Consensus views on competencies and teaching methods for an interprofessional curriculum on complementary and integrative medicine:A Delphi study

Removal of endocrine disrupting chemicals from water through urethane functionalization of microfiltration membranes via electron beam irradiation

Polyethersulphone(PES)membranes modified with urethane functional groups were prepared through an interfacial reaction using electron beam *** removal of eight endocrine disrupting chemicals(EDCs)was studied using both pristine and functionalized PES *** prepared membranes underwent characterization using several techniques,including attenuated total reflectance-Fourier transform infrared(ATR-FTIR)spectroscopy,scanning electron microscopy,contact angle analysis,and measurements of pure water ***,dynamic adsorption experiments were conducted to evaluate the adsorption mechanism of the prepared membrane toward the eight *** urethane functionalized membranes were hydrophilic(52°contact angle)and maintained a high permeate flux(26000 L/h m^(2) bar)throughout the filtration *** adsorption results demonstrated that the introduction of urethane functional groups on the membranes significantly enhanced the removal efficiency of 17β-estradiol,estriol,bisphenol A,estrone,ethinylestradiol,and *** adsorption loading of 17β-estradiol on the functionalized PES membrane was 6.7±0.7 mg/m^(2),exhibiting a 5-fold increase compared to the unmodified PES *** membranes were successfully regenerated and reused for three adsorption cycles without experiencing any loss of adsorption capacity.

目的:本论文基于针刺镇痛方面的神经解剖和神经生物学研究文献,以及其导师所研究的《黄帝内经》的针灸皮脉肉筋骨层次理论,探讨了针刺治疗中针刺深度与疗效的相关性。腰痛(LBP)的患病率高达84%,是最常见的肌肉骨骼疾病之一,也是针灸临床的常见有效病症。本研究在古今文献研究基础上,选择导师临床慢性腰痛典型病例,真实记录治疗全部经过,探讨针灸层次理论对针灸师在临床中选穴、决定进针深度、选取针具及手法的重要指导价值。方法:文献部分,通过PubMed MEDLINE数据库进行了全面的文献综述。文献检索中使用的关键词是“针灸”或“针灸疗法”或“穴位”与“治疗”和/或“疗法”、“腰痛”或“慢性腰痛”、“疼痛机制”、“针灸机制”、“针刺深度”、“干针”和/或“肌筋膜触发点”。文献检索仅限于以英语或德语发布的结果,同时还检索了书籍。如果检索结果中出现了一篇重要的带有英文摘要的中文文章,那么这篇文章就会被翻译并收录。临床部分,根据预定义的纳入和排除标准招募了 3名受试者。临床病例报告分析中包含的信息是根据CARE指南(针对CAse REports)进行的。临床数据收集中通过设置可视化流程图系统记录治疗的全部经过,包括得气感觉,还确定了有效治疗的关键因素:适当(正确)的针刺深度、针具(针灸针的种类)、针刺技术和穴位/阿是穴的选择。疗效评估,采用数值评定量表(NRS)主观疼痛评分量表,记录受试者在治疗前和治疗后立即对疼痛程度进行评分。结果:文献的分析以及对病例报告的评估表明,针刺深度与针刺治疗LBP的疗效之间密切相关。现代针刺镇痛研究可以从神经解剖学神经生物学原理,尤其是神经分布和A-δ和C-神经纤维给予解释,这些结论一定程度上与2000多年前《黄帝内经》中的层次论治理论有一定的相关性。临床病例观察显示,3名慢性LBP的受试者表现出不同的症状特征和疼痛强度,治疗中依据据患者的症状特征进行层次诊断,并给予相应的治疗。因此,3位患者根据其病变的不同层次(肉、筋、骨)选取不同的穴位、针刺深度、乃至针具和手法,但都取得较好的显著的临床疗效。在受试者1中,病变位于于肉层和筋层。选取的每一穴位都产生了得气感觉,最强烈的针感出现在阿是穴(大致在胸最长肌和腰髂肋肌之间的区域),并且在整个治疗过程中均使用该穴位;受试者2也表现出局部疼痛,没有向其他区域辐射,受影响的层层次仅限于肉层,但与受试者1相比更浅;受试者3不仅是局部疼痛,而且还向下辐射到膝盖,受影响最大的层是骨层。因此,与其他两名受试者相比,受试者3的治疗更加复杂,针刺了三个不同的层(肉、筋和骨)。局部穴位和远端穴位的配合以及针刺层次的选择可能得到了良好的疗效。目前的文献也证实了这一结果。结论:有关针灸机制的神经解剖学和神经生理学理论模型可以与现代研究结果在一定程度上与《黄帝内经》中基于古代解剖学总结出的皮脉肉筋骨层次理论相一致。也就是说,在大多数情况下,经络的运行过程反映了人体内的神经分布。同时,A-δ和C-神经纤维对最佳治疗效果(镇痛效果)起着关键作用,因此必须刺激这两种神经纤维。得气感觉也与这些纤维有关。一些研究还表明,针刺感觉可能会随着深度的增加而增加。由此可以进一步推断,神经分支的密度也随着深度的增加而增加,因此得气感觉更强。这反过来可能导致与疼痛级联下降相关的大脑区域更强激活。这项研究也表明,针刺不同组织能够下调疼痛级联。案例研究中使用的一些穴位具有高密度的神经结构,这可能会大大减轻疼痛并放松紧张的肌肉。由于对不同组织的更大刺激,对更深的层给予刺激,可以提高临床疗效。除了穴位和针刺深度(层)的选择,针灸师的针具和针刺技术也对临床疗效起着关键作用。在阐释针刺镇痛机制时,可以考虑层次论治和神经解剖学、神经生理学理论模型。由于这一主题的复杂性,需要进行进一步的研究。

