Objective： To investigate the effects of Korean red ginseng （KRG） on semen parameters in male infertility patients in a randomized, double-blind, placebo-controlled study. Methods： A total of 80 male infertility patients with varicocele were recruited from April 2011 to February 2012. The subjects were then divided into the following four groups： non-varicocelectomy （V）＋placebo （P） group, V＋P group, non-V＋KRG group （1.5-g KRG daily）, and V＋KGR group （1.5-g KRG daily）. Semen analysis was performed and hormonal levels were measured in each treatment arm after 12 weeks. Results： All groups but not the non-V＋P group, showed significant improvements in sperm concentrations, motility, morphology, and viability at the end of the study. However, there were no significant differences in serum follicle-stimulating hormone, luteinizing hormone, and testosterone among groups. The incidence of adverse events was low, and all events were assumed to be unrelated to the treatments administered. Conclusions： Although the exact mechanism by which KRG improves spermatogenesis remains unclear, KRG may be a useful agent for the treatment of male infertility. Nevertheless, additional studies to evaluate the optimal dose and duration of treatment are needed.

The roles of E-cadherin and α-catenin were evaluated in the development of varicocele-induced infertility. Analysis of the association between the expression of E-cadherinla-catenin and clinical/pathological parameters was performed. Thirty lO-week-old male rats （experimental group） were used for the experiments; the left renal vein was ligated to form a varicocele. The abdomen was incised in 30 rats （control group） and no procedure was performed on 10 rats （baseline group）. The weights of the left testis, serum reactive oxygen species （ROS）, testosterone, luteinizing hormone （LH）, follicle-stimulating hormone （FSH） and degenerative changes in the seminiferous tubules after 4 and 8 weeks were recorded. The expression of E-cadherin and α-catenin was evaluated by immunohistochemical （IHC） staining and Western blot analysis. The ROS increased in the 8-week experimental group, compared with the baseline and control groups （P〈0.001 for both）. Additionally, FSH significantly increased in the 4- and 8-week experimental group compared with the control groups （P= 0.013 and P= 0.032, respectively）. The ratio of degenerative changes in the seminiferous tubules of the experimental groups increased. The IHC staining showed that the expression of E-cadherin and a-catenin decreased in the 4- and 8-week experimental groups. Similar to the IHC staining, the experimental group had decreased reactivity on Western blot analysis. The expression of E-cadherin and a-catenin was significantly associated with the ROS and degenerative changes in the seminiferous tubules. The results of this study suggest that damage to the blood-testis barrier （BTB） is associated with varicocele-induced male infertility, and that ROS may cause damage to the BTB.

Varioocelectomny is a management option for patients with painful varicocele. In this study, we assessed the effectiveness of varicocelectomy for painful varicocele and examined the factors that might be predictive of outcome. All patients who underwent a varicocelectomy for pain between February 2007 and July 2009 were includecL A review of patient medical records was conducted; patient age,body mass index （BMI）, uede, iocatien of the vericocele, tasticular volume, duration and quality of the pain （dull, draaing, throbbing or sharp） and surgical technique （ingulnal versus subinguinal） were documented. All parameters were compared with the rasolution of pain （complete, partial or failure）. We followed up on 53 of 104 patients （51.0%）. Complete postopanntive resolution of pain was reported by 28 patients （52.8%）, whereas 22 （41.5%） reported partial resolution, Only three patients （5.7%） mpaded failure. No relationship was observed between postoperative pain resolution and age, BMI, grade of varicoceio, location of varicocele, ipsilateral testicular, quality of pain or surgical technique. The duration of pain before surllery was the only factor that correlated with postopaative pain resolution （univariate, P=-0.004; multivariate, P=0.002）. Our rasults indicatethat varicocelectomy isan effective treatmentfar painful varicocele in properly selected patients, and that duration of pain before surgery may be predictive of outcome.

Objective: To investigate the effect of hydrogen-rich Korean Red Ginseng(KRG) water(HRGW) mixture on the spermatogenesis and sperm motility of mice of different ages. Methods: Eighty young(3 month-old) and aged(12 month-old) male mice were randomly assigned to 4 groups(n=10 per group) including control group, hydrogen-rich water(HRW) group(10 m L/kg daily), KRG group(50 mg/kg daily) and HRGW group(10 m L/kg HRW and 50 mg/kg KRG daily) by an oral zoned needle for 4 weeks. Sperm count and motility were measured using sperm suspension released from cauda epididymis. Serum follicle stimulating hormone(FSH), luteinizing hormone(LH), testosterone, and reactive oxygen species(ROS) in serum have also been estimated. Tubular changes were examined through histological hematoxylin and eosin staining. Expression of antioxidation(PPx3, PPx4, GSTm5 and GPx4), spermatogenesis(inhibin-α, neptin-2 and CREM), antiaging(SIRT1 and SIRT2), and angiogenesis [visfatin and vascular endothelial growth factor(VEGF)] related genes were examined through realtime polymerase chain reaction. Results: HRW and KRG treatment stimulated spermatogenesis followed by increasing sperm production and sperm motility(P<0.05). These effects were strengthened synergistically by a HRGW mixture(P<0.05 or P<0.01). HRGW greatly increased the expressions of antioxidation, antiaging, spermatogenesis related genes and VEGF especially in aged mice(P<0.05). Serum testosterone and FSH levels also increased, while serum ROS level decreased(all P<0.05). Conclusion: HRGW increases sperm production and motility by enhancing antioxidation and stimulating spermatogenesis and sex hormone production, particularly in aged mice.

