The Role of Endoscopic Retrograde Cholangiopancreatography (ERCP) in the Management of Intra-Biliary Rupture of Liver Hydatid Cysts (IBRH): Follow-Up of 12 Cases

Background: Intra-biliary rupture of hydatid cyst [IBRH] is not a rare complication of hydatid disease of the liver and surgery until recently is the only definitive treatment. With the introduction of ERCP preoperatively or postoperatively there was a great reduction in the operative complications. There were reports in which the ERCP was the sole treatment of this condition. Objective: To evaluate role of ERCP in the management of cases of IBRH. Design: A retrospective study of 7 cases of IBRH and prospective study of 5 cases of IBRH managed by ERCP. Setting: The ERCP Unit of Kurditan Center for Gastroenterology & Hepatology, Asulaimaneyah-Iraqi, Kurdistan, Iraq. Main Outcome Measurements: Improvement in the symptoms, obstructive liver functions pattern and ultrasonic findings in these patients following ERCP management. Methods: This is a study of 12 cases of IBRH managed by ERCP in the Kurdistan Center of Gastroenterology (KCGE) in Asulaimaneyah-Iraq, from 2007-2010. Seven cases of these were studied retrospectively from the available information on the center computers and on follow-up of these patients, while the other five patients were studied prospectively during the study period. We collected the available information: laboratory investigations, trans-abdominal ultrasonography (TAUS), computed tomography (CT) or magnetic resonance images (when available), before and after ERCP. We depended on the liver functions, trans-abdominal ultrasound and the clinical presentation before and after the ERCP;some patients were contacted via mobile phone for follow-up. Results: Cases were more males than females (58% vs 42%);most were from the ages between 30 - 50 years. The clinical presentation was fever, jaundice, itching and right hypochondrial pain in most patients. The liver function tests were obstructive pattern with elevated direct bilirubin and alkhaline phosphates in most patients. Flue gas heat loss accounts for a significant component of theoverall heat loss for coal-fired boilers in power plants. The flue gas absorbsmore heat as the exhaust gas temperature rises, which reduces boiler efficiencyand raises coal consumption. Additionally, if the exhaust gas temperatureis too high, a lot of water must be used to cool the flue gas for the wetflue gas desulfurization system to function well, which has an impact onthe power plant’s ability to operate profitably. It is consequently vital totake steps to lower exhaust gas temperatures in order to increase boilerefficiency and decrease the amount of coal and water used. Desulfurizationperformance may be enhanced and water use can be decreased by reasonableflue gas characteristics at the entry. This study analyzed the unit’s energyconsumption, investment, and coal savings while proposing four couplingstrategies for regulating flue gas temperature and waste heat recovery. Agraded flue gas conditioning and waste heat recovery plan was presentedunder the condition of ensuring high desulfurization efficiency, along withthe notion of minimizing energy loss owing to energy inflow temperaturedifference. Numerical results show that the proposed methods improved thesystem performance and reduced the water consumption and regulated theboiler temperature.

Evaluation of the Influence of Fixed Orthodontic Treatment Duration on the Severity of Inflammatory Gingival Enlargement (Fixed Orthodontic Induced Gingival Enlargements) and Some Properties of Saliva

Background: Inflammatory gingival enlargement is a more common clinical feature with orthodontic therapy than other features. Therefore, this study was designed to the evaluation of the influence of fixed orthodontic treatment duration on the severity of inflammatory gingival enlargement (fixed orthodontic induced gingival enlargements) and some properties of saliva. Material and Methods: The sample size comprised 145 patients undergoing fixed orthodontic treatment for at least 6 months aged 13 - 32 years. They were divided according to orthodontic treatment duration into three groups. Group I (n = 47) included the patients who were treated for less than 6 months, group II (n = 51) included the patients who were treated for a period of 6 - 12 months, and group III (n = 47) included the patients who were managed for more than 12 months. Data were obtained from the outpatient clinics, college of dentistry, King Khalid University, Abha, Saudi Arabia, and some dental centers in Sana’a city, the Republic of Yemen. This study was conducted from October 2021 G to January 2022 G. Clinical examination was done for plaque index (PLI), gingival index (GI), and gingival enlargement indexes (GEI). Saliva was collected in sterile test tubes then salivary flow and pH were measured. Statistical analysis was done with SPSS (version 23) and ANOVA test to evaluate the impact of orthodontic treatment duration on the severity of inflammatory gingival enlargement and some properties of saliva. Results: The statistical analysis demonstrated the highest mean plaque index (PLI) was among groups III and I participants whereas, the highest mean gingival index and mean gingival enlargement were among groups II and III participants. The present study revealed an increase in salivary flow with decreased salivary pH values with an increase in orthodontic therapy duration. Framework for Random Power Allocation of Wireless Sensor Networks in Fading Channels

In naturally deaf wireless sensor networks or generally when there is no feedback channel, the fixed-level transmit power of all nodes is the conventional and practical power allocation method. Using random power allocation for the broadcasting nodes has been recently proposed to overcome the limitations and problems of the fixed power allocation. However, the previous work discussed only the performance analysis when uniform power allocation is used for quasi-static channels. This paper gives a general framework to evaluate the performance (in terms of outage and average transmit power) of any truncated probability density function of the random allocated power. Furthermore, dynamic Rayleigh fading channel is considered during the performance analysis which gives more realistic results that the AWGN channels assumed in the previous work. The main objective of this paper is to evaluate the communication performance when general random power allocation is used. Furthermore, the truncated inverse exponential probability distribution of the random power allocation is proposed and compared with the fixed and the uniform power allocations. The performance analysis for the proposed schemes are given mathematically and evaluated via intensive simulations.

