Clinical and laboratory features of juvenile idiopathic arthritis with wrist involvement:Results of a retrospective cohort study

BACKGROUND Previous studies in the pre-biological era showed an association of wrist inflammation in juvenile idiopathic arthritis(JIA)with progressive disease course,polyarticular involvement and failure of methotrexate *** To describe features of JIA,associated with wrist *** Data from about 753 JIA patients were included in this retrospective cohort *** clinical and laboratory features of patients with and without wrist involvement were *** Wrist involvement was found in oligoarthritis(5.8%),RF(−)/RF(+)polyarthritis(44.9%/15.0%),enthesitis-related arthritis(17.7%),and systemic(58.6%)JIA *** wrist involvement was typical for oligoarthritis patients,bilateral involvement was either equal to that of unilateral involvement or was more frequent in other *** arthritis was found to be associated with female sex,a low incidence of uveitis,and more indications of systemic inflammation,including elevated levels of C-reactive protein,erythrocyte sedimentation rate,and platelets,as well as involvement of the cervical spine,temporomandibular,shoulder,elbow,metacarpophalangeal,proximal interphalangeal,distal interphalangeal,hip,ankle,and tarsus *** number of patients with hip osteoarthritis and hip replacement was also *** arthritis was associated with a lower probability of achieving remission[hazard ratio(HR)=1.3(95%CI:1.0-1.7),P=0.055],and a higher probability of being treated with biologics[HR=1.7(95%CI:1.3-2.10,P=0.00009)].CONCLUsION Wrist arthritis in JIA patients is a marker of a severe disease course,characterized by more intensive inflammation,unfavorable outcomes,*** more intensive treatment with early administration of *** monitoring of wrist inflammation with ultrasound and MR assessment with early biological treatment might improve the outcomes.

Efficacy of Mometasone Furoate Nasal spray in Delayed Antibiotic Prescriptions for Acute Rhinosinusitis: A Multicenter, Randomized, Open-Label, Comparative, Prospective, Parallel Group study

While their clinical presentation is similar, post-viral rhinosinusitis does not indicate the possible development of bacterial rhinosinusitis which occurs in only 0.5% to 5% of all cases of ARs. However, unreasonable antibacterial therapy is prescribed in 54% to 77% of cases. The perspective is to use delayed antibacterial therapy management. The purpose of the study was to investigate the efficacy of mometasone furoate nasal spray within the delayed prescription approach. Methods: A multicenter, randomized, open-label, comparative, prospective, parallel group study enrolled a total of 156 patients with ARs aged 18 to 60 years, of them 155 patients completed the study. The patients were randomized into the main group of Mometasone Furoate (Flix) Nasal spray or into the control group of amoxicillin clavulanate tablets while both groups were administered a standard ARs therapy. Findings: The use of MF nasal spray within the delayed antibiotics prescription approach has a significantly higher efficacy compared to the immediate antibiotics prescription in the first days of treatment (p s prescribed to 42.3% of patients in the main group compared to 100.0% of patients in the control group (p s shown that there were statistically significant differences in the dynamics of changes in the total symptom score between the patients administered antibiotics and the antibiotic-naive patients. Conclusion: The use of MF within the delayed antibiotics prescription approach reliably improves the pattern of symptoms over time at the disease onset and significantly reduces the need for NsAIDs administration and antibiotics prescription frequency.

Background: The Marburg virus (MARv) is the causative agent of Marburg virus disease (MvD). This filovirus first appeared in 1967 and has since caused several outbreaks with case fatality rates between 23% and 90%. The earliest cases of MvD are thought to be caused by exposure to an infected animal, either a reservoir host (some bat species, e.g., Rousettus aegyptiacus) or a spill-over host, such as non-human primates. The virus is spread between people by direct contact with blood or other bodily fluids (including saliva, sweat, faeces, urine, tears, and breast milk) from infected individuals. Despite the high fatality rate, the Marburg virus has no vaccine or drug treatment. Recent outbreaks of the virus in 2023 in Tanzania and Equatorial Guinea have reignited the need to develop effective therapeutics, especially in the wake of the COvID-19 pandemic. Purpose: This review seeks to highlight the drug discovery efforts aimed at developing vaccines or possible treatments as potential therapeutics. several existing antiviral agents are being probed, and vaccines are in pre-clinical and clinical stages. Natural products are also an important source of possible drugs or lead compounds and when coupled with computational techniques, these strategies offer possible therapeutics for the Marburg virus, especially in Africa, which has a high disease burden. Methods: Using the search engines Google scholar and PubMed;keywords e.g. Marburg virus, Marburg treatments, Marburg virus drug discovery were utilized. several results were yielded, and articles published in recent years were accepted into the final *** and Conclusion: This study shows there is a growing interest in therapeutics for the Marburg virus, especially with the recent outbreaks and pandemic preparedness. Initiatives that to support vaccine development and access like the MARvAC consort time are critical to fighting this public health threat.

Electric field distributions along helium plasma jets

Results of simulations of streamers propagating along the atmospheric-pressure helium plasma jets ejected from a thin tube into ambient air are presented. The computational model includes the equations governing the helium-air mixture composition along and across the jet, as well as the equations, describing streamer propagation in the mixture, for the electric field and the charged species densities. streamer velocity and spatial-temporal profiles of plasma parameters are obtained for various values of the helium flow rate. It is shown, in agreement with available experimental data, that the electric field values in the heads of streamers increase nearly linearly with the distance from the tube exit and that the slope of this dependence decreases with the growth of the helium flow rate.
