Utility of pancreatography for diagnosing autoimmune pancreatitis

AIM: To identify pancreatographic findings that facilitate differentiating between autoimmune pancreatitis (AIP) and pancreatic cancer (PC) on endoscopic retrograde cholangiopancreatography (ERCP) and magnetic resonance cholangiopancreatography (MRCP). METHODS: ERCP findings of 48 AIP and 143 PC patients were compared. Diagnostic accuracies for AIP by ERCP and MRCP were compared in 30 AIP patients. RESULTS: The following ERCP findings suggested a diagnosis of AIP rather than PC. Obstruction of the main pancreatic duct (MPD) was more frequently detected in PC (P < 0.001). Skipped MPD lesions were detected only in AIP (P < 0.001). Side branch derivation from the narrowed MPD was more frequent in AIP (P < 0.001). The narrowed MPD was longer in AIP (P < 0.001), and a narrowed MPD longer than 3 cm was more frequent in AIP (P < 0.001). Maximal diameter of the upstream MPD was smaller in AIP (P < 0.001), and upstream dilatation of the MPD less than 5 mm was more frequent in AIP (P < 0.001). Stenosis of the lower bile duct was smooth in 87% of AIP and irregular in 65% of PC patients (P < 0.001). Stenosis of the intrahepatic or hilar bile duct was detected only in AIP (P = 0.001). On MRCP, diffuse narrowing of the MPD on ERCP was shown as a skipped non-visualized lesion in 50% and faint visualization in 19%, but segmental narrowing of the MPD was visualized faintly in only 14%. CONCLUSION: Several ERCP findings are useful for differentiating AIP from PC. Although MRCP cannot replace ERCP for the diagnostic evaluation of AIP, some MRCP findings support the diagnosis of AIP.

Postsurgical radiation therapy for gastric carcinosarcoma with c-kit expression: A case report

Ansa pancreatica as a predisposing factor for recurrent acute pancreatitis

AIM To determine the non-biased prevalence and clinical significance of ansa pancreatica in patients with acute pancreatitis using magnetic resonance imaging(MRI).METHODS Our institutional review board approved this crosssectional study, which consisted of a community-based cohort of 587 consecutive participants in a whole-body health-check program, and 73 subjects with episode of acute pancreatitis(55 patients with a single episode of acute pancreatitis, and 18 patients with recurrent acute pancreatitis). All of the subjects underwent abdominal MRI including magnetic resonance cholangiopancreatography, medical examinations, and blood tests. Two board-certified, diagnostic, abdominal radiologists evaluated the images, and ansa pancreatica was diagnosed based on its characteristic anatomy on *** Compared with the community group [5/587(0.85%)], patients with recurrent acute pancreatitis had a significantly higher frequency of ansa pancreatica [2/18(11.1%)](P = 0.016; OR = 14.3; 95%CI: 1.27-96.1), but not compared with patients with single-episode acute pancreatitis [1/55(1.8%)](P = 0.42; OR = 2.1; 95%CI: 0.44-19.7). Multiple logistic regression analysis using age, alcohol intake, presence of ansa pancreatica, and presence of autoimmune disease as independent covariates, revealed a significant relationship between the presence of ansa pancreatica and recurrent acute pancreatitis. The presence of autoimmune disease was also significantly associated with the onset of recurrent acute pancreatitis. On the other hand, neither age nor alcohol intake were significantly related to the onset of recurrent acute *** The present study is the first to provide robust evidence that the presence of ansa pancreatica is significantly associated with recurrent acute pancreatitis.

Magnetostratigraphy of Late Tertiary sediments from the Chinese Loess Plateau and its paleoclimatic significance

The Late Tertiary red clay/bed sediments underlying the Quaternary loess-paleosol in the Chinese Loess Plateau possesses high-resolution paleoclimatic changes related to the uplift of the Tibetan Plateau. Magnetostratigra-phy and susceptibility measurements are discussed in this paper. The paleomagnetic results show that the red clay/bed began to accumulate at about 8.1 Ma, which represents the oldest Late Tertiary deposit continuously in the central Loess Plateau. The magnetic susceptibility curves show stepwise increases since the initiation of red clay, superimposed on several peaks. Moreover, the magnetic susceptibility increased abruptly since 3.8 Ma, which probably indicates the inception of the modern East Asia monsoon system. Between 3.8 and 2.6 Ma, the stepwise increases of susceptibility may imply progressively intensified East Asia summer monsoon activity related with the stepwise uplift process of the Tibetan Plateau.

