Congenital Diaphragmatic Hernia with Delayed Diagnosis: Report of Two Cases

Introduction: Advances in prenatal imaging studies have allowed early diagnosis of Congenital Diaphragmatic Hernia (CDH), although neonatal mortality remains at high levels (60% survival). Despite advances, this study found delayed diagnosis, demonstrating subdiagnosis, possibly caused by lack of resources and/or prenatal diagnostic failure. Case Report 1: Infant, 4 months and 22 days old, dyspneic, cyanotic, with fever and vomiting for 3 days, treated as pneumonia. After the second chest radiography, a right CDH was revealed. Surgical correction was performed and removal of Meckel’s diverticulum was surgical findings. The patient was discharged on the 10th days after admission. Case Report 2: Infant, 11 months and 3 days old, distended abdomen, with fever and dyspnea for 2 days, treated as pneumonia. After the second chest radiography, a left CDH was revealed. The patient was submitted to surgery, intestinal loop incarceration without necrosis was observed, and the correction of CDH was performed. The patient was discharged on the 7th day after admission. Discussion: Three cases, between July 2012 and July 2013, were diagnosed at Dom Malam Hospital;2 Cases of CDH, with delayed diagnosis, showed incidence of 66.66%. Delayed diagnosis of these patients with initial hypotheses of pneumonia demonstrated high risk, emphasizing the importance of diagnosis research through observation of the clinical course, the correct imaging interpretation, in order to establish early diagnosis and treatment.

Overcoming antibiotic-resistant Helicobacter pylori infection:Current challenges and emerging approaches

Recent studies have shown a noticeable increase in global Helicobacter pylori(***)resistance,with clarithromycin resistance surpassing 15%in various ***,inadequate epidemiological monitoring,especially in developing countries,and the absence of uniform testing methods lead to discrepancies between regions and a possible underestimation of resistance *** complexity of treating *** is driven by its highly dynamic genome,which is prone to frequent mutations contributing to phenotypical *** usual course of action in empirical treatment involves using a combination of various drugs simultaneously,leading to significant resistance selection pressure and potential side *** emergence of *** strains resistant to multiple drugs is closely tied to failures in first-line treatment,highlighting the need to prevent further resistance by using optimal initial empirical therapy or regimens guided by antibiotic susceptibility testing,requiring a collection of mixed samples and multiple isolates for accurate *** emergence of new treatments like potassium-competitive acid blockers offers a hopeful approach to decrease antimicrobial usage while still ensuring effectiveness in comparison to traditional therapies with proton pump ***,the use of probiotics is under investigation to identify specific strains and formulations that may mitigate therapy-associated adverse effects.

The inflammatory pattern during Helicobacter pylori(***)infection is changeable and *** childhood,it is possible to observe a predominantly regulatory response,evidenced by high concentrations of key cytokines for the maintenance of Treg responses such as TGF-β1 and IL-10,in addition to high expression of the transcription factor *** the other hand,there is a predominance of cytokines associated with the Th1 and Th17 responses among ***-positive *** the last few years,the participation of the Th17 response in the gastric inflammation against *** infection has been highlighted due to the high levels of TGF-β1 and IL-17 found in this infectious scenario,and growing evidence has supported a close relationship between this immune response profile and unfavorable outcomes related to the ***,this cytokine profile might play a pivotal role in the effectiveness of *** *** is evident that age is one of the main factors influencing the gastric inflammatory pattern during the infection with ***,and understanding the immune response against the bacterium can assist in the development of alternative prophylactic and therapeutic strategies against the infection as well as in the comprehension of the pathogenesis of the outcomes related to that microorganism.

Tertiary peritonitis: A disease that should not be ignored

Intra-abdominal infections can be classified into uncomplicated or complicated(peritonitis). Peritonitis is divided into primary, secondary, and tertiary. Tertiaryperitonitis is the less common but the most severe among peritonitisstratifications, being defined as a recurrent intra-abdominal infection that occurs48 h after a well-succeeded control of a secondary peritonitis. This disease has acomplex pathogenesis that is closely related to the capacity of the peritonealcavity to activate immunological processes. Patients who progress to persistentperitonitis are at an increased risk of developing several infectious complicationssuch as sepsis and multiple organ failure syndrome. Moreover, tertiary peritonitisremains an important cause of hospital death mainly among patients withassociated risk factors. The microbiological profile of organisms causing tertiaryperitonitis is often different from that observed in other types of peritonitis. Inaddition, there is a high prevalence of multidrug-resistant pathogens causing thiscondition, and an appropriate and successful clinical management depends on anearly diagnosis, which can be made easier with the use of clinical scorespresenting a good prediction value during the intensive care unit ***, immediate therapy should be performed to control theinfectious focus and to prevent new recurrences. In this sense, the treatment isbased on initial antimicrobial therapy and well-performed peritoneal drainage.

