Septic abortions usually result from intrauterine maneuvers using non-sterile instruments or materials;they are much more common when abortion is illegal. We report the clinical case of a 28-year-old patient following a complicated abortion of endometritis and bilateral abscess of the shoulders, requiring surgical drainage and antibiotic therapy. The bacterium identified was Escherichia coli, sensitive to ceftriaxone.

Introduction: Dental pathologies during pregnancy can compromise the prognosis of pregnancy and that of the mother. We report a first-study of cases of association of pregnancy and oral pathology supported in the Department of Odonto-stomatology of the National Hospital of Niamey. Objective: To report epidemiology, treatment and prognosis of dental origin cervicofacial cellulitis associated with pregnancy. Patients and Methods: This is a retrospective and prospective study over 5 years from August 1st, 2013 to July 31, 2018 involving 21 pregnant women in the Odonto-stomatology Department of the National Hospital of Niamey (HNN). Results: During the study period, 585 patients were consulted in the Odonto-stomatology Department for cervicofacial tooth cellulitis of dental origin, 21 of them were pregnant women (3.39%). The average age of the pregnant patients was 29.57 years old, extrems 25 - 29 years old. The average delay before consultation was 28.76 days. Majority patients were rural inhabitants The pathology happened in the third trimester of pregnancy (52.39%), second trimester (33.33%), in first (14.29%). This pathology was observed among disadvantaged socioeconomic class as housewives (85.71%) and uneducated women (61.90%). The lake of oral hygien was the main cause of cellulitis. We noted a history of periodontal disease (52.38%), fever upper 38°C (61.90%), trismus (85.71%,) dysphonia (42.86%), odynophagia (23.81%), dysphagia (42.86%), dyspnea (33.33%), and cervicofacial swelling (100%.) The treatment was medico-surgical. The prognosis depended on the precocious treatment with 76.19% cured. But unfortunately 4 cases died (19.05%), still for birth (52.38%) and 9.52% abortion. Foreign body retained for long duration sometimes causes vesico-vaginal and recto-vaginal fistula. We report a 60-year-old woman with vaginal foreign body causing vesico-vaginal and recto-vaginal fistula;she died after foreign body removal due to septic shock. The patient had vaginal purulent discharge, abdominal pain, and a septic shock. A fragment of stone (limestone) was present in the vagina, which was removed. The patient developed urinary incontinence and fecal incontinence after removal of the foreign body. The examination demonstrated the presence of vesico-vaginal and recto-vaginal fistula. The patient died four days after removal of the foreign body due to septic shock and multi-visceral failure. During the extraction foreign body, bacteria could be disseminated into the systemic circulation and might cause septic shock. Thus, wide-spectrum antibiotic therapy must be used before the procedure, which may decrease the chance of septic shock.

Introduction: Menopause is poorly documented in Africa. For most of the women living there, the experience of this period does not seem to be very problematic. For women living in rural areas, other concerns are more prominent. Methods: The study was prospective, quantitative and qualitative about 500 cases collected over a period of 6 months. The study involved postmenopausal women in the Niamey region of public services, in certain residential areas, or accompanying people in health structures. Parameters taken into account were sociocultural, antecedents, experience, knowledge, and management of menopause. The data was entered and analyzed using Microsoft Office Word 2010 and Microsoft Office Excel 2010 software. Results: The average age was 56.4 years with extremes of 40 and 80 years;57% were still living with their husbands and 39% widows;60.2% housewives;53% uneducated, 17% have higher education;60.2% had had at least 6 children and 39.6% had used contraception. The comorbidities observed were arterial hypertension (24.4%) and diabetes (7%). As for the representation that women made of menopause, for all of them, this stage is the beginning of old age. The most frequent signs were asthenia (86.2%) and mood disorders (70.6%). Almost all of the patients (99.8%) had not used hormonal treatment for the menopause and among them 27% the management of the menopause was with traditional means. Conclusion: Menopausal disorders are certainly a reality in our socio-cultural context. They are not a primary concern either through ignorance or resignation of the women concerned.

