A case series of precipitous cardiac tamponade from suspected perimyocarditis in COVID-19 patients

A 71yearold well controlled hypertensive female presented on day 4 of acute COVID19 illness with atypical chest pain and worsening exertional dyspnea.On examination,heart rate was 110 beats/min,blood pressure was 125/82 mmHg and there were decreased heart sounds with visible jugular venous distention.She was not in respiratory distress at rest and was not requiring supplemental oxygen.Chest radiography showed bilateral diffuse opacities(Figure 1).transthoracic echocardiography(TTE)confirmed a moderate pericardial effusion with right ventricular systolic compression,paradoxical right ventricular septal motion,enddiastolic right atrial collapse,and a plethoric inferior vena cava(IVC)with no respiratory variation.Her ejection fraction(EF)was 55%.Initial workup revealed mildly elevated troponin T levels of 0.14 ng/mL,NTPro BNP of 2500 pg/mL.

Atrial flutter(AFL)is a detrimental cardiac arrhythmia caused by multiple pathologic conditions.Certain unsuspecting cases,however,may be an inciting factor.Among them is blunt cardiac injury(BCI),characterized by nonpenetrating mediastinal trauma,leading to arrhythmias due to myocardial tissue injury.AFL is a rare sequela of BCI that has seldom been reported.We present a case of a healthy 50-year-old lady who was incidentally diagnosed with AFL in the setting of BCI.

A valve-in-valve approach to manage severe bioprosthetic tricuspid valve stenosis

severe symptomatic tricuspid stenosis often requires intervention.When the etiology is bioprosthetic valve failure,a challenging situation is met.Surgical valve replacement is the gold standard of treatment for severe tricuspid valve(TV)disease.[1]However,a redo sternotomy is rarely performed for isolated TV disease due to very high perioperative mortality.[2]Evolving percutaneous technology presents new frontiers in treatment,with the valveinvalve procedure emerging as an alternative.We present a case of a 65year woman with progressive symptoms due to failure of a bioprosthetic valve in the tricuspid position who was not a surgical candidate but underwent percutaneous intervention with significant clinical improvement.
