Wear-driven tool failure is one of the main hurdles in the industry.This issue can be addressed through surface coating with ceramic-reinforced metal matrix composites.However,the maximum ceramic content is limited by cracking.In this work,the tribological behaviour of the functionally graded WC-ceramic-particlereinforced Stellite 6 coatings is studied.To that end,the wear resistance at room temperature and 400°C is investigated.Moreover,the tribological analysis is supported by crack sensitivity and hardness evaluation,which is of utmost importance in the processing of composite materials with ceramic-particle-reinforcement.Results indicate that functionally graded materials can be employed to increase the maximum admissible WC content,hence improving the tribological behaviour,most notably at high temperatures.Additionally,a shift from abrasive to oxidative wear is observed in high-temperature wear testing.

Sodium-oxygen batteries(SOBs) have the potential to provide energy densities higher than the state-ofthe-art Li-ion batteries. However, controlling the formation of sodium superoxide(NaO_(2)) as the sole discharge product on the cathode side is crucial to achieve durable and efficient SOBs. In this work, the discharge efficiency of two graphene-based cathodes was evaluated and compared with that of a commercial gas diffusion layer. The discharge products formed at the surface of these cathodes in a glyme-based electrolyte were carefully studied using a range of characterization techniques. NaO_(2) was detected as the main discharge product regardless of the specific cathode material while small amounts of Na_(2)O_(2).2H_(2)O and carbonate-like side-products were detected by X-ray diffraction as well as by Raman and infrared spectroscopies. This work leverages the use of X-ray diffraction to determine the actual yield of NaO_(2)which is usually overlooked in this type of batteries. Thus, the proper quantification of the superoxide formed on the cathode surface is widely underestimated;even though is crucial for determining the efficiency of the battery while eliminating the parasitic chemistry in SOBs. Here, we develop an ex-situ analysis method to determine the amount of NaO_(2) generated upon discharge in SOBs by transmission X-ray diffraction and quantitative Rietveld analysis. This work unveils that the yield of NaO_(2) depends on the depth of discharge where high capacities lead to very low discharge efficiency, regardless of the used cathode. We anticipate that the methodology developed herein will provide a convenient diagnosis tool in future efforts to optimize the performance of the different cell components in SOBs.

目的本研究旨在调查组合锻炼项目对居住在长期养老院的老年人的健康相关生活质量和抑郁症状的影响。方法采用类实验研究设计。在西班牙北部巴斯克地区最大的长期养老院中便利选取41名老年人,并分为干预组(21名)和对照组(20名)。干预组接受50 min中等强度的组合锻炼干预课程(力量和平衡,每周3次,为期3个月)。对照组继续进行他们在长期疗养院中的常规活动。在干预前(基线时)、干预后(12周后)采用健康调查简表(the Short Form Health Survey,SF-36)和简版老年抑郁量表(Geriatric Depression Scale,GDS)对老年人的健康相关生活质量和抑郁症状进行调查,并比较两组的干预效果。结果共38名老年人完成研究(每组19例)。干预组中,干预后老年人身体功能得分比干预前增加11.06分(增加17.2%);角色情感方面得分比干预前提高5.27分(增加29.1%)(P0.05)。结论尽管本研究结果中组合锻炼对养老院院老年人健康相关的生活质量和抑郁症状的改善没有显示统计学意义,但增加样本量可能证实其有效性。这些结果有助于为未来的研究设计提供信息。

Anaphylactic Shock Due to Psyllium (<i>Plantago ovate</i>Seed) Allergy: A Case Report

The seeds of Plantago ovata, called psyllium or ispaghula, have been used in the preparation of bulk laxatives. Nowadays, it is also used as an ingredient in bakery and breakfast cereals to increase dietary fiber and to give baking products fluffiness and viscosity. Mostly, there are described cases of occupational allergy that handle Plantago ovata seeds in powder laxatives and some cases of anaphylaxis after eating products containing psyllium. This case report is about a geriatric assistant who had an anaphylactic shock after eating a multigrain gluten-free piece of bread containing psyllium, who presented a positive skin test with psyllium in the allergy study. We recommended to the patient to avoid the ingestion and the inhalation at work of Plantago ovata seeds and to carry self-injectable adrenaline. With this case, we would like to highlight the growing uses of psyllium, especially in bakery products, and that it can behave like a hidden allergen.

Epilepsy surgery has constantly evolved in various fields of knowledge. Surgical criteria have shifted from standard procedures to individualized forms of treatment, depending on physiological tests and specific imagenology findings in an individual patient. New instruments and applications based upon older instruments have been described in the treatment of epilepsy surgery, including the use of endoscopes. Frequent indications of neuroendoscopy in epilepsy surgery have been mostly to assist in open procedures, particularly when fluid-filled spaces are present within the surgical field, such as cystic parasites, tumors, arachnoid, or other types of cysts. Other indications certainly include cases of temporal lobe epilepsy, where ventricular exploration precedes intraventricular electrode placing as a tool to localize epileptogenic zones. Although described several years ago, there has been a recent trend in performing endoscopy-assisted section of the corpus-callosum in patients with generalized seizures. As neurosurgical instruments and techniques continue their progress, endoscopy will be included more frequently as part of the armamentarium in epilepsy surgery.

