Comparison with Surgical Findings for the Accuracy of Routine MRI in Rotator Cuff Tears

Objective: To evaluate the diagnostic efficacy of magnetic resonance imaging (MRI) for the detection of partial-thickness rotator cuff tears (PTT) and full-thickness rotator cuff tears(FTT) by comparing its findings with surgical findings as the gold standard and to improve the previous MRI accuracy in diagnosing rotator cuff tears (RCT) considering more variables. Methods: In 45 months, 804 patients underwent MRI shoulder joint. Among them, only 95 cases had undergone both MRI imaging and surgery accordingly. The patient records were evaluated retrospectively if MRI and surgery were performed within 40 days of MRI. MRI findings were categorized into PTT, FTT and no tears which were further divided into different types according to four main nominal data as variables viz. site, size, shape and muscle involvement in RCT and were correlated with surgical findings for statistical calculation by using Kappa coefficient and McNemar Bowker test. Results: 81 patients (86 RCTs) underwent surgery within 40 days. On the basis of site as variable, MRI correctly depicted 100% of full thickness tears(FTT), 85% of bursal partial thickness tears(PTT), 80.4% of articular partial thickness tears(PTT). The consistency in diagnosis of RCT between MRI and surgery was moderate (Kappa coefficient 0.645). Overall sensitivity, specificity and accuracy of MRI for diagnosing PTT was 87.3%, 53.3% and 81.3%;and that for FTT was 100%, 98.7% and 98.8% respectively. Likewise on the basis of size, shape and muscles involved, the consistency between MRI and surgery was poor for size and shape and moderate for muscles involved;and the difference in diagnosing RCT by MRI and surgery was significant for shape (P = 0.002) only, but not significant for size (P = 0.16) and for muscles involved (P = 0.206) respectively. 甘达基河流域(Gandaki River Basin,GRB)是喜马拉雅中部地区的一部分,该地区栖息着许多珍稀的野生动物。由于气候和人类活动的影响,许多珍稀保护物种的生境处于危险之中。本研究基于最大熵(MaxEnt)模型,运用生物气候、土地覆被和DEM数据,分析各环境要素对棕尾虹雉(Lophophorusimpejanus)的生境适宜性的影响,评估棕尾虹雉现在状况和未来栖息地分布的变化。研究表明,目前棕尾虹雉的高度适宜栖息地面积约为749 km^2,主要分布在流域北部、东部和西部,尤其是郎塘国家公园、马纳斯卢峰自然保护区和安纳布尔纳峰自然保护区等保护区内。到2050年,棕尾虹雉的高度适宜栖息地面积将减少至561 km^2,主要在流域北部和西北部(即Chhyo,Tatopani,Humde和Chame地区)。未来环境变化的模拟表明,由于适宜栖息地面积的减少,棕尾虹雉面临的生存风险将增加。

Treating morbid obesity in cirrhosis: A quest of holy grail

The problem of obesity is increasing worldwide in epidemic proportions; the situation is similarly becoming more common in patients with cirrhosis which negatively affect the prognosis of disease and also makes liver transplantation difficult especially in the living donor liver transplantation setting where low graft to recipientweight ratio negatively affects survival. Treatment of obesity is difficult in cirrhosis due to difficulty in implementation of lifestyle measures, limited data on safety of anti-obesity drugs and high risk of surgery. Currently approved anti-obesity drugs have limited data in patients with cirrhosis. Bariatric surgery remains an option in selected compensated cirrhotic patients. Endoscopic interventions for obesity are emerging and are quite promising in patients with cirrhosis as these are minimally invasive. In present review, we briefly discuss various modalities of weight reduction in obese patients and their applicability in patients with cirrhosis.

