BACKGROUND Healthcare workers(HCWs)are at an increased risk for exposure to infections.Serratia marcescens(S.marcescens)is a gram-negative,opportunistic and nosocomial pathogen belonging to the Enterobacterieae family.A few case reports have been published of chorioamnionitis caused by S.marcescens infection.Immunological changes during pregnancy can also affect the risk of infection.However,few studies have examined hospital-acquired bacterial infection in pregnant HCWs.CASE SUMMARY A 33-year-old woman,a resident in anesthesiology,was admitted at 14 wk gestation for fever with chills.She had no medical history other than contact dermatitis of both hands that started from the beginning of the trainee.There was no obvious infection focus and no bacterial growth in blood cultures.She was discharged after 1 wk of empirical antibiotic treatment.At three weeks before the fever started,she had a blister on the site of contact dermatitis on both hands,she applied antibiotic ointment for three days and the blisters had healed.At 19 wk gestation,she had a high fever and was readmitted.Physical examination and image studies were nonspecific and the patient had no other symptoms.S.marcescens grew in blood cultures at 19 wk gestation.Treatment with intravenous antibiotics was started.However,she suffered a miscarriage at 224/7 wk gestation.Pathologically,the amniotic membrane showed chorioamnionitis with a focal infarct.Subsequently,a placenta tissue culture grew S.marcescens.CONCLUSION HCWs can be exposed to pathogens that can cause opportunistic infections such as S.marcescens.Pregnancy affects the immune system,making it susceptible to opportunistic infections.Therefore,pregnant HCWs may require more preventive measures,including hand hygiene and avoid risk factors(ex.wrapping the skin).

BACKGROUND Dedifferentiated liposarcoma(DDLPS)is an extremely rare neoplasm that exhibits various morphologies.The tumor is characterized by immunoreactivity to MDM2 and CDK4 and can be confirmed by detecting MDM2 amplification via fluorescence in situ hybridization(FISH).Herein,we report an unusual case of DDLPS arising from the duodenum.CASE SUMMARY A 64-year-old man presented with repeated abdominal pain and weight loss.Radiologic studies revealed a mass of the duodenum involving the pancreas.The patient was treated with pylorus-preserving pancreaticoduodenectomy.Histologically,the tumor showed a high-grade sarcoma.Immunohistochemistry demonstrated that the tumor cells were positive for MDM2 and CDK4 expression.MDM2 amplification was detected via FISH,leading to the final diagnosis of DDLPS.Following surgery,the patient was treated in the intensive care unit due to peritonitis,and died 60 d after surgery.CONCLUSION To the best of the authors’knowledge,this is the first case of primary duodenal DDLPS in Korea and the third case in the English-language literature.Care must be taken not to misdiagnose DDLPS as another high-grade tumor.Liposarcoma should be in the differential diagnosis list.

BACKGROUND Perivascular epithelioid cell tumor(PEComa)is a rare mesenchymal tumor that exhibits an epithelioid and spindle cell morphology.The tumor is characterized by immunoreactivity for melanocytic and myogenic markers but can be misdiagnosed as more common tumors with similar characteristics,including gastrointestinal stroma tumors or leiomyosarcomas.Recently,a subset of PEComas has been reported to harbor a transcription factor binding to TFE3 fusion.Herein,we report a rare case of TFE3-expressing malignant PEComa arising from the mesentery.CASE SUMMARY A 50-year-old woman presented with abdominal discomfort for 3 months.Results of laboratory tests were all within the normal ranges,and the patient had no notable medical history.Magnetic resonance imaging revealed a large tumor on the right side of the pelvic floor,which was originally suspected to be a primary ovarian tumor.However,during surgery,the tumor was revealed to have originated from the mesentery.Histologically,the tumor was composed of bundles of spindle cells and sheets of epithelioid cells.Extensive coagulative necrosis and numerous mitotic figures were observed.Immunohistochemistry revealed that the tumor cells were positive for smooth muscle actin,HMB-45,and TFE3 expression.Tumor involvement of the rectal serosa was identified,leading to a final diagnosis of malignant PEComa of the mesentery.Surgical resection was followed by adjuvant chemotherapy.No recurrence or metastasis was observed over a 6-month follow-up period.CONCLUSION Malignant PEComa of the mesentery is extremely rare and should be distinguished from morphological mimics through differential diagnosis and immunohistochemistry.

BACKGROUND Cytomegalovirus(CMV)colitis is usually seen in immunocompromised patients with risk factors such as human immunodeficiency virus infection,solid organ transplant,inflammatory bowel disease,or malignancy.Therefore,many clinicians usually do not consider the possibility of CMV colitis in immunocompetent patients.We reported a rare case of segmental colonic hypoganglionosis associated with CMV colitis in an immunocompetent patient.CASE SUMMARY A 48-year-old woman with no underlying disease was admitted to our hospital for severe abdominal pain and constipation.Computed tomography of the abdomen showed diffuse dilatation of the small intestine and the entire colon.Initial sigmoidoscopic findings and result of polymerase chain reaction(PCR)for CMV revealed the compatible findings of CMV colitis,the patient was treated with intravenous ganciclovir.After treatment,sigmoidoscopic findings and CMV PCR results improved.However the patient continued to suffered from constipation.Eight months after the initial admission,patient visited the emergency department with severe abdominal pain and imaging revealed aggravation of fecal impaction and bowel dilatation.We performed subtotal colectomy to control patient’s symptom.Histological examination of the resected specimen showed significantly reduced number of mature ganglion cells in the sigmoid colon compared to that in the proximal colon.CONCLUSION Our case demonstrates that CMV colitis can develop even in patients with no other underlying disease,and that CMV colitis can be one of the causes for developing colonic hypoganglionosis.

