Pancreatectomy and splenectomy for a splenic aneurysm associated with segmental arterial mediolysis

Segmental arterial mediolysis(SAM) is characterized by intra-abdominal, retroperitoneal bleeding or bowel ischemia, and the etiology is unknown. A 44-year-old man complaining of abdominal pain was admitted to our hospital. He had been admitted for a left renal infarction three days earlier and had a past medical history of cerebral aneurysm with spontaneous remission. The ruptured site of the splenic arterial aneurysm was clear via a celiac angiography, and we treated it using trans-arterial embolization. Unfortunately, the aneurysm reruptured after two weeks, and we successfully treated it with distal pancreatomy and splenectomy. We recommended a close follow-up and prompt radiological or surgical intervention because SAM can enlarge rapidly and rupture.

We report two cases of delayed esophageal perforation occurring with endoscopic submucosal dissection. Our cases involved delayed perforation after 10 d in case 1 and after 6 d in case 2. Both cases were related to solid food. We performed subtotal esophagectomy with gastric tube reconstruction of the esophagus via the subcutaneous route anterior to the thoracic wall without conservative treatment because both cases involved chest pain and major leakage of food into the mediastinum. Postoperative complications were a local factor(including suture failure and esophageal stricture) in case 1, and we performed endoscopic balloon dilatation five times for esophageal stricture. There was no intrathoracic and mediastinal infection in either case. Surgical treatment for delayed esophageal perforation can be performed safely and surely if diagnosis and assessment are not delayed.

Rice is a staple food for half of the world’s population,but it is a poor dietary source of calcium(Ca)due to the low concentration.It is an important issue to boost Ca concentration in this grain to improve Ca deficiency risk,but the mechanisms underlying Ca accumulation are poorly understood.Here,we obtained a rice(Oryza sativa)mutant with high shoot Ca accumulation.The mutant exhibited 26%-53% higher Ca in shoots than did wild-type rice(WT)at different Ca supplies.Ca concentration in the xylem sap was 36% higher in the mutant than in the WT.There was no difference in agronomic traits between the WT and mutant,but the mutant showed 25% higher Ca in the polished grain compared with the WT.Map-based cloning combined with a complementation test revealed that the mutant phenotype was caused by an 18-bp deletion of a gene,OsK5.2,belonging to the Shaker-like K+channel family.OsK5.2 was highly expressed in the mature region of the roots and its expression in the roots was not affected by Ca levels,but upregulated by low K.Immunostaining showed that OsK5.2 was mainly expressed in the pericycle of the roots.Taken together,our results revealed a novel role for OsK5.2 in Ca translocation in rice,and will be a good target for Ca biofortification in rice.

Soft robotics is a breakthrough technology to support human-robot interactions.The soft structure of a soft robot can increase safety during human and robot interactions.One of the promising soft actuators for soft robotics is dielectric elastomer actuators(DEAs).DEAs can operate silently and have an excellent energy density.The simple structure of DEAs leads to the easy fabrication of soft actuators.The simplicity combined with silent operation and high energy density make DEAs interesting for soft robotics researchers.DEAs actuation follows the Maxwell-pressure principle.The pressure produced in the DEAs actuation depends much on the voltage applied.Common DEAs requires high voltage to gain an actuation.Since the power consumption of DEAs is in the milli-Watt range,the current needed to operate the DEAs can be neglected.Several commercially available DC-DC converters can convert the volt range to the kV range.In order to get a voltage in the 2-3 kV range,the reliable DC-DC converter can be pricy for each device.This problem hinders the education of soft actuators,especially for a newcomer laboratory that works in soft electric actuators.This paper introduces an entirely do-it-yourself(DIY)Ultrahigh voltage amplifier(UHV-Amp)for education in soft robotics.UHV-Amp can amplify 12 V to at a maximum of 4 kV DC.As a demonstration,we used this UHV-Amp to test a single layer of powdered-based DEAs.The strategy to build this educational type UHV-Amp was utilizing a Cockcroft-Walton circuit structure to amplify the voltage range to the kV range.In its current state,the UHV-Amp has the potential to achieve approximately 4 kV.We created a simple platform to control the UHV-Amp from a personal computer.In near future,we expect this easy control of the UHV-Amp can contribute to the education of soft electric actuators.