The Effect of Metformin versus Vitamin E on the Testis of Adult Diabetic Albino Rats: Histological, Biochemical and Immunohistochemistry Study

Background: Impairment of testicular function is one of complications of Diabetes mellitus (D.M) and this may result from increased oxidative stress. Vitamin E (Vit E) has antioxidant properties. Objective: To determine the microscopic changes in the diabetic testis with metformin alone or with Vit E and which of them are better to preserve the testicular structure. Materials and Methods: Fifty adult albino rats, were equally divided into five groups: Group 1: control group;group 2: diabetic group (diabetes was induced by (65 mg/kg;i.p) single dose of streptozotocin (STZ);group 3: vit.E-treated diabetic group (200 mg/kg/day) ip;group 4: Metformin-treated diabetic group (100 mg/kg/day) and group 5 Metformin and vit.E-treated diabetic group for eight weeks. Levels of testosterone, insulin and blood sugar were analyzed and testes specimens were prepared for light microscope and immunohistochemical detection of androgen receptor (AR) and caspase-3. Results: In diabetic group showed irregular seminiferous tubules with depletion, separation, vacuolation of the spermatogenic cells with perivascular and intertubular fibrosis also, the mean diameter of tubules significantly decreases. There are negative immunoexpressions of AR with positive caspase-3. Sections of metformin treated diabetic group showed incomplete or partial regeneration of germ cells while in group 4 and 5 showed almost complete regeneration of spermatogenic cells with increase in mean diameter of tubules, significantly increases in AR and decreases in caspase-3 expression. Conclusion: D.M produce damage in the testicular tissue and the concomitant administration of vit-E with metformin was shown better effect than metformin alone.

Three-Dimensional Dynamic Based Borrowing Scheme for Wireless Cellular Networks

Mobility metrics of wireless networks such as link availability, number of neighboring nodes, link duration, link state, and link stability make it difficult to provide a node with quality of services guarantee. In previous research on Quality of Service (QoS) for cellular networks especially for handling handoff connections, the design was based on a flat 2D hexagon cells. However, in reality Base Station antenna coverage is in a 3D space and there exists a blind spot;the area which is just above and bellow the radiated antenna. In this paper we introduce the concept of Blind Spot (BS) in which there is no signals to initiate a call or accepting a handoff one. In BS, the signal power equal zero. Even if there is enough bandwidth to initiate or accept a handoff call, it will be blocked or dropped respectively. We present an implementation of Static Borrowing Scheme (SBS) and we extend the dynamic-rate based borrowing scheme [1] into 3-Dimentional structure and call it 3-Dimensional Dynamic Based Borrowing Scheme (3D DBBS). The proposed new technique for resource sharing is to ensure the continuity for both originating and handoff connections in 3-D cellular networks based on Dynamic-Based Borrowing Scheme (3D BBS). This technique aims to minimize the blocking probability of the originating calls by minimizing the dropping probability of the handoff requests and maximizing the channel utilization. The results revealed that 3D DBBS outperformed the static based schemes by 5% on average even when the blind spot of the base station antenna is taken into consideration. When moving to a 3D space, the results of the simulation showed the 3D DBBS outperformed the static scheme by 2% on average. As a result, considering nodes in a 3D space will have better QoS guarantee as the blocking and dropping probabilities are decreased. Thus, the bandwidth utilization is increased.

Assessment of Sub-Endometrial Junction Zone by 3-Dimensional Transvaginal Ultrasound in Unexplained Recurrent Implantation Failure and Its Effect on ICSI Outcomes

BACKGROUND: Sub-endometrial junction zone (JZ) plays an important role in most of reproductive functions. OBJECTIVE: To find out the effect of the sub-endometrial JZ thickness assessment by 3-D trans-vaginal ultrasound (TVUS) on intracytoplasmic sperm injection (ICSI) outcomes in patients with unexplained recurrent implantation failure (RIF). SETTING: ART Unit of Obstetrics and Gynecology Department, Faculty of Medicine, South Valley University, Qena, Egypt. DURATION: From April 2016 to October 2018. STUDY DESIGN: A prospective observational study. METHDS: Fifty couples with history of unexplained RIF in previous ICSI cycles and prepared for another ICSI cycle (group I) and fifty couples with unexplained infertility prepared for ICSI for the first time (group II) had been included in this study. At time of ovum pick up, 3-DTVUS was done for all cases in both groups for assessment of sub endometrial junction zone thickness and correlated with ICSI outcome. RESULTS: There were statistically significant differences between group I and group II in JZ thickness in the 3 uterine regions (fundus, anterior and posterior walls) with p 0.001, but there were mildly statistically significant differences between both groups in chemical and clinical pregnancy rates with p < 0.01. CONCLUSIONS: The thickness of JZ in patients with history of unexplained RIF was higher than those with unexplained infertility scheduled for ICSI. The JZ thickness was inversely correlated with increased embryo implantation rates in ICSI procedures, the thinner the JZ thickness was associated with higher pregnancy rates (both chemical and clinical pregnancy rates).
