Pharmacodynamic and kinetic effect of rabeprazole on serum gastrin level in relation to CYP2C19 polymorphism in Chinese Hans

AIM： To observe the pharmacokinetics and pharmacodynamics of rabeprazole and compare serum gastrin concentrations in different CYP2C19 genotype groups. METHODS： The CYP2C19 genotype status of Chinese Han healthy volunteers was determined by polymerase chain reaction-restriction fragment length polymorphism method. Twenty H pylori-negative healthy subjects voluntary participated in the study. They were divided into the following three groups： homozygous extensive metabolizers （homEM）, heterozygous extensive metabolizers （hetEM） and poor metabolizers （PM）. After they orally received rabeprazole 20 mg once daily in the morning of d 1 and d 8, blood samples were collected at various time-points until 24 h after administration and intragastric pH values were monitored for 24 h by Digitrapper pH. Serum gastrin concentrations were measured by radioimmunoassay. Serum concentrations of rabeprazole were measured by high performance liquid chromatography. RESULTS： The mean AUC values for rabeprazole after a single and repeated doses were significantly different between the homEM and PM groups, but not between the homEM and hetEM, or the hetEM and PM groups. No significant differences in intragastric pH medians were observed among the three different genotype groups after a single dose or repeated doses. The ratio of pH medians between d 1 and d 8 ranged from 84% to 108%. The mean gastrin AUC values were also different among the three genotype groups, with a relative ratio of 1.0, 1.2 and 1.5 after a single dose and 1.0, 1.5 and 1.6 after repeated doses in the homEM, hetEM and PM groups, respectively. The gastrin AUC values among the three different genotype groups showed no significant difference either after a single dose or repeated doses. The subject who had lower intragastric acidity showed higher serum gastrin levels and concentrations of rabeprazole. CONCLUSION： In Chinese Han healthy people, the pharmacokinetics of rabeprazole are dependent on the CYP2C19 genotype

In this study,the Betts-Miller-Janjic(BMJ)convective adjustment scheme in the Weather Research and Forecasting(WRF)model version 4.0 was used to investigate the effect of itsα-parameter,which influences the first-guess potential temperature reference profile on the Madden-Julian oscillation(MJO)propagation and *** study diagnosed the MJO active phase composites of the MJO-filtered outgoing longwave radiation(OLR)during the December-to-February(DJF)period of 2006-2016 over the Indian Ocean(IO),Maritime Continent(MC),and western Pacific(WP).The results show that the MJO-filtered OLR intensity,propagation pattern,and MJO classification(standing,jumping,and propagating clusters)are sensitive to theα-value,but the phase speeds of propagating MJOs are ***,with an increasingα-value,the simulated MJO-filtered OLR intensity increases,and the simulated propagation pattern is *** also show that the intensity and propagation pattern of an eastward-propagating MJO are associated with MJO circulation structures and thermodynamic ***αincreases,the front Walker cell and the low-level easterly anomaly are enhanced,which premoistens the lower troposphere and triggers more active shallow and congestus *** enhanced shallow and congestus convection preconditions the lower to middle troposphere,accelerating the transition from congestus to deep convection,thereby facilitating eastward propagation of the ***,the simulated MJO tends to transfer from standing to eastward propagating asα*** summary,increasing theα-value is a possible way to improve the simulation of the structure and propagation of the MJO.

Organic matter(OM)derived from the decomposition of crop residues plays a key role as a sorbent for cadmium(Cd)*** studies have explored the straw decomposition processes with the presence of minerals,and the effect of newly generated organomineral complexes on heavy metal *** this study,we investigated the variations in structure and composition during the rice straw decomposition with or without minerals(goethite and kaolinite),as well as the adsorption behavior and mechanisms by which straw decomposition affects Cd *** degree of humification of extracted straw organic matter was assessed using excitation-emission matrix(EEM)fluorescence and Ultraviolet-visible spectroscopy(UV-vis),while employing FTIR spectroscopy and XPS to characterize the adsorption *** spectra analysis revealed the enrichment of highly aromatic and hydrophobic components,indicating that the degree of straw decomposition and humification were further intensified during ***,the existence of goethite(SG)accelerated the humification of *** experiments revealed that the straw humification increased Cd adsorption ***,SG exhibited significantly higher adsorption performance compared to the organic matter without minerals(RS)and the existence of kaolinite(SK).Further analysis using FT-IR spectroscopy and XPS verified that the primarymechanisms involved in Cd immobilization were complexion with—OH and—COOH,as well as the formation of Cd-πbinds with aromatic C=C on the surface of solid *** findings will facilitate understanding the interactions of the rice straw decomposing with soil minerals and its remediation effect on Cd-contaminated farmland.

