Diagnostic performance of serum interleukin-6 in pediatric acute appendicitis:a systematic review

Background Despite the radiological and analytical advances of the last decades,there is still a significant rate of diagnostic error in pediatric acute appendicitis.In recent years,multiple biomarkers have emerged as potential diagnostic tools.This study aimed to examine the diagnostic performance of serum interleukin-6 in pediatric acute appendicitis.Methods We conducted a systematic review of the literature that involved an extensive search in the main databases of medical bibliography(Medline,PubMed,Web of Science and SciELO).Two independent reviewers selected the relevant articles based on the previously defined inclusion and exclusion criteria.Methodological quality of the selected article was rated using the QUADAS2 index.Data extraction was performed by two independent reviewers.Results The research in the medical bibliography databases resulted in 68 articles.We removed 26 duplicates.Among the remaining 42 articles,we excluded 33 following the inclusion and exclusion criteria.Of the final 9 studies included in this review,8 provided measured serum interleukin-6 values,and all of them reported significant differences between groups,but inconsistent results regarding sensitivity and specificity.Conclusions The diagnostic performance of interleukine-6 alone for the diagnosis of acute appendicitis in children is limited.The sensitivity and specificity of interleukine-6 for the diagnosis of non-complicated acute appendicitis in the pediatric population are moderate but increased in complicated appendicitis.There seems to be a direct relationship between serum level of interleukin-6 and the hours of evolution of abdominal pain in children with acute appendicitis.The increasingly widespread use of non-operative management of acute appendicitis warrants further exploration of the classificatory potential of this marker between complicated and uncomplicated appendicitis.We consider that this may be an avenue of research to explore in the future.

Currently,Salmonella spp.is the bacterium causing the highest number of food-borne diseases(FADs)in the world.It is primarily associated with contaminated water used to that irrigates crops from intensive livestock farming.However,literature emphasizes that the reservoirs for Salmonella spp.remain in wildlife and there are unconventional sources or secondary reservoirs,such as soil.Human soil-borne diseases have not been modeled in spatial scenarios,and therefore it is necessary to consider soil and other climatic factors to anticipate the emergence of new strains or serotypes with potential threat to public and animal health.The objective of this research was to investigate whether edaphic and climatic factors are associated with the occurrence and prevalence of Salmonella spp.in Northwestern Mexico.We estimated the potential distribution of Salmonella spp.with an interpolation method of unsampled kriging areas for 15 environmental variables,considering that these factors have a seasonal dynamic of change during the year and modifications in longer periods.Subsequently,a database was generated with human salmonellosis cases reported in the epidemiological bulletins of the National System of Epidemiological Surveillance(SIVE).For the Northwest region,there were 30,595 human cases of paratyphoid and other salmonellosis reported have been reported in Baja California state,71,462 in Chihuahua,and 16,247 in Sonora from 2002 to 2019.The highest prevalence was identified in areas with higher temperatures between 35 and 37℃,and precipitation greater than 1000 mm.The edaphic variables limited the prevalence and geographical distribution of Salmonella spp.,because the region is characterized by presenting a low percentage of organic matter(≤4.3),and most of the territory is classified as aridic and xeric,which implies that the humidity comprises≤180 days a year.Finally,the seasonal time series indicated that in the states of Baja California and Chihuahua the rainy quarter of the year is 18.7%and 17.01%above a typical quar

Effects on physiologic measures of appetite from intragastric balloon and endoscopic sleeve gastroplasty: results of a prospective study