Usefulness of biopsying the major duodenal papilla to diagnose autoimmune pancreatitis:A prospective study using IgG4-immunostaining

AIM： TO examine the histological and immunohistochemical findings of biopsy specimens taken from the major duodenal papilla of autoimmune pancreatitis （AIP） patients. METHODS： The major duodenal papilla in the resected pancreas of 3 patients with AIP and of 5 control patients [pancreatic carcinoma （n = 3） and chronic alcoholic pancreatitis （17 = 2）] was immunostained using anti-CD4-T cell, CD8-T cell and IgG4 antibodies. Forceps biopsy specimens taken from the major duodenal papilla of 2 patients with AIP and 5 control patients with suspected papillitis were prospectively taken during duodenoscopy and immunohistochemically examined. RESULTS： Moderate or severe Iymphoplasmacytic in- filtration including many CD4-positive or CD8-positive T lymphocytes and IgG4-positive plasma cells （≥10/HPF）, was observed in the major duodenal papilla of all 3 patients with AIR The same findings were also detected in the biopsy specimens taken from the major duodenal papilla of 2 patients with/kiP, but in controls, there were only a few （≤3/HPF） IgG4-positive plasma cells infiltrating the major duodenal papilla. CONCLUSIONS： An abundant infiltration of IgG4-positive plasma cells is specifically detected in the major duodenal papilla of patients with A/P. Although this is a preliminary study, IgG4-immunostaining of biopsy specimens taken from the major duodenal papilla may support the diagnosis of AIR

Malignancies associated with intraductal papillary mucinous neoplasm of the pancreas

AIM： As intraductal papillary mucinous neoplasm （IPMN） has a favorable prognosis, associated malignancies have potential significance in these patients. We examined the incidence and characteristics of pre-existing, coexisting and subsequent malignancies in patients with IPMN. METHODS： Seventy-nine cases of IPMN were diagnosed by detection of mucous in the pancreatic duct during endoscopic retrograde pancreatography. Histological diagnosis was confirmed in 30 cases （adenoma （n = 19） and adenocarcinoma （n = 11）. Other primary malignancies associated with IPMN, occurring in the prediagnostic or postdiagnostic period, were investigated. Postdiagnostic follow-up period was 3.3±0.5 years （range, 0.2-20 years）. RESULTS： Other 40 malignancies occurred in 28 patients （35%）. They were found before （n = 15）, at （n = 19） and after （n = 6） the diagnosis of IPMT. Major associated malignancies were gastric cancer （n = 12）, colonic cancer （n = 7）, esophageal cancer （n = 4）, pulmonary cancer （n = 4）, and independent pancreatic cancer （n = 3）. Pancreatic cancer was synchronous with IPMN in two patients and metachronous in one （3 years after diagnosis of IPMN）. Thirty-one lesions were treated surgically or endoscopically. Fourteen patients died of associated cancers. Development of other malignancies was related to age （71.9±8.2 v566.8±9.3, P〈0.05）, but not to gender or site of the tumor. CONCLUSION： IPMN is associated with a high incidence of other malignancies, particularly gastric and colonic cancers. Common genetic mechanisms between IPMN and other associated malignancies might be present. Clinicians should pay attention to the possibility of associated malignancies in preoperative screening and follow-up of patients with IPMN. 2005 The WJG Press and Elsevier Inc. All rights reserved

Crustal Signature of δ^(13)C and Nitrogen Content in Microdiamonds from Erzgebirge, Germany: Ion Microprobe Studies