Structural and Electrical Characteristics of Ba(Fe0.5Nb0.5)O3-SrTiO3Ceramic System

A complex structure of barium iron niobate, Ba(Fe0.5Nb0.5)O3 (BFN) and strontium titanate SrTiO3 (ST) was fabricated by a solid-state reaction method. The phase formation of Ba(Fe0.5Nb0.5)O3-SrTiO3 was checked using X-ray diffraction (XRD) technique. The X-ray structural analysis of BFN and BFN-ST ceramics, showed the formation of single-phase compound in the monoclinic system, which is a distorted structure of an ideal cubic perovskite. Careful examination of microstructures of the individual compounds of the system was done by the scanning electron micrograph (SEM), and confirms the polycrystalline nature of the systems. Detailed studies of dielectric and electrical impedance properties of the systems in a wide range of frequency (100Hz - 5MHz) and different temperatures (30°C-285°C) showed that these properties are strongly dependent on temperature and frequency.

Planetary-Scale Wave Structures of the Earth's Atmosphere Revealed from the COSMIC Observations

GPS radio occultation（GPS RO） method,an active satellite-to-satellite remote sensing technique,is capable of producing accurate,all-weather,round the clock,global refractive index,density,pressure,and temperature profiles of the troposphere and *** study presents planetary-scale equatorially trapped Kelvin waves in temperature profiles retrieved using COSMIC（Constellation Observing System for Meteorology,Ionosphere,and Climate） satellites during 2006-2009 and their interactions with background atmospheric *** is found that the Kelvin waves are not only associated with wave periods of higher than 10 days（slow Kelvin waves） with higher zonal wave numbers（either 1 or 2）,but also possessing downward phase progression,giving evidence that the source regions of them are located at lower altitudes.A thorough verification of outgoing longwave radiation（OLR） reveals that deep convection activity has developed regularly over the Indonesian region,suggesting that the Kelvin waves are driven by the convective *** derived Kelvin waves show enhanced（diminished） tendencies during westward（eastward） phase of the quasi-biennial oscillation（QBO） in zonal winds,implying a mutual relation between both of *** El Nino and Southern Oscillation（ENSO） below 18 km and the QBO features between 18 and 27km in temperature profiles are observed during May 2006-May 2010 with the help of an adaptive data analysis technique known as Hilbert Huang Transform（HHT）.Further,temperature anomalies computed using COSMIC retrieved temperatures are critically evaluated during different phases of ENSO,which has revealed interesting results and are discussed in light of available literature.

Expression of PCNA in oral gingival epithelium of aggressive and, chronic periodontitis—A pilot study

Periodontal disease leads to loss of tissue homeostasis of tooth supporting structures. The mechanism of fibrous tissue degradation by various enzymes in chronic periodontitis and aggressive periodontitis was well documented but there is paucity of information regarding epithelial response. The aim of the present study was to evaluation of epithelial cell proliferation in Chronic Periodontitis, Aggressive Periodontitis and normal patients using Proliferating Cell Nuclear Antigen (PCNA). The study consists of 30 individuals who attended the Periodontia clinic of Vishnu Dental College. They were divided equally into 3 groups: chronic periodontitis (10) aggressive periodontitis (10) and normal individuals requiring gingival excision for crown lengthening (10). Excised gingival tissues were routinely processed and subjected to Immuno-histochemical evaluation using Proliferating Cell Nuclear Antigen (PCNA). The mean proliferation index was almost equal in Aggressive periodontitis patients (36.55) and normal individuals (37.02) but it was high in Aggressive Periodontitis patients when compared to chronic periodontitis group (18.86). Observations of this study indicate the possibility of subclinical inflammation in clinically normal gingiva and difference in the types of microorganism associated with chronic periodontitis and aggressive periodontitis.

A Study of Differential Gene Expression and Core Canonical Pathways Involved in Rhenium Ligand Treated Epithelial Mesenchymal Transition (EMT) Induced A549 Lung Cancer Cell Lines by INGENUITY Software System

Rhenium compounds have shown anti-cancer properties against many different types of cancer cell lines;however, the cellular signaling mechanisms involved in the cytotoxic properties of rhenium-based compounds were never deciphered or reported. In this manuscript, we report the results of an investigation done by RNA sequencing of rhenium treated A549 lung cancer cell lines along with an untreated vehicular control, analyzed by the Ingenuity Pathway Analysis (IPA) software system to decipher the core canonical pathways involved in rhenium induced cancer cell death. A549 EMT lung cancer cell lines were treated with rhenium ligand (Tricarbonylperrhenato(bathocuproine)rhenium(I), PR7) for seven days along with vehicular control. RNA was isolated from the treated and control cells and sequenced by a commercial company (PrimBio Corporation). The RNA sequencing data was analyzed by the INGNUITY software system and the core canonical pathways involved with differential gene expression were identified. Our report is showing that there are several cellular pathways involved in inducing cell death by rhenium-based compound PR7.

