Increased incidence of chemoport-related thrombosis in patients with colorectal cancer receiving bevacizumab: A single-institutional experience

Objective： Chemoport-related thrombosis （CRT） is a serious complication that causes morbidities and interruptsadministration of intravenous cancer therapy. We investigated the incidence and risk of CRT in colorectal cancer（CRC） patients treated with bevacizumab （BEV）.Methods： We retrospectively reviewed 1,534 CRC patients who received chemotherapy with or without BEVusing a chemoport between 2014 and ***： The participants had a median age of 58 （18-85） years, and 60.3% were male. All participants werestratified into three groups： adjuvant chemotherapy （AC）（n=670）, palliative chemotherapy （PC） without BEV（n=356）, and PC with BEV （n=508）. The median follow-up was 20.19 （interquartile range, 14.07-27.19） *** occurred in 3.8% of all patients; incidence of symptomatic and asymptomatic CRT was 2.9% and 0.9%,respectively. CRT occurred more in patients with BEV （5.7%） than in patients without BEV （2.9%, P=0.008）. Thecumulative incidence of CRT in patients administered PC with BEV was significantly higher than that in thoseadministered AC （P=0.011） and there was a trend toward increased CRY in patients administered PC with BEVcompared with those administered PC without BEV （P=0.044）. Multivariate analysis found that BEV treatment wasthe only variable that was significandy associated with CRT （hazard ratio, 2.06; 95% confidence interval, 1.24-3.43;P=0.006）.Conclusions： BEV treatment was significandy associated with increased incidence of CRT in CRC patients.

Transforming growth factor-β1 （TGF-β1） has been identified as one of the most important fibrogenic cytokines associated with Peyronie＇s disease （PD）. The mothers against decapentaplegic homolog 7 （SMAD7） is an inhibitory Smad protein that blocks TGF-J3 signaling pathway. The aim of this study was to examine the anti-fibrotic effect of the SMAD7 gene in primary fibroblasts derived from human PD plaques. PD fibroblasts were pretreated with the SMAD7 gene and then stimulated with TGF-β1. Treated fibroblasts were used for Western blotting, fluorescent immunocytochemistry, hydroxyproline determination, and terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick-end labeling assays. Overexpression of the SMAD7 gene inhibited TGF-β1-induced phosphorylation and nuclear translocation of SMAD2 and SMAD3, transdifferentiation of fibroblasts into myofibroblasts, and quashed TGF-β1-induced production of extracellular matrix protein and hydroxyproline. Overexpression of the SMAD7 gene decreased the expression of cyclin D1 （a positive cell cycle regulator） and induced the expression of poly （ADP-ribose） polymerase 1, which is known to terminate Smad-mediated transcription, in PD fibroblasts. These findings suggest that the blocking of the TGF-β pathway by use of SMAD7 may be a promising therapeutic strategy for the treatment of PD.

Clinical outcomes of self-expandable metal stents in palliation of malignant anastomotic strictures caused by recurrent gastric cancer

AIM： To examine the technical feasibility and clinical outcomes of the endoscopic insertion of a selfexpandable metal stent （SEMS） for the palliation of a malignant anastomotic stricture caused by recurrent gastric cancer. METHODS： The medical records of patients, who had obstructive symptoms caused by a malignant anastomotic stricture after gastric surgery and underwent endoscopic insertion of a SEMS from January 2001 to December 2007 at Kangnam St Mary＇s Hospital, were reviewed retrospectively. RESULTS： Twenty patients （15 male, mean age 63 years） were included. The operations were a total gastrectomy with esophagojejunostomy （n = 12）, subtotal gastrectomy with Billroth-Ⅰ reconstruction （n = 2） and subtotal gastrectomy with Billroth- Ⅱ reconstruction （n = 8）. The technical and clinical success rates were 100% and 70%, respectively. A small bowel or colon stricture was the reason for a lack of improvement in symptoms in 4 patients. Two of these patients showed improvement in symptoms after another stent was placed. Stent reobstruction caused by tumor ingrowth or overgrowth occurred in 3 patients （15%） within 1 mo after stenting. Stent migration occurred with a covered stent in 3 patients who underwent a subtotal gastrectomy with Billroth-Ⅱ reconstruction. Two cases of partial stent migration were easily treated with a second stent or stent repositioning. The median stent patency was 56 d （range, 5-439 d）. The median survival was 83 d （range, 12-439 d）. CONCLUSION： Endoscopic insertion of a SEMS provides safe and effective palliation of a recurrent anastomotic stricture caused by gastric cancer, A meticulous evaluation of the presence of other strictures before inserting the stent is essential for symptom improvement.

Poly Adenylate Binding Protein Interacting protein 1(PAIP1) plays a critical role in translation initiation and is associated with the several cancer types. However, its function and clinical significance have not yet been described in oral squamous cell carcinoma(OSCC) and its associated features like lymph node metastasis(LNM). Here, we used the data available from Gene Expression Omnibus(GEO), The Cancer Genome Atlas(TCGA), and Clinical Proteomic Tumor Analysis Consortium(CPTAC) to analyze PAIP1 expression in oral cancer. The publicly available data suggests that PAIP1 mRNA and protein levels were increased in OSCC. The high PAIP1 expression was more evident in samples with advanced stage, LNM, and worse pattern of invasion. Moreover, the in vitro experiments revealed that PAIP1 knockdown attenuated colony forming, the aggressiveness of OSCC cell lines, decreasing MMP9 activity and SRC phosphorylation. Importantly, we found a correlation between PAIP1 and pSRC through the analysis of the IHC scores and CPTAC data in patient samples. Our findings suggest that PAIP1 could be an independent prognostic factor in OSCC with LNM and a suitable therapeutic target to improve OSCC patient outcomes.

