Psychological Preparation of Children for Surgery: Awareness Survey Targeting Medical Professionals

The objective of this study was to conduct a nation-wide survey to investigate the state of: 1) the prevalence of psychological preparation of children undergoing surgery;and 2) the awareness of psychological preparation of children by medical professionals (physicians and nurses). We also aimed to identify the issues that need to be addressed next. A total of 178 physicians and 291 nurses working in hospitals where children undergo surgeries participated in this study. Anonymous self-administered questionnaire survey. Physicians and nurses in Japan are aware of the need to provide age-appropriate psychological preparation for children undergoing medical procedures. In current practice, however, our study revealed that surgical orientations are given solely to parents in the absence of the child in approximately 30% of cases. While approximately 60% of respondents felt that the uses of “children’s picture books and booklets” are good methods for delivering explanations to children, only about 20% of respondents were implementing psychological preparation specific to each age group, and half of the respondents were providing verbal explanations alone. Meanwhile, 77% responded that it is “time-consuming” and 48% said, “on the contrary, it may heighten their anxiety”. Also 34.3% said that they “do not know how to explain to a child” and 54.8% of these medical professionals worked in mixed wards. We observed a gap between ideal requirements and current practices. We found that it is essential to: 1) organize training sessions for knowledge acquisition;2) consciously strive to be informed of the actual post-surgery conditions and outcomes of the children;and 3) acquire physical and financial support.

Role of home monitoring with iCare ONe rebound tonometer in glaucoma patients management

AIM: To evaluate intraocular pressure(IOP) measurements and fluctuations using the i Care ONe rebound tonometer(RT-ONe), during home monitoring, in diagnosed and suspected glaucoma ***: A retrospective case series of consecutive patients with known glaucoma or glaucoma suspects who were followed-up and treated between January 2016 and January 2017. The study included 80 eyes of 40 patients with a mean age of 59.1±14.6 y(range, 24-78). All patients have undergone 4-5 d of IOP home monitoring with RT-ONe at morning, noon, afternoon, and night ***: Baseline mean IOP, as measured in the clinic(8 a.m.-12 p.m.), was 17.4±5.1 mm Hg, compared to RT-ONe home monitoring mean IOP of 15.6±4.1 mm Hg(P=0.002). Mean IOP was significantly lower at noon, afternoon and night times compared to clinic measured IOP and morning measurements(P=0.005). IOP peak measured during home monitoring was significantly higher compared to the clinic measured IOP(21.3±5.6 mm Hg and 17.4±5.1 mm Hg, Peaks during home monitoring demonstrated a majority of 47 peaks during morning measurements, compared to 23 at noon, 19 at afternoon and only 12 at night(Pe home monitoring results led to treatment modification of 44 eyes(55%), treatment regime was insufficient for 40(50%) ***: Home monitoring IOP with RT-ONe can provide good assessment of mean IOP, IOP fluctuations and peaks throughout the hours of the day, which lead to an accurate treatment for glaucoma patients.

Background Although recent data in healthy humans suggestthat treatment with intranasal oxytocin(0T)may facilitate extinction recall,to date,little is known about the effects of 0T on memory *** To examine the effect of intranasal administration of OT compared with placebo on memory consolidation blockade of a de novo fear memory in a classical 2-day fear conditioning *** There were no significant differences between the 0T and the placebo groups on the first two extinction trials(mean(SD)=0.01(0.39)vs 0.15(0.31),t=-1.092,p=0.28).Similarly,during early extinction,analysis of variance for repeated measures failed to show significant main effects of extinction trials:trials(F(4,112)=1.58,p=0.18),drug(F(1,112)=0.13,p=0.72)or drug x trials interaction(F(4,112)=0.76,p=0.56).Conclusion Our results suggest that 0T administered in a double-blind fashion immediately after fear conditioning does not significantly reduce consolidation of fear learning as measured by a differential skin conductance response tested at the beginning of extinction.

Literature Review of Nursing education Promoting Professional Socialization among Nursing Students

The aim of this review was to clarify the elements that promote socialization of nursing professionals from the findings of previous studies, with keywords related to socialization among undergraduate nursing students, and obtain indicators for promoting professional socialization in nursing students in the current nursing education. The literature review was conducted using a qualitative analysis of the promotion of professional socialization among nursing students. The results were reported according to the reporting checklist of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement. The authors conducted a literature search using electronic databases in July 2021, such as CINAHL (eBSCOhost), MeDLINe (eBSCOhost) for english articles, and Ichushi for Japanese articles. Included studies were from 1985 to 2021. Previous studies were integrated to current studies. A total of 23 articles in english and 34 articles in Japanese were analyzed. The methods for promoting professional socialization in nursing students include self-learning, online media, extracurricular elements, and clinical practice. Socialization process differed for each school year;the first-year students tend to have ideological answers, while the fourth-year students tend to have practical answers as a result of education. The process needs supports from instructors and mentors. Nursing education promoting professional socialization in nursing students has developed diverse modeling, positive effects for improving social skills, and impact of self-efficacy and self-esteem and has fostered professional commitment and professional identity. However, loss of idealism of professional identity after graduation should be considered with supports from instructors and mentors. To establish nursing professional identity smoothly, nursing instructors supporting skills should be developed by understanding students’ stage of professional socialization.