Chronic spontaneous urticaria (CSU) is defined as the spontaneous appearance of wheals, angioedema, or both, for at least 6 weeks, due to known or unknown causes [1]. In some patients who present a CSU with daily or almost daily symptoms a type I allergy could be the underlying cause. We present one adult patient with chronic urticaria who was finally diagnosed as a non-occupational case of IgE-mediated wheat allergy manifested following exposure by 3 different routes: inhalation (rhinitis and bronchial asthma), dermal absorption (contact urticaria) and ingestion (systemic chronic urticaria). We were able to detect the culprit proteins by immunoblotting. Serum IgE binds mainly to alpha-amylase/trypsin inhibitors and, to a lesser extent, to other proteins associated with food allergy to grains (e.g. beta-glucanase, serpin, peroxidase). In our opinion, skin prick tests with a food standard battery could help in the etiological diagnosis of chronic urticaria. The identification of responsible allergens could be difficult because only a few complex in vitro techniques allow detecting the allergy to several proteins.

SonoVue® (Bracco;Milan, Italy) is an ultrasound contrast medium which contains polyethylene glycol (PEG) or macrogol, an excipient responsible for severe allergic reactions, especially through the parenteral route, and widely used, especially in the health field. We present an adult patient who experienced an IgE-mediated anaphylactic shock due to macrogol contained in SonoVue®, as demonstrates the skin tests realized. The allergy study has been useful in our case because it has guided us in the preventive measures to advice to the patient, who has to avoid not only SonoVue® but also other products that contain macrogol. We consider this case of interest because it is an allergic adverse reaction to SonoVue® in which its excipient, macrogol, has acted like the allergen responsible. We have to add SonoVue® to the list of the products that contain macrogol in their composition and keep in mind the possible allergenic capacity of this excipient for suspecting its responsibility, especially in those patients with multiple drug allergy and/or reactions to products of different natures like ultrasound gels, topical products (e.g. toothpastes, antiseptics, sunscreens, etc.), plasma expanders and others.

Prurigo pigmentosa is a rare inflammatory disease of unknown origin, first reported from Japan, with only 33 cases described in non-Japanese patients. We describe a 13-year-old girl with a pruriginous symmetrical eruption of papules and vesicles affecting her back, neck and chest of 1 month duration. She remembered a similar, but lighter eruption, 2 months before. As the initial diagnosis was of a vesiculobullous form of Darier disease, treatment with isotretinoin 40 mg/day was started with good response. Histological study showed a superficial perivascular and interstitial dermatitis composed predominantly of lymphocytes. The epidermiswas spongiotic,with exocytosis of lymphocytes and some neutrophils and necrotic keratinocytes. All these findings were consist ent with prurigo pigmentosa. The lesions resolved leaving a light brown reticulate hyperpigmentation. Prurigo pigmentosa has never been reported in prepubescent patients, the vesiculobullous forms are unusual, and the only treatments used previously are sulphonamides, tetracyclines and macrolides. We report a 13-year -old Caucasian girl with vesiculobullous prurigo pigmentosa successfully treated with isotretinoin.

Background: Melanotic pigmentations in scars consecutive to the excision of melanocytic tumours can be secondary to a reactive phenomenon related to the scar tissue or to a recurrence of the melanocytic lesion excised in the first case. Recurrent naevi may sometimes adopt unusual features that make them difficult to differentiate from a melanoma. Objectives: To describe the clinical, dermoscopic and histological features of melanotic pigmentations in scars consecutive to the excision of melanocytic tumours, and to correlate the histological diagnosis with the dermoscopic features. Methods: This was a prospective cohortstudy using macrophotography, dermoscopy and histopatho- logical study. Ninety-five melanotic pigmentations (77 patients) in scars secondary to the excision of melanocytic tumours were prospectively collected in the Department of Dermatology at the Instituto Valenciano de Oncologa in Valencia, Spain. Histopathological study was performed in 57 scars. Results: Thirteen dermoscopic structures were identified. Four criteria allowed a differentiation between reactive and specific melanocytic pigmentations. Presence of globules and presence of heterogeneous pigmentation were features associated with specific melanocytic pigmentations (P < 0.0001). Presence of a regular network and presence of streaks weremore frequently found in reactive pigmentations (P = 0.023 and 0.026, respectively). Conclusions: Dermoscopic examination of melanotic pigmentations in excision scars of melanocytic tumours provides useful information about the origin of that pigmentation. Based on such information, recurrent naevi can be differentiated from reactive pigmentations in most cases. Excision and histopathological diagnosis continue to be imperative in some cases of recurrent naevi with atypical clinical features.

Background: Location of the tumour is a well-known prognostic factor for recurrence of basal cell carcinoma (BCC). We have observed that tumours located in the nasolabial fold may deserve a particular attention. Objective: The aim of this study is to evaluate the behaviour of BCC located on the nasolabial fold in comparison to a control group of patients with BCC in lower risk sites, and to detect characteristics which may imply a more complicated treatment or the consideration of adjuvant treatment. Methods: A series of 24 patients with BCC located on the nasolabial fold were retrospectively recruited from our database of Mohs surgery from January 1997 to December 2000. A control group of 37 patients with BCC located on the cheek or forehead were selected from the same database. The differences between both groups in clinical, surgical and histological terms were evaluated. Results: BCC on the nasolabial fold presented with shorter evolution time from first symptom, had a smaller size, had been less frequently treated before and showed mostly non-aggressive histologic subtype, but needed more surgical stages and more complex reconstruction. Conclusion: We have observed that BCC on the nasolabial fold, despite presenting with better clinical and histological features, needed more complicated surgery. In our opinion, this location warrants a particular attention and further prospective studies should be made to corroborate our findings.
