Evaluation of urea breath test as a diagnostic tool for Helicobacter pylori infection in adult dyspeptic patients

In this editorial,we discuss the article in the World Journal of Gastroenterology.The article conducts a meta-analysis of the diagnostic accuracy of the urea breath test(UBT),a non-invasive method for detecting Helicobacter pylori(H.pylori)infection in humans.It is based on radionuclide-labeled urea.Various methods,both invasive and non-invasive,are available for diagnosing H.pylori infection,inclu-ding endoscopy with biopsy,serology for immunoglobulin titers,stool antigen analysis,and UBT.Several guidelines recommend UBTs as the primary choice for diagnosing H.pylori infection and for reexamining after eradication therapy.It is used to be the first choice non-invasive test due to their high accuracy,specificity,rapid results,and simplicity.Moreover,its performance remains unaffected by the distribution of H.pylori in the stomach,allowing a high flow of patients to be tested.Despite its widespread use,the performance characteristics of UBT have been inconsistently described and remain incompletely defined.There are two UBTs available with Food and Drug Administration approval:The 13C and 14C tests.Both tests are affordable and can provide real-time results.Physicians may prefer the 13C test because it is non-radioactive,compared to 14C which uses a radioactive isotope,especially in young children and pregnant women.Although there was heterogeneity among the studies regarding the diagnostic accuracy of both UBTs,13C-UBT consistently outperforms the 14C-UBT.This makes the 13C-UBT the preferred diagnostic approach.Furthermore,the provided findings of the meta-analysis emphasize the significance of precise considerations when choosing urea dosage,assessment timing,and measurement techniques for both the 13C-UBT and 14C-UBT,to enhance diagnostic precision.

An illness known as pneumonia causes inflammation in the lungs.Since there is so much information available fromvarious X-ray images,diagnosing pneumonia has typically proven challenging.To improve image quality and speed up the diagnosis of pneumonia,numerous approaches have been devised.To date,several methods have been employed to identify pneumonia.The Convolutional Neural Network(CNN)has achieved outstanding success in identifying and diagnosing diseases in the fields of medicine and radiology.However,these methods are complex,inefficient,and imprecise to analyze a big number of datasets.In this paper,a new hybrid method for the automatic classification and identification of Pneumonia from chest X-ray images is proposed.The proposed method(ABOCNN)utilized theAfrican BuffaloOptimization(ABO)algorithmto enhanceCNNperformance and accuracy.The Weinmed filter is employed for pre-processing to eliminate unwanted noises from chest X-ray images,followed by feature extraction using the Grey Level Co-Occurrence Matrix(GLCM)approach.Relevant features are then selected from the dataset using the ABO algorithm,and ultimately,high-performance deep learning using the CNN approach is introduced for the classification and identification of Pneumonia.Experimental results on various datasets showed that,when contrasted to other approaches,the ABO-CNN outperforms them all for the classification tasks.The proposed method exhibits superior values like 96.95%,88%,86%,and 86%for accuracy,precision,recall,and F1-score,respectively.

The behavior of the quantum correlations, information scrambling and the non-Markovianity of three entangling qubits systems via Rashba is discussed. The results showed that, the three physical quantities oscillate between their upper and lower bounds, where the number of oscillations increases as the Rashba interaction strength increases. The exchanging rate of these three quantities depends on the Rashba strength, and whether the entangled state is generated via direct/indirect interaction. Moreover, the coherence parameter can be used as a control parameter to maximize or minimize the three physical quantities.