The study of δ13CCpDB （Pee Dee Belemnite） and nitrogen contents in 1 to 5-pm-diameter microdinmonds included in garnets from the qnartz-feldspathic gneisses （Erzgehirge, Germany） was performed in situ with the Nano-scale Secondary Ion Mass Spectrometer. The results revealed that there were two stages of diamond crystallization from a C-O-H supeeritical fluid rich in biogenic carbon and diverse minor elements of crustal origin. The δ13CCpDB Of the Erzgebirge diamond of the first stage falls in the range -17‰ to 19‰, with an average value of -17.8‰; the average content of nitrogen is 820 ppm. Diamonds of the second stage are characterized by δ13CPDB=-21.5‰ to -25.5‰, with an average value of -23.24‰; the average nitrogen content is non-homogeneously scattered from 740 ppm to 3 370 ppm among 6 diamonds situated in garnets within the same polished rock slide. Both diamond of the first stage and diamond of the second stage carbon reservoirs belong to biogenic matter, therefore confirming deep subduction of the continental crust sediments and their subsequent exhumation during the Variscan orogeny.

Nicotine Treatment Induces Expression of Matrix Metalloproteinases in Human Osteoblastic Saos-2 Cells

Tobacco smoking is an important risk factor for the development of severe ***,we showed that nicotine affected mineralized nodule formation,and that nicotine andlipopolysaccharide stimulated the formation of osteoclast-like cells by increasing production of macrophagecolony-stimulating factor （M-CSF） and prostaglandin E2（PGE2） by human osteoblastic Saos-2 *** thepresent study,we examined the effects of nicotine on the expression of matrix metalloproteinases （MMPs）,tissue inhibitors of matrix metalloproteinases （TIMPs）,the plasminogen activation system including thecomponent of tissue-type plasminogen activator （tPA）,urokinase-type PA （uPA）,and PA inhibitor type 1（PAI-1）,α7 nicotine receptor,and *** also examined the effect of the nicotine antagonist D-tubocurarineon nicotine-induced expression of *** expression was examined using real-time polymerase chainreaction （PCR） to estimate mRNA *** addition,expression of the MMP,TIMP,uPA,tPA,and PAI-1proteins was determined by Western blotting *** treatment caused expression of MMP-1,2,3,and 13,but not MMP-14,to increase significantly after 5 or 10 d of culture;MMP-14 expression did notchange through day *** of MMP-1 expression by nicotine treatment was eliminated bysimultaneous treatment with *** the presence of nicotine,expression of uPA,PAI-1,orTIMP-1,2,3,or 4 did not change over 14 d of culture,whereas expression of tPA increased significantly byday *** also increased expression of the α7 nicotine receptor and c-fos *** results suggestthat nicotine stimulates bone matrix turnover by increasing production of tPA and MMP-1,2,3,and 13,thereby tipping the balance between bone matrix formation and resorption toward the latter process.

Previous studies have indicated that lipopolysaccharide（LPS）from Gram-negative bacteria inplaque induces the release of prostaglandin E2（PGE2）,which promotes alveolar bone resorption in periodontitis,and that tobacco smoking might be an important risk factor for the development and severity of *** determined the effect of nicotine and LPS on alkaline phosphatase（ALPase）activity,PGE2production,and the expression of cyclooxygenase（COX-1,COX-2）,PGE2receptors Ep1-4,and macrophage colonystimulating factor（M-CSF）in human osteoblastic Saos-2 *** cells were cultured with 10-3M nicotinein the presence of 0,1,or 10μg/ml LPS,or with LPS *** activity decreased in cells cultured withnicotine or LPS alone,and decreased further in those cultured with both nicotine and LPS,whereas PGE2production significantly increased in the former and increased further in the *** itself,nicotine did notaffect expression of COX-1,COX-2,any of the PGE2receptors,or M-CSF,but when both nicotine and LPSwere present,expression of COX-2,Ep3,Ep4,and M-CSF increased *** addition of10-4M indomethacin eliminated the effects of nicotine and LPS on ALPase activity,PGE2production,and M-CSF *** of protein kinase A was high in cells cultured with nicotine and *** suggest that LPS enhances the production of nicotine-induced PGE2by an increase in COX-2 expres-sion in osteoblasts,that nicotine-LPS-induced PGE2interacts with the osteoblast Ep4 receptor primarily inautocrine or paracrine mode,and that the nicotine-LPS-induced PGE2then decreases ALPase activity andincreases M-CSF expression.