Adnexal torsion is defined by the existence of a partial or complete torsion of the ovary and a tubal portion around its vascular pedicle. It is a rare gynecological emergency. We report the case of adnexal torsion on a large postpartum ovarian cyst in a 40-year-old patient. The diagnosis was suspected based on clinical and ultrasound signs and confirmed by exploratory laparotomy. The appearance of the cyst was suggestive of a mucinous cystadenoma of the ovary. The treatment was radical by performing an adnexectomy. The surgical piece weighed 5.2 kg. The postoperative course was simple. Anatomopathological examination had not been carried out;the parents had buried the operating piece.

Pregnancy in a sickle cell patient is a source of maternal and perinatal morbidities. Thromboembolic diseases of which Deep Vein Thrombosis (DVT) is one of the complications. The frequency of the latter is poorly assessed. Very few studies have been done on this topic. We report a case of DVT in a 35-year-old patient with sickle cell SS pregnant with 27SA diagnosed and cared for at the Maternity Issaka Gazobi (MIG) in Niamey. The management was based on low molecular weight heparin combined with preventive measures against sickle cell anemia and a planned cesarean section which led to a favorable maternal and perinatal prognosis.

Introduction: Obstetric fistula (OF) is an abnormal communication between the genital and urinary tracts and/or associated with a recto-vaginal communication resulting from prolonged dystocic delivery. It is a frequent pathology in underdeveloped countries. In Zinder no study has been carried out on obstetric fistula. The aim of this study was to evaluate the epidemiological, social impact and therapeutic aspects of obstetric fistula at the CSME of Zinder. Patient and Method: This was a descriptive cross-sectional study of obstetric fistula care at CSME during the period of (January 2018 to June 2020). Results: A total of 196 cases were collected over 30 months. This represents an annual incidence of 78.4 fistulas. The median age of the patients was 18.63 years, and more than 65% were over 20 years old. One hundred and ninety-one patients (97.45%) did not attend school, and 56.12% (n = 110) lived in polygamous households. One hundred and fifty-one women (n = 151), 77.04% had full assistance from their husband during the first four (4) months of the pathology. Twenty-four (12.24%) had been notified of repudiation. Obstetrical risk of dystocia was found in 39.79% (n = 78) of the patients, of whom 24.49% (n = 48) had a focused antenatal consultation (CPNR). The labour lasted more than 24 hours in 100%. The majority of women are multiparous and 60% had at least 2 pregnancies. vesicovaginal fistula is the main pathological type with 86.23%. Fistulas were closed in 83.16% and 68.87% exited without urine loss. Conclusion: Obstetric fistula is a devastating disease affecting girls and young women in Niger, as in other countries in Africa and Asia. It is a major concern worldwide and is a social tragedy because it is disabling and depressing.

A Ricardian Analysis of the Impact of Temperature and Rainfall Variability on Ag-riculture in Dosso and Maradi Regions of Niger Republic

The impact of climate variability on Maradi and Dosso agriculture was estimated taking into account farmer adaptations. The study used a Ricardian analysis of 200 farms to explore the effects of climate variability on net revenue. It also simulates the impact of different climate scenarios on agriculture incomes. This analysis bespeaks that if temperature increases 1°C annually, the annual crop net revenues for both frameworks will decrease up to 582170.7 FCFA2 for model without adaptation (M1) and up to 1316 FCFA for model with adaptation (M2). An increase of Precipitation of 1 mm/month will increase crop receipts for the frameworks up to 721,917 FCFA for M1 and 1,861,455 FCFA for M2. In order to predict climate change impacts for these regions, the RCP 4.5 and RCP 8.5 of IPCC scenarios were examined. The crop net receipts will fall between 10% and 26% if the scenarios happen. Another finding of this study is that each farmer who is practicing adaptation is able to cover the potential loss from climate variability up to 8.95% and 12.71% per ha respectively in Maradi and in Dosso. The study proposes that these regions should start planning measures for unexpected event of climate conditions. Irrigated systems need to be encouraged in order to minimize the vulnerability of the agricultural sector.