构建利用废弃物作资源的循环经济社会已经成为一个全世界的重要课题。日本政府在1997至2006年间,在全国26个地区实施了建设循环经济社会的示范性工程——生态工业园区工程。这一工程的目标在于发展循环利用产业的同时将地方建设成为环境和谐型社区。要实现将地方建设成环境协调性社区的目标,针对是否应该将作为原料的废弃物的确保问题完全依赖于市场竞争的做法需要作充分的探讨,循环利用相关法律制度的运用与结构中还有应当改进的问题。

BACKGROUND Goblet cell carcinoid(GCC)of the appendix is a rare tumor characterized by neuroendocrine and adenocarcinoma features.Accurate preoperative diagnosis is very difficult,with most patients complaining mainly of abdominal pain.Computed tomography shows swelling of the appendix,so diagnosis is usually made incidentally after appendectomy based on a preoperative diagnosis of appendicitis.Even if a patient undergoes preoperative colonoscopy,accurate endoscopic diagnosis is very difficult because GCC shows a submucosal growth pattern with invasion of the appendiceal wall.CASE SUMMARY Between 2017 and 2022,6 patients with GCC were treated in our hospital.The presenting complaint for 5 of these 6 patients was abdominal pain.All 5 patients underwent appendectomy,including 4 for a preoperative diagnosis of appendicitis and the other for diagnosis and treatment of an appendiceal tumor.The sixth patient presented with vomiting and underwent ileocecal resection for GCC diagnosed from preoperative biopsy.Although 2 patients with GCC underwent colonoscopy,no neoplastic changes were identified.Two of the six patients showed lymph node metastasis on pathological examination.As of the last followup(median:15 mo),all cases remained alive without recurrence.CONCLUSION As preoperative diagnosis of GCC is difficult,this possibility must be considered during surgical treatments for presumptive appendicitis.

Gustav Senn(1875–1945): The pioneer of chloroplast movement research

Gustav Senn analyzed for the first time light- induced movement and arrangement of chloroplasts. Using many plant species he performed physiological analyses of chloroplast migration in response to external stimuli, with emphasis on light. He determined light paths within a cell by measuring refractive indices and optical thickness of cellular compartments and confirmed that chloroplasts migrate towards the region where the light intensity is optimum. After 6 to 7 years＇ concentrated study, Senn published the famous monograph ＂Die Gestalts- und Lageveranderung der Pflanzen- Chromatophoren＂（The Changes in Shape and Position of Plant Chloroplasts） in 1908. This book has stimulated many plantphysiologists and photobiologists, because Senn not only thoroughly classified and defined various types of light-induced chloroplast migration but also already described possible interaction of different photoreceptor systems in Nlougeotia more than 50 years before the discovery of phytochrome. This book also contains still useful experimental hints and over- looked findings on the interaction between light and other factors, such as temperature, water content, and nourishment. After publishing this book, Senn retreated from the study of chloroplasts and became a researcher of the Greek philoso- pher, Theophrastus. In this review, I introduce his biographical background and then summarize some of his key research accomplishment.

Global Existence and Stability of Solutions to River Flow System

In this short note,we are concerned with the global existence and stability of solutions to the river flow system.We introduce a new technique to set up a relation between the Riemann invariants and the finite mass to obtain a time-independent,bounded solution for any adiabatic exponent.The global existence of solutions was known long ago[Klingenberg and Lu in Commun.Math.Phys.187:327-340,1997].However,since the uncertainty of the function b(x),which corresponds physically to the slope of the topography,the L∞estimates growed larger with respect to the time variable.As a result,it does not guarantee the stability of solutions.By employing a suitable mathematical transformation to control the slope of the topography by the friction and the finite mass,we prove the uniformly bounded estimate with respect to the time variable.This means that our solutions are stable.