Background: Endoscopic bariatric therapies can help address widening management gaps in obesity. Their ability to facilitate weight loss is largely tied to influences on appetite through perturbations of gastric emptying and accommodation. As these tools gain traction in obesity therapy, their physiologic underpinnings require exploration, which may enhance efficacy, tolerance, and patient-tailored care.Methods: We prospectively assessed consecutive subjects with fluid-filled intragastric balloons (IGBs) (n = 18) placed between October 2016 and June 2017 or underwent endoscopic sleeve gastroplasty (ESG) (n = 23) from March 2018 to June 2018. Patients underwent physiologic appraisal at 3 months with13C-spirulina-based gastric emptying breath test to determine time to half emptying (T50), as well as maximum tolerated volume (MTV) of a standard nutrient drink test. Changes in T50 and MTV at 3 months were compared with percent total body weight loss (%TBWL) at 3 and 6 months using best-fit linear regression.Results: The change in T50 at 3 months correlated with %TBWL at 3 months for IGB (P = 0.01) and ESG (P = 0.01) but with greater impact on %TBWL in IGB compared to ESG (R2 = 0.42vs. 0.26). Change in T50 at 3 months was predictive of weight loss at 6 months for IGB (P = 0.01) but not ESG (P = 0.11). ESG was associated with greater decrease in MTV compared to IGB (340.25 ± 297.97 mLvs. 183.00 ± 217.13 mL,P = 0.08), indicting an enhanced effect on satiation through decreased gastric accommodation. Changes in MTV at 3 months did not correlate with %TBWL for either IGB (P = 0.26) or ESG (P = 0.49) but trended toward significance for predicting %TBWL at 6 months for ESG (P = 0.06) but not IGB (P = 0.19).Conclusion: IGB and ESG both induce weight loss but likely through distinct gastric motor function phenotypes, and gastric emptying may predict future weight loss in patients with IGB.

COVID-19,a disease caused by the novel coronavirus SARS-Co V-2,has produced a serious emergency for global public health,placing enormous stress on national health systems in many countries.Several studies suggest that cytokine storms(interleukins)may play an important role in severe cases of COVID-19.Neutralizing key inflammatory factors in cytokine release syndrome(CRS)could therefore be of great value in reducing the mortality rate.Tocilizumab(TCZ)in its intravenous(IV)form of administration-Ro Actemra?20 mg/m L(Roche)-is indicated for treatment of severe CRS patients.Preliminary investigations have concluded that inhibition of IL-6 with TCZ appears to be efficacious and safe,with several ongoing clinical trials.This has led to a huge increase in demand for IV TCZ for treating severe COVID-19 patients in hospitals,which has resulted in drug shortages.Here,we present a comparability study assessing the main critical physicochemical attributes of TCZ solutions used for infusion,at 6 mg/m L and 4 mg/m L,prepared from Ro Actemra?20 mg/m L(IV form)and from Ro Actemra?162 mg(0.9 m L solution pre-filled syringe,subcutaneous(SC)form),to evaluate the use of the latter for preparing clinical solutions required for IV administration,so that in a situation of shortage of the IV medicine,the SC form could be used to prepare the solutions for IV delivery of TCZ.It is important to remember that during the current pandemic all the medicines are used off-label,since none of them has yet been approved for the treatment of COVID-19.

Mixing Efficiency across Rayleigh-Taylor and Richtmeyer-Meshkov Fronts

Mixing generated by gravitational acceleration and the role of local turbulence measured through multifractal methods is examined in numerical experiments of Rayleigh-Taylor and Richtmyer-Meshkov driven front occurring at density interfaces. The global advance of the fronts is compared with laboratory experiments and Nusselt and Sherwood numbers are calculated in both large eddy simulation (LES) and kinematic simulation KS models. In this experimental method, the mixing processes are generated by the evolution of a discrete set of forced turbulent plumes. We describe the corresponding qualitative results and the quantitative conclusions based on measures of the density field and of the height of the fluid layers. We present an experimental analysis to characterize the partial mixing process. The conclusions of this analysis are related to the mixing efficiency and the height of the final mixed layer as functions of the Atwood number, which ranges from 9.8 × 10−3 to 1.34 × 10−1.