目的制备抗白细胞介素1β(IL-1β)纳米抗体, 并观察其对缺氧小鼠心肌细胞凋亡的影响。方法利用DNA重组技术, 使用pET-16b和pHEN1表达载体构建抗IL-1β纳米抗体的重组质粒(pET-16b-4G6M-VHH、pET-16b-5BVP-VHH、pET-16b-5MVZ-VHH和pHEN1-4G6M-VHH、pHEN1-5BVP-VHH、pHEN1-5MVZ-VHH, 其中VHH为重链单域抗体, 4G6M-VHH、5BVP-VHH、5MVZ-VHH分别为3种抗IL-1β的纳米抗体), 将构建成功的质粒分别转入大肠杆菌Rosetta2(DE3)表达菌中进行诱导表达及镍柱纯化, 用十二烷基硫酸钠-聚丙烯酰胺凝胶电泳(SDS-PAGE)和Western印迹对表达产物及纯化产物进行鉴定, 并通过酶联免疫吸附试验(ELISA)测定其亲和力。将纯化所得亲和力最高的抗IL-1β纳米抗体(pHEN1-5MVZ-VHH)应用于小鼠HL-1心肌细胞缺氧模型(分为正常对照组、缺氧模型组、空白质粒组及12.5、25.0、50.0 μg/ml pHEN1-5MVZ-VHH处理组), 并检测细胞存活率及凋亡率。结果 SDS-PAGE和Western印迹结果显示, 成功得到相对分子质量约15 000的抗IL-1β纳米抗体;ELISA结果显示, 与pET-16b载体组相比, pHEN1载体组所表达的纳米抗体与IL-1β抗原的亲和力更高(4G6M-VHH组:3.20±0.03比1.20±0.03, P<0.001;5BVP-VHH组:3.18±0.06比1.21±0.02, P<0.001;5MVZ-VHH组:3.38±0.05比1.62±0.04, P<0.001);细胞存活及凋亡检测结果显示, 与缺氧模型组相比, 25.0 μg/ml及50.0 μg/ml pHEN1-5MVZ-VHH处理组HL-1细胞活性增加[(75.55±2.23)%比(46.90±2.51)%, P<0.001;(74.36±1.96)%比(46.90±2.51)%, P<0.001], 凋亡率降低[(6.83±0.27)%比(10.24±0.76)%, P<0.001;(6.68±0.38)%比(10.24±0.76)%, P<0.001]。结论 pET-16b和pHEN1载体均能表达出抗IL-1β纳米抗体, 且pHEN1载体表达的纳米抗体具有更好的抗原亲和作用。此外, pHEN1-5MVZ-VHH治疗可减少缺氧小鼠心肌细胞凋亡。

Software Defined Network(SDN)and Network Function Virtualization(NFV)technology promote several benefits to network operators,including reduced maintenance costs,increased network operational performance,simplified network lifecycle,and policies management.Network vulnerabilities try to modify services provided by Network Function Virtualization MANagement and Orchestration(NFV MANO),and malicious attacks in different scenarios disrupt the NFV Orchestrator(NFVO)and Virtualized Infrastructure Manager(VIM)lifecycle management related to network services or individual Virtualized Network Function(VNF).This paper proposes an anomaly detection mechanism that monitors threats in NFV MANO and manages promptly and adaptively to implement and handle security functions in order to enhance the quality of experience for end users.An anomaly detector investigates these identified risks and provides secure network services.It enables virtual network security functions and identifies anomalies in Kubernetes(a cloud-based platform).For training and testing purpose of the proposed approach,an intrusion-containing dataset is used that hold multiple malicious activities like a Smurf,Neptune,Teardrop,Pod,Land,IPsweep,etc.,categorized as Probing(Prob),Denial of Service(DoS),User to Root(U2R),and Remote to User(R2L)attacks.An anomaly detector is anticipated with the capabilities of a Machine Learning(ML)technique,making use of supervised learning techniques like Logistic Regression(LR),Support Vector Machine(SVM),Random Forest(RF),Naïve Bayes(NB),and Extreme Gradient Boosting(XGBoost).The proposed framework has been evaluated by deploying the identified ML algorithm on a Jupyter notebook in Kubeflow to simulate Kubernetes for validation purposes.RF classifier has shown better outcomes(99.90%accuracy)than other classifiers in detecting anomalies/intrusions in the containerized environment.