In this editorial,we comment on an article by Tang et al published in the World journal of *** and diabetes are two pathological situations that are intrinsically *** lifestyle changes nor pharmacological treatments have achieved diabetes *** this perspective,bariatric surgery has been widely used as an approach for weight loss in obese patients and as a strategy to promote metabolic *** main effects of bariatric surgery involve direct action in improving cardiovascular function and endothelial function and reducing insulin resistance,leading to diabetes remission in the short term following *** this context,it has been observed that hormones from the gastrointestinal tract and endothelium play a prominent role in this *** reversing endothelial dysfunction,it is possible to balance pro-inflammatory cytokine production,improving the availability of nitric oxide and inhibiting vascular oxidative ***,it can be considered an efficient anti-inflammatory strategy,alleviating interferon-gamma-mediated adipose tissue *** current challenge must be to unravel the pathophysiological mechanisms and potential targets for treating metabolic diseases.

BACKGROUND：Liver ischemia reperfusion（IR）injury triggers a systemic inflammatory response and is the main cause of organ dysfunction and adverse postoperative outcomes after liver ***（PTX）and hypertonic saline solution（HTS）have been identified to have beneficial effects against IR *** study aimed to investigate if the addition of PTX to HTS is superior to HTS alone for the prevention of liver IR ***： Male Wistar rats were allocated into three groups. Control rats underwent 60 minutes of partial liver ischemia, HTS rats were treated with 0.4 mL/kg of intravenous 7.5% NaCl 15 minutes before reperfusion, and HPTX group were treated with 7.5% NaC1 plus 25 mg/kg of PTX 15 minutes before reperfusion. Samples were collected after reperfusion for determination of ALT, AST, TNF-α, IL-6, IL-10, mitochondrial respiration, lipid peroxidation, pulmonary permeability and myeloperoxidase. RESULTS： HPTX significantly decreased TNF-α 30 minutes after reperfusion. HPTX and HTS significantly decreased ALT,AST, IL-6, mitochondrial dysfunction and pulmonary myelo- peroxidase 4 hours after reperfusion. Compared with HTS only, HPTX significantly decreased hepatic oxidative stress 4 hours after reperfusion and pulmonary permeability 4 and 12 hours after reperfusion. CONCLUSION： This study showed that PTX added the beneficial effects of HTS on liver IR injury through decreases of hepatic oxidative stress and pulmonary permeability.

Influence of biological maturation on cardiac autonomic recovery in female volleyball players during&after repeated sprints training:An experimental trial

Previously,it was suggested that biological maturation(BM)could be linked to cardiac autonomic recovery(CAR)in the pediatric ***,this influence hasn’t been confirmed *** aim was to investigate the impact of BM on CAR in female volleyball *** study with a sample of 38 volleyball players,comprising 20 girls(age:[11.62.1]years)and 18 women(age:[24.55.5]years),we analyzed BM,comparing maturing subjects(girls)with mature subjects(women).Additionally,we assessed peak height velocity(PHV)in *** conducted a training session involving repeated sprints(3 rounds of 6 sprints interspersed by 5 min[min]of passive rest).Using short-range radio telemetry,we analyzed CAR during(at the end of the 1st and 2nd rounds)and after(following the 3rd round)the training session of repeated sprints by applying the 60-s to 300-s heart rate recovery index(HRR-Index).Girls exhibited superior CAR compared to women(round 2:60-s,120-s,240-s,and 300-s,p0.4,p0.4,p<0.05).We have concluded that biological maturation has a significant impact on cardiac autonomic recovery.

Oxaliplatin induced disseminated intravascular coagulation: A case report and review of literature

Oxaliplatin in combination with a fluoropyrimide is a treatment option for colorectal cancer patients in the adjuvant and metastatic settings. Very few hematological emergencies have been reported associated with Oxaliplatin. These include autoimmune hemolytic anemia, thrombocytopenia and pancytopenia. We present a case report of a patient who developed hematuria and disseminated intravascular coagulation while receiving the second cycle of FOLFOX and bevacizumab for metastatic colon cancer.

Safety and Efficacy of Carboxytherapy in Obese Patients

Obesity is a highly prevalent disease worldwide, associated with substantial economic and social costs. Besides the traditional treatment approach with low-calorie diet and regular physical exercise, some adjunct therapies have been investigated to assist body fat loss in obese patients. Carboxytherapy is an invasive intervention already studied for a range of conditions, from wound healing to the treatment of abdominal adiposity, due to its oxidative effect on adipocytes, in addition to contributing to improved skin elasticity. Thus, this study aimed to investigate the efficacy and safety of carboxytherapy in reducing abdominal circumference in patients hospitalized for obesity treatment. Fifty-eight adult inpatients with obesity underwent an individualized treatment (low-calorie diet and daily physical exercise for 40 - 90 days) associated with a mean of 11 sessions of carboxytherapy in the abdominal region and were assessed for body mass index (BMI), abdominal circumference (AC), waist to hip ratio (WHR) and percent body fat (%BF). After the intervention, significant reductions were observed in all studied variables (p nation with an inpatient program involving a low-calorie diet and regular exercise, contributed to the reduction of abdominal adiposity.

How to improve colon cancer screening rates

Colorectal carcinoma is a common cause of death throughout the world and may be prevented by routine control, which can detect precancerous neoplasms and early cancers before they undergo malignant transformation or metastasis. Three strategies may improve colon cancer screening rates: convince the population about the importance of undergoing a screening test; achieve higher efficacy in standard screening tests and make them more available to the community and develop new more sensitive and efficacious screening methods and make them available as routine tests. In this light, the present study seeks to review these three means through which to increase colon cancer screening rates.