BACKGROUND Suicide is a leading cause of preventable death worldwide,with its peak of maximum incidence in later life.Depression often puts an individual at higher risk for suicidal behaviour.In turn,depression deserves particular interest in old age due to its high prevalence and dramatic impact on health and wellbeing.AIM To gather integrated evidence on the potential risk factors for suicide behaviour development in depressive older adults,and to examine the effects of depression treatment to tackle suicide behaviour in this population.METHODS A systematic review of empirical studies,published from 2000 onwards,was conducted.Suicidal behaviour was addressed considering its varying forms(i.e.,wish to die,ideation,attempt,and completed suicide).RESULTS Thirty-five papers were selected for review,comprising both clinical and epidemiological studies.Most of studies focused on suicidal ideation(60%).The studies consistently pointed out that the risk was related to depressive episode severity,psychiatric comorbidity(anxiety or substance use disorders),poorer health status,and loss of functionality.Reduced social support and loneliness were also associated with suicide behaviour in depressive older adults.Finally,the intervention studies showed that suicidal behaviour was a robust predictor of depression treatment response.Reductions in suicidal ideation were moderated by reductions in risk factors for suicide symptoms.CONCLUSION To sum up,common and age-specific risk factors seem to be involved in suicide development in depressive older adults.A major effort should be made to tackle this serious public health concern so as to promote older people to age healthily and well.

Multiobjective optimization and multivariable control of the beer fermentation process with the use of evolutionary algorithms

This paper describes empirical research on the model, optimization and supervisory control of beer fermentation.Conditions in the laboratory were made as similar as possible to brewery industry conditions. Since mathematical models that consider realistic industrial conditions were not available, a new mathematical model design involving industrial conditions was first developed. Batch fermentations are multiobjective dynamic processes that must be guided along optimal paths to obtain good results.The paper describes a direct way to apply a Pareto set approach with multiobjective evolutionary algorithms (MOEAs).Successful finding of optimal ways to drive these processes were reported.Once obtained, the mathematical fermentation model was used to optimize the fermentation process by using an intelligent control based on certain rules.

Enhanced Ferromagnetism in ZnO Nanoribbons and Clusters Passivated with Sulfur

Inspired by recent experimental results,the electronic and magnetic properties of sulfur-passivated ZnO clusters and zigzag nanoribbons have been studied using fi rst principles calculations in the framework of the local spin density approximation.In the case of the ZnO nanoribbons,the sulfur atoms or thiol groups were attached in different ways to the zinc or oxygen atoms located at the edges,whereas in clusters,the sulfur atoms were set on the surface,mainly interacting with atoms with low-coordinate number.After an exhaustive atomic relaxation,we found that a magnetic moment emerges in zigzag nanoribbons both with and without sulfur-passivation on the edges.However,the magnitude of the magnetic moment is very sensitive to sulfur passivation.In particular,we found that when sulfur is attached to the zinc atoms in an alternating fashion along the ribbon edges,the magnetic moment is a maximum(1.4μB/unit cell).In the case of clusters,we found that the Zn_(15)O_(15) cluster exhibits a high spin moment of 5.5μB when capped with sulfur atoms.Our calculations indicate that sulfur-passivating of ZnO nanosystems could be responsible for recently observed ferromagnetic responses.

Environmental Issues of a Marine Protected Area in a Tectonic Estuary in the Tropical Eastern Pacific:Uramba(Malaga Bay Colombia):Context,Biodiversity,Threats and Challenges

The environmental protection of tropical marine and coastal areas faces different challenges due to the diversity of aspects related to these areas, which include natural, social and economical issues. Despite these challenges, efforts for the protection of these areas are urgent nowadays because of the dramatical increase of human related threats like habitat destruction and population growth. Malaga Bay (MB) is a tectonic estuarine system located in Panama Bight (central region of the Colombian Pacific coast), which due to its origin shows important environmental characteristics of few sites in the Tropical Eastern Pacific (TEP) match. For these reasons, the government of Colombia declared in 2010, the bay’s marine area as a Marine National Natural Park in order to preserve its unique estuarine marine biodiversity. Despite this measure, MB presents several conditions that make environmental protection a very difficult task. In this paper, we present the geographical context, biodiversity and natural resources, environmental threats, the complexity of economic and social context, and the institutional and legal context of MB, to exemplify the difficulty that the protection of marine areas face in the TEP.