Aim: To study the effect of rebamipide added to semen samples and cryoprotectant on reactive oxygen species (ROS) production. Methods: Semen samples from 30 fertile and healthy volunteers were collected by masturbation after 2 days - 3 days of abstinence. After liquefaction, the specimens were diluted with sperm wash media to a uniform density of 20×106/mL. Rebamipide was added to semen samples and cryoprotectant to a final concentration of 10 μmol/L, 30 μmol/L, 100 μmol/L or 300 μmol/L. Specimens were incubated at 37 ℃ in a 0.5 % CO2 incubator for 1 h or cryopreserved at -196 ℃ LN2 for 3 days. The sperm motility and viability and the levels of ROS and lipid peroxidation of sperm membrance were assessed before and after incubation and cryopreservation by means of computer assisted semen analyzer, eosin-nigrosin stain, chemiluminescence and thiobarbituric acid assay, respectively. Results: The sperm motility was significantly increased after incubation with 100 μmol/L and 300 μmol/L rebamipide (P0.05). The levels of ROS and lipid peroxidation in semen were significantly decreased in proportion to the concentrations of rebamipide both after incubation and cryopreservation (P<0.05). Conclusion: Rebamipide is an effective free radical scavenger in semen in vitro.

In this study, we aimed to investigate the prevalence and perception of premature ejaculation （PE） in young and middle-aged Korean men. The study was conducted using an Internet-assisted questionnaire. A total of 2 037 Korean male adults, aged 20 years or older, were randomly sampled based on age and residency. The questionnaire developed by the PE Study Group of the Korean Andrological Society includes four categories （overall sexual function, symptoms, distress and treatment） with a total of 16 questions. For each question, symptoms were evaluated by a scale ranging from 0 to 10. Intravaginal ejaculation latency time was ‘5-10 min＇ in 38.6%, followed by ‘longer than 10 min＇ in 2919%, ‘2-5 min＇ in 23.6%, ‘1-2 min＇ in 5.4% and ‘shorter than 1 min＇ in 2.5%. In our series, 27.5% of respondents reported having PE. Control over ejaculation within a recent 3-month period was 6.2 points on average. Respondent complaints of PE-related stress averaged 7.1 points and stress-related complaints from sexual partners averaged 7.1 points. The effect of PE on sexual life was 6.8 points. Of the respondents determined as having PE, 42.6% responded that they were inclined to receive treatment. Results from this study suggest that the prevalence of PE diagnosed by the respondent on his own was approximately 27.5% in young and middle-aged men in Korea. PE-related stress had a significant effect on the stress, sexual activity and quality of life of the respondent and his sexual partner.

Aim： To evaluate the efficacy and safety of SK3530, a newly developed type 5 phosphodiesterase inhibitor （PDE5I）, in Korean men with erectile dysfunction （ED）. Methods： A total of 119 patients were randomized at 10 centers in Korea to receive either SK3530 （50, 100, or 150 mg; n = 89） or placebo （n = 30） taken 1 h before anticipated sexual activity for an 8-week period. The patients were evaluated at baseline and 4 and 8 weeks after beginning therapy. Efficacy was assessed using the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP）, and the Global Assessment Question （GAQ）. Safety was analyzed by adverse events, laboratory values and vital signs. Results： At the end of the study, all the primary and secondary efficacy end-points were statistically significantly improved by SK3530 compared with placebo （P 〈 0.05）. Of the 89 patients in the treatment arm, 36 （42.3 %） achieved normal erectile function after treatment, including six patients with severe ED. Treatment-related adverse events occurred in 32 patients. The most common adverse events were flushing, headache, dizziness and eye redness （10.9%, 7.6%, 2.5% and 2.5%, respectively）, and most were mild. Only two patients discontinued treatment during the study period because of adverse events. Conclusion： The results of our phase Ⅱ study have confirmed the efficacy and safety of SK3530 in a broad population of men with ED of various etiologies and severity. The optimal doses in terms of efficacy and safety were determined to be 50 mg and 100 mg, respectively.

