Association of post-traumatic stress disorder and work performance: A survey from an emergency medical service, Karachi, Pakistan

BACKGROUND: The purpose of the study was to explore the association between posttraumatic stress disorder(PTSD) and work performance of emergency medical services personnel in Karachi, Pakistan.METHODS: Emergency medical service personnel were screened for potential PTSD using Impact of Event Scale-Revised(IES-R). Work performance was assessed on the basis of fi ve variables: number of late arrivals to work, number of days absent, number of days sick, adherence to protocol, and patient satisfaction over a period of 3 months. In order to model outcomes like the number of late arrivals to work, days absent and days late, negative binomial regression was applied, whereas logistic regression was applied for adherence to protocol and linear for patient satisfaction scores.RESULTS: Mean scores of PTSD were 24.0±12.2. No association was found between PTSD and work performance measures: number of late arrivals to work(RRadj 0.99; 0.98–1.00), days absent(RRadj 0.98; 0.96–0.99), days sick(RRadj 0.99; 0.98–1.00), adherence to protocol(ORadj 1.01; 0.99–1.04) and patient satisfaction(β 0.001%–0.03%) after adjusting for years of formal schooling, living status, coping mechanism, social support, working hours, years of experience and anxiety or depression.CONCLUSION: No statistically significant association was found between PTSD and work performance amongst EMS personnel in Karachi, Pakistan.

Frame Length Dependency for Fundamental Frequency Extraction in Noisy Speech

The fundamental frequency plays a significant part in understanding and perceiving the pitch of a sound. The pitch is a fundamental attribute employed in numerous speech-related works. For fundamental frequency extraction, several algorithms have been developed which one to use relies on the signal’s characteristics and the surrounding noise. Thus, the algorithm’s noise resistance becomes more critical than ever for precise fundamental frequency estimation. Nonetheless, numerous state-of-the-art algorithms face struggles in achieving satisfying outcomes when confronted with speech recordings that are noisy with low signal-to-noise ratio (SNR) values. Also, most of the recent techniques utilize different frame lengths for pitch extraction. From this point of view, This research considers different frame lengths on male and female speech signals for fundamental frequency extraction. Also, analyze the frame length dependency on the speech signal analytically to understand which frame length is more suitable and effective for male and female speech signals specifically. For the validation of our idea, we have utilized the conventional autocorrelation function (ACF), and state-of-the-art method BaNa. This study puts out a potent idea that will work better for speech processing applications in noisy speech. From experimental results, the proposed idea represents which frame length is more appropriate for male and female speech signals in noisy environments.

Screening of Psychiatric Disorders among Hearing-Impaired Children and Adolescents Aged 4 - 16 Years Attending Special Education Institutions in Karachi

Aim: The aim of this study was to estimate the prevalence of psychiatric problems and to identify associated factors among hearing-impaired children and adolescents (aged 4 - 16 years) attending special education institutions in Karachi, Pakistan. Methods: A cross-sectional study, using stratified random sampling was conducted in two special education institutions in Karachi from September 2010 to July 2011 on 272 hearing-impaired children aged 4 - 16 years. Multi-informant rating (responding parent and teacher) was used to assess the prevalence of psychiatric problems on Strengths and Difficulties Questionnaire (SDQ). Kappa statistic was computed to assess agreement between parents/teachers ratings. Multivariable logistic regression analysis was conducted to identify the factors associated with psychiatric problems of children. Results: An overall parent-rated prevalence of psychiatric problems among hearing-impaired children was estimated as 18% and teacher-rated prevalence as 32.7%;poor inter-rater agreement was found between the two raters (Kappa statistic = 0.09). Multivariate model indicated protective effect of child’s age greater than 6 months at the time of diagnosis AOR: 0.49;95% CI (0.25, 0.96). Depression among responding parents AOR: 2.61;95% CI (1.34, 5.11) was identified as a risk factor. Children with good performance AOR: 3.09;95% CI (1.04, 9.25) and fair/poor performance AOR: 3.43;95% CI (1.17, 10.04) were more likely to have psychiatric problems compared to children with outstanding/excellent performance. Cronbach’s alpha of 0.71 on overall SDQ indicated an acceptable internal consistency. Conclusion: Awareness programs should be held for parents and teachers for better dealing with hearing-impaired children.

Assessment of Physico-Chemical and Microbial Quality of Sugarcane Juice Sold by Street Vendors in Three Regions of Bangladesh