A study was undertaken for the prediction of runoff flow from 0.8 ha field-sized agricultural watershed in South Korea using Soil and Water Assessment Tool (SWAT) sub-daily. The SWAT model with sub-daily configuration predicted flow from the watershed within the range of acceptable accuracy. The SWAT sub-daily simulations were carried out for a total of 18 rainfall events, 9 each for calibration and validation. Overall trend and extent of matching simulated flow for the rainfall events in 2007–2008 with measured data during the calibration process were coefficient of determination (R 2) value of 0.88 and Nash and Sutcliffe Efficiency (E NS) value of 0.88. For validation, R 2 and E NS values were 0.9 and 0.84, respectively. Whereas R 2 and E NS values for simulation results using daily rainfall data were 0.79 and −0.01, respectively, that were observed to be out of acceptable limits for the model simulation. The importance of higher time resolution (hourly) precipitation records for flow simulation were evaluated by comparing R 2 and E NS with 15 min, 2 h, 6 h and 12 h precipitation data, which resulted in lower statistics with increases in time resolution of precipitation data. The SWAT sub-daily sensitivity analysis was performed with the consideration of hydraulic parameter and was found as in the rank order of CN2 (curve number), ESCO (soil evaporation compensation factor), GW_DELAY (ground water delay time), ALPHA_BF (base flow alpha factor), GWQMN (a threshold minimum depth of water in the shallow aquifer required for return flow to occur), REVAPMN (minimum depth of water in shallow aquifer for re-evaporation to occur), LAT_TIME (lateral flow travel time) respectively. These sensitive parameters were evaluated at 10% higher and lower values of the parameters, corresponding to 70.5% higher and 23.2% lower in simulated flow out from the SWAT model. Large-area polydimethylsiloxane(PDMS)films with variably sized moth-eye structures were fabricated to improve the efficiency of perovskite solar *** approach that incorporated photolithography,bilayer PDMS deposition and replication was used in the fabrication *** simply attaching the moth-eye PDMS films to the transparent substrates of perovskite solar cells,the optical properties of the devices could be tuned by changing the size of the moth-eye *** device with 300-nm moth-eye PDMS films greatly enhanced power conversion efficiency of ~21 % due to the antireflective effect of the moth-eye ***,beautiful coloration was observed on the 1000-nm moth-eye PDMS films through optical interference caused by the diffraction grating *** results imply that moth-eye PDMS films can greatly enhance the efficiency of perovskite solar cells and building-integrated photovoltaics.

Community detection in social networks is a hard problem because of the size,and the need of a deep understanding of network structure and *** several methods with significant effort in this direction have been devised,an outstanding open problem is the unknown number of communities,it is generally believed that the role of influential nodes that are surrounded by neighbors is very *** addition,the similarity among nodes inside the same cluster is greater than among nodes from other ***,the global and local methods of community detection have been getting more ***,in this study,we propose an advanced communitydetection model for social networks in order to identify network communities based on global and local *** proposed model initially detects the most influential nodes by using an Eigen score then performs local expansion powered by label *** process is conducted with the same color till nodes reach maximum ***,the communities are formed,and a clear community graph is displayed to the *** proposed model is completely parameter-free,and therefore,no prior information is required,such as the number of communities,*** perform simulations and experiments using well-known synthetic and real network benchmarks,and compare them with well-known state-of-the-art *** results prove that our model is efficient in all aspects,because it quickly identifies communities in the ***,it can easily be used for friendship recommendations or in business recommendation systems.

The denitdfication and P-removal in the sorption-denitrification-P-removal （S-DN-P） process were carded out under various wastewater compositions. It is noted that P-removal largely depends on the wastewater composition as well to the quantity of the substrates present in wastewater fraction. Three different wastewater fractions are obtained as： raw wastewater, dissolved wastewater （obtained with filtration using 0.45 μm filter）, and undissolved wastewater （i.e., infiltrate obtained by above filtration）. The ratio of P-release/CODtotal-consumption clearly inferred that undissolved wastewater possess very low value i.e., 0.0008 followed by raw wastewater 0.008 and dissolved wastewater 0.03. When this ratio was nearby 0.01, enhanced P-removal was observed. Moreover, the ratio of P-uptake to NO3^--N decomposition for raw wastewater was two times for dissolved wastewater. Interestingly, it was observed that the P-removal and denitrification depend not only on the dissolved substrates but also the undissolved substrates present in the wastewater. The result of the P-removal obtained with this S-DN-P process did not show a big difference of 36%, 34% and 30%, respectively, for raw, dissolved and undissolved wastewater.

An 81-year-old man presented with epigastric pain and weight loss for one month. He had a past history of pulmonary tuberculosis, 10 years ago. We performed a gastroscopy, which showed a linear depressed whitish gastric ulcer scar (0.8 cm in length) in the posterior wall of the prepyloric antrum. The result of biopsy was reported as squamous epithelium. Immunohistochemical staining using an antibody to high molecular weight cytokeratin (HMC) revealed positive staining in the squamous epithelium. Two years later, the lesion was followed up. The lesion remained at same site endoscopically, but no squamous epithelium could be seen microscopically.