AIM: To investigate the role of cyclooxygenase-2(COX-2)and vascular endothelial growth factor (VEGF) in the development of gastric carcinoma and correlation between expression of COX-2 and VEGF and clinicopathologic features in tissues from patients with gastric ***: 281 patients with gastric carcinoma who underwent surgical resection between 1990 and 1999 at the First Affiliated Hospital, Anhui Medical University, PRC, were followed up. Expression of COX-2 and VEGF was investigated retrospectively in 232 gastric carcinoma tissues and 60noncancerous specimens by using ***: The 5-year survival rates of early gastric carcinoma (EGC) and advanced gastric carcinoma (AGC) were 93.4 %and 59.0 %, respectively. Survival time was highly correlated with lymph node metastasis, vascular invasion, depth of invasion and treatment with chemotherapy. Compared with paired noncancerous tissues, expression of COX-2 and VEGF and microvessel density (MVD) value in carcinoma tissue were significantly higher. The MVD value was much higher in COX-2-positive group and VEGF-positive group than that in COX-2-negative group and VEGF-negative *** of COX-2 and VEGF, as well as MVD value were highly correlated with lymph node metastasis and vascular invasion. The 5-year survival rate of patients with expression of COX-2 or VEGF was significantly lower than that of patients without COX-2 or VEGF expression. Multivariate analysis revealed that VEGF overexpression, lymph node metastasis,COX-2 overexpression, depth of invasion and vascular invasion were all independent prognostic factors of gastric ***: Overexpression of COX-2 and VEGF in patients with gastric carcinoma can enhance the possibility of invasion and metastasis, implicating a poor *** may serve as the fairly good prognostic factors to indicate biologic behaviors of gastric carcinoma.

The mechanism of cell damage during acute pancreatitis (AP) has not been fully elucidated, and there is still a lack of specific or effective treatments. Increasing evidence has implicated mitochondrial dysfunction as a key event in the pathophysiology of AP. Mitochondrial dysfunction is closely related to calcium (Ca^(2+)) overload, intracellular adenosine triphosphate depletion, mitochondrial permeability transition pore openings, loss of mitochondrial membrane potential, mitophagy damage and inflammatory responses. Mitochondrial dysfunction is an early triggering event in the initiation and development of AP,and this organelle damage may precede the release of inflammatory cytokines, intracellular trypsin activation and vacuole formation of pancreatic acinar cells. This review provides further insight into the role of mitochondria in both physiological and pathophysiological aspects of AP, aiming to improve our understanding of the underlying mechanism which may lead to the development of therapeutic and preventive strategies for AP.

Polythiocarbonates with similar structures to polycarbonates, have emerged as a group of promising sulfur-containing polymers with a range of practical applications, taking advantages of their excellent optical properties, good thermal stability and filmforming ability, and so on. However, the synthesis of polythiocarbonates generally suffered from mixed isomeric structures of repeating units, and hazardous sulfur-containing reagents. In this work, an efficient multicomponent reaction of elemental sulfur,difluoromethylene phosphobetaine（PDFA）, and aryl silyl ethers was designed with the assistance of organic base 7-methyl-1,5,7-triazabicyclo[4.4.0]dec-5-ene（MTBD） to produce 12 kinds of O,O′-monothiocarbonate compounds in up to 93% yields,taking advantages of the unique chemical property of difluorocarbene, and the deprotection of silyl group by F-generated from PDFA. Most importantly, the MTBD-assisted multicomponent polymerization of bis（aryl silyl ether）s, elemental sulfur, and PDFA was developed, affording 12 kinds of poly（O,O′-monothiocarbonate）s with well-defined structures, high Mws of up to16900 g/mol and high yields（up to 99%）. These poly（O,O′-monothiocarbonate）s possessed good film-forming properties and thermal stability, high light refractive indices and transmittance. The facile synthesis of these polymonothiocarbonates might pave the way for the development of promising sulfur-containing polymer materials.