Sugar cane juice is a popular refreshing drink in most part of Bangladesh. It has great taste and health benefits;also it is available most of the public places at reasonable prices which consumed by road side customers including general public, shopping personals, tourists, students. In our country, street vendors crushing sugarcane between roller crusher and sold without any heat treatment or preservative, also served with or without added ice and lemon juice. The study aimed to identify and compare the physico-chemical and microbial quality of sugarcane juice. The chemical qualities of juices including moisture, PH, ash, total soluble solid, total sugar, reducing sugar and titrable acidity were found slightly different in Mymensingh, Gazipur and Narayangonj areas. The highest and lowest value of moisture, ash, PH, total soluble solids, total sugar, reducing sugar and titrable acidity were found in 84.33% - 79.26% (Gazipur-Mymensingh), 0.57% - 0.04% (Mymensingh-Gazipur), 5.9 - 2.9 (Gazipur-Narayangonj), 17.48% - 4.98% (Mymensingh-Narayangonj), 21.9% - 6.56% (Mymensingh-Gazipur), 3.7 - 2.1 (Gazipur-Mymensingh) and 0.523% - 0.007% (Narayangonj-Gazipur) respectively. For microbiological analysis, the total viable count of sugar cane juice in Mymensingh, Gazipur and Narayangonj were ranged from 0.6 × 105 - 43.6 × 105 cfu/ml, 4.6 × 105 - 21.6 × 105 cfu/ml and 3.6 × 105 - 36.6 × 105 cfu/ml respectively, where the permitted value is 1.0 × 104 cfu/ml, whereas the total coliform count was ranged from 0.4 × 105 - 6.4 × 105 cfu/ml, 0.6 × 105 - 8.4 × 105 cfu/ml and 0.00 - 8.4 × 105 cfu/ml, where the permitted value is 100 cfu/ml and total fungal count was ranged from 5.5 × 105 - 56.5 × 105 cfu/ml, 21.5 × 105 - 54.5 × 105 cfu/ml and 32.5 × 105 - 68.5 × 105 cfu/ml, where the permitted value is 1000 cfu/ml. Association of Depression and Life Satisfaction with Low Resilience among Married Women of Karachi, Pakistan

Background: The concept of resilience is very crucial in promoting positive psychological well-being. However, this construct was never looked among married women of Karachi, Pakistan. Therefore, this study aimed to assess the prevalence and the associated risk factors of resilience in Pakistan. Methods: It was a cross-sectional survey, using the Wagnild Resilience Scale (RS) to assess resilience, Beck Depression Inventory II (BDI-II) for measuring depression and Trait Wellbeing Inventory for determining Life Satisfaction. Systematic sampling was employed to enroll 636 participants of aged 20 to 40 years living in two urban squatter settlements of Karachi, Pakistan. Prevalence ratio was computed with their 95% confidence interval. Results: A total of 636 married women participated in the study. The average age of females with low resilience was 29.8 (5.7) whereas the mean age of females with high resilience was 31.1 (5.7). Around 90% of all the participants could speak in Urdu. The prevalence of low resilience among women was 21.9%. Moreover, the prevalence of depression among low resilience group was 43.9% whereas the mean life satisfaction score among females with low resilience was lower than females with high resilience. The females who had low resilience were younger and had no formal/informal education as compared to their counterparts. After controlling for other variables, the prevalence of low resilience was 1.78 times more among depressed females as compared to the non-depressed with a 95% CI: (1.27-2.51). Moreover with every one unit increase in the life satisfaction scores, the prevalence of low resilience decreased 9%. Furthermore, age and informal schooling were also found to be significantly associated with resilience. Conclusion: Depression and life satisfaction are the potential modifiable risk factors for resilience and hence we can improve resilience through interventions that may focus on reducing depression and improving satisfaction towards life. Factors Affecting Depression among Married Women Living in Urban Squatter Settlements of Karachi, Pakistan

Background: Depression is one of the growing public health concerns among women worldwide. This is one of the most under-recognized and under-treated mental illnesses worldwide. Women of developing countries are inexplicably affecting with depression. Purpose: The purpose of study is to assess the prevalence and associative factors of depression among women of aged 20 to 40 years living in urban squatter settlements of Karachi, Pakistan. The identification of all the potential determinants will potentially help in formulating preventive strategies in order to decline the prevalence of depression among women and improve the wellbeing of women. Methods: An analytical cross-sectional survey was employed among women living in Reta Plot and Kala Board communities of Karachi, Pakistan. Beck Depression Inventory II (BDI-II) was used to measure depression. Systematic sampling was used to recruit 636 married women. Cox proportional hazard was run to compute prevalence ratios and their 95% confidence interval. Results: Our study estimated 33.3% of depression among study participants. The current study showed that age, possession of own vehicle, years of schooling and recent deaths in family were significantly associated with depression. Conclusion: This research was a bold step to address the issue of increasing rate of depression among Pakistani population. Our study results can convince policy makers to extend mental health support to women by improving accessibility of services. Recommendation: Our study recommends that the women of our society should be encouraged for attaining education. This study also recommends that at primary health care setting, every health professional should be trained to screen the depression. Moreover, the results of our study can also encourage policy makers to expand mental health support services for women.