Methodological impact of starch determination on starch content and ileal digestibility of starch in grain legumes for growing pigs

Background： Grain legumes represent a valuable energy source in pig diets due to their high starch content. The present study was conducted to determine the content and apparent ileal digestibility（AID） of starch in different grain legume cultivars for pigs by means of both a polarimetric and enzymatic method for starch ***： Three experiments were conducted with six barrows each which were fitted with ileal T-cannulas. In total, 18 diets including six different cultivars of faba beans（Vicia faba L.） and peas（Pisum sativum L.）, five different cultivars of lupins（Lupinus luteus L., Lupinus angustifolius L.）, and one diet with a soybean meal（SBM） were ***： The starch content of faba beans and peas was greater（P ＆lt; 0.05） when determined polarimetrically than enzymatically（438 vs. 345 g/kg dry matter（DM） in faba beans and 509 vs. 390 g/kg DM in peas, respectively）.Considerable lower starch contents were obtained in lupins and SBM, with 82 and 48 g/kg DM（analyzed polarimetrically） and ＆lt;1.1 and 3 g/kg DM（analyzed enzymatically）, respectively. Mean values for contents of neutral detergent fiber（NDF） and acid detergent fiber（ADF） in grain legumes ranged from 111 and 79 g/kg DM in peas to248 and 207 g/kg DM in lupins, respectively. Contents of condensed tannins in the colored flowered faba bean cultivars ranged from 2.1 to 7.4 g/kg DM. The AID of starch was greater（P ＆lt; 0.05） in pea than in faba bean cultivars,and using the polarimetric starch determination method resulted in greater（P ＆lt; 0.05） digestibility values than using enzymatic starch analysis（84 vs. 80% in faba beans and 86 vs. 83% in peas）. Background： During processing in a desolventizer/toaster（DT）, rapeseed meal（RSM） is heated to evaporate the hexane and to reduce the level of heat-labile anti-nutritional factors such as glucosinolates（GSL）. However, excessive heat treatment may reduce amino acid（AA） content in addition to lower AA digestibility and availability in RSM. The objective of the present study was to produce from one batch of a 00-rapeseed variety（17 μmol GSL/g dry matter（DM）, seed grade quality） five differently processed RSM under standardized and defined conditions in a pilot plant,and to determine the impact of these different treatments on protein solubility and chemical composition, in particular with regard to contents of AA including reactive Lys（rL ys） and levels of total and individual ***： Four RSM were exposed to wet toasting conditions（WetT C） with increasing residence time in the DT of 48,64, 76, and 93 min.
