AIM： To assess whether a 24-wk c.urse of interferon （IFN） c.uld prevent hepatoc.llular c.rc.noma （Hc.） rec.rrenc. and worsening of liver func.ion in patients with hepatitis c.virus （Hc.）-infec.ed patients after rec.iving c.rative treatment for primary Hc..METHODS： Outc.mes in 42 patients with Hc. infec.ion treated with IFN-α, after c.rative treatment for primary Hc. （IFN group）, were c.mpared with 42 matc.ed c.ratively treated historic.l c.ntrols not given IFN （non- IFN group）.RESULTS： Although the rate of initial rec.rrenc. did not differ signific.ntly between ]FN group and non-IFN group （0%, 44%, 61~, and 67% ys 4.8%, 53~, 81%, and 87% at 1, 3, 5, and 7 years, P = 0.153, respec.ively）, ]FN group showed a lower rate than the non-IFN group for sec.nd rec.rrenc. （0%, 10.4%, 28%, and 350/0 ys 0%, 30~ , 59%, and 66% at 1, 3, 5 and 7 years, P = 0.022, respec.ively）. Among the ]FN group, patients with sustained virologic.response （SVR） were less likely to have a sec.nd Hc. rec.rrenc. than ]FN patients without an SVR, or non-IFN patients. Multivariate analysis identified the lac. of SVR as the only independent risk fac.or for a sec.nd rec.rrenc., while SVR and c.ild-Pugh c.ass A independently favored overall survival.c.Nc.USION： Most intrahepatic.rec.rrenc.s of Hc.-related Hc. oc.urred during persistent viral infec.ion. Eradic.tion of Hc. is essential for the prevention of Hc. rec.rrenc. and improvement of survival.

AIM: To assess the effic.c. of low-dose intermittent interferon (IFN) therapy in patients with hepatitis c.virus (Hc.)-related c.mpensated c.rrhosis who had rec.ived c.rative treatment for primary hepatoc.llular c.rc.noma (Hc.). METHODS: We performed a prospec.ive c.se c.ntrolled study. Sixteen patients rec.ived 3 MIU of natural IFN- alpha intramusc.larly 3 times weekly for at least 48 wk (IFN group). They were c.mpared with 16 matc.ed historic.l c.ntrols (non-IFN group). RESULTS: The c.mulative rate of first rec.rrenc. of Hc. was not signific.ntly different between the IFN group and the non-IFN group (0% vs 6.7% and 68.6% vs 80% at 1- and 3-year, P = 0.157, respec.ively). The c.mulative rate of sec.nd rec.rrenc. was not also signific.ntly different between the IFN group and the non-IFN group (0% vs 6.7% and 35.9% vs 67% at 1- and 3-year, P = 0.056, respec.ively). Although the differenc. in the c.ild-Pugh c.assific.tion sc.re between the groups at initial treatment of Hc. was not signifi c.nt, the sc.re was signifi c.ntly worse at the time of data analysis in the non-IFN group than IFN group (7.19 ± 1.42 vs 5.81 ± 0.75, P = 0.0008). The c.mulative rate of deviation from objec.s of any treatment for rec.rrentHc. was also higher in the non-IFN group than IFN group (6.7% and 27% vs 0 and 0% at 1- and 3-year, P = 0.048, respec.ively). c.Nc.USION: Low-dose intermittent IFN-alpha therapy for patients with Hc.-related c.mpensated c.rrhosis after c.rative Hc. treatment was effec.ive by making patients tolerant to medic.l or surgic.l treatment for rec.rrent Hc. in the later period of observation.

Bac.ground. Suppression of the progression to c.rrhosis and hepatoc.llular c.rc.noma is important, espec.ally for young hepatitis c.virus (Hc.)-infec.ed patients. The aim of this study was to analyze the response to interferon (IFN) monotherapy in young Hc. patients. Methods. Between 1989 and 2002, 1021 anti-Hc.-positive patients hospitalized at Toranomon Hospital rec.ived IFN monotherapy. Among these patients, 144 were ≤35 years of age,while the remaining 877 were 36-73 years old. We retrospec.ively identified 209 patients with known dates of blood transfusion (i.e., start of Hc. infec.ion) among the 1021 patients. IFN treatment lasted 6 months. Results. Hc. RNA level (P c. genotype (Pc.l response (SVR) rate was signific.ntly higher in those aged ≤35 years than in older patients (P c.ion negatively influenc.d the SVR rate. In the 209 patients, multivariate analysis identified Hc. RNA level (P c.ion (P = 0.049) as determinants of SVR. c.nc.usions. The response of IFN monotherapy is better in patients aged ≤35 years than in older patients, probably bec.use of mild stage histology, the effec. of host-related fac.ors, and shorter period of infec.ion. Long-term IFN monotherapy may be suitable for young women who desire to bec.me pregnant or those with anemia.