Source identification, contamination status and health risk assessment of heavy metals from road dusts in Dhaka, Bangladesh

In this study, concentrations of Cr, Mn, Ni, Cu, Zn, Cd and Pb were determined in road dusts collected from different locations in Dhaka to assess source, contamination status and health risk. Energy-dispersive X-ray fluorescence spectroscopy and energy-dispersive X-ray spectroscopy were used to determine Cr, Mn, Ni, Cu, Zn, Cd and Pb and their mean concentrations were 162.27 ± 29.46, 721.18 ± 180.14, 35.65 ± 12.55, 104.56 ± 128.33, 515.32 ± 321.90,BDL, and 342.82 ± 591.20 mg/kg, respectively. Among the heavy metals, highest concentrations of Cu, Zn and Pb were found at urban sites-7(municipal waste dumping) and 8(medical waste incineration). Highest concentration of Cr followed by Cu and Zn was found at site-5(Tejgaon, urban). Principal component analysis revealed that anthropogenic activities are the potential sources for Cr, Ni, Cu, Zn and Pb while earth crust for Mn. Pollution index and pollution load index results suggested that all the sites were contaminated and/or degraded by Cr, Cu, Zn and Pb except sites-9(urban), 10(sub-urban), 11(rural) while sites-7 and 8(urban) were extremely degraded. For noncarcinogenic health risk, hazard quotient values for dermal were higher compared to that of inhalation/ingestion. Though hazard index values were less than 1 at all the sites, these were at least one order of magnitude higher for children group than that of adult group, thus the children group may face more noncarcinogenic health risk at sites -7 and 8. Values of incremental lifetime cancer risk were from 10to 10showed no carcinogenic health risk by road dusts contaminated with the heavy metals.

Adherence in Patients with Type 2 Diabetes and Depression and the Role of Physician-Patient Relationship: A Cross-Sectional Study

Background: Diabetes mellitus (DM) challenges health and quality of life of patients, families, and communities. Patients with comorbid depression are more likely to develop macrovascular and microvascular complications. The aim was to assess glycemic control and adherence in diabetic patients with comorbid depression. Further, the study evaluated the relationship between adherence and the physician-patient relationship. Methods: The study was conducted at Al-Agouza Family Medicine Center (AFMC) between February 2018 and March 2020. The included patients were between 35 - 80 years of age;had type 2 diabetes with hemoglobin A1c (HbA1c) ≥ 6.5%, fasting plasma glucose ≥ 126 mg/dl, and scored between 11 - 30 on the Beck Depression Inventory (BDI). Logistic regression, chi-square, and analysis of variance (ANOVA) were used to assess the relationship between depression, adherence, physician-patient relationship, and other variables. Results: The study included 100 eligible patients with a median BDI score of 20 (10 - 30). The median diabetic panel for patients was FBS 188 (126 - 348) mg/dl, PPS 282.50 (162 - 448) mg/dl, and HbA1c 9.5 (6.6 - 14.0)%. Depression and regular follow-up visits were statistically associated with improvement of diabetes symptoms (p = 0.019). There was a significant relationship (p 0.001) between adherence, regular follow-up visits, and knowledge of DM. Further, there was a significant relationship between the physician-patient relationship and DM improvement (p = 0.047). Conclusion: Physician-patient relationship was paramount to improving adherence and positive diabetes care. Our findings suggest a shift to a physician-patient relationship model with mutual agreement on medical decisions is highly recommended.

Warped product manifolds are known to have applications in physics. For instance, they provide an excellent setting to model space-time near a black hole or a massive star （cf. [9]）. The studies on warped product manifolds with extrinsic geometric point of view were intensified after the B.Y. Chen＇s work on CR-warped product submanifolds of Kaehler manifolds （cf. [6], [7]）. Later on, similar studies were carried out in the setting of 1.c.K. manifolds and nearly Kaehler manifolds （el. [3], [11]）. In the present article, we investigate a larger class of warped product submanifolds of 1.c.K. manifolds, ensure their existence by constructing an example of such manifolds and obtain some important properties of these submanifolds. With regard to the CR-warped product submanifold, a special case of generic warped product submanifolds, we obtain a characterization under which a CR-submanifold is reducesd to a CR-warped product submanifold.

Computational Determination of the Binding Mode of α-Conotoxin to Nicotinic Acetylcholine Receptor

Conotoxins belong to the large families of disulfide-rich peptide toxins from cone snail venom, and can act on a broad spectrum of ion channels and receptors. They are classified into different subtypes based on their targets. The α-conotoxins selectively inhibit the current of the nicotinic acetylcholine receptors. Because of their unique selectivity towards distinct n ACh R subtypes, α-conotoxins become valuable tools in n ACh R study. In addition to the X-ray structures of α-conotoxins in complex with acetylcholine-binding protein, a homolog of the n ACh R ligand-binding domain, the high-resolution crystal structures of the extracellular domain of the α1 and α9 subunits are also obtained. Such structures not only revealed the details of the configuration of n ACh R, but also provided higher sequence identity templates for modeling the binding modes of α-conotoxins to n ACh R. This mini-review summarizes recent modeling studies for the determination of the binding modes of α-conotoxins to n ACh R. As there are not crystal structures of the n ACh R in complex with conotoxins, computational modeling in combination of mutagenesis data is expected to reveal the molecular recognition mechanisms that govern the interactions between α-conotoxins and n ACh R at molecular level. An accurate determination of the binding modes of α-conotoxins on ACh Rs allows rational design of α-conotoxin analogues with improved potency or selectivity to n ACh Rs.