DEAD盒蛋白家族的ATP依赖性的RNA解旋酶类参与细胞内几乎所有的RNA代谢过程 ,在几乎所有生物的细胞生长发育过程中扮演着众多不可或缺的角色。在本实验中 ,通过PCR和探针杂交相结合的筛选方法 ,筛选恶性疟原虫 (Plasmodiumfalciparum)的基因组文库 ,克隆了FH1F———abstrakt同源基因的完整序列。通过搜索已经完成测序的恶性疟原虫基因组数据库 ,推测FH1F序列定位在第 5条染色体上。FH1F全长2 80 4bp,包含一个 1 1 6 1bp的完整阅读框 ,编码一个由 386个氨基酸组成的蛋白。对FH1F蛋白序列用BlastP进行搜索和分析以及用DNAStar与许多典型的DEAD盒蛋白序列进行比对分析 ,结果均提示FH1F蛋白应该是DEAD盒家族的一个Abstrakt蛋白。另一方面 ,用DNAStar对已知所有完整的DEAD盒蛋白进行详细的序列分析以及用Mega对这些序列进行系统发育研究的结果都显示 :DEAD盒家族的蛋白聚类成为若干不同的亚群 ;与DEAD盒蛋白的一般保守序列相比 ,Abstrakt,eIF 4A ,Vasa ,P6 8等不同亚群的DEAD盒蛋白在保守区具有各自不同的结构特征。本文对不同的DEAD盒蛋白的结构特征进行了总结并试图给出不同亚群分类上的结构标准。

尼泊尔的社区森林计划是十分成功的森林保护计划之一。森林生态系统中的树种在维持植被结构、复杂性和异质性方面起着重要的基础性的作用。本研究分析了尼泊尔拉姆琼县(Lamjung District)亚热带地区的5个社区森林(Community Forest,CF)中树种的组成和更新,为今后的长期研究提供了基础数据,并帮助社区科学地管理社区森林。本研究使用35个样方(20 m×20 m),通过分层随机抽样生成树种数据。结果表明,森林中成年树的密度在575 Ind ha^(-1)(Tilahar CF)到1196 Ind ha^(-1)(Deurali Thadopakha CF)之间,幼树的生长范围为2533 Ind ha^(-1)(Tilahar CF)到4000 Ind ha^(-1)(Thuliban CF),幼苗为19583 Ind ha^(-1)(Tilahar CF)到37500 Ind ha^(-1)(Thuliban CF);成年树种的胸高断面积为28.34 m^(2)ha^(-1)(Tilahar CF)至49 m^(2)ha^(-1)(Deurali CF);成年树种的Shannon-Weiner‘sH指数为1.08(ThulibanCF)到1.88(TilaharCF);异序乌桕(Sapium insigne)、孟加拉榕树(Ficus benghalensis)、西藏长叶松(Pinus roxburghii)、合欢属(Albizia sp.)以及紫薇属的Lagerstroemia parviflore更新较差。总的来说,除了Tilahar社区森林之外,其余社区森林均具有良好的更新,但根据胸径大小等级分布图来看,均没有可持续的更新。在5个社区森林中,成年植株的胸径大小有显著差异,其中Deurali Thadopakha社区森林的成年植株胸径最小。社区森林中的大多数树种更新能力较弱,较少的树种如粗娑罗双(Shorea robusta)、印度锥(Castanopsis indica)以及西南木荷(Schima wallichii)以优势地位维持了社区森林的整体再生,因此在维护社区森林时需要对更新能力较差的物种进行进一步的区分以制定科学的种植管理计划。