This study was undertaken to determine the impact on ejaculatory function of tamsulosin （0.2 mg） given once daily （OD） for 12 weeks and to identify risk factors for ejaculatory dysfunction in patients undergoing this treatment. Males with an International Prostatic Symptom Score （IPSS） ≥ 8 were enrolled in this study. All participants completed questionnaires, including the IPSS and the Male Sexual Health Questionnaire （MSHQ）, and serum prostate-specific antigen, transrectal ultrasound and uroflowmetry with post-void residual were measured. After initiating 0.2 mg OD tamsulosin, patients were re-evaluated on the fourth and twelfth weeks of medication. The chi-squared test, the independent t-test and one-way ANOVA were used to compare means. Binary logistic regression analysis was used to calculate the odds ratio for all risk factors. A total of 177 men constituted the study cohort. No significant difference was observed between baseline and follow-up for the erectile function, ejaculatory function, satisfaction, sexual activity and desire domains （EFD, EjFD, SDA and ADD） or for erectile or ejaculatory bother mean scores. After 12 weeks, the overall incidence of ejaculatory dysfunction （EjD） was 13.4%. Incidences of the seven different types of EjD （decreased frequency, delay, dryness, decreased strength/force, decreased volume, decreased pleasure and pain at ejaculation） were 2.4%, 3.1%, 3.9%, 3.9%, 6.3%, 7.1% and 3.1%, respectively. Baseline EjFD scores were higher for I PSS responders than for non-responders （26.09 vs. 24.06, P=0.03）. An EjFD score reduction was more frequent in IPSS responders. The incidence of EjD was small, but not negligible and was more frequent in patients with less lower urinary tract symptoms, a smaller prostate, higher baseline MSHQ totals and higher EjFD scores.

Previous studies have demonstrated that male hypogonadism is associated with a low level of vitamin D. However, no reports have investigated the effects of vitamin D on testosterone levels in Korean men. Our aim was to investigate whether testosterone levels are associated with serum vitamin D levels and whether seasonal variation exists. This cross-sectional study analyzed serum 25-hydroxyvitamin D [25（OH）D], total testosterone （TT）, and free testosterone （FT） in 652 Korean men over 40 years of age who had undergone a comprehensive medical examination. The average age of the subjects was 56.7± 7.9 years, and the mean serum 25（OH）D, TT and FT levels were 21.23 ± 7.9 ng m1-1, 4.70 ±1.6 ng m1-1, and 8.12± 3.3 pg m1-1, respectively. In the multiple linear regression model, 25（OH）D showed positive association with TT （β =0.137, P〈 0.001） and FT （p =0.103, P= 0.008）. 25（OH） D and FT showed similar seasonal or monthly variation after adjustment for age. A vitamin D deficiency [25（OH）D 〈 20 ng m1-1] was associated with an increased risk of deficiencies of TT （〈2.30 ng ml-1）（odds ratio [OR]： 2.65; 95% confidence interval [CI]. 1.21-5.78, P = 0.014） and FT （〈6.50 pg ml-1）（OR： 1.44; 95% Ch 1.01-2.06 P = 0.048） after adjusting for age, season, body mass index, body composition, chronic disease, smoking, and alcohol use. In conclusion, we demonstrated a positive correlation between 25（OH）D and testosterone, which showed similar seasonal variation in Korean men.

To evaluate patient preferences for sildenafil citrate or tadalafil （PDE-5 inhibitors available for the treatment of erectile dysfunction [ED]） and assess potential reasons for these preferences. Methods： This open-label study was conducted on Korean men taking sildenafil, at least 6 weeks prior to study entry, for ED. Following screening, patients continued sildenafil treatment for 4 weeks, then after a 1-week washout period, switched to tadalafil for 8 weeks. Patients then continued with their treatment of choice during an extension phase. Psychosocial factors （time concern, spontaneity, sexual self-confidence） were evaluated using Psychological and Interpersonal Relation- ship Scales （PAIRS）, while timing of dose to sexual attempt patterns were assessed from patient diaries. Results： The present study enrolled 160 Korean men （mean age 55 years） with prior median sildenafil use of 585 days. During the extension phase, 73.7% of patients elected to take tadalafil, whereas 26.3% chose sildenafil （P 〈 0.001）. After switching from sildenafil to tadalafil, mean PAIRS time concern scores decreased from 2.54 to 2.42 （P = 0.002）, with no statistically significant differences observed between the sildenafil and tadalafil assessment phases in sexual spontaneity and self-confidence scores. Sexual attempts made 〉 4 h to 〈 36 h post-dose occurred in 4.5% of patients during the sildenafil assessment phase compared with 17.5% during the tadalafil assessment phase. Conclusion： After experiencing both sildenafil and tadalafil, the majority of patients exhibited a preference for tadalafil. This preference might be influenced by psychosocial factors, such as decreased time concerns, and a broader window of opportunity available for sexual activity.
