions of phosphorylated serine and tyrosine in the peptide backbone provides insights on how the specific molecular structures influence self-assembling behaviors of small peptides and the subsequent cellular responses. Moreover, the utilization of D-amino acids largely enhances the biostability of the peptides, thus providing a unique soft material for potential biomedical applications.

Porphyromonas gingivalis,a periodontal pathogen,impairs post-infarcted myocardium by inhibiting autophagosome–lysosome fusion

While several previous studies have indicated the link between periodontal disease (PD) and myocardial infarction (MI), theunderlying mechanisms remain unclear. Autophagy, a cellular quality control process that is activated in several diseases, includingheart failure, can be suppressed by Porphyromonas gingivalis (P.g.). However, it is uncertain whether autophagy impairment byperiodontal pathogens stimulates the development of cardiac dysfunction after MI. Thus, this study aimed to investigate therelationship between PD and the development of MI while focusing on the role of autophagy. Neonatal rat cardiomyocytes(NRCMs) and MI model mice were inoculated with wild-type P.g. or gingipain-deficient P.g. to assess the effect of autophagyinhibition by P.g. Wild-type P.g.-inoculated NRCMs had lower cell viability than those inoculated with gingipain-deficient P.g. Thisstudy also revealed that gingipains can cleave vesicle-associated membrane protein 8 (VAMP8), a protein involved in lysosomalsensitive factor attachment protein receptors (SNAREs), at the 47th lysine residue, thereby inhibiting autophagy. Wild-type P.g.-inoculated MI model mice were more susceptible to cardiac rupture, with lower survival rates and autophagy activity thangingipain-deficient P.g.-inoculated MI model mice. After inoculating genetically modified MI model mice (VAMP8-K47A) with wildtype P.g., they exhibited significantly increased autophagy activation compared with the MI model mice inoculated with wild-typeP.g., which suppressed cardiac rupture and enhanced overall survival rates. These findings suggest that gingipains, which arevirulence factors of P.g., impair the infarcted myocardium by cleaving VAMP8 and disrupting autophagy. This study confirms thestrong association between PD and MI and provides new insights into the potential role of autophagy in this relationship.

Study of the Conditions Surrounding Fatigue That Are Common to Medical Professionals Working in Emergency and Critical Care Centers in Japan

This study aimed to clarify the conditions surrounding fatigue that are common to medical professionals working in emergency and critical care centers in Japan. Semi-structured interviews were conducted with eight professionals ranging from doctors, nurses and pharmacists to clinical engineering technologists and radiation technologists. Their narratives were analyzed using the qualitative descriptive approach to determine fatigue common to all professionals and the reasons behind it. The five categories that emerged as the reasons for fatigue common to the subjects were [playing one’s role in treatment and procedures for emergency and critical patients], [accommodating the patient’s background and coming to terms with the outcome], [difficulties in liaising with other professionals], [feeling pressure as a responsible professional in emergency care] and [loss of sense of time caused by variable working hours]. The results revealed that fatigue common to all of the subjects was related to dealing with patients, coordinating with other professionals, having professional responsibilities and the working environment. This study suggests that arrangements to improve the working environment, ensure adequate staffing, and provide mental health support for the well-being of medical professionals working in emergency and critical care centers are necessary.

Background: The aim of this study was to investigate Japanese students and employed adults’ characteristic features, and sexual situation, and to find the correlated factors of lesbian, gay, bisexual, transgender (LGBT) recognition in order to obtain suggestions for improved relationships with sexual minorities in a diversifying society. Methods: This quantitative cross-sectional descriptive correlational study occurred during 2018 to 2019 when 2210 questionnaires were distributed to a purposive sample of eligible participants. A total of 1534 completed questionnaires (69.4%) were returned, of which 1439 (65.1%) were suitable for analyses. The survey included respondents’ 1) characteristics, 2) sexual situation, and 3) LGBT recognition scale. Data were analyzed with descriptive statistics, factor analysis, reliability analysis, and structural equation modeling. SPSS ver. 26.0 (SPSS, Chicago, IL, USA) was used for data analysis at a 5% significance level. Results: The LGBT recognition score was significantly affected by suffering from gender identity (standardized partial regression coefficient: β = 0.17, p −0.19, p Conclusions: Suffering from gender identity, learning about LGBT experience, experience of disclosure from LGBT person, sexual problem with intimate relationship, and diversity penetration had a positive effect, whereas age had negative effects on the LGBT recognition score.

