Introduction: Food allergies are on the constant increase worldwide. Among them is peanut allergy, which also affects children. The aim of this study was to determine the profile of peanut sensitization in children attending a pneumo-allergology consultation. Methods: This was a cross-sectional study, which ran from January 1, 2018, to December 31, 2022, on children sensitized to peanuts seen in pneumo-allergology consultations at the Teaching Hospital Campus of Lomé. Results: The sample included 137 children aged 3 months to 18 years. The frequency of peanut sensitization was 25.3%. The mean age of patients was 6.3 ± 4.3 years (minimum 6 months and maximum 18 years). The 6 months to 5 years age group was the most represented (43.1%). The sex ratio was 1.3. The main reasons for consultation were rhinorrhea (67.9%), sneezing (36.5%) and cough (35.8%). Allergic rhinitis was identified as a personal history in 75.2% of patients. Peanut allergy was manifested as rhinorrhea (62.5%), asthma (26.8%) and eczema (8.9%). The risk of onset of symptoms within 15 - 30 minutes was 1.87 times (p = 0.001, CI = [1.2 - 2.1]) for peanuts consumed in roasted form with shell. Severe clinical signs such as Quincke’s Edema and anaphylactic shock were found in 1.4% of cases. Conclusion: Peanut allergy was common in children. Severe clinical signs were rare.

Introduction: Respiratory distress in neonates is a neonatal emergency that can lead to serious complications if not treated appropriately. The aim of this study was to describe the epidemiology, the diagnostic, and the outcomes of neonatal respiratory distress. Methods: This was a cross-sectional study carried out in the pediatric wards of Lomé Teaching Hospitals (CHU Sylvanus Olympio and CHU Campus), including neonates treated for respiratory distress (dyspnea associated with the use of accessory muscles of respiration, noisy breathing and with or without cyanosis) from January 1, 2021 to December 31, 2021. Data were entered using Epi Data 3.1 and SPSS software version 12.0. Results: The total number of neonates hospitalized for respiratory distress was 353, with a frequency of 12.5% and a sex ratio of 1.5. The mean age was 0.82 ± 3.20 days;the 0 - 6-day age group accounted for 92.4% of cases. Neonates had been resuscitated at birth in 46.7% of cases. Dyspnea was tachypnea in 94% of cases and bradypnea in 6%. Dyspnea was associated with cyanosis in 21.5% of cases. The severity of the respiratory distress was moderate in 64.9% of cases. Perinatal asphyxia (49.1%), inhalation pneumonitis (17.1%) and neonatal bacterial infection (14.1%) were the main etiologies. The mortality rate was 20.4%. Age greater than or equal to seven days, no neonatal resuscitation were protective factors against death. Prematurity, no antenatal consultations follow up, neonatal resuscitation, severe respiratory distress were risk factors of death. Conclusion: Neonatal respiratory distress was common in the early neonatal period and its mortality was high.