Corrosiveness or scaling is an inherent threat to oil operations. The primary cause for this threat is the presence of water having complex geochemical matrix. Unanticipated water production, particularly if it contains unwanted impurities, can significantly impact hydrocarbon production. The current paper discusses the degree of threat posed to oil operation facilities based on the water characteristics. Methodology involved the collection and analysis of water samples from major sources in oil industry such as groundwater, wellheads and seawater. The parameters tested include geochemical, microbiological and pollutants. The lab data was used to develop scaling and corrosion prediction indices such as Langelier Saturation Index (LSI), Ryznar Stability Index (RSI) and Puckorius Scaling Index (PSI). The study indicated varying water chemistry for different sources. Mixing of those waters may lead to ionic saturation and scaling in different facilities. Presence of the SRB and GAB in some water sources also posed threat to water system by forming fouling and corrosion. Seawater used for offshore oilfields water injection to maintain reservoir pressure and improve oil recovery showed scaling tendency, whereas under different reservoir pressure, it can cause corrosion. Some of the samples also had corrosion residuals such as iron, which indicated active corrosion. Current study showed higher alkalinity with high sulfate for one groundwater sample with presence of active corrosion residuals such as dissolved iron and manganese. The study showed positive value for LSI which indicated supersaturation of the water samples with respect to calcium carbonate (CaCO3) and scale forming. Similarly, for RSI and PSI, the value was below six which confirmed the scaling potential for all the samples. Even though the index value was pointing towards scaling potential, the geochemistry, microbiology and presence of other impurities indicated corrosion threat to the oil and gas industrial facilities. An analysis of the molecular dynamics of ethanol solvated by water molecules in the absence and presence of a Pt surface has been performed using DL_POLY_2.19 code. The structure and diffusion properties of an ethanol–water system have been studied at various temperatures from 250 to 600 K. We have measured the self-diffusion coefficients of the 50:50% ethanol–water solution;in the absence of a Pt surface our results show an excellent agreement–within an error of 7.4% – with the experimental data. An increase in the self-diffusion coefficients with the inclusion of a Pt surface has been observed. The estimation of the diffusion coefficients of both water and ethanol in the presence of a Pt surface shows that they obey the Arrhenius equation;the calculated activation energies of diffusion of ethanol and water are 2.47 and 2.98 Kcal/mole, respectively. The radial distribution function graphs and density profiles have been built;their correlations with the self-diffusion coefficients of both ethanol and water molecules are also illustrated.

Introduction: In the last thirty years, brain neuromodulation techniques have been used as an alternative to pharmacological treatment of neurological disorders. Parkinson’s disease (PD) is a neurodegenerative disorder leading to bradykinesia, rest tremor, postural changes, and non-motor symptoms such as depression, anxiety, sleep disorders, pain, and cognitive decline that compromises executive functions (EFs), responsible for the orderly execution of behaviors and tasks of daily life and intentional and directed actions. To this date, a few studies with transcranial direct current stimulation (tDCS) have shown beneficial effects in PD patients concerning specific motor and non-motor symptoms, targeting the motor cortex and/or prefrontal regions. Objective: The main objective of this study was to evaluate the effects of left prefrontal tDCS across a broad spectrum of motor and non-motor symptoms of PD using established validated scales. Method: Single-blind randomized clinical trial with 18 volunteers with PD, aged between 45 and 80 years (66.1 ± 9.65), who met inclusion and exclusion criteria. Participants were submitted to assessments of motor and non-motor functions employing psychometric scales and tests to evaluate EFs and were randomly divided into two groups: control (sham stimulation) and experimental (active stimulation). All participants were involved in three separate tDCS sessions. The anode was positioned over the left dorsolateral prefrontal cortex and the cathode over the right supraorbital region, with a direct current intensity of 2 mA, lasting 20 minutes. At the end of the three sessions, all participants were reassessed. Results: Significant effects of tDCS on non-motor functions were observed for cognition (verbal fluency of actions, clock copy test, appointment by visual confrontation, and verbal memory with immediate free recall) and subjective assessment of sleep quality (overall restlessness and discomfort in the arms and legs at night, leg and arm cramps at night and distressing dreams).

BACKGROUND A post-bulbar duodenal ulcer(PBDU)is an ulcer in the duodenum that is distal to the duodenal bulb.PBDU may coexist with a synchronous posterior ulcer in rare occurrences,resulting in a kissing ulcer(KU).Duodenocaval fistula(DCF)is another uncommon but potentially fatal complication related to PBDU.There is limited knowledge of the scenarios in which PBDU is complicated by KU and DCF simultaneously.CASE SUMMARY A 22-year-old man was admitted to the emergency department with abdominal pain,stiffness,and vomiting.The X-ray showed pneumoperitoneum,suggesting a perforated viscus.Laparotomy revealed a KU with anterior perforation and a DCF.After Kocherization,venorrahphy was used to control caval bleeding.Due to the critical condition of the patient,only primary duodenorrahphy with gastrojejunostomy was performed as a damage control strategy.However,later,the patient developed obstructive jaundice and leakage,and two additional jejunal perforations were detected.Due to the poor condition of the duodenum and the involvement of the ampulla in the posterior ulcer,neither primary repair nor pancreatic-free duodenectomy and ampull-oplasty/ampullary reimplantation were considered viable;therefore,an emergency pancreaticoduodenectomy was performed,along with resection and anastomosis of the two jejunal perforations.The patient had a smooth recovery after surgery and was discharged after 27 d.CONCLUSION The timely diagnosis of PBDU and radical surgery can aid in the smooth recovery of patients,even in the most complex cases.