Predictors of Mothers’ Early Postpartum Fatigue: A Cross-Sectional Study

Background: Most postpartum mothers experience fatigue. However, it was unclear which day after birth during hospitalization mothers are the most fatigued, and if the mothers’ background is a factor. The aim of this study was to investigate hospitalized mothers’ characteristic features, fatigue, and relaxation, and then to find the correlated factors of fatigue in order to obtain suggestions for mothers’ early postpartum care. Methods: This quantitative cross-sectional descriptive correlational study occurred during May to December 2016. A purposive sample of early postpartum hospitalized mothers at a postpartum unit in a Tokyo hospital completed an anonymous self-filled questionnaire survey. The survey included respondents’ characteristics, fatigue scale, and relaxation scale. Data were analyzed with descriptive statistics, independent t-test, one-way analysis of variance, multiple regression analyses, and structural equation modeling. Results: Returned questionnaires were 251 (83.4%). Fatigue was significantly higher for mothers with longer postpartum days compared to shorter postpartum days (F = 5.7, p β = −0.30, p β = 0.26, p β = 0.14, p Conclusions: No relaxation, longer postpartum days, and previous disease were associated with Fatigue for early postpartum mothers. These results suggest that focusing on previous disease, length of postpartum days, and mother’s relaxation is needed to provide full support to early postpartum mothers.

Efficacy of direct-acting antiviral treatment for chronic hepatitis C: A single hospital experience

AIM To evaluate the efficacy of direct-acting antivirals(DAAs) in Kanto Rosai Hospital. METHODS All patients with hepatitis C virus(HCV) who underwent DAA prescription were enrolled in this study. The present study was a single center retrospective analysis using patients infected with HCV genotype 1 or 2. Resistance analysis was performed by using direct sequencing and cycleave PCR in genotype 1 patients treated with interferon(IFN)-free DAA. The primary endpoint was sustained virologic response at 12 wk after therapy(SVR12).RESULTS A total of 117 patients participated in the study, including 135 with genotype 1 and 42 with genotype 2. Of the 135 patients with genotype 1, 16 received protease inhibitor + IFN + ribavirin and all achieved SVR. Of the 119 patients who received IFN-free DAA(in different combinations), 102 achieved SVR and 9 failed(7/9 were on daclatasvir/asunaprevir and 2/9 on ledipasvir/sofosbuvir). Efficacy analysis was done only for 43 patients who received daclatasvir/asunaprevir. From this analysis, Y93 resistance-associated substitutions were significantly correlated with *** The SVR rate was 98% for genotype 1 and 100% for genotype 2. However, caution is needed for HCV NS5 A resistance-associated substitutions that are selected by HCV NS5 A inhibitors because cerebrovascular adverse events are induced by some DAA drugs.

The leaf extract of Mallotus japonicus and its major active constituent, rutin,suppressed on melanin production in murine B16F1 melanoma

Objective: To find anti-melanogenesis materials used in whitening ***: The ethanolic leaf extract of Mallotus japonicus(M. japonicus) having an anti-melanogenesis activity was separated by a sephadex LH-20 chromatography. Each fraction was measured for its tyrosinase inhibitory activity together with its polyphenol content using the Folin–Ciocalteu method. The anti-melanogenesis activity of the active fractions was determined by the melanin content in the murine B16F1 melanoma. The active fractions were put together due to similar constituents, and then separated by high performance liquid chromatography using a C-18 ODS column. The major antimelanogenesis compound was identified using1 H and13C-NMR and liquid chromatography-mass ***: The ethanolic leaf extract of M. japonicus showed an anti-tyrosinase activity with a high polyphenol content, resulting in suppression of melanin production in the B16F1 melanoma. The extract was separated and the active compound was identical as rutin based on the1 H,13C-NMR and liquid chromatography-mass spectrometry analysis data. In addition, the rutin treatment with cells reduced the melanin content in a concentration dependent manner without any cell toxicity. The leaf extract of M. japonicus containing rutin would be useful in whitening cosmetics for protection from UV-light exposure to be limiting the accumulation of melanin in ***: The leaf extract of M. japonicus and/or rutin isolated from the extract as a key whitening agent would be useful as a whitening cosmetic material for protecting against disorder skin due to melanin accumulation.