Understanding the characteristics of extreme rainfall is crucial for effective flood management planning, as it enables the incorporation of insights from past extreme rainfall patterns and their spatiotemporal distribution. This work investigated the changes in the frequency and pattern of extreme rainfall over Uganda, using daily datasets sourced from Climate Hazard Group InfraRed Precipitation with Station (CHIRPS-v2) for the period 1981 to 2022. The study utilized the extreme weather Indices provided by the Expert Team on Climate Change Detection and Indices (ETCCDI). Attention was directed towards September to November (SON) rainfall season with precise analysis of four indices (Rx1day, Rx5day, R95p, and R99p). The Sequential Mann-Kendall (SQMK) non-parametric test was applied to identify abrupt changes in SON extreme rainfall trends. Results showed that October consistently recorded the highest count of extreme rainfall days across all four indices. The long-term analysis revealed fluctuations in extreme rainfall events across years, with certain periods exhibiting heightened intensity. The analysis portrayed a shift in the decadal variations and region-specific distribution of extreme rainfall, with Eastern Uganda and areas around Lake Victoria standing out compared to other regions. The findings further revealed an increase in extreme rainfall for all indices in the recent decade (2011-2022) with 2019/2020 standing out as the extreme years of SON for the study period. While trendlines suggested a slight increase in intense daily rainfall events, the SQMK tests revealed statistical significance in the trend of prolonged periods of intense daily rainfall. This study contributes to the understanding of the spatiotemporal variability and trends of extreme rainfall events over Uganda during the SON season, which is crucial for the assessment of climate change impacts and adaptation strategies. Climate change has been a global pandemic with its adverse impacts affecting environments and livelihoods. This has been largely attributed to anthropogenic activities which generate large amounts of Green House Gases (GHGs), notably carbon dioxide (CO2), methane (CH4), and nitrous oxide (N2O) among others. In the Upper East of Ghana, climate change manifests in erratic rainfalls, drought, high temperatures, high wind speeds, high intensity rainfall, windstorms, flooding, declining vegetation cover, perennial devastating bushfires etc. Practices such as burning farm residues, use of dung as fuel for cooking, excessive application of nitrogenous fertilizers, and deforestation that are prevalent in the region exacerbate the situation. Although, efforts made by governmental and none-governmental organizations to mitigate climate change through afforestation, agroforestry and promotion of less fuelwood consuming cook stoves, land management practices antagonize these efforts as more CO2 is generated than the carrying capacity of vegetation in the region. Research findings have established the role of trees and soil in carbon sequestration in mitigating climate. However, there is limited knowledge on how the vegetation and soil in agroforestry interplay in mitigation climate change. It is against this background that this review seeks to investigate how vegetation and soil in an agroforestry interact synergistically to sequester carbon and contribute to mitigating climate change in Upper East region of Ghana. In this review, it was discovered soil stored more carbon than vegetation in an agroforestry system and is much effective in mitigating climate change. Flora, Structure and Carbon Sequestration of Vegetation in the Southeast of the Mono Biosphere Reserve in Togo Amidst Environmental Challenges

In most Sub-Saharan African countries such as Togo, people’s heavy dependence on ecosystem services is a major factor in accelerating the degradation of natural resources, which are already suffering as a result of climatic factors. This study was initiated to contribute to the sustainable and rational management of forest resources in the south-east of the Mono Biosphere Reserve in Togo. It specifically aims to identify and characterize the flora of the residual forest ecosystems within the reserve through their specific diversity, demographic structure, and carbon sequestration potential. The study was carried out in the forest ecosystems of Avévé. The methodology used was based on the analysis of phytosociological, forestry, ecological, and regeneration inventory data. Overall, the study revealed that the RBMT still has floristically viable habitats, despite the anthropogenic pressures it is subject to revealed a floristic diversity of 160 plant species divided into 52 families and 135 genera. The most represented families are Rubiaceae (29.09%), followed by Fabaceae (27.94%). The most represented species are Mitragyna inermis (Willd.) Kuntze (24.38%), Lecaniodiscus cupanioides Planch. Ex Benth (X) and Lonchocarpus sericeus (Poir.) Kunth (10.93%). The high presence of Mitragyna inermis observed in all the ecological groups identified makes it the characteristic species of the flooded marshy areas of southeast Togo and contributes to the resilience of the ecosystems and populations in the study area. The Shannon index for the formation groups varies between (3.03 and 5.16) bits. Pielou’s equitability varies between (0.43 and 0.63) bits. The overall average density is estimated at 210 stems/ha, with an average diameter of 25.57 ± 21.77 m and an average height of 7.93 ± 3.83 m. Introduction: Traumatic Brain Injury (TBI) is a major public health problem causing significant morbidity and mortality in young adults. This study aimed to describe the epidemiological, diagnostic, therapeutic, and evolutionary aspects of TBI. Materials and Methods: This was a prospective, descriptive study conducted from 1 April 2022 to 31 March 2023 on patients admitted to and treated for cranioencephalic trauma in the General Surgery department of Kara Regional Hospital. Results: Eighty-three (83) patients with cranioencephalic trauma were managed out of 773 patients admitted to the department during the study period. The mean age was 34 ± 14.98 years and the sex ratio was 3.6 in favour of men. Motorbike taxi drivers were the social group most affected (n = 33, 40%). The causes of trauma were dominated by public road accidents (n = 80;96%). TBI was mild (n = 40;48%), moderate (n = 35;42%) and severe (n = 8;10%). Cerebral CT scans were performed in 19 patients (23%). Cerebral contusion (n = 4) was the most frequent cerebral lesion. Six patients (7%) with severe head injuries were transferred to Kara University Hospital. Six deaths (7%) occurred in patients with severe head injuries. The main sequelae were intermittent headaches in all patients reviewed, and memory problems (6%). Conclusion: Traumatic brain injuries are common at Kara Regional Hospital. Severe cranial trauma is less frequent but leads to death because of financial difficulties and limited technical facilities.