To the Editor:A supraglottic airway device(SAD)can be used instead of an endotracheal tube(ET)in emergency situations.There are two types of SAD,cuffed and non-cuffed,the ease of use and success rates of which are similar.Compared to ET insertion,SAD insertion is quick and simple,while providing comparable ventilation.The 2019 American Heart Association guideline recommends SAD insertion in emergency situations,such as out-of-hospital cardiac arrest.[1,2]However,the success of SAD insertion is not easily ascertained.According to one study,when a SAD was used in an emergency medical service(EMS)setting,airway failure was not recognized in 13.8%of patients.[3]Incorrect positioning of a SAD is evidenced by bite block malalignment with the incisors and ineffective gas exchange(characterized by thoracic and“front of neck”excursions,inadequate tidal volume,low arterial oxygen saturation[SpO_(2)],poor capnograph trace,high airway pressure,and air leak).[1,4]Failure to insert a SAD to the proper depth typically results from the resistance of structures around the hypopharynx.[5]In this study,we introduce the thyroid cartilage elevation(TCE)maneuver,a simple and novel method that facilitates correct insertion of a SAD.

This review is focused on carbon nanotube(CNT)-elastomeric polymer nanocom-posites,which have attracted industrial and academic interest over the years due to their enhanced properties.Major factors notably CNT type,surface modifica-tion,dispersion of CNT,and processing techniques that affect the physical properties of CNT-elastomeric polymer nanocomposites are reviewed,and several key physical properties,including tensile,electrical,and thermal properties,were also included in this review.Some of the key challenges that undermine the effectiveness of CNTs and their composites with elastomeric polymers,and the potential applications of CNT-elastomeric composites are also captured.

Nanoporous silicon is a promising anode material for high energy density batteries due to its high cycling stability and high tap density compared to other nanostructured anode materials.However,the high cost of synthesis and low yield of nanoporous silicon limit its practical application.Here,we develop a scalable,low-cost top-down process of controlled oxidation of Mg2Si in the air,followed by HCl removal of MgO to generate nanoporous silicon without the use of HF.By controlling the synthesis conditions,the oxygen content,grain size and yield of the porous silicon are simultaneously optimized from commercial standpoints.In situ environmental transmission electron microscopy reveals the reaction mechanism;the Mg2Si microparticle reacts with O2 to form MgO and Si,while preventing SiO2 formation.Owing to the low oxygen content and microscale secondary structure,the nanoporous silicon delivers a higher initial reversible capacity and initial Coulombic efficiency compared to commercial Si nanoparticles(3,033 mAh/g vs.2,418 mAh/g,84.3%vs.73.1%).Synthesis is highly scalable,and a yield of 90.4%is achieved for the porous Si nanostructure with the capability to make an excess of 10 g per batch.Our synthetic nanoporous silicon is promising for practical applications in next generation lithium-ion batteries.

Type-Ⅱsuperlattice(T2SL)materials are the key element for infrared(IR)detectors.However,it is well known that the characteristics of the detectors with the T2SL layer are greatly affected by the strain developed during the growth process,which determines the performance of IR detectors.Therefore,great efforts have been made to properly control the strain effect and develop relevant analysis methods to evaluate the strain-induced dark current characteristics.In this work,we report the strain-induced dark current characteristics in InAs/GaSb T2SL MWIR photodetector.The overall strain of InAs/GaSb T2SL layer was analyzed by both high-resolution X-ray diffraction(HRXRD)and the dark current measured from the absorber layer at the elevated temperatures(≥110 K),where the major leakage current component is originated from the reduced minority carrier lifetime in the absorber layer.Our findings indicate that minority carrier lifetime increases as the tensile strain on the InAs/GaSb T2SL is more compensated by the compressive strain through‘InSb-like’interface,which reduces the dark current density of the device.Specifically,tensile strain compensated devices exhibited the dark current density of less than 2×10^-5 A/cm^2 at 120 K,which is more than one order of magnitude lower value compared to that of the device without tensile strain relaxation.

存在句是汉语的特殊句式之一，但目前韩国学生的存在句偏误分析研究跟其他的特殊句式相比并不多。这可能是存在句跟其他汉语的特殊句式相比偏误的严重性较低，偏误出现的几率也较低，因此目前对外汉语教学方面存在句的偏误分析不是热门的话题。但偏误的严重性较低并不等于说，存在句是可以被忽视的语法点，或者学生较容易习得的语法点。通过目前关于韩国学生习得存在句的研究情况能得出一个共同的结论。存在句确实是韩国学生习得的难点，当中特别是“V着”存在句是韩国学生最大难点。因此本文的研究是通过问卷调查找出韩国学生习得“V着”存在句的偏误类型以及找出产生偏误的原因，并且会提出一些相应的教学对策。同时本文为了找出以下两个问题的答案：1.在使用目的语的环境下学习的学生是否有利于只在使用母语的环境下学目的语的学生习得“V着”存在句和2.正规大学的系统性教育和专业对外汉语教师对学生习得的影响如何，调查对象分成3组。第一组是正在中国受正规大学教育的韩国学生。第二组是只在韩国国内学习汉语的韩国学生。第三组是正在中国工作，但没有机会受正规大学教育的韩国人。这是本研究的特点当中之一。希望本研究有助于以下三方面;教材编写、教师的教学和韩国学生的“V着”存在句的习得。