Landslide dams,as frequent natural hazards,pose significant risks to human lives,property,and ecological *** grading characteristics and density of dam materials play a crucial role in determining the stability of landslide dams and the potential for dam *** explore the failure mechanisms and evolutionary processes of landslide dams with varying soil properties,this study conducted a series of flume experiments,considering different grain compositions and material *** results demonstrated that grading characteristics significantly influence landslide dam stability,affecting failure patterns,breach processes,and final breach ***-graded materials exhibited a sequence of surface erosion,head-cut erosion,and subsequent surface erosion during the breach process,while well-graded materials typically experienced head-cut erosion followed by surface *** coarse-graded dams,the high permeability of coarse particles allowed the dam to remain stable,as inflows matched *** dam breach model experiments also showed that increasing material density effectively delayed the breach and reduced peak breach flow ***,higher fine particle content led to a reduction in the residual dam height and the base slope of the final breach profile,although the relationship between peak breach discharge and the content of fine and coarse particles was *** better understand breach morphology evolution under different soil characteristics and hydraulic conditions,three key points were identified—erosion point,control point,and scouring *** study,by examining the evolution of failure patterns,breach processes,and breach flow discharges under various grading and density conditions,offers valuable insights into the mechanisms behind landslide dam failures.

目的:评估^(18)F-脱氧葡萄糖(^(18)F-fluorodeoxyglucose,^(18)F-FDG)正电子发射计算机断层显像(positron emission tomography,PET)及PET/计算机断层扫描(computed tomography,CT)在胰腺癌(pancreatic cancer)中的诊断价值.方法:计算机检索2 0 1 5-0 4-0 1之前在Pubmed、EMBASE、Webofscience、Cochrane协作网数据库、中国知网、谷歌学术搜索、万方及维普等数据库公开发表的有关^(18)F-FDG PET及^(18)F-FDG PET/CT诊断胰腺癌的文献,并手工检索国内外相关医学杂志.严格按照纳入排除标准筛选文献,采用QUADAS量表评价文献质量,利用MetaD i s c 1.4统计软件进行异质性分析和定量合成,计算相应验后合并灵敏度和特异度,并绘制SROC曲线评估诊断效能,同时运用S t a t a12.0软件检测纳入研究是否存在发表偏倚.结果:共纳入51篇英文文献,Meta分析表明:^(18)F-FDGPET诊断胰腺癌的合并灵敏度(pooled sensitivity)为87%(95%CI:85%-89%)、合并特异度(pooled specificity)为78%(95%CI:74%-81%)、诊断比值比(diagnosis odds ratio,DOR)为21.91(95%CI:14.15-33.93)、阳性似然比(positive likelihood r a t i o,+L R)为3.38(95%C I:2.64-4.33)、阴性似然比(negative likelihood ratio,-LR)为0.^(18)(95%CI:0.14-0.23)、SROC曲线下面积为0.8930;^(18)F-F D G P E T/C T诊断胰腺癌的合并的灵敏度为91%(95%CI:88%-93%)、合并特异度为77%(95%CI:72%-82%)、DOR为28.52(95%CI:19.63-41.42)、+LR为3.57(95%CI:2.96-4.31)、-LR为0.14(95%C I:0.11-0.^(18))、S R O C曲线下面积为0.9315.结论:^(18)F-FDGPET/CT及^(18)F-FDGPET诊断胰腺癌的诊断效能均显著高于C T,且^(18)F-FDG PET/CT诊断灵敏度较^(18)F-FDG PET高,漏诊率低,具有更高的诊断价值,两者均可作为传统检查不能发现的胰腺癌的辅助诊断工具.