This study focuses on the landscape dynamics of the savannahs’ region in the far north of Togo. Based on a literature review and satellite images analysis using GIS and remote sensing, the study aims to ascertain the effects of anthropogenic threats on the forest coverage of the Savannahs’ Region between 1984 to 2020. The objective is to clarify the dynamics of land use in the region from 1984 to 2000 and from 2000 to 2020. The findings indicate a significant decline in forest coverage within the region from 1984 to 2020, a trend attributed to land use patterns. Dry forests in the Savannah region are largely converted to farmlands, housing, dry savannahs or agroforestry parks, leading to a steady reduction in forest areas.

Skin and Soft Tissue Infections in the Surgical Area at the Kara Teaching Hospital

Objective: To report the management of skin and soft tissue infections in the surgical area of Kara University Hospital in Togo. Material and Methods: This study was conducted retrospectively from January 1, 2021, to December 31, 2022, in the general surgery and orthopedic trauma departments. The study focused on soft tissue infections of the pelvic and thoracic limbs and analyzed epidemiological, clinical, paraclinical, therapeutic, and evolutionary data. Results: We registered 165 patients, comprising 109 men and 56 *** sex ratio (F/H) were 0.51. The mean age was 45 years with extremes ranging from 23 to 90 years. Farmers (64.8%) followed by housewives (34.0%) were the social strata most affected. The consultation period varied between 1 and 90 days. The pathologies found were necrotizing fasciitis (53.3%), erysipelas (18.2%), infected limb wounds (12.1%), pyomyositis (9.7%), and necrotizing dermo-hypodermitis (1.8%). The main procedures performed were necrosectomy and grafting (62.9%), sample necrosectomy (18.8%), drainage (9.7%), and pelvic limb amputation (1.2%). Follow-up was favorable in 86.7% of cases. The study noted a death rate of 13.3% due to septic shock secondary to a delay in consultation. Conclusion: Skin and soft tissue infections were a common reason for surgical hospitalization at Kara University Hospital, with a high mortality rate due to delayed consultations.

Introduction: The acronym SAPHO (Synovitis, Acne, Pustulosis, Hyperostosis and Osteitis) is a syndrome combining osteoarticular and cutaneous manifestations. It occurs mainly between the ages of 30 and 50. Sternocostoclavicular hyperostosis is one of the main distinguishing features. We report a case of SAPHO in Dakar diagnosed by bone scintigraphy. Observation: 28-year-old Senegalese women presented with left shoulder pain and relative functional impotence for over 2 years. Examination revealed right sternoclavicular hyperostosis and left shoulder pain on palpation. Questioning revealed a history of acne and hyperostosis of the right first toe. Bone scintigraphy, performed after injection of 630 MBq of 99mTc-HMDP, revealed: hyperfixation of the bilateral (right++) manubrio-sternal and sternoclavicular junction, producing the classic bull’s horn image;hyperfixation of the left shoulder with an inflammatory appearance;hyperfixation of the sacroiliac joints suggestive of bilateral sacroiliitis;hyperfixation of the right first toe;two mandibular hyper fixations probably related to dental damage. This scintigraphic appearance in one was strongly suggestive of SAPHO syndrome. Conclusion: SAPHO syndrome, related to spondyloarthropathy, associates cutaneous and osteoarticular signs. It is characterized by frequent delays in diagnosis due to poor recognition. Soy is an invaluable diagnostic tool, enabling us to assess the extent of the disease and its evolution.
