Changes in insomnia severity with advanced PAP therapy in patients with posttraumatic stress symptoms and comorbid sleep apnea:A retrospective,nonrandomized controlled study

Background:Sleep disorders frequently occur in posttraumatic stress disorder(PTSd)*** insomnia is a common feature of and criteria for the diagnosis of *** sleep disorder,obstructive sleep apnea(OSA),also occurs frequently in PTSd,and emerging research indicates OSA fuels chronic *** research has investigated the impact of OSA treatment on insomnia outcomes(Insomnia Severity Index,ISI)in trauma ***:OSA patients with moderately severe posttraumatic stress symptoms were studied in a retrospective chart ***-six patients who failed CPAP therapy due to expiratory pressure intolerance or complex sleep apnea or both underwent manual titration with advanced PAP modes[autobilevel(ABPAP);adaptive servo-ventilation(ASV)],which were subsequently *** use measured by objective data downloads divided the sample into three groups:compliant regular users(C-RU):n=68;subthreshold users(SC-RU):n=12;and noncompliant users(NC-MU):n=*** average follow-up was 11.89±12.22 *** and posttreatment ISI scores were analyzed to assess residual insomnia symptoms as well as cure ***:The C-RU group showed significant improvements in insomnia with very large effects compared to those in the NC-MU reference group(P=0.019).Insomnia severity significantly decreased in all three groups with large effects(C-RU,P=0.001;SC-RU,P=0.027;NC-MU,P=0.007).Hours of weekly PAP use and insomnia severity were inversely correlated(P=0.001,r=–0.321).However,residual insomnia symptoms based on established ISI cut-offs were quite common,even among the C-RU *** hoc analysis showed that several categories of sedating medications reported at baseline(hypnotics,anti-epileptic,opiates)as well as actual use of any sedating medication(prescription or nonprescription)were associated with smaller insomnia improvements than those in patients not using any sedating ***:In a retrospective,nonrandomized analysis of a selec

Hepatocellular carcinoma(HCC) is the 3^(rd) leading cause of cancer-related death worldwide. More than 80% of HCCs arise within chronic liver disease resulting from viral hepatitis, alcohol, hemochromatosis, obesity and metabolic syndrome or genotoxins. Projections based on Western lifestyle and its metabolic consequences anticipate a further increase in incidence, despite recent breakthroughs in the management of viral hepatitis. HCCs display high heterogeneity of molecular phenotypes, which challenges clinical management. However, emerging molecular classifications of HCCs have not yet formed a unified corpus translatable to the clinical practice. Thus, patient management is currently based upon tumor number, size, vascular invasion, performance status and functional liver reserve. Nonetheless, an impressive body of molecular evidence emerged within the last 20 years and is becoming increasingly available to medical practitioners and researchers in the form of repositories. Therefore, the aim this work is to review molecular data underlying HCC classifications and to organize this corpus into the major dimensions explaining HCC phenotypic diversity. Major efforts have been recently made worldwide toward a unifying "clinically-friendly" molecular landscape. As a result, a consensus emerges on three major dimensions explaining the HCC heterogeneity. In the first dimension, tumor cell proliferation and differentiation enabled allocation of HCCs to two major classes presenting profoundly different clinical aggressiveness. In the second dimension, HCC microenvironment and tumor immunity underlie recent therapeutic breakthroughs prolonging patients' survival. In the third dimension,metabolic reprogramming, with the recent emergence of subclass-specific metabolic profiles, may lead to adaptive and combined therapeutic approaches. during the last decade, transient receptor potential （TRP） channels emerge as key proteins in central mechanisms of the carcinogenesis such as cell proliferation, apoptosis and migration. Initial studies showed that expression profile of some TRP channels, notably TRP melastatin 8 （TRPM8）, TRP vanilloid 6 （TRPV6）,TRP canonical （TRPC6） and TRPV2, is changing during the development and the progression of prostate cancer towards the hormone-refractory stages. The link between the change in expression levels and the functional role of these channels in prostate cancer is step by step being elucidated. These recent advances are here described and discussed.

Cognitive rehabilitation is a potential and promising treatment for cognitive impairment in Parkinson＇s disease （Pd） that has shown efficacy in diverse studies. In addition, some few studies have found brain changes after cognitive rehabilitation in Pd, which supports the existence of brain plasticity associated to cognitive training in a degen- erative disease.

Characterization and modeling of the temperature-dependent thermal conductivity in sintered porous silicon-aluminum nanomaterials

Nanostructured silicon and silicon-aluminum compounds are synthesized by a novel synthesis strategy based on spark plasma sintering(SPS)of silicon nanopowder,mesoporous silicon(pSi),and aluminum *** interplay of metal-assisted crystallization and inherent porosity is exploited to largely suppress thermal *** and temperaturedependent thermal conductivity studies allow us to elucidate the impact of porosity and nanostructure on the macroscopic heat *** electron microscopy along with quantitative image analysis is applied to characterize the sample morphology in terms of domain size and interpore distance *** demonstrate that nanostructured domains and high porosity can be maintained in densified mesoporous silicon *** contrast,strong grain growth is observed for sintered nanopowders under similar sintering *** observe that aluminum agglomerations induce local grain growth,while aluminum diffusion is observed in porous silicon and dispersed nanoparticles.A detailed analysis of the measured thermal conductivity between 300 and 773 K allows us to distinguish the effect of reduced thermal conductivity caused by porosity from the reduction induced by phonon scattering at nanosized *** a modified Landauer/Lundstrom approach the relative thermal conductivity and the scattering length are *** relative thermal conductivity confirms the applicability of Kirkpatrick’s effective medium *** extracted scattering lengths are in excellent agreement with the harmonic mean of log-normal distributed domain sizes and the interpore distances combined by Matthiessen’s rule.

The significance of U-Pb zircon ages in zoned plutons:the case of the Flamenco pluton, Coastal Range batholith, northern Chile

Compositionally zoned plutons, both layered and concentrically arranged, provide granitic exposures where the mechanisms and timing of the magmatic emplacement processes can be studied. The importance of in-situ geochemical differentiation and the magma replenishment rates are revealed by geochemistry and field relations, together with the increasingly accurate U-Pb geochronology, which has promoted the knowledge about the pluton incremental assembly *** Flamenco pluton, located in the Coastal Range of northern Chile, is part of the Upper Triassic to Early Cretaceous Andean intrusives formed in the western active margin of South America, and present a normal zoned structure with mafic magmatic facies(mostly gabbros and Qtz-diorites) close to the contacts with the host metasediments, and tonalites, granodiorites and granites in the inner areas. A combined study of the field relations, geochemistry and zircon geochronology of the magmatic facies was applied to determine the emplacement sequence of the Flamenco pluton, revealing three distinguishable domains separated by metasedimentary septa. The SW area is constituted by mostly homogeneous leucocratic granodiorites that yielded an age of 213 Ma as the best estimation for their emplacement age. distinctive geochemical characteristics, such as the absence of an Eu anomaly, the depletion in HREE, or the highest Sr, Sr/Y and Ce/Yb values among the granodioritic facies of the pluton,involve lower T and/or higher P conditions at the magmatic source according to experimental *** conditions were established during an early stage of the Andean magmatic arc building that is firstly defined here as Upper Triassic. The NW and E domains of the pluton were sequentially emplaced between 194 Ma and 186 Ma and both the field relations and the detailed geochronological results suggest that the mafic facies intruded latter in the emplacement sequence.
